1. Instabilität des Kniegelenks – medial oder anteromedial?

Abstract: Zusammenfassung Hintergrund Verletzungen des medialen Bandkomplexes gehören zu den häufigsten Knieverletzungen. Sie heilen zwar meist mit konservativer Therapie problemlos aus, persistierende Instabilitäten erhöhen aber die Belastung der Kreuzbänder und benötigen speziell bei deren Beteiligung eine adäquate Therapie. Anatomie und Biomechanik Der mediale Seitenbandkomplex besteht im Wesentlichen aus dem oberflächlichen Seitenband (sMCL), welches der primäre Stabilisator gegen Valgus ist, dem tiefen Seitenband (dMCL) mit dessen schräg verlaufendem ventralem Anteil (AML), die nur sekundäre Stabilisatoren gegen Valgus darstellen, aber primär gegen Außenrotation stabilisieren, und dem hinteren Schrägband (POL), das in vollständiger Streckung gegen Valgus sowie gegen Innenrotation stabilisiert. Therapie Chronische Instabilitäten bzw. höhergradige Verletzungen mit Dislokation der Bandstümpfe oder multiligamentäre Verletzungen bedürfen einer operativen Versorgung. Im Akutstadium zeigen Avulsionsverletzungen bei anatomischer Refixation gutes Heilungspotenzial, während bei intraligamentären Verletzungen zusätzlich zur Naht der Bandanteile eine Augmentation mit Sehnenmaterial empfohlen wird. Bei chronischen Instabilitäten ist die Differenzierung des Instabilitätsmusters ausschlaggebend für die Wahl der Rekonstruktionstechnik (reine sMCL-Rekonstruktion oder kombinierte Rekonstruktion von sMCL und AML). In beiden Fällen kommt die hier beschriebene Technik mit flachem Transplantat der Anatomie näher als bei konventionellen Verfahren. Diskussion Rekonstruktionstechniken unter Verwendung flacher Sehnentransplantate, die alle betroffenen Bandanteile adressieren, haben sich biomechanisch als sehr effektiv erwiesen, komplexe mediale Instabilitäten suffizient zu behandeln. Ob diese auch klinisch überlegen sind, werden zukünftige klinische Studien zeigen müssen.

Abstract: The study of hot corinos in solar-like protostars has been so far mostly limited to the Class 0 phase, hampering our understanding of their origin and evolution. In addition, recent evidence suggests that planet formation starts already during Class I phase, which therefore represents a crucial step in the future planetary system chemical composition. Hence, the study of hot corinos in Class I protostars has become of paramount importance. Here, we report the discovery of a hot corino towards the prototypical Class I protostar L1551 IRS5, obtained within the ALMA (Atacama Large Millimeter/submillimeter Array) Large Program FAUST (Fifty AU STudy of the chemistry in the disc/envelope system of solar-like protostars). We detected several lines from methanol and its isotopologues (13CH3OH and CH2DOH), methyl formate, and ethanol. Lines are bright towards the north component of the IRS5 binary system, and a possible second hot corino may be associated with the south component. The methanol lines' non-LTE analysis constrains the gas temperature (∼100 K), density (≥1.5 × 10^8 per cu.cm), and emitting size (∼10 au in radius). All CH3OH and 13CH3OH lines are optically thick, preventing a reliable measure of the deuteration. The methyl formate and ethanol relative abundances are compatible with those measured in Class 0 hot corinos. Thus, based on this work, little chemical evolution from Class 0 to I hot corinos occurs.

Abstract: Ziel dieser Operation ist es, das mediale patellofemorale Ligament mittels distal gestieltem Quadricepssehnenstreifen zu ersetzen. Hierzu wird mit einem minimal invasiven Instrumentarium ein Quadricepssehnenstreifen uber einen kleinen superomedialen Zugang entnommen und dann nach medial zwischen zweiter und dritter Schicht des medialen Retinakulums geshuttelt. Einer der entscheidendsten Schritte ist das bildwandergestutzte Aufsuchen des Isometriepunkts, sodass physiologische Langenveranderungen und ein zufriedenstellendes postoperatives Ergebnis resultieren. Hiernach kann das Transplantat unter dosiertem Zug und Zentralisierung der Patella femoral fixiert werden. Eine fruhfunktionelle Nachbehandlung fuhrt hierbei zu einem guten Outcome.

Abstract: C. Vastel et al., [Aims] Methanol is a ubiquitous species commonly found in the molecular interstellar medium. It is also a crucial seed species for the build-up of chemical complexity in star forming regions. Thus, understanding how its abundance evolves during the star formation process and whether it enriches the emerging planetary system is of paramount importance., [Methods] We used new data from the ALMA Large Program FAUST (Fifty AU STudy of the chemistry in the disc/envelope system of solar protostars) to study the methanol line emission towards the [BHB2007] 11 protobinary system (sources A and B), where a complex structure of filaments connecting the two sources with a larger circumbinary disc has previously been detected., [Results] Twelve methanol lines have been detected with upper energies in the [45–537] K range along with one 13CH3OH transition and one methyl formate (CH3OCHO) line blended with one of the methanol transitions. The methanol emission is compact (FWHM ~ 0.5″) and encompasses both protostars, which are separated by only 0.2″ (28 au). In addition, the overall methanol line emission presents three velocity components, which are not spatially resolved by our observations. Nonetheless, a detailed analysis of the spatial origin of these three components suggests that they are associated with three different spatial regions, with two of them close to 11B and the third one associated with 11A. A radiative transfer analysis of the methanol lines gives a kinetic temperature of [100–140] K, an H2 volume density of 106–107 cm−3 and column density of a few 1018 cm−2 in all three components with a source size of ~0.15″. Thus, this hot and dense gas is highly enriched in methanol with an abundance as high as 10−5. Using previous continuum data, we show that dust opacity can potentially completely absorb the methanol line emission from the two binary objects., [Conclusions] Although we cannot firmly exclude other possibilities, we suggest that the detected hot methanol is resulting from the shocked gas from the incoming filaments streaming towards [BHB2007] 11A and B, respectively. Abstract: There is an ever-increasing need to design better methods to selectively connect two molecules under mild aqueous conditions on a small scale. The process of finding such methods significantly relies on the employment of an appropriate assay. We report here a modular FRET-based assay to monitor such reactions and illustrate how the assay is used to monitor two particular reactions: native chemical ligation (NCL) and oxime ligation. For both reactions we show that by employing appropriately designed probes FRET measurements could be used to monitor the reaction's progress. We additionally demonstrate the usefulness of the developed probe system to study the mechanisms of the ligation reactions, for example, in monitoring the formation of a trimeric intermediate in the NCL reaction. Finally, we demonstrate that FRET measurements conducted in our system allow the quantification of the reaction yield and we show the application of our FRET-based assay to catalyst screening for the oxime ligation.

ACL re-rupture including special demands (e.g., high-performance athletes, hyperlaxity) RELATIVE CONTRAINDICATIONS: Osteoarthritis, additional instability of the knee, which should be treated independently; non-anatomic ACL reconstruction with persisting instability should be treated first with anatomic ACL reconstruction.General contraindications for surgery (e. g. septic arthritis), acute irritation of the affected knee.Supine position. Incision along the proximal lateral femoral epicondyle. Marking of the needed width and length of the iliotibial band (ITB) graft. Passing the ITB graft underneath the lateral collateral ligament. Find and mark the isometric point for fixation next to the lateral femoral epicondyle. Fixation of the ITB graft. Layered wound closure.Knee brace for at least 6 weeks. Range of motion (RoM): from postoperative day 1: flexion-extension 90-0-0°; first 2 weeks after surgery: partial weight bearing (20 kg).An anterolateral extra-articular reconstruction may reduce a persistent anterolateral rotatory instability as well as the re-rupture rate following ACL reconstruction with good patient-reported short-term outcomes. Based on current (biomechanical) data, anterolateral tenodesis seems to be superior to a reconstruction of the anterolateral ligament. If a tenodesis is performed, the graft should be fixed in an isometric position, with neutral rotation of the knee and low graft tension to avoid extraphysiologic load within the lateral compartment. Indications for such a procedure may include a high-grade pivot shift or revision ACL reconstruction as well as a persistent anterolateral rotatory instability following anatomic ACL reconstruction.

Abstract: In this review I introduce readers to three exemplary ethnographies. All three contribute not only to anthropological and queer studies literatures but also to discourses and critiques of globalization, transnationalism, and neoliberalism. Further, these works destabilize notions of what constitutes ethnography in the general field of queer studies and demonstrate that queer anthropology is imperative to consider in present and future developments in queer theory, methods, and analyses. These texts argue that it is time to move beyond the search for that which is “queer” as emerging from “tradition.” They demonstrate that social constructions of queer subjectivities are ever-changing and emerging in contemporary historical contexts, often in relation to the nation-state and against hegemonic Euro-American notions of what is considered “queer.”

Abstract: By means of Montessori education, children aged from 3 to 6 years were introduced to the basics of earth-sciences in general and of glaciology in particular. For this purpose, a one-year-program was established under the principles of Cosmic Education (Sensu Montessori) in order to provide an insight in some parts of the afore mentioned sciences via 3 steps. Step 1 brought up introductory informations about earth sciences, historical geology, planetology, and climatology. In Step 2, specific information about the Ice Ages and glaciology was given by introducing a story about a personified glacier which included information on mass budget (ablation/accumulation), dynamics, features (crevasses), and in relation to those, the dangers of a glacier. Step 3 completed the program by giving the children the opportunity for depicting and acting out their knowledge via glacier-“modelling”, painting, dancing, and making music. 17. Organic molecular anions in interstellar and circumstellar environments

Abstract: The synthesis of organic molecular anions in TMC-1 and IRC+10216 is investigated. Modelled C2H−, CN−, C3N−, C5N− and C7N− column densities are sufficiently great that these species might be observable in IRC+10216. Density-enhanced shells in the outer envelope of IRC+10216 are found to enhance the C2H− and CN− column densities by shielding these anions from destruction by UV radiation. From a newly-derived upper column density limit of 6.6 × 1010 cm−2 for C2H− in IRC+10216 we deduce the primary production mechanism for this anion to be C2H2 + H− ⇒ C2H− + H2. In TMC-1, due to the low radiative electron attachment rates calculated for C2H−, CN− and CH2CN−, these species have modelled column densities below the detection threshold. They could, however, be produced in reactions we have not yet considered.

Abstract: We have used the Herschel-HIFI instrument to observe both nuclear spin symmetries of amidogen (NH2) towards the high-mass star-forming regions W31C (G10.6-0.4), W49N (G43.2-0.1), W51 (G49.5-0.4) and G34.3+0.1. The aim is to investigate the ratio of nuclear spin types, the ortho-to-para ratio (OPR), of NH2. The excited NH2 transitions are used to construct radiative transfer models of the hot cores and surrounding envelopes in order to investigate the excitation and possible emission of the ground state rotational transitions of ortho-NH2 N_(K_a,K_c} J=1_(1,1) 3/2 - 0_(0,0) 1/2 and para-NH2 2_(1,2) 5/2 - 1_(0,1) 3/2$ used in the OPR calculations. Our best estimate of the average OPR in the envelopes lie above the high temperature limit of three for W49N, specifically 3.5 with formal errors of \pm0.1, but for W31C, W51, and G34.3+0.1 we find lower values of 2.5\pm0.1, 2.7\pm0.1, and 2.3\pm0.1, respectively. Such low values are strictly forbidden in thermodynamical equilibrium since the OPR is expected to increase above three at low temperatures. In the translucent interstellar gas towards W31C, where the excitation effects are low, we find similar values between 2.2\pm0.2 and 2.9\pm0.2. In contrast, we find an OPR of 3.4\pm0.1 in the dense and cold filament connected to W51, and also two lower limits of >4.2 and >5.0 in two other translucent gas components towards W31C and W49N. At low temperatures (T \lesssim 50 K) the OPR of H2 is 20-25 K, depending on time, and values above three at lower temperatures., 21 pages, 23 figures. Chemistry also occurs in giant clouds of gas and dust particles that exist in the rarefied regions among the stars. In the so-called dense interstellar clouds, nearly 200 different molecules, most of them organic in nature, have been discovered in the gas. Molecules have also been discovered to exist in ice mantles that surround dust particles. In this article, we discuss the chemistry that produces these diverse molecules, especially in portions of dense clouds that are collapsing to form new stars and planets.

Abstract: The N = 2 ← 1 rotational transition of the PH radical in its ground electronic (X3 Σ- ) and vibrational states has been measured with the Cologne terahertz spectrometer in the frequency region between 920 and 1070 GHz. The PH radical was generated by immersing red phosphorus in a dc glow discharge of molecular hydrogen buffered with argon. Transition frequencies of the five J′ - J″ fine structure components together with the associated hyperfine structure patterns were precisely measured and analyzed to derive highly accurate molecular parameters, which are more extensive than those reported in the existing literature. Among the determined parameters are the rotational constant B0 = 252200.8099(63) MHz and the centrifugal distortion constant D0 = 13.2915(33) MHz, as well as two fine structure constants - the spin-rotation constant γ0 and the spin-spin interaction constant λ0 - together with their centrifugal distortion contributions Dγ0 and Dλ0. Furthermore, each of the magnetic hyperfine parameters bF, c, and the nuclear spin-rotation constants CI were obtained for both nuclei. The accuracy of previously obtained molecular constants has been improved by up to one order of magnitude. This new set of molecular parameters allows highly reliable frequency predictions of the rotational spectrum extending into the far infrared region; such transition frequencies may be of interest for interstellar spectroscopy.

Abstract: In this paper, we discuss the chemistry that occurs in the cold dense interstellar medium and as temperatures rise above standard interstellar cloud values due to star formation or other energetic processes. During the warm up to the hot core stage (100–300 K), a radical photochemistry on grain surfaces leads to the formation of complex organic molecules (COMs), which then desorb into the gas, where they are detected. At still higher temperatures, it is necessary to consider a large number of additional gas-phase reactions that become important. We emphasize a new high-temperature gas-phase network that is capable of being used in simulations of the chemistry that occurs at temperatures up to 800–1000 K. Examples are given. It is also possible that a very high temperature surface chemistry, in which species other than atomic hydrogen stick to grains via strong chemisorption forces, occurs.

Abstract: We have used the Herschel-HIFI instrument to observe interstellar nitrogen hydrides along the sight-lines towards W49N and G10.6-0.4 in order to elucidate the production pathways leading to nitrogen-bearing species in diffuse gas. All detections show absorption by foreground material over a wide range of velocities, as well as absorption associated directly with the hot-core source itself. As in the previously published observations towards G10.6-0.4, the NH, NH2 and NH3 spectra towards W49N show strikingly similar and non-saturated absorption features. We decompose the absorption of the foreground material towards W49N into different velocity components in order to investigate whether the relative abundances vary among the velocity components, and, in addition, we re-analyse the absorption lines towards G10.6-0.4 in the same manner. Abundances, with respect to molecular hydrogen, in each velocity component are estimated using CH. The analysis points to a co-existence of the nitrogen hydrides in diffuse or translucent interstellar gas with a high molecular fraction. Towards both sources, we find that NH is always at least as abundant as both o-NH2 and o-NH3, in sharp contrast to previous results for dark clouds. We find relatively constant N(NH)/N(o-NH3) and N(o-NH2)/N(o-NH3) ratios with mean values of 3.2 and 1.9 towards W49N, and 5.4 and 2.2 towards G10.6-0.4, respectively. The mean abundance of o-NH3 is ~2x10^-9 towards both sources. The nitrogen hydrides also show linear correlations with CN and HNC towards both sources, and looser correlations with CH. The upper limits on the NH+ abundance indicate column densities < 2 - 14 % of N(NH). Surprisingly low values of the ammonia ortho-to-para ratio are found in both sources, ~0.5 - 0.7 +- 0.1. Abstract: After rapid cooling in a supersonic jet expansion, the populations of the lowest rotational levels of methanol have been determined from the intensities of IR absorption transitions of the C-O stretching band at 10 μm. A newly constructed multireflection system (White type, 16 passes) has been employed for the detection of weak absorption lines. The rotational temperature of methanol has been found to be lower for the E -symmetry substate of the ground ( v t = 0) torsional state than for the A -symmetry substate: T rot ( A -Sym) = (15.8 ± 0.5) K and T rot ( E -Sym) = ( 13.3 ± 0.9) K. No torsional hot-band transitions have been observed in the jet spectrum, indicating an efficient cooling of the torsional degree of freedom. The population distribution of the probed rotational levels is strongly nonthermal, or "normal," in the sense that the A / E population ratio maintains its room temperature value of near unity.

Abstract: We report the first detection of chloronium, H$_2$Cl$^+$, in the interstellar medium, using the HIFI instrument aboard the \emph{Herschel} Space Observatory. The $2_{12}-1_{01}$ lines of ortho-H$_2^{35}$Cl$^+$ and ortho-H$_2^{37}$Cl$^+$ are detected in absorption towards NGC~6334I, and the $1_{11}-0_{00}$ transition of para-H$_2^{35}$Cl$^+$ is detected in absorption towards NGC~6334I and Sgr~B2(S). The H$_2$Cl$^+$ column densities are compared to those of the chemically-related species HCl. The derived HCl/H$_2$Cl$^+$ column density ratios, $\sim$1--10, are within the range predicted by models of diffuse and dense Photon Dominated Regions (PDRs). However, the observed H$_2$Cl$^+$ column densities, in excess of $10^{13}$~cm$^{-2}$, are significantly higher than the model predictions. Our observations demonstrate the outstanding spectroscopic capabilities of HIFI for detecting new interstellar molecules and providing key constraints for astrochemical models., Comment: Accepted for publication in A&A, HIFI Special Issue; 4 pages, 2 figures, 1 table

Abstract: We report the detection of absorption by interstellar hydroxyl cations and water cations, along the sight-line to the bright continuum source W49N. We have used Herschel's HIFI instrument, in dual beam switch mode, to observe the 972 GHz N = 1 - 0 transition of OH+ and the 1115 GHz 1(11) - 0(00) transition of ortho-H2O+. The resultant spectra show absorption by ortho-H2O+, and strong absorption by OH+, in foreground material at velocities in the range 0 to 70 km/s with respect to the local standard of rest. The inferred OH+/H2O+ abundance ratio ranges from ~ 3 to ~ 15, implying that the observed OH+ arises in clouds of small molecular fraction, in the 2 - 8% range. This conclusion is confirmed by the distribution of OH+ and H2O+ in Doppler velocity space, which is similar to that of atomic hydrogen, as observed by means of 21 cm absorption measurements, and dissimilar from that typical of other molecular tracers. The observed OH+/H abundance ratio of a few E-8 suggests a cosmic ray ionization rate for atomic hydrogen of (0.6 - 2.4) E-16 s-1, in good agreement with estimates inferred previously for diffuse clouds in the Galactic disk from observations of interstellar H3+ and other species., Accepted for publication in A&A Letters

Abstract: We present spectrally resolved observations of triatomic carbon (C3) in several ro-vibrational transitions between the vibrational ground state and the low-energy nu2 bending mode at frequencies between 1654-1897 GHz along the sight-lines to the submillimeter continuum sources W31C and W49N, using Herschel's HIFI instrument. We detect C3 in absorption arising from the warm envelope surrounding the hot core, as indicated by the velocity peak position and shape of the line profile. The sensitivity does not allow to detect C3 absorption due to diffuse foreground clouds. From the column densities of the rotational levels in the vibrational ground state probed by the absorption we derive a rotation temperature (T_rot) of ~50--70 K, which is a good measure of the kinetic temperature of the absorbing gas, as radiative transitions within the vibrational ground state are forbidden. It is also in good agreement with the dust temperatures for W31C and W49N. Applying the partition function correction based on the derived T_rot, we get column densities N(C3) ~7-9x10^{14} cm^{-2} and abundance x(C3)~10^{-8} with respect to H2. For W31C, using a radiative transfer model including far-infrared pumping by the dust continuum and a temperature gradient within the source along the line of sight we find that a model with x(C3)=10^{-8}, T_kin=30-50 K, N(C3)=1.5 10^{15} cm^{-2} fits the observations reasonably well and provides parameters in very good agreement with the simple excitation analysis., Accepted for publication in Astronomy and Astrophysics (HIFI first results issue)

Abstract: We identify a prominent absorption feature at 1115 GHz, detected in first HIFI spectra towards high-mass star-forming regions, and interpret its astrophysical origin. The characteristic hyperfine pattern of the H2O+ ground-state rotational transition, and the lack of other known low-energy transitions in this frequency range, identifies the feature as H2O+ absorption against the dust continuum background and allows us to derive the velocity profile of the absorbing gas. By comparing this velocity profile with velocity profiles of other tracers in the DR21 star-forming region, we constrain the frequency of the transition and the conditions for its formation. In DR21, the velocity distribution of H2O+ matches that of the [CII] line at 158\mu\m and of OH cm-wave absorption, both stemming from the hot and dense clump surfaces facing the HII-region and dynamically affected by the blister outflow. Diffuse foreground gas dominates the absorption towards Sgr B2. Abstract: Despite the low elemental deuterium abundance in the Galaxy, enhanced molecular D/H ratios have been found in the environments of low-mass star forming regions, and in particular the Class 0 protostar IRAS 16293-2422. The CHESS (Chemical HErschel Surveys of Star forming regions) Key Program aims at studying the molecular complexity of the interstellar medium. The high sensitivity and spectral resolution of the HIFI instrument provide a unique opportunity to observe the fundamental 1,1,1 - 0,0,0 transition of the ortho-D2O molecule, inaccessible from the ground, and to determine the ortho-to-para D2O ratio. We have detected the fundamental transition of the ortho-D2O molecule at 607.35 GHz towards IRAS 16293-2422. The line is seen in absorption with a line opacity of 0.62 +/- 0.11 (1 sigma). From the previous ground-based observations of the fundamental 1,1,0 - 1,0,1 transition of para-D2O seen in absorption at 316.80 GHz we estimate a line opacity of 0.26 +/- 0.05 (1 sigma). We show that the observed absorption is caused by the cold gas in the envelope of the protostar. Using these new observations, we estimate for the first time the ortho to para D2O ratio to be lower than 2.6 at a 3 sigma level of uncertainty, to be compared with the thermal equilibrium value of 2:1., 5 pages, 5 figures, accepted the A&A HIFI Special Issue as a letter

Abstract: We report the detection of more than 48 velocity-resolved ground rotational state transitions of H2(16)O, H2(18)O, and H2(17)O - most for the first time - in both emission and absorption toward Orion KL using Herschel/HIFI. We show that a simple fit, constrained to match the known emission and absorption components along the line of sight, is in excellent agreement with the spectral profiles of all the water lines. Using the measured H2(18)O line fluxes, which are less affected by line opacity than their H2(16)O counterparts, and an escape probability method, the column densities of H2(18)O associated with each emission component are derived. We infer total water abundances of 7.4E-5, 1.0E-5, and 1.6E-5 for the plateau, hot core, and extended warm gas, respectively. In the case of the plateau, this value is consistent with previous measures of the Orion-KL water abundance as well as those of other molecular outflows. In the case of the hot core and extended warm gas, these values are somewhat higher than water abundances derived for other quiescent clouds, suggesting that these regions are likely experiencing enhanced water-ice sublimation from (and reduced freeze-out onto) grain surfaces due to the warmer dust in these sources., Comment: 5 pages, 2 figures

Abstract: In contrast with numerous studies on the physical and chemical structure of low- and high-mass protostars, much less is known about their intermediate-mass counterparts, a class of objects that could help to elucidate the mechanisms of star formation on both ends of the mass range. We present the first results from a rich HIFI spectral dataset on an intermediate-mass protostar, OMC2-FIR4, obtained in the CHESS (Chemical HErschel SurveyS of star forming regions) key programme. The more than 100 methanol lines detected between 554 and 961 GHz cover a range in upper level energy of 40 to 540 K. Our physical interpretation focusses on the hot core, but likely the cold envelope and shocked regions also play a role in reality, because an analysis of the line profiles suggests the presence of multiple emission components. An upper limit of 10^-6 is placed on the methanol abundance in the hot core, using a population diagram, large-scale source model and other considerations. This value is consistent with abundances previously seen in low-mass hot cores. Furthermore, the highest energy lines at the highest frequencies display asymmetric profiles, which may arise from infall around the hot core., Comment: 2 figures, accepted for publication in the A&A Herschel-HIFI special issue

Abstract: We report the detection of strong absorption by interstellar hydrogen fluoride along the sight-line to the submillimeter continuum source G10.6-0.4 (W31C). We have used Herschel's HIFI instrument, in dual beam switch mode, to observe the 1232.4763 GHz J=1-0 HF transition in the upper sideband of the Band 5a receiver. The resultant spectrum shows weak HF emission from G10.6-0.4 at LSR velocities in the range -10 to -3 km/s, accompanied by strong absorption by foreground material at LSR velocities in the range 15 to 50 km/s. The spectrum is similar to that of the 1113.3430 GHz 1(11)-0(00) transition of para-water, although at some frequencies the HF (hydrogen fluoride) optical depth clearly exceeds that of para-H2O. The optically-thick HF absorption that we have observed places a conservative lower limit of 1.6E+14 cm-2 on the HF column density along the sight-line to G10.6-0.4. Our lower limit on the HF abundance, 6E-9 relative to hydrogen nuclei, implies that hydrogen fluoride accounts for between ~ 30 and 100% of the fluorine nuclei in the gas phase along this sight-line. This observation corroborates theoretical predictions that - because the unique thermochemistry of fluorine permits the exothermic reaction of F atoms with molecular hydrogen - HF will be the dominant reservoir of interstellar fluorine under a wide range of conditions., Comment: Accepted for publication in Astronomy and Astrophysics (Herschel special issue). Abstract: Ultraviolet photons are created in the interior of dense interstellar clouds by the impact excitation of molecular hydrogen by secondary electrons generated by cosmic ray ionization. The resulting photodissociation and photoionization rates of a wide range of interstellar molecules are calculated. The effects on the equilibrium chemical composition of dense clouds are briefly discussed.

Abstract: The chemical processes that occur in the gas‐phase and on the surfaces of dust particles in regions of dense interstellar clouds known as cold cores are discussed. These regions represent the first stage in the formation of low‐mass stars such as our sun. Although the chemical processes that occur in cold cores appear to be reasonably well understood, detailed gas‐phase models of the chemistry do not reproduce 100% of the observed molecular abundances, possibly because our knowledge of gas‐grain interactions and the dynamics of the physical conditions is insufficient. The chemistry of stages in star formation following cold cores is discussed briefly.

Abstract: Gisbert Fritz Winnewisser was born in 1936 in Karlsruhe, Germany into a family whose roots can be traced back to the Huguenots. This was not an auspicious year for a birth, because the ravages of the Second World War were soon to engulf the world. Most but not all of Gisbert’s family survived the war, while Gisbert attended elementary school in the picturesque village of Bad Peterstal, in the Black Forest. Later, the family moved back to Karlsruhe, where Gisbert first attended the Goethe Gymnaisum (1947-1956) and subsequently studied physics at the Technische Hochscule Karlsruhe (1956- 1963). After obtaining his Diplom (Masters), he crossed the Atlantic Ocean in 1963 to study molecular spectroscopy at Duke University, in Durham, North Carolina. At that time, Durham was a small, parochial southern town, and it is hard at first to fathom why Gisbert would choose to study there. But, Gisbert was following in his elder brother Manfred’s footsteps; Manfred was already at Duke University pursuing postdoctoral studies. Both Manfred and later Gisbert were attracted to this remote location because Walter Gordy, a very famous microwave spectroscopist, was a professor there. A pioneer in the use of ultra-high frequency microwaves, Walter Gordy can indeed be called the originator of millimeter-wave and even submillimeter-wave spectroscopy.

Abstract: A 45-year-old man with type I diabetes mellitus was admitted to hospital because of colicky abdominal pain and 5-6 watery stools daily. Upper gastrointestinal endoscopy showed nearly total atrophy of the villi in the duodenum and jejunum suggesting coeliac disease. However, gluten-free diet for 2 weeks brought no improvement. Another examination of the biopsy 6 weeks after the first endoscopy revealed extensive collagen deposition in the lamina propria of the small intestine, giving the diagnosis of collagenous sprue.Parenteral nutrition, lactulose, cisapride, cholestyramine, doxycycline, paromomycin, vancomycin and octreotide failed to affect the loss of fluid from the gut which 12 weeks after admission had increased to 221 daily. However, it was stopped after prednisolone was administered (100mg daily). 7 months after starting the steroid treatment the collagen layer had disappeared and the villous atrophy had partially regressed. Over the next 6 months the prednisolone dosage was decreased to 10 mg daily. Shortly thereafter a perimembranous glomerulonephritis occurred, with proteinuria (up to 60 g/d) and oedema. It regressed to 6 g/d when the steroid dose was increased and cyclosporin, 0.5 g/d, had been added. On maintenance dosage of cyclosporin the histological and clinical remission of the collagenous sprue has now lasted for over 2 years.This case suggests that steroid administration is an effective treatment of collagenous sprue. The presence of diabetes and other immune-related diseases in this case also suggests that an immunological mechanism may play a causative role in collagenous sprue.

Abstract: Cancer vaccines genetically engineered to produce interleukin 2 have been investigated intensively in a series of animal models and are at the point of entering into clinical trials. In this study we demonstrate a strong correlation between the rate of interleukin 2 production and the protection efficiency of murine S91 melanoma cell (clone M-3) vaccines. Best immunization is achieved with vaccines producing medium interleukin 2 levels of 1000-3000 units per 10(5) cells per day. Reduced interleukin 2 production evokes a corresponding decline in the number of successfully treated animals. Unexpectedly, when interleukin 2 expression is raised to high levels of 5000-7500 units per 10(5) cells per day, protection is completely absent because of impaired generation of tumor-specific cytotoxic T lymphocytes. In comparison, granulocyte-macrophage colony-stimulating factor as immunomodulator induces substantial immunization even at a moderate level of secretion and protects all animals at the maximal obtainable level of secretion. Our findings demonstrate the importance of the interleukin 2 level produced by genetically modified tumor cells and may have substantial impact for the clinical application of cancer vaccines.

50. Tuberous sclerosis complex with end-stage renal failure

Abstract: Renal angiomyolipoma is common in the tuberous sclerosis complex (TSC), the classic features of which are facial angiofibroma, seizures, and mental retardation. We report a family with three affected members demonstrating the wide spectrum of TSC-associated lesions ranging from asymptomatic findings to life-threatening complications. The predominant symptoms of the index patient were hypertension and mild renal insufficiency at age 48, resulting in end-stage renal failure at age 63 due to giant bilateral angiomyolipoma of the kidneys. The two TSC-affected siblings had died years previously, one from pulmonary lymphangioleiomyomatosis and the other during an epileptic state; the latter had situs inversus totalis as another remarkable finding. The diagnosis of TSC may be overlooked if CNS symptoms are absent and if cutaneous lesions are masked by cosmetic procedures, as occurred in the index case. Chronic renal failure due to angiomyolipoma is not widely known to clinical nephrologists, but develops in approximately 15% of TSC patients. Displacement of functional renal parenchyma by abnormal tissue appears to be the major pathogenetic mechanism leading to end-stage renal failure. Angiomyolipomas can be diagnosed from this characteristic sonographic pattern and the demonstration of fatty tissue in CT or MRI. Multiple renal cysts are also common in TSC. Therefore TSC should be considered in the differential diagnosis of polycystic kidney disease.
