1. Evaluation of an HIV homecare program for lost-to-follow-up populations: a mixed methods study in Detroit, Michigan

Abstract

Abstract Background Maintaining people living with HIV (PLWHIV) in clinical care is a global priority. In the Metro Detroit area of Michigan, approximately 30% of PLWHIV are out of care. To re-engage lost-to-follow-up patients, Wayne Health Infectious Disease clinic launched an innovative Homecare program in 2017. In addition to home healthcare delivery, the program included links to community resources and quarterly community meetings. We aimed to evaluate Homecare’s impact on participants’ ability to stay engaged in HIV care and reach viral suppression. We included data from PLWHIV and their healthcare workers. Methods We used a convergent mixed-methods design, including first year program record review, semi-structured interviews, and a validated Likert scale questionnaire rating illness perception before and after Homecare. Interview data were collected from 15 PLWHIV in Metro Detroit and two healthcare workers responsible for program delivery. Semi-structured interviews focused on obstacles to clinic-based care, support networks, and illness perceptions. Interview data were transcribed and analyzed using a thematic approach. A fully coded analysis was used to create a conceptual framework of factors contributing to Homecare’s success. Means in eight categories of the Brief Illness Perception (IPQ) were compared using paired T-tests. Results In the first year of Homecare, 28 of 34 participants (82%) became virally suppressed at least once. The program offered (1) social support and stigma reduction through strong relationships with healthcare workers, (2) removal of physical and resource barriers such as transportation, and (3) positive changes in illness perceptions. PLWHIV worked towards functional coping strategies, including improvements in emotional regulation, acceptance of their diagnosis, and more positive perspectives of control. Brief-IPQ showed significant changes in six domains before and after Homecare. Conclusion Homecare offers an innovative system for successfully re-engaging and maintaining lost-to-follow-up PLWHIV in care. These findings have implications for HIV control efforts and could inform the development of future programs for difficult to reach populations.

Abstract

Introduction: Alzheimer disease risk polymorphisms have been studied in patients with dementia, but have not yet been explored in mild cognitive impairment (MCI) in our population; nor have they been addressed in relation to cognitive variables, which can be predictive biomarkers of disease. Objective: To evaluate cognitive performance and presence of polymorphisms of the genes SORL1(rs11218304), PVRL2(rs6859), CR1(rs6656401), TOMM40(rs2075650), APOE (isoforms ε2, ε3, ε4), PICALM(rs3851179), GWAS_14q(rs11622883), BIN1(rs744373), and CLU(rs227959 and rs11136000) in patients with MCI and healthy individuals. Methodology: We performed a cross-sectional, exploratory, descriptive study of a prospective cohort of participants selected by non-probabilistic sampling, evaluated with neurological, neuropsychological, and genetic testing, and classified as cognitively healthy individuals and patients with MCI. Cognition was evaluated with the Neuronorma battery and analysed in relation to the polymorphic variants by means of measures of central tendency, confidence intervals, and nonparametric statistics. Results: We found differences in performance in language and memory tasks between carriers and non-carriers of BIN1, CLU, and CR1 variants and a trend towards poor cognitive performance for PICALM, GWAS_14q, SORL1, and PVRL2 variants; the APOE and TOMM40 variants were not associated with poor cognitive performance. Discussion: Differences in cognitive performance associated with these polymorphic variants may suggest that the mechanisms regulating these genes could have an effect on cognition in the absence of dementia; however, this study was exploratory and hypotheses based on these results must be explored in larger samples. Resumen: Introducción: Los polimorfismos de riesgo para el desarrollo de enfermedad de Alzheimer se han estudiado en pacientes con demencia, pero aún no se han explorado en trastorno neurocognitivo leve (TNL) en nuestra población, ni se han considerado en relación con variables cognitivas, las cuales pueden ser biomarcadores predictivos de enfermedad. Objetivo: Evaluar los desempeños cognitivos y los polimorfismos en los genes SORL1(rs11218304), PVRL2(rs6859), CR1(rs6656401), TOMM40(rs2075650), APOE(isoformas ε2, ε3, ε4), PICALM(rs3851179), GWAS_14q(rs11622883), BIN(rs744373), CLU (rs227959 y rs11136000) en pacientes con TNL y en sujetos sanos. Metodología: Estudio descriptivo, exploratorio y transversal, en una cohorte prospectiva de participantes seleccionados mediante muestreo no probabilístico, evaluados por neurología, neuropsicología y genética, y clasificados como cognitivamente sanos y pacientes con TNL, según criterios. La cognición se evaluó por medio de la batería Neuronorma y se analizó en relación con las variantes polimórficas por medio de medidas de tendencia, intervalos de confianza y estadísticos no paramétricos. Resultados: Se identificaron diferencias en los desempeños en tareas de lenguaje y memoria en relación con las variantes de BIN1, CLU y CR1, junto con tendencias en las variantes de PICALM, GWArs, SORL y PVRL2, mientras que en APOE y TOMM40 no se encontraron tendencias. Discusión: Las tendencias en los desempeños cognitivos en relación con variantes polimórficas podrían indicar que, en ausencia de demencia, los mecanismos que regulan estos genes podrían tener un efecto sobre la cognición; sin embargo, esta aproximación tiene un carácter exploratorio y sus resultados permiten generar hipótesis que requieren ser exploradas en muestras de mayor tamaño.

Abstract

Digital active disturbance rejection controllers (DADRC) have a wide scope as they have inherent advantages such as: easy hardware implementation, the entire system's mathematical model it is not necessary, is a linear controller dealing with uncertainty and is intended to ensure that energy savings are achieved. This document provides a procedure for designing a DADRC for a dc motor drive from the perspective of a Generalized Proportional Integral observer and differential flatness. The angular speed of a dc motor drive is the main purpose of the controller. The proposed DADRC was implemented using floating point operations on a field programmable gate array Artix-7™ using a Nexys 4 board. The system is subject to current demands, parameter variations, dc power supply changes and external load torque variations. The coupling of buck converter and dc motor resulting in external and internal disturbances that are actively counteracts the linear controller, as shown in the results.

Abstract

ABSTRACT Clostridioides difficile is an obligately anaerobic, spore-forming, Gram-positive pathogenic bacterium that is considered the leading cause of nosocomial diarrhea worldwide. Recent studies have attempted to understand the biology of the outermost layer of C. difficile spores, the exosporium, which is believed to contribute to early interactions with the host. The fundamental role of the cysteine-rich proteins CdeC and CdeM has been described. However, the molecular details behind the mechanism of exosporium assembly are missing. The underlying mechanisms that govern exosporium assembly in C. difficile remain poorly studied, in part due to difficulties in obtaining pure soluble recombinant proteins of the C. difficile exosporium. In this work, we observed that CdeC was able to form organized inclusion bodies (IBs) in Escherichia coli filled with lamella-like structures separated by an interspace of 5 to 15 nm; however, CdeC expression in an E. coli strain with a more oxidative environment led to the loss of the lamella-like organization of CdeC IBs. Additionally, dithiothreitol (DTT) treatment of CdeC inclusion bodies released monomeric soluble forms of CdeC. Deletions in different portions of CdeC did not affect CdeC’s ability to aggregate and form oligomers stable under denaturation conditions but affected CdeC’s self-assembly properties. Overall, these observations have important implications in further studies elucidating the role of CdeC in the exosporium assembly of C. difficile spores. IMPORTANCE The endospore of Clostridioides difficile is the vehicle for transmission and persistence of the pathogen, and, specifically, the exosporium is the first contact between the host and the spore. The underlying mechanisms that govern exosporium assembly in C. difficile remain understudied, in part due to difficulties in obtaining pure soluble recombinant proteins of the C. difficile exosporium. Understanding the exosporium assembly’s molecular bases may be essential to developing new therapies against C. difficile infection.

Abstract

Pancreatic ductal adenocarcinoma contributes significantly to global cancer-related deaths, featuring only a 10% survival rate over five years. The quest for novel tumor markers is critical to facilitate early diagnosis and tailor treatment strategies for this disease, which is key to improving patient outcomes. In pancreatic ductal adenocarcinoma, these markers have been demonstrated to play a crucial role in early identification, continuous monitoring, and prediction of its prognosis and have led to better patient outcomes. Nowadays, biopsy specimens serve to ascertain diagnosis and determine tumor type. However, liquid biopsies present distinct advantages over conventional biopsy techniques. They offer a noninvasive, easily administered procedure, delivering insights into the tumor's status and facilitating real-time monitoring. Liquid biopsies encompass a variety of elements, such as circulating tumor cells, circulating tumor DNA, extracellular vesicles, microRNAs, circulating RNA, tumor platelets, and tumor endothelial cells. This review aims to provide an overview of the clinical applications of liquid biopsy as a technique in the management of pancreatic cancer.

Abstract

The aim of this research was to determine the efficiency of activated carbon obtained from coconut (Cocos nucifera) endocarp in the reduction of color in runoff water to improve its quality. For this purpose, activated carbon was produced at a carbonization temperature of 700 °C in a time of 30 minutes and phosphoric acid at a concentration of 85% was used as an activating agent. A completely randomized design was used, consisting of 3 treatments (100, 50 and 25 g of activated carbon) and a control (without treatment), with three replicates of each treatment, where 1 L of water from the uptake was filtered. It was determined that the best percentage removal was obtained with treatment 3 (T3) in the reduction of color and turbidity with average values of 97.56% and 97.11%, respectively. An activated carbon filter was installed in the San Lorenzo - Moyobamba sector in a similar way to the laboratory tests, in which the efficiency of this product in reducing the color and turbidity of the water was demonstrated, achieving values suitable for human consumption.

Background: Renal dysfunction is frequently encountered in patients with aortic prosthesis degeneration requiring valve-in-valve (VIV) transcatheter aortic valve replacement (TAVR). The effect of VIV TAVR on renal function in patients with bioprosthetic aortic regurgitation (AR) and stenosis (AS) is unknown. Objectives: The aims of this study were to describe the change in renal function after VIV TAVR and to compare differences in renal function changes in those with predominant prosthetic regurgitation compared to stenosis. Methods: All VIV TAVR between June of 2014, and October 2019 (n = 141) at a single institution were reviewed. Baseline renal function parameters including estimated glomerular filtration rate (eGFR) were compared with post-discharge follow-up values in both prosthetic AR and AS patient groups. Linear regression analysis was performed to determine correlates of renal function change. Results: Mean baseline eGFR was lower in the AR group (55 SD21 vs. 64 SD24 ml/min/1.73 m2 p = 0.0495). At post-discharge follow-up there was an increase in mean eGFR in the AR group which was not present in the AS group (8 SD12 vs. 0 SD11 ml/min/1.73 m2 respectively p = 0.0006). Abstract

Currently, there are no reliable biomarkers for the diagnosis of pancreatic cancer (PaC). Glycoproteomic approaches that analyze the glycan determinants on specific glycoproteins have proven useful to develop more specific cancer biomarkers than the corresponding protein levels. In PaC, mesothelin (MSLN) is a neo-expressed glycoprotein. MSLN glycosylation has not been described and could be altered in PaC. In this work, we aimed to characterize MSLN glycans from PaC cells and serum samples to assess their potential usefulness as PaC biomarkers. First, we analyzed MSLN glycans from PaC cell lines and then we developed an enzyme-linked lectin assay to measure core fucosylated-MSLN (Cf-MSLN) glycoforms. MSLN glycans from PaC cells were analyzed by glycan sequencing and through Western blotting with lectins. All of the cell lines secreted MSLN, with its three N-glycosylation sites occupied by complex-type N-glycans, which were mainly α2,3-sialylated, core fucosylated and highly branched. The Cf-MSLN glycoforms were quantified on PaC serum samples, and compared with MSLN protein levels. The Cf-MSLN was significantly decreased in PaC patients compared to control sera, while no differences were detected by using MSLN protein levels. In conclusion, Cf-MSLN glycoforms were differently expressed in PaC, which opens the way to further investigate their usefulness as PaC biomarkers.

Abstract

The MITRAL (Mitral Implantation of Transcatheter Valves) trial is the first prospective study for valve-in-mitral annular calcification (ViMAC), mitral valve-in-ring (MViR), and mitral valve-in-valve (MViV) using balloon-expandable aortic transcatheter heart valves. Procedural outcomes beyond 1 year are not well described.This study evaluated 2-year outcomes in ViMAC, MViR, and MViV in the MITRAL trial.This multicenter prospective study enrolled patients with severe MAC, prior failed mitral annuloplasty ring repair, or prior failed bioprosthetic MV replacement who were at high surgical risk at 13 U.S. sites.Between February 1, 2015, and December 31, 2017, 91 patients were enrolled (31 with ViMAC, 30 with MViR, and 30 with MViV). In the ViMAC group, 2-year all-cause mortality was 39.3%, 66.7% were New York Heart Association (NYHA) functional class I-II, and mean MV gradient was 5.6 ± 2.0 mm Hg. In the MViR group, 2-year all-cause mortality was 50%, 65% were NYHA functional class I-II, and mean MV gradient was 6.5 ± 2.7 mm Hg. In the MViV group, 2-year all-cause mortality was 6.7%, 85% were NYHA functional class I-II, and mean MV gradient was 6.9 ± 2.4 mm Hg. At 2 years, all patients had ≤mild mitral regurgitation and survivors in all 3 arms showed sustained improvement in Kansas City Cardiomyopathy Questionnaire scores compared to baseline.Use of balloon-expandable aortic transcatheter heart valves in selected patients with severe MAC, failed annuloplasty ring, and bioprosthetic MV dysfunction is associated with improvements in symptoms, quality of life, and stable prosthesis function at 2-year follow-up. Between 1 and 2 years, the MViR group experienced higher mortality rates than the MViV and ViMAC groups.

20. Comprehensive transcriptomic analysis of heat shock proteins in the molecular subtypes of human breast cancer

Abstract

Abstract Background Heat Shock Proteins (HSPs), a family of genes with key roles in proteostasis, have been extensively associated with cancer behaviour. However, the HSP family is quite large and many of its members have not been investigated in breast cancer (BRCA), particularly in relation with the current molecular BRCA classification. In this work, we performed a comprehensive transcriptomic study of the HSP gene family in BRCA patients from both The Cancer Genome Atlas (TCGA) and the Molecular Taxonomy of Breast Cancer International Consortium (METABRIC) cohorts discriminating the BRCA intrinsic molecular subtypes. Methods We examined gene expression levels of 1097 BRCA tissue samples retrieved from TCGA and 1981 samples of METABRIC, focusing mainly on the HSP family (95 genes). Data were stratified according to the PAM50 gene expression (Luminal A, Luminal B, HER2, Basal, and Normal-like). Transcriptomic analyses include several statistical approaches: differential gene expression, hierarchical clustering and survival analysis. Results Of the 20,531 analysed genes we found that in BRCA almost 30% presented deregulated expression (19% upregulated and 10% downregulated), while of the HSP family 25% appeared deregulated (14% upregulated and 11% downregulated) (|fold change| > 2 comparing BRCA with normal breast tissues). The study revealed the existence of shared HSP genes deregulated in all subtypes of BRCA while other HSPs were deregulated in specific subtypes. Many members of the Chaperonin subfamily were found upregulated while three members (BBS10, BBS12 and CCTB6) were found downregulated. HSPC subfamily had moderate increments of transcripts levels. Various genes of the HSP70 subfamily were upregulated; meanwhile, HSPA12A and HSPA12B appeared strongly downregulated. The strongest downregulation was observed in several HSPB members except for HSPB1. DNAJ members showed heterogeneous expression pattern. We found that 23 HSP genes correlated with overall survival and three HSP-based transcriptional profiles with impact on disease outcome were recognized. Conclusions We identified shared and specific HSP genes deregulated in BRCA subtypes. This study allowed the recognition of HSP genes not previously associated with BRCA and/or any cancer type, and the identification of three clinically relevant clusters based on HSPs expression patterns with influence on overall survival.

Abstract

In this work, we present a teaching experience based on emerging pedagogies and gamification to improve leadership skills and achieve efficient teamwork in scientific-technical areas of Engineering Projects. We designed the contents of a workshop and a virtual tool to create emotional contexts that improved the acquisition of soft skills. The combination of multi-sensory pedagogical strategies and hybrid inductive-deductive methodologies based on tests and team games, inspired by the story line of the classic film "The Seven Samurai" (Kurosawa, 1954), allowed us both to carry out, over a period of two academic years, an “on-the-ground” workshop-game, and a virtual tool to interact in the workshop, while under the COVID-19 restrictions. Despite the different formats and gamification elements used over the two academic years, the metrics for evaluating the efficiency of acquiring these soft skills produced very positive results and, on the whole, did not show significant differences. The cinematographic language and empathy with the story and the characters led the students to identify with and internalize the elements of efficient teamwork and leadership. Movies can provide simple but effective narrative lines on which to build games. Here, we present the contents and details of both workshops as a transferable experience, as well as making the open virtual tool available for the educative community.

Abstract

Fiber-reinforced cementitious composites (FRCC) are a class of materials made by adding randomly distributed fibers to a cementitious matrix, providing better material toughness through the crack bridging behavior of the fibers. One of the primary concerns with FRCCs is the behavior of the fiber when a crack is formed. The fibers provide a stress-bridging mechanism, which is largely determined by the bond that exists between the concrete and the fiber’s outer surface. While many studies have determined the properties of FRCCs and potential benefits of using specific fiber types, the effects of low temperature or cold plasma treatment of polymer fibers on the mechanical behavior of the composite material are limited. Polymer fibers are notable for their low density, ductility, ease of manufacture, and cost-effectiveness. Despite these advantages, the surface properties of polymers do not enable the bonding potential given by steel or glass fibers when used in untreated FRCC, resulting in pull-out failures before the full displacement capacity of the fiber is utilized. For this reason, modification of the surface characteristics of polymer fibers can aid in higher bonding potential. Plasma treatment is a process wherein surfaces are modified through the kinetics of electrically charged and reactive species in a gaseous discharge environment. This paper is a preliminary study on the use of atmospheric pressure plasma generated at approximately room temperature. This atmospheric, cold plasma treatment is a method for improving the mechanical properties of FRCC using polymeric fibers. In this study, polypropylene and polyvinyl-alcohol fibers were cold plasma treated for 0, 30, 60, and 120 s before being used in cementitious mortar mixtures. Compression and flexure tests were performed using a displacement-based loading protocol to examine the impact of plasma treatment time on the corresponding mechanical performance of the fiber-reinforced cementitious composite. The experimental results obtained from this study indicate that there is a positive correlation between fiber treatment time and post-peak load-carrying capacity, especially for specimens subjected to flexural loading.

Abstract

Electrical power quality is one of the main elements in power generation systems. At the same time, it is one of the most significant challenges regarding stability and reliability. Due to different switching devices in this type of architecture, different kinds of power generators as well as non-linear loads are used for different industrial processes. A result of this is the need to classify and analyze Power Quality Disturbance (PQD) to prevent and analyze the degradation of the system reliability affected by the non-linear and non-stationary oscillatory nature. This paper presents a novel Multitasking Deep Neural Network (MDL) for the classification and analysis of multiple electrical disturbances. The characteristics are extracted using a specialized and adaptive methodology for non-stationary signals, namely, Empirical Mode Decomposition (EMD). The methodology’s design, development, and various performance tests are carried out with 28 different difficulties levels, such as severity, disturbance duration time, and noise in the 20 dB to 60 dB signal range. MDL was developed with a diverse data set in difficulty and noise, with a quantity of 4500 records of different samples of multiple electrical disturbances. The analysis and classification methodology has an average accuracy percentage of 95% with multiple disturbances. In addition, it has an average accuracy percentage of 90% in analyzing important signal aspects for studying electrical power quality such as the crest factor, per unit voltage analysis, Short-term Flicker Perceptibility (Pst), and Total Harmonic Distortion (THD), among others.

25. How to measure inFLAtion volatility. A note

Abstract

This paper proposes a statistical model and a conceptual framework to estimate inflation volatility assuming rational inattention, where the decay in the level of attention reflects the arrival of news in the market. We estimate trend inflation and the conditional inflation volatility for Germany, Spain, the euro area and the United States using monthly data from January 2002 to March 2022 and test whether inflation was equal to or below 2% in this period in these regions. We decompose inflation volatility into positive and negative surprise components and characterise different inflation volatility scenarios during the Great Financial Crisis, the Sovereign Debt Crisis, and the post-COVID period. Our volatility measure outperforms the GARCH(1,1) model and the rolling standard deviation in one-step ahead volatility forecasts both in-sample and out-of-sample. The methodology proposed in this article is appropriate for estimating the conditional volatility of macro-financial variables. We recommend the inclusion of this measure in inflation dynamics monitoring and forecasting exercises.

26. Implementation of a method of extraction conditions of pectin obtained fromagroindustrial by-product cocoa husks

Abstract

Pectin is a polysaccharide with hydrocolloidal, gelling and stabilizing characteristics. It is widely used in the manufacture of cosmetics, food products, and pharmaceuticals, among others products. Taking into account this wide range of applications in several industries, this study sought to evaluate national cocoa husks (Theobroma cacao Linneo), a waste of difficult disposal, as a source of this natural polymer. The study also attempted to determine the best conditions for the extraction process. The cocoa husks are from a national industry and comprise different materials (Criollo, Forastero and Trinitario). A reflux extraction method using citric acid was implemented. Preliminary tests took different periods of time (100, 95, 85 and 75 minutes) and were conducted at different temperatures (95, 90, 80 and 70 ºC). A test of variance was made using 22 design and employing the highest percentages of methoxyl and galacturonic acid. The pectin extraction yield increased proportionally with time and temperature. However, the percentage of methoxyl did not show an important variation (2.65%±0.16 and 2.90%±0.03). The pectin obtained was characterized as one of low methoxyl pectin content. The best extraction temperature condition using a citric acid solution of pH 3 was 70 °C hold during 95 minutes. Under this condition a yield of 8.82 g pectin / 100 g cocoa husks was obtained, the esterification degree was 71.88%±0.78, and the percentage of galacturonic acid was 26.86%±0.86. These results show that the husks from the national cocoa industry can be included among the alternative sources to obtain pectin.

27. Modeling Truck Parking Demand at Commercial and Industrial Establishments

Abstract

In urban areas, there exists a mismatch between where trucks need to park and the availability of spaces, forcing many truck drivers to park in undesignated locations that are unsafe, disrupt traffic, and cause a nuisance to the community. This mismatch also decreases the productivity of the sector, as drivers spend more time searching for spaces, take longer detours, and end their workday early. Most commercial and industrial establishments currently do not allow trucks to park at their facilities beyond the loading or unloading of cargo, which generates parking demand in the surrounding areas. This paper quantifies the truck parking requirements of different commercial and industrial establishments in Phoenix, AZ, to inform land use and development decision-making. GPS data were combined with land use and employment data to identify where, when, and for how long trucks are parking. Parking rates were calculated at 2-digit and 3-digit NAICS level, for three parking metrics: stops per weekday, cumulative stop duration per weekday, and peak occupancy. Linear and negative binomial models were estimated, along with other types of models, to improve the prediction of parking demand. Results show that parking demand is the highest for establishments that store trucks overnight, such as truck terminals, and establishments with large in-house fleets. Parking demand is also high for establishments moving bulky commodities or having significant transportation needs, like in warehousing and the wholesale trade. Manufacturing establishments have a wide range of parking demand rates, based on the bulkiness of products and use of in-house fleets.

Abstract

Global trade continues to increase in volume, speed, geographic scope, diversity of goods, and types of conveyances, which has resulted in a parallel increase in both quantity and types of pathways available for plant pests to move via trade. Wood packaging material (WPM) such as dunnage, pallets, crates, and spools, is an integral part of the global supply chain due to its function in containing, protecting, and supporting the movement of traded commodities. The use of untreated solid wood for WPM introduces the risk of wood boring and wood-infesting organisms into the supply chain, while the handling and storage conditions of treated WPM presents risk of post-treatment contamination by surface-adhering or sheltering pests. The wood-boring and -infesting pest risks intrinsic to the solid wood packaging pathway were addressed in the 2002 adoption and 2009 revision of ISPM 15, which was first implemented in 2005–2006 in North America. Although this global initiative has been widely implemented, some pest movement still occurs due to a combination of factors including; fraud, use of untreated material, insufficient- or incomplete- treatment, and post-treatment contamination. Here we examine the forest-to-recycling production and utilization chain for wood packaging material with respect to the dynamics of wood-infesting and contaminating pest incidence within the environments of the international supply chain and provide opportunities for improvements in pest risk reduction. We detail and discuss each step of the chain, the current systems in place, and regulatory environments. We discuss knowledge gaps, research opportunities and recommendations for improvements for each step. This big picture perspective allows for a full system review of where new or improved pest risk management strategies could be explored to improve our current knowledge and regulations.

Abstract

The tumor microenvironment (TME) is constituted by a great diversity of highly dynamic cell populations, each of which contributes ligands, receptors, soluble proteins, mRNAs, and miRNAs, in order to regulate cellular activities within the TME and even promote processes such as angiogenesis or metastasis. Intravasated platelets (PLT) undergo changes in the TME that convert them into tumor-educated platelets (TEP), which supports the development of cancer, angiogenesis, and metastasis through the degranulation and release of biomolecules. Several authors have reported that the deregulation of PF4, VEGF, PDGF, ANG-1, WASF3, LAPTM4B, TPM3, and TAC1 genes participates in breast cancer progression, angiogenesis, and metastasis. The present work aimed to analyze the expression levels of this set of genes in tumor tissues and platelets derived from breast cancer patients by reverse transcription-quantitative polymerase chain reaction (RTqPCR) assays, in order to determine if there was an expression correlation between these sources and to take advantage of the new information to be used in possible diagnosis by liquid biopsy. Data from these assays showed that platelets and breast cancer tumors present similar expression levels of a subset of these genes’ mRNAs, depending on the molecular subtype, comorbidities, and metastasis presence.

Abstract

Introduction: Endodontic retreatment seeks reentry to the root canal system to remove all filling material, identification and correction of pathological or iatrogenic defects. Objective: This study aimed to compare the dissolving capacity of eucalyptus oil, orange oil, and distilled water versus three types of endodontic cements. Materials and methods: Two hundred and four stainless steel molds were fabricated, where each cement sample was placed to the edge and left to set in the incubator, simulating normal oral conditions for 7 days. Subsequently, the solubility of each cement was analysed by immersing the samples in eucalyptus and orange oil for ten minutes in a static and ultrasonic environment. The solubility of the cement was evidenced by the difference between the weight before and after contact with the oils, data were statistically analysed with the ANOVA test and the post hoc Tukey bootstrap means test with 1,000 repetitions. Abstract

In recent years, nanofluids have captured great attention due to their enhanced thermal properties in comparison to conventional pristine base fluids. Among them, TiO2 nanofluids have shown outstanding improvements on heat exchange transfer and thermal conductivity at extremely low TiO2 volume fractions. However, there is a lack of research on nanofluid applications in cooling systems of internal combustion engine vehicles and their influence on the pollutant emissions from these mobile sources. Thus, in this work a nanofluid, composed of 0.5 vol.% TiO2 nanoparticles and a commercial ethylene glycol-based coolant, was prepared to assess their effect on CO2, CO, NOx, hydrocarbon (HC) and fine particulate matter (PM2.5) emissions from a diesel passenger van under the IM-240 cycle, at Quito ambient conditions (2800 m.a.m.s.l.). Therefore, the use of TiO2-based nanofluids allowed improving the thermal efficiency of the engine, as demonstrated by the rise of CO2 emissions (2%), along with the consequent reduction of CO, HC and mainly PM2.5 up to 16, 17 and 60%, respectively. The substantial decrease in PM2.5 by using TiO2-nanocoolant could drop the impact of diesel vehicles on human health, particularly on cardiovascular and respiratory diseases.

Background: Social determinants of health (SDOH) contribute to over 50% of health outcomes and inequities. Healthcare institutions are increasingly implementing SDOH screenings and referrals. St. Mary Medical Center (SMMC) in Northwest Indiana was the first acute care hospital to implement a comprehensive SDOH pilot screening and referral program in Indiana using the validated Protocol for Responding to and Assessing Patients’ Assets, Risks, and Experiences (PRAPARE). This study is part of a three-phased Community-Based Participatory Research (CBPR) partnership between IUSM-NW and SMMC to examine and address SDOH within SMMC’s service area from January 2022-January 2025. 36. Descriptive cross-sectional study on major bovine diseases and associated risk factors in north-eastern Ecuadorian Amazon

Abstract

Cattle raising is a crucial element of production systems in the tropics and subtropics. However, in recent years, global public health security has been threatened by disease emergence. In Orellana Province, livestock is the most important activity to generate economic income. Nevertheless, there is no available data about Animal Health status. With this objective, a study was performed to describe the major Bovine diseases recorded between 2011 to 2019, and the main Risk factors associated. Data on main Bovine diseases were retrieved from the World Animal Health Information System database. Whereas Bovine population data used to calculate the prevalence rates and confidence intervals were obtained from Ecuador's Ministry of Agriculture. By contrast, the Risk factors identified with an epidemiological questionnaire were applied to 300 livestock farmers. As a result, from 2011 to 2019 in Orellana has been confirmed: 90 cases of Infectious Bovine Rhinotracheitis (31.58%), Bovine Rabies by hematophagous bats (Desmodus rotundus), 83 cases (29.12%), Bovine viral diarrhea with 43 cases (15.10%), Brucellosis by Brucella abortus 35 cases, which was (12.28%), and 34 cases related to Enzootic bovine leukosis (11.92%). Overall, the prevalence rates ranged from (0.24 to 15.37%). In addition, farm size, presence of forest, herd, and paddock sizes, cutting frequency of forages, and other animal species were involved as Risk factors (OR = 3.15 to 11.75; 95% CI, 0.01 to 0.69). In conclusion, there are animal diseases with reproductive and neurologic symptomology and high-Risk factors implicated in the transmission. Consequently, space-temporal and seroprevalence epidemiological studies should be performed in Orellana.

Abstract

Hypersialylation is a feature of pancreatic ductal adenocarcinoma (PDA) and it has been related to tumor malignancy and immune suppression. In this work, we have evaluated the potential of the sialyltransferase inhibitor, Ac3FNeu5Ac, to decrease tumor sialoglycans in PDA and to revert its malignant phenotype. Sialoglycans on PDA cells were evaluated by flow cytometry, and the functional impact of Ac3FNeu5Ac was assessed using E-selectin adhesion, migration, and invasion assays. PDA tumors were generated in syngeneic mice from KC cells and treated with Ac3FNeu5Ac to evaluate tumor growth, mice survival, and its impact on blocking sialic acid (SA) and on the tumor immune component. Ac3FNeu5Ac treatment on human PDA cells decreased α2,3-SA and sialyl-Lewis, which resulted in a reduction in their E-selectin adhesion, and in their migratory and invasive capabilities. Subcutaneous murine tumors treated with Ac3FNeu5Ac reduced their volume, their SA expression, and modified their immune component, with an increase in CD8 T-lymphocytes and NK cells. In conclusion, Ac3FNeu5Ac treatment weakened PDA cells’ malignant phenotype, thereby reducing tumor growth while favoring anti-tumor immune surveillance. Abstract

The Internet of Things (IoT) paradigm allows the connection and exchange of information between millions of smart devices. This paradigm grows and develops exponentially as do the risks and attacks on IoT infrastructures. Security, privacy, reliability, and autonomy are the most important requirements in IoT Systems. If these issues are not guaranteed, the IoT system could be susceptible to malicious users and malicious use. In centralized IoT systems, attacks and risks are greater, especially when data is transmitted between devices and shared with other organizations. To avoid these types of situations, this work presents a decentralized system that guarantees the autonomy and security of an IoT system. The proposed methodology helps to protect data integrity and availability based on the security advantages provided by blockchain and the use of cryptographic tools. The accuracy of the proposed methodology was measured on a temperature and humidity sensing IoT-based Wireless Sensor Network (WSN). The obtained results prove that the proposal fulfils the main requirements of an IoT system. It is autonomous, secure to share and send information between devices and users, has privacy, it is reliable, and the information is available in the infrastructure. Furthermore, this research demonstrates that the proposal is less susceptible to the most frequent attacks against IoT systems, such as linking attack, man in the middle, and Distributed Denial of Service (DDoS) attack.

Abstract

We argue that making accept/reject decisions on scientific hypotheses, including a recent call for changing the canonical alpha level from p = 0.05 to p = 0.005, is deleterious for the finding of new discoveries and the progress of science. Given that blanket and variable alpha levels both are problematic, it is sensible to dispense with significance testing altogether. There are alternatives that address study design and sample size much more directly than significance testing does; but none of the statistical tools should be taken as the new magic method giving clear-cut mechanical answers. Inference should not be based on single studies at all, but on cumulative evidence from multiple independent studies. When evaluating the strength of the evidence, we should consider, for example, auxiliary assumptions, the strength of the experimental design, and implications for applications. To boil all this down to a binary decision based on a p-value threshold of 0.05, 0.01, 0.005, or anything else, is not acceptable.

Resumen Introduccion Los polimorfismos de riesgo para el desarrollo de enfermedad de Alzheimer (se han estudiado en pacientes con demencia, pero aun no se han explorado en trastorno neurocognitivo leve (TNL) en nuestra poblacion, ni se han considerado en relacion con variables cognitivas, las cuales pueden ser biomarcadores predictivos de enfermedad. Objetivo Evaluar los desempenos cognitivos y los polimorfismos en los genes SORL1(rs11218304), PVRL2(rs6859), CR1(rs6656401), TOMM40(rs2075650), APOE(isoformas ɛ2, ɛ3, ɛ4), PICALM(rs3851179), GWAS_14q(rs11622883), BIN(rs744373), CLU (rs227959 y rs11136000) en pacientes con TNL y en sujetos sanos. Metodologia Estudio descriptivo, exploratorio y transversal, en una cohorte prospectiva de participantes seleccionados mediante muestreo no probabilistico, evaluados por neurologia, neuropsicologia y genetica, y clasificados como cognitivamente sanos y pacientes con TNL, segun criterios. La cognicion se evaluo por medio de la bateria Neuronorma y se analizo en relacion con las variantes polimorficas por medio de medidas de tendencia, intervalos de confianza y estadisticos no parametricos. Resultados Se identificaron diferencias en los desempenos en tareas de lenguaje y memoria en relacion con las variantes de BIN1, CLU y CR1, junto con tendencias en las variantes de PICALM, GWArs, SORL y PVRL2, mientras que en APOE y TOMM40 no se encontraron tendencias. Discusion Las tendencias en los desempenos cognitivos en relacion con variantes polimorficas podrian indicar que, en ausencia de demencia, los mecanismos que regulan estos genes podrian tener un efecto sobre la cognicion; sin embargo, esta aproximacion tiene un caracter exploratorio y sus resultados permiten generar hipotesis que requieren ser exploradas en muestras de mayor tamano.

Abstract

Background Restrictive Cardiomyopathies (R-CMP) represent a wide array of conditions characterized by diastolic heart failure with important impairment in left ventricle filling pressures. These entities require different treatments, rendering etiological diagnosis a key component during the evaluation of these patients. Purpose Report the mortality of a select group of patients with R-CMP referred to CMR in a tertiary care center in Latin-America. Methods We analyzed all patients with CMR performed in our institution, including hospitalized and ambulatory patients, between 2016 and 2021. We extracted data of patients diagnosed by CMR with amyloidosis, hypertrophic cardiomyopathy (HCM), and endomyocardial fibrosis (EMF). All-cause mortality was extracted from national census data and analyzed using Kaplan-Meier survival estimates and log-rank test. Results Between June 1st, 2016, and December 31st, 2021, 5000 CMRs were performed at our institution. Among these, 317 (6.3%) patients were diagnosed with R-CMP by CMR as follows: 52 patients (16.4%) had amyloidosis, 250 patients had HCM (78.9%) and 15 patients (4.7%) had EMF. Of these, 40.7% were female (n=129), the median age was 63 yar-old (IQR 51–71) and 64.7% (n=205) were out-patients. At a mean follow-up of 33.3 months, there was a statistically significant difference in mortality between patients with amyloidosis (49,0%) and those with HCM and EMF (7,1% and 13,3%, respectively; P Conclusions R-CMP represent a meaningful percentage of patients referred for CMR in our institution with different survival rates. Amyloidosis patients had a significantly higher mortality rate compared to HCM and EMF, at a relatively short follow-up. Abstract

The dynamic network of chaperone interactions known as the chaperome contributes significantly to the proteotoxic cell response and the malignant phenotype. To bypass the inherent redundancy in the network, we have used a microRNA (mir) approach to target multiple members of the chaperome simultaneously. We identified a potent microRNA, miR-570 that could bind the 3′untranslated regions of multiple HSP mRNAs and inhibit HSP synthesis. Transfection of cells with this miR species reduced expression of multiple HSPs, inhibited the heat shock response and reduced tumor cell growth while acted additively in combination with cytotoxic drugs. As overexpression of miR-570 elicited tumor suppressive effects, we inferred that this miR could play a potential role in inhibiting tumorigenesis and cancer cell growth. In accordance with this hypothesis, we determined a significant role for miR-570 in regulating markers of mammary tumor progression, including cell motility and invasion. Our data provide a proof of the principle that the tumor chaperome can be targeted by microRNAs suggesting a potential therapeutic avenue towards cancer therapy.

Abstract

Myocardial infarction (MI) is one of the leading causes of death worldwide. Prognosis and mortality rate are directly related to infarct size and post-infarction pathological heart remodeling, which can lead to heart failure. Hypoxic MI-affected areas increase the expression of hypoxia-inducible factor (HIF-1), inducing infarct size reduction and improving cardiac function. Hypoxia translocates HIF-1 to the nucleus, activating carbonic anhydrase IX (CAIX) transcription. CAIX regulates myocardial intracellular pH, critical for heart performance. Our objective was to investigate CAIX participation and relation with sodium bicarbonate transporters 1 (NBC1) and HIF-1 in cardiac remodeling after MI. We analyzed this pathway in an "in vivo" rat coronary artery ligation model and isolated cardiomyocytes maintained under hypoxia. Immunohistochemical studies revealed an increase in HIF-1 levels after 2 h of infarction. Similar results were observed in 2-h infarcted cardiac tissue (immunoblotting) and in hypoxic cardiomyocytes with a nuclear distribution (confocal microscopy). Immunohistochemical studies showed an increase CAIX in the infarcted area at 2 h, mainly distributed throughout the cell and localized in the plasma membrane at 24 h. Similar results were observed in 2 h in infarcted cardiac tissue (immunoblotting) and in hypoxic cardiomyocytes (confocal microscopy). NBC1 expression increased in cardiac tissue after 2 h of infarction (immunoblotting). CAIX and NBC1 interaction increases in cardiac tissue subjected to MI for 2h when CAIX is present (immunoprecipitation).
