Currently, refractive surgery is a safe and effective procedure, and considered as a risk for development of dry eye. The aim of study is to analyze the scientific publications in the field of ocular dryness secondary to refractive surgery through a bibliometric approach. The temporal period goes since 2001-2019, years in which first references appeared and search limited selection is done, respectively. The set of publications ranges from the first publication appeared in 2001, to the last one selected in 2019.A search of references was made through Scopus, using "refractive surgery" as main descriptor, and «dry eye» as secondary one; both descriptors were limited to those available in the chosen field for the title, abstract, and keywords. The most common indicators and bibliometric maps were applied for to the selected publications.A total of 78 original articles were collected from the timeframe 2001-2019. According to the Price's law, the growth of literature production was linear turned out in a linear growth of literature production. The annual growth rate was 8.6% with a literature doubling time of 8.4 years. The Bradford core, preferred journals chosen by authors were 4 with offered four preferred journals by the authors, all of them with an impact factor2. These were Ophthalmology, Investigative OphthalmologyVisual Science, Journal of Glaucoma and British Journal of Ophthalmology. Regarding geographical distribution, the United States had the highest production.The scientific production of dry eye after refractive surgery follows a linear growth. In this instance, postulates of the Price's growth law of science are not fulfilled. In addition, there is a high rate of transience. That may indicate low productivity or presence of researchers from other related subjects disciplines, who have published occasionally in this topic.

Novo capítulo da conturbada relação entre o Estado brasileiro e os sindicatos se iniciou com o golpe que ocasionou a queda da presidente Dilma Rousseff em 2016. O governo de seu vice-presidente e sucessor foi marcado pela criação de normas jurídicas antissindicais e anti-trabalhistas, característica também presente no primeiro ano de governo do presidente eleito em 2018. O período, peculiar pela quantidade de normas desmanteladoras, pode ser definido como retorno de defensores do neoliberalismo ortodoxo ao poder do país. O presente trabalho visa analisar a relação entre Estado brasileiro, sindicatos e neoliberalismo, em termos históricos e legislativos, a fim de demonstrar a atuação estatal, em conjunto com o capital, para a aplicação da lógica neoliberal na legislação infraconstitucional. O panorama teórico e histórico precedente possibilita a verificação do impacto das normas entre 2016 e 2019 na capacidade de atuação e representação dos sindicatos, seja pela via direta, com ataques frontais, ou indireta, com a precarização do mundo do trabalho. A análise de legislações, de exposições de motivos e outras declarações governamentais, de trabalhos teóricos específicos de cada tema e de textos doutrinários sobre a interação entre eles é a metodologia adotada para responder à pergunta sobre a forma como as alterações legislativas do período escolhido aprofundaram a limitação da atuação do sindicato em prol da classe trabalhadora. A resposta preliminar, confirmada pela pesquisa, é de que as entidades sindicais foram estranguladas em sua capacidade de atuação, com ataque às suas responsabilidades de representação, e em sua capacidade financeira, com a retirada de apenas um dos pilares que submetem o sindicalismo ao controle estatal. Mais que a resposta prevista, contudo, a análise histórica da relação entre sindicato, Estado, empresa e neoliberalismo revelou processo contínuo – ainda que em variados graus de intensidade – de castração da atuação coletiva dos trabalhadores, independentemente do modelo político ou do sistema de acumulação capitalista adotado. A new chapter in the troubled relationship between the brazilian State and unions began with the coup that caused the fall of President Dilma Rousseff in 2016. The government of her vicepresident and successor was marked by antiunion and antiworkers legislation, a feature also present in the first year of government of the president elected in 2018. The period, peculiar due to the amount of dismantling rules, can be defined as the return of defenders of orthodox neoliberalism to power in the country. The present work aims to analyze the relationship between the Brazilian State, trade unions, enterprises and neoliberalism, in historical and legislative terms, in order to demonstrate state action, together with capital, for the application of neoliberal logic in infraconstitutional legislation. The preceding theoretical and historical panorama makes it possible to verify the impact of the rules between 2016 and 2019 on the ability of unions to act and represent, either directly by frontal attacks or indirectly by the evergrowing precariousness of the labor sphere. The analysis of legislation, of explanatory statements and other governmental statements, of theoretical works specific to each theme and of doctrinal texts on the interaction between them is the methodology adopted to answer the question about how the legislative changes of the chosen period deepened the limitation of union action on behalf of the working class. The preliminary answer, confirmed by the research, is that unions were strangled in their capacity to act, with an attack on their representation responsibilities, and in their financial capacity, with the removal of only one of the pillars that subject unionism to State control. More than the expected response, however, the historical analysis of the relationship between unions, State, companies and neoliberalism revealed a continuous process – albeit in varying degrees of intensity – of castration of the collective action of workers, regardless of the political model or the system of accumulation adopted.

Objetivo: Analisar o que se compreende como causas para lesões miocárdicas nos infectados pelo SARS-CoV-2, investigando o quanto essa ocorrência tem capacidade para impactar nos internados o aumento de morbimortalidade, considerando se esta injúria cardíaca apresenta potencial para gerar sequelas, deixando-os sujeitos a desfechos negativos no longo prazo. Metodologia: Esta revisão foi conduzida de acordo com as diretrizes PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-analyses). Critérios de inclusão contemplaram trabalhos que analisam as causas, manifestações e consequências da relação entre COVID-19, publicados no ano de 2020 e de janeiro a março 2021nas bases bibliográficas Science Direct, LILACS, MEDLINE, Scielo e PUBMED. Resultados: Após remoção de duplicatas e triagem de resumos com base nos critérios de inclusão e exclusão, obteve-se 22 artigos potencialmente elegíveis para inclusão nesta revisão. Após análise dos textos na íntegra, 16 foram incluídos na pesquisa. Seis são revisões de literatura, sete são estudos retrospectivos, um é relato de caso, um é estudo experimental e um é estudo observacional. Conclusões: Biomarcadores cardíacos trazem dados fundamentais no desfecho da doença, principalmente porque têm capacidade de predizer o risco de complicações ou morte. Pouco se sabe até o momento se os recuperados da COVID-19 que cursaram com dano miocárdico apresentarão disfunções cardíacas relevantes em longo prazo. É preciso mais tempo e maior pesquisa sobre o tema.

Our understanding of strong gravity near supermassive compact objects has recently improved thanks to the measurements made by the Event Horizon Telescope (EHT). We use here the M87∗ shadow size to infer constraints on the physical charges of a large variety of nonrotating or rotating black holes. For example, we show that the quality of the measurements is already sufficient to rule out that M87∗ is a highly charged dilaton black hole. Similarly, when considering black holes with two physical and independent charges, we are able to exclude considerable regions of the space of parameters for the doubly-charged dilaton and the Sen black holes. © 2021 authors. Esophageal squamous papilloma is thought to arise from a chronic inflammatory reaction due to mechanical or chemical irritant. ESP is usually considered to have a benign clinical course; however, some reports highlight the potential development of a malignancy. The development of extensive esophageal squamous papillomas also known as squamous papillomatosis of the esophagus is even less frequent. Presentation of the case We present the case of an esophageal papillomatosis that developed into an invasive esophageal squamous cell carcinoma that was only diagnosed in the surgical specimen after minimally invasive Ivor-Lewis esophagectomy and in this case report, we discuss its etiology, diagnosis and treatment. Conclusion Extensive papillomatosis with continuous symptoms, especially persistent dysphagia, should always prompt a thorough analysis into a possible underlying malignancy.

The peripheral arteries and veins of the extremities are among the most commonly injured vessels in both civilian and military vascular trauma. Blunt causes are more frequent than penetrating except during military conflicts and in certain geographic areas. Physical examination and simple bedside investigations of pulse pressures are key in early identification of these injuries. In stable patients with equivocal physical examinations, computed tomography angiograms have become the mainstay of screening and diagnosis. Immediate open surgical repair remains the first-line therapy in most patients. However, advances in endovascular therapies and more widespread availability of this technology have resulted in an increase in the range of injuries and frequency of utilization of minimally invasive treatments for vascular injuries in stable patients. Prevention of and early detection and treatment of compartment syndrome remain essential in the recovery of patients with significant peripheral vascular injuries. The decision to perform amputation in patients with mangled extremities remains difficult with few clear indicators. Abdominal vascular trauma accounts for a small percentage of military and a moderate percentage of civilian trauma, affecting all age ranges and impacting young adult men most frequently. Penetrating causes are more frequent than blunt in adults, while blunt mechanisms are more common among pediatric populations. High rates of associated injuries, bleeding, and hemorrhagic shock ensure that, despite advances in both diagnostic and therapeutic technologies, immediate open surgical repair remains the mainstay of treatment for traumatic abdominal vascular injuries. Because of their devastating nature, abdominal vascular injuries remain a significant source of morbidity and mortality among trauma patients. Among the numerous published studies on COVID-19 in a pandemic year, few listed asthma as a comorbidity, making it therefore difficult to draw any solid conclusions. The respiratory allergy and controlled exposures to allergens are associated with significant reductions in the expression of angiotensin-converting enzyme 2 (ACE2), receptor for SARS-CoV-2 entry into human cells. There is a hypothesis that patients with chronic asthma, due to the type 2 inflammatory profile, may be potentially resistant to developing a severe clinical course of COVID-19. The low IFN-g-mediated response in the respiratory tract of asthmatic patients could limit ACE2 expression in the target cells of SARS-CoV-2 infection. The inflammatory profile of the airways in patients with chronic asthma is mainly related to a Th2 response in type-2 asthma, with production of IL-4, IL-13 and IL-5, presenting antagonistic relationship with pro-inflammatory cytokines such as IFN-g, produced at high levels in severe COVID-19. Published studies, for the most part, are retrospective and may have loss of information or present material with limited possibility for more robust and conclusive analysis. It is important to discuss how patients with atopic or nonatopic asthma seem to protect themselves from new coronavirus infection and how asthma affects COVID-19 and the course of the disease, since there is no increased mortality in asthmatic patients with COVID-19 compared to non-asthmatic patients.

Duodeno-pancreatic and extrahepatic biliary tree injuries are rare in both adult and pediatric trauma patients, and due to their anatomical location, associated injuries are very common. Mortality is primarily related to associated injuries, but morbidity remains high even in isolated injuries. Optimal management of duodeno-bilio-pancreatic injuries is dictated primarily by hemodynamic stability, clinical presentation, and grade of injury. Endoscopic and percutaneous interventions have increased the ability to non-operatively manage these injuries. Late diagnosis and treatment are both associated to increased morbidity and mortality. Sequelae of late presentations of pancreatic injury and complications of severe pancreatic trauma are also increasingly addressed endoscopically and with interventional radiology procedures. However, for moderate and severe extrahepatic biliary and severe duodeno-pancreatic injuries, immediate operative intervention is preferred as associated injuries are frequent and commonly present with hemodynamic instability or peritonitis. The aim of this paper is to present the World Society of Emergency Surgery (WSES) and American Association for the Surgery of Trauma (AAST) duodenal, pancreatic, and extrahepatic biliary tree trauma management guidelines. © 2019 The Author(s).

This declaration, signed by an interdisciplinary task force of 234 experts from 83 different countries with different backgrounds, highlights the threat posed by antimicrobial resistance and the need for appropriate use of antibiotic agents and antifungal agents in hospitals worldwide especially focusing on surgical infections. As such, it is our intent to raise awareness among healthcare workers and improve antimicrobial prescribing. Background The development of metabolic syndrome (MS) after liver transplantation (LT) is a major source of mortality derived from cardiovascular events. The aim of the present study was to determine the prevalence and risk factors of MS after LT. Methods One-hundred seventy-four consecutive LT patients from January 2004 to June 2010 surviving longer than 1 year after LT were included. Median follow-up after LT was 48 months. Independent predictors of MS were obtained by means of multivariate logistic regression. Results At 3 years after LT, 25.5% of patients reached a body mass index (BMI) ≥30 kg/m 2 , 35.6% of patients developed arterial hypertension, 54.2% showed impaired fasting glucose, 22.5% had serum cholesterol >200 mg/dL, and 22.5% showed hypertriglyceridemia >150 mg/dL. The prevalence of MS ranged from 49% to 86% depending on the considered period. The pre-LT variables associated with MS were age at LT (odds ratio [OR], 1.08; P = .002), BMI of recipient before LT (OR, 1.23; P = .001), serum glucose (OR, 1.02; P = .005), and non–heart-beating donor (OR, 1.02; P = .046). The post-LT predictors of MS were body weight (OR, 1.04; P = .005), arterial hypertension (OR, 1.02; P = .047), and serum glucose (OR, 1.02; P = .011) at 6 months. Conclusions LT patients develop MS in a high proportion and progressively despite current efforts (ie, lifestyle modifications and aggressive management of hypertension, diabetes, and hyperlipidemia). The associated risk factors include age, increased BMI, and pre- and post-LT glucose.

Objective: To determine the knowledge level of undergraduate dentistry students of UESB on hematological disorders and their implications in the treatment of the patient. Methods: This is a descriptive, quantitative and cross-sectional study in which a questionnaire with 16 questions was used as a data collection instrument. The study subjects were 192 students enrolled at UESB Dentistry Course. For data analysis, SPSS statistical software (version 16.0) was used. Results: The response rate was 87.5%. Only 54.2% of respondents reported having information about blood disorders and 78% considered themselves not enough informed, fact observed through the low percentage of right answers in a knowledge test. Conclusion: The level of knowledge of UESB Dentistry academics on hematological disorders is insufficient and there isn't a gradual increment of this knowledge throughout the semesters, polarized mainly in the 6th and 10th semester. RESUMO Objetivo: Verificar o nível de conhecimento de graduandos do curso de Odontologia da UESB sobre os distúrbios hematológicos e suas implicações para o tratamento do paciente em âmbito odontológico. Métodos: Trata-se de um estudo descritivo, quantitativo e de corte transversal, onde adotou-se como instrumento de coleta de dados um questionário, composto por 16 perguntas. Os sujeitos da pesquisa foram os 192 estudantes regularmente matriculados no curso de Odontologia da UESB. Para análise dos dados foi utilizado o software estatístico SPSS (versão 16.0). Resultados: A taxa de resposta foi de 87,5%. Apenas 54,2% dos entrevistados relataram ter informações acerca dos distúrbios hematológicos e 78% consideraram-se insuficientemente informados, fato corroborado pelas baixas porcentagens de acertos nos testes de conhecimento. Conclusão: O nível de conhecimento dos acadêmicos de Odontologia da UESB sobre os distúrbios hematológicos mostrou-se insuficiente e não houve um crescimento gradual deste conhecimento ao longo dos semestres, estando o mesmo polarizado principalmente nos 6º e 10º semestres.

Objetivo: Avaliar a prevalência de lesões orais em usuários de prótese dentária. Material e método: 110 usuários de próteses dentárias foram avaliados quanto ao sexo, faixa etária, tipo de prótese, tempo de uso de prótese e tempo de uso da prótese atual, frequência e métodos de higienização bucal e da prótese, uso noturno da prótese e consumo de bebidas alcoólicas e fumo. Para coleta dos dados realizou-se o exame clínico dos participantes e posterior orientação de higiene bucal e da prótese. Resultado: 34,6% dos usuários de prótese exibiram algum tipo de patologia, sendo a estomatite protética (27,3%) a mais prevalente. 2,7% dos participantes desenvolveram queilite e 4,6% uma associação de estomatite e queilite. Não houve diferença significativa entre os sexos quanto à presença de lesão. 46,4% dos usuários apresentavam higiene bucal deficiente e desses 54,9% possuíam algum tipo de lesão bucal. A frequência de higienização demonstrou ser um dos fatores determinantes no processo de desenvolvimento de lesões. 71,9% dos participantes relataram fazer uso noturno da prótese. 30% dos que bebem e 30% dos fumantes ativos possuíam lesão bucal. Conclusão: Há ainda um grande número de pessoas acometidas por lesões orais de simples diagnóstico e intervenção. Assim, faz-se extremamente necessário a contínua orientação, conscientização e acompanhamento profissional da população para uma melhor qualidade de vida.Descritores: Prótese Dentária; Prevalência; Mucosa Bucal; Patologia Bucal.

Arterial complications after total knee arthroplasty have an incidence of 0.25%, but the risk to the limb requires aggressive management. We present three cases of popliteal lesions after such surgery. One was a case of acute ischaemia due to popliteal thrombosis managed by an autologous vein by-pass. The second case consisted of a popliteal pseudoaneurysm with symptoms of knee pain and late functional impotence treated with a venous patch. The last case consisted of a non-occlusive intimal dissection managed with conservative treatment. In agreement with published data we believe that an early diagnosis and aggressive treatment of acute arterial thrombosis secondary to knee arthroplasty is necessary, surgical by-pass has the best results. Endovascular technics for popliteal pseudoaneurysm have good results, but due to the lack of long-term follow it is difficult to generalise their indication. Lastly, there is a group of non- occlusive, asymptomatic lesions which can be conservatively managed.

The stresses generated in the laser cladding technique result from the high thermal gradients involved. They could lead to undesirable distortions or even crack formation, therefore the prediction of the temperatures, strains and stresses during the process results in its better understanding, besides, it could allow some optimization. A transient nonlinear thermomechanical FEM model was developed for this task. In order to validate its predictions, measurements by the energy dispersive X-ray diffraction (EDXRD) technique were performed at the European Synchrotron Radiation Facility (ESRF), for several samples with the Stellite 6B alloy on the AISI 304 stainless steel, and compared with the predictions.

Objetivo Valorar retrospectivamente el grado de necrosis en el hepatocarcinoma producido por la quimioembolizacion hepatica correlacionando el estudio histologico de los higados explantados en el trasplante hepatico con factores morfologicos del tumor (numero, tamano, tipo histologico, encapsulacion e infiltracion vascular) y tecnicos (patron angiografico, numero de sesiones y nivel de la quimioembolizacion). Material y metodos Se estudian 17 pacientes cirroticos trasplantados hepaticos por hepatocarcinoma y a los que previamente se les realizo quimioembolizacion. Este procedimiento se realizo introduciendo en ramas de la arteria hepatica adriamicina, lipiodol y particulas de alcohol de polivinilo. Los higados explantados se estudiaron macro y microscopicamente, valorando el grado de necrosis alcanzado en cada uno de los nodulos encontrados. Resultados En el higado explantado se detectaron 32 nodulos en 16 pacientes: 25 hepatocarcinomas y 7 nodulos displasicos tipo II; en el paciente restante fueron multiples hepatocarcinomas. El tamano medio de los hepatocarcinomas fue de 3,2 cm (0,4-7,5) y el de los nodulos displasicos de 1,2 cm (1-1,7). En relacion con los 25 hepatocarcinomas se consiguio una necrosis mayor del 90% en 17 de ellos (68%), 6 de los cuales estaban encapsulados y ninguno presento infiltracion vascular. En los pacientes con multiples hepatocarcinomas la necrosis fue menor del 30% en cada uno de ellos (todos sin capsula e hipervascularizados). En tres pacientes donde aparecio infiltracion vascular por alguno de los nodulos coincidio con multiplicidad de los mismos, necrosis del 0-70% de estos y aparicion de metastasis extrahepaticas en el periodo postrasplante. Dieciocho de los hepatocarcinomas tenian un patron angiografico hipervascularizado y 7 eran hipo/avasculares. En relacion con los 7 nodulos displasicos tipo II (todos no encapsulados) la necrosis fue nula en todos ellos; aparecieron en tres pacientes, todos (excepto uno) eran avasculares y coincidieron con hepatocarcinomas simultaneos en dos pacientes. Conclusion Las mayores necrosis parecen obtenerse en hepatocarcinonas unicos, encapsulados, hipervascularizados y sin infiltracion vascular. La necrosis por el procedimiento es nula en los nodulos displasicos.

Vitamin E (alpha-tocopherol) has demonstrated antioxidant activity and gene-regulatory properties. d-Galactosamine (D-GalN)-induced cell death is mediated by nitric oxide in hepatocytes, and it is associated with hepatic steatosis. The beneficial properties of alpha-tocopherol and their relation to oxidative stress and gene regulation were assessed in D-GalN-induced cell death. Hepatocytes were isolated from human liver resections by a collagenase perfusion technique. alpha-Tocopherol (50 microM) was administered at the advanced stages (10 h) of D-GalN-induced cell death in cultured hepatocytes. Cell death, oxidative stress, alpha-tocopherol metabolism, and NF-kappaB-, pregnane X receptor (PXR)-, and peroxisome proliferator-activated receptor (PPAR-alpha)-associated gene regulation were estimated in the hepatocytes. D-GalN increased cell death and alpha-tocopherol metabolism. alpha-Tocopherol exerted a moderate beneficial effect against apoptosis and necrosis induced by D-GalN. Induction (rifampicin) or inhibition (ketoconazole) of alpha-tocopherol metabolism and overexpression of PXR showed that the increase in PXR-related CYP3A4 expression caused by alpha-tocopherol enhanced cell death in hepatocytes. Nevertheless, the reduction in NF-kappaB activation and inducible nitric oxide synthase expression and the enhancement of PPAR-alpha and carnitine palmitoyl transferase gene expression by alpha-tocopherol may be relevant for cell survival. In conclusion, the cytoprotective properties of alpha-tocopherol are mostly related to gene regulation rather than to antioxidant activity in toxin-induced cell death in hepatocytes.

Orthotopic liver transplantation (OLT) is the best treatment for nonresectable hepatocellular carcinoma (HCC), but tumor recurrence reduces long-term and medium-term survival. The effectiveness of adjuvant chemotherapy to prevent tumor recurrence has not been fully established. Methods Three hundred eighty-seven consecutive patients, including 43 with HCC superimposed on liver cirrhosis, underwent OLT. Twelve patients with one or more prognostic criteria for HCC recurrence were entered into a prospective prophylaxis protocol with monthly cycles of cisplatin (60 mg/m2) and adriamycin (30 mg/m2), beginning the fourth week post-OLT for a maximum of seven sessions. Results The 5-year survival of the non-HCC patients was 65.7% and that of the HCC patients was 60.46% (P = NS). Chemotherapy was reasonably well tolerated, but the 9 patients with hepatitis C- or B-associated cirrhosis showed viral and histological recurrence of the primary disease. A high proportion of patients (7 of 12) developed tumor recurrence during the first year after OLT. Six of these patients died, all but one due to HCC relapse. Five patients remain healthy and tumor free at 58 to 130 months. Post-OLT adjuvant chemotherapy does not avoid tumor recurrence and its fatal consequences but may contribute to prolonged tumor-free survival among a significant proportion of patients with high-risk HCC. However, the uncertain implications on viral recurrence and the lack of control groups do not allow post-OLT chemotherapy to be recommended outside controlled clinical trials, which are clearly warranted.

Resonance Raman spectra for zucchini, cucumber, bean, and lettuce plastocyanins, blue copper proteins involved in plant photosynthetic electron transport, have been measured at wavelengths throughout their intense 600-nm absorption band. A comparison of the resonance Raman spectra of these four plastocyanins, and two others previously quantified, demonstrates that they all exhibit vibrational bands with similar frequencies but significantly different relative intensities. Self-consistent analysis of the absorption band and resulting resonance Raman excitation profiles using a time-dependent wave packet formalism for these four plastocyanins demonstrates that many of the derived molecular parameters are similar to those determined previously for two other species of plastocyanin. However, significant differences are observed in the mode-specific displacements and reorganization energies, although the total reorganization energy is 0.16 ± 0.01 eV for all six plastocyanins. The low-field complex AC permeability, μ=μ′−jμ″, of a nearly non-magnetostrictive amorphous wire has been measured after 470°C annealing as a function of frequency f (=0.1 –102 kHz) and DC bias field HDC(=0–7.2 A/m). With increasing HDC, the low-f eddy-current anomaly factor η increases from 2.8 to 11.3, and after a low-f rise, the loss factor tan δ(=μ″/μ′) versus f curve changes from one showing a maximum 1.28 and a minimum 1.01 at 4.5 and 22 kHz, respectively, with a final value of 1.49 at 102 kHz to one with a slow increase from 0.34 at 7 kHz to 0.44 at 102 kHz. These anomalous behaviors are explained by a helical magnetization core-shell model.
