1. Accuracy evaluation of orthodontic movements with aligners: a prospective observational study

Abstract: Abstract Background Since their introduction in orthodontics, clear aligners have been appreciated by patients, including adults, for their comfort and low aesthetic impact. Despite the enormous mobilization of financial resources all over the world aimed at producing new product lines, few clinical studies or high-quality evidence have been produced regarding the real effectiveness of such treatment. Given the few limited kinds of research on the subject, this study aims to produce and critically evaluate other data, to establish the concrete reliability of clear aligners in orthodontic therapy. Results Significant sample sizes were obtained for intrusion, vestibulo/lingual (V/L) crown tipping, and rotation. The overall accuracy for rotation resulted in 86%, ranging from 96% for maxillary central incisors to 70.4% for mandibular first premolars. The intrusion was registered only for anterior teeth; mean predictability was 92%, with the worst result being 86.7% for mandibular canines and the best being 98% for mandibular central incisors. V/L tipping was the most accurate movement: 93.1% of the prescribed movement was completed. Maxillary central incisors showed the lowest accuracy (80.7%), while mandibular central incisors were the highest (97.5%). Conclusions The present study provided reassuring data in support of the accuracy of the Invisalign® system. Vestibulo/lingual tipping was the most predictable movement, while rotation of canines, premolars, and lateral incisors were the least predictable. Intrusion resulted highly predictable up to 2 mm. When careful treatment planning follows a correct diagnosis, together with the use of auxiliary features and refinements, the planned results can be achieved in a clinically successful way. Authors believe that there is a major need for greater samples to overcome bias related to variables if we want to answer the unsolved questions, such as the predictability of severe malocclusions treatment.

Abstract: Open conduit volcanoes like Stromboli can display elusive changes in activity before major eruptive events. Starting on December 2020, Stromboli volcano displayed an increasing eruptive activity, that on 19 May 2021 led to a crater-rim collapse, with pyroclastic density currents (PDCs) that spread along the barren NW flank, entered the sea and ran across it for more than 1 km. This episode was followed by lava flow output from the crater rim lasting a few hours, followed by another phase of lava flow in June 2021. These episodes are potentially very dangerous on island volcanoes since a landslide of hot material that turns into a pyroclastic density current and spreads on the sea surface can threaten mariners and coastal communities, as happened at Stromboli on 3 July and 28 August 2019. In addition, on entering the sea, if their volume is large enough, landslides may trigger tsunamis, as occurred at Stromboli on 30 December 2002. In this paper, we present an integration of multidisciplinary monitoring data, including thermal and visible camera images, ground deformation data gathered from GNSS, tilt, strainmeter and GBInSAR, seismicity, SO2 plume and CO2 ground fluxes and thermal data from the ground and satellite imagery, together with petrological analyses of the erupted products compared with samples from previous similar events. We aim at characterizing the preparatory phase of the volcano that began on December 2020 and led to the May–June 2021 eruptive activity, distinguishing this small intrusion of magma from the much greater 2019 eruptive phase, which was fed by gas-rich magma responsible for the paroxysmal explosive and effusive phases of July–August 2019. These complex eruption scenarios have important implications for hazard assessment and the lessons learned at Stromboli volcano may prove useful for other open conduit active basaltic volcanoes.

3. Novel buffer for long-term preservation of DNA in biological material at room temperature

Abstract: The collection and preservation of biological material before DNA analysis is critical for inter alia biomedical research, medical diagnostics, forensics and biodiversity conservation. In this study, we evaluate an in-house formulated buffer called the Forensic DNA Laboratory-buffer (FDL-buffer) for preservation of biological material for long term at room temperature. Human saliva stored in the buffer for 8 years, human blood stored for 3 years and delicate animal tissues from the jellyfish Pelagia noctiluca comb jelly Beroe sp., stored for 4 and 6 years respectively consistently produced high-quality DNA. FDL-buffer exhibited compatibility with standard organic, salting out and spin-column extraction methods, making it versatile and applicable to a wide range of applications, including automation.

Abstract: The ASTRI Mini-Array (MA) is an INAF project to build and operate a facility to study astronomical sources emitting at very high-energy in the TeV spectral band. The ASTRI MA consists of a group of nine innovative Imaging Atmospheric Cherenkov telescopes. The telescopes will be installed at the Teide Astronomical Observatory of the Instituto de Astrofisica de Canarias (IAC) in Tenerife (Canary Islands, Spain) on the basis of a host agreement with INAF. Thanks to its expected overall performance, better than those of current Cherenkov telescopes' arrays for energies above \sim 5 TeV and up to 100 TeV and beyond, the ASTRI MA will represent an important instrument to perform deep observations of the Galactic and extra-Galactic sky at these energies., Comment: 19 pages, 22 figures

5. An optimized histological proceeding to study the female gametophyte development in grapevine

Abstract: Abstract Background Reproductive success in seed plants depends on a healthy fruit and seed set. Normal seed development in the angiosperms requires the production of functional female gametophytes. This is particularly evident in seedless cultivars where defects during megagametophyte’s developmental processes have been observed through cytohistological analysis. Several protocols for embryo sac histological analyses in grapevine are reported in literature, mainly based on resin- or paraffin-embedding approaches. However their description is not always fully exhaustive and sometimes they consist of long and laborious steps. The use of different stains is also documented, some of them, such as hematoxylin, requiring long oxidation periods of the dye-solution before using it (from 2 to 6 months) and/or with a differentiation step not easy to handle. Paraffin-embedding associated to examination with light microscope is the simplest methodology, and with less requirements in terms of expertise and costs, achieving a satisfactory resolution for basic histological observations. Safranin O and fast green FCF is an easy staining combination that has been applied in embryological studies of several plant species. Results Here we describe in detail a paraffin-embedding method for the examination of grapevine ovules at different phenological stages. The histological sample preparation process takes 1 day and a half. Sections of 5 µm thickness can be obtained and good contrast is achieved with the safranin O and fast green FCF staining combination. The method allows the observation of megasporogenesis and megagametogenesis events in the different phenological stages examined. Conclusions The histological sample preparation process proposed here can be used as a routine procedure to obtain embedded ovaries or microscope slides that would require further steps for examination. We suggest the tested staining combination as a simple and viable technique for basic screenings about the presence in grapevine of a normally and fully developed ovule with embryo sac cells, which is therefore potentially functional.

6. Pericapsular nerve group (PENG) block for hip arthroscopy: a randomized, double-blinded, placebo-controlled trial

Abstract: Introduction Arthroscopic hip surgery is associated with significant postoperative pain. Femoral nerve blocks have been shown to improve postoperative analgesia at the expense of quadriceps weakness. The pericapsular nerve group (PENG) block could be an alternative that may improve postoperative analgesia while preserving quadriceps strength. Our hypothesis was that a PENG block would provide superior postoperative analgesia compared with a sham block following arthroscopic hip surgery. Methods Subjects presenting for arthroscopic hip surgery were randomized in a 1:1 ratio to either an ultrasound-guided unilateral, single-injection PENG block (PENG group) with 20 mL of 0.5% ropivacaine or a sham injection with 5 mL of 0.9% normal saline (Sham group) prior to receiving general anesthesia. The primary outcome of this study was worst pain score within 30 min of emergence from anesthesia. Secondary outcomes included opioid consumption, patient satisfaction with analgesia, opioid-related adverse events, and persistent opioid use at 1 week. Results Sixty-eight subjects, 34 from the PENG group and 34 from the Sham group, completed the study per protocol. Analysis of the primary outcome demonstrated a mean difference in pain scores of −0.79 (95% CI −1.96 to 0.37; p=0.17) between the PENG and Sham groups immediately following surgery. No secondary outcomes showed statistically significant differences between groups. Discussion This study demonstrates that a preoperative PENG block does not improve analgesia following arthroscopic hip surgery. Trial reigstration number NCT04508504

Abstract: The ASTRI Mini-Array is a next-generation system of nine imaging atmospheric Cherenkov telescopes that is going to be built at the Observatorio del Teide site. After a first phase, in which the instrument will be operated as an experiment prioritizing a schedule of primary science cases, an observatory phase is foreseen in which other significant targets will be pointed. We focus on the observational feasibility of extragalactic sources and on astrophysical processes that best complement and expand the ASTRI Mini-Array core science, presenting the most relevant examples that are at reach of detection over long-term time scales and whose observation can provide breakthrough achievements in the very-high energy extragalactic science. Such examples cover a wide range of $\gamma$-ray emitters, including the study of AGN low states in the multi-TeV energy range, the possible detection of Seyfert galaxies with long exposures and the searches of dark matter lines above 10 TeV. 8. Deciphering the nature of the pulsar wind nebula CTB 87 with XMM-Newton

Abstract: CTB 87 (G74.9+1.2) is an evolved supernova remnant (SNR) which hosts a peculiar pulsar wind nebula (PWN). The X-ray peak is offset from that observed in radio and lies towards the edge of the radio nebula. The putative pulsar, CXOU J201609.2+371110, was first resolved with Chandra and is surrounded by a compact and a more extended X-ray nebula. Here, we use a deep XMM–Newton observation to examine the morphology and evolutionary stage of the PWN and to search for thermal emission expected from a supernova shell or reverse shock interaction with supernova ejecta. We do not find evidence of thermal X-ray emission from the SNR and place an upper limit on the electron density of 0.05 per cu.cm for a plasma temperature kT ∼ 0.8 keV. The morphology and spectral properties are consistent with a ∼20-kyr-old relic PWN expanding into a stellar wind-blown bubble. We also present the first X-ray spectral index map from the PWN and show that we can reproduce its morphology by means of 2D axisymmetric relativistic hydrodynamical simulations.

Abstract: A deep survey of the Large Magellanic Cloud at ~0.1-100TeV photon energies with the Cherenkov Telescope Array is planned. We assess the detection prospects based on a model for the emission of the galaxy, comprising the four known TeV emitters, mock populations of sources, and interstellar emission on galactic scales. We also assess the detectability of 30 Doradus and SN 1987A, and the constraints that can be derived on the nature of dark matter. The survey will allow for fine spectral studies of N157B, N132D, LMC P3, and 30 Doradus C, and half a dozen other sources should be revealed, mainly pulsar-powered objects. The remnant from SN 1987A could be detected if it produces cosmic-ray nuclei with a flat power-law spectrum at high energies, or with a steeper index 2.3-2.4 pending a flux increase by a factor >3-4 over ~2015-2035. Large-scale interstellar emission remains mostly out of reach of the survey if its >10GeV spectrum has a soft photon index ~2.7, but degree-scale 0.1-10TeV pion-decay emission could be detected if the cosmic-ray spectrum hardens above >100GeV. The 30 Doradus star-forming region is detectable if acceleration efficiency is on the order of 1-10% of the mechanical luminosity and diffusion is suppressed by two orders of magnitude within, Accepted for publication in MNRAS. 10. Comparing bupivacaine alone to liposomal bupivacaine plus bupivacaine in interscalene blocks for total shoulder arthroplasty: a randomized, non-inferiority trial

Abstract: IntroductionInterscalene brachial plexus blocks are a commonly performed procedure to reduce pain following total shoulder arthroplasty. Liposomal bupivacaine has been purported to prolong the duration of brachial plexus blocks for up to 72 hours; however, there has been controversy surrounding the analgesic benefits of this drug. Our hypothesis was that an interscalene block performed with bupivacaine alone would be non-inferior to a combination of liposomal bupivacaine and bupivacaine with respect to opioid consumption following total shoulder arthroplasty.MethodsSubjects presenting for primary total shoulder arthroplasty were randomized in a 1:1 ratio to an ultrasound-guided, single-injection interscalene block with either a combination of liposomal bupivacaine and bupivacaine (LB group) or bupivacaine without additive (Bupi group). The primary outcome of this study was 72-hour postoperative cumulative opioid consumption (in oral morphine equivalents) with a non-inferiority margin of 22.5 mg. Secondary outcomes included pain scores, patient satisfaction with analgesia and patient reported duration of sensory block.ResultsSeventy-six subjects, 38 from the Bupi group and 38 from the LB group, completed the study. Analysis of the primary outcome showed a 72-hour cumulative geometric mean oral morphine equivalent consumption difference of 11.9 mg (95% CI −6.9 to 30.8) between groups (calculated on the log scale). This difference constitutes approximately 1.5 tablets of oxycodone over 3 days. No secondary outcomes showed meaningful differences between groups.DiscussionInterscalene brachial plexus blocks performed with bupivacaine alone did not demonstrate non-inferiority compared to a mixture of liposomal bupivacaine plus bupivacaine with regards to 72-hour cumulative opioid consumption following total shoulder arthroplasty. However, the difference between groups did not appear to be clinically meaningful.

Abstract: We have investigated the oxidative behaviour of natural compounds such as methyl abietate (1), farnesyl acetate (2), α-ionone (3), β-ionone (4), methyl linolelaidate (5), methyl linolenate (6) and bergamottin (7) with the oxidant system methyltrioxo-rhenium/ H2O2/pyridine. The reactions, performed in CH2Cl2/H2O at 25 °C, have shown good regio- and stereoselectivity. The oxidation products were isolated by HPLC or silica gel chromatography and characterized by MS(EI), 1H-, 13C-NMR, APT, gCOSY, HSQC, TOCSY and NOESY measurements. The selectivity seems to be controlled by the nucleophilicity of double bonds and by stereoelectronic and steric effects.

12. Regional Anesthesia for Hip Arthroscopy

Abstract: Pain after hip arthroscopy can be severe, yet we lack a consensus method for non-narcotic analgesia. Here we describe anatomic elements of hip arthroscopy and our current understanding of the relevant sensory innervation as a prelude to the evaluation of locoregional analgesic techniques. Many regional nerve blocks and local anesthetic infiltration techniques are reviewed, including 2 newer ultrasound fascial plane blocks. Further study of targeted, motor-sparing approaches, either ultrasound-guided or under direct surgical visualization is needed.

Abstract: Very-high Energy (VHE) gamma-ray astroparticle physics is a relatively young field, and observations over the past decade have surprisingly revealed almost two hundred VHE emitters which appear to act as cosmic particle accelerators. These sources are an important component of the Universe, influencing the evolution of stars and galaxies. At the same time, they also act as a probe of physics in the most extreme environments known - such as in supernova explosions, and around or after the merging of black holes and neutron stars. However, the existing experiments have provided exciting glimpses, but often falling short of supplying the full answer. A deeper understanding of the TeV sky requires a significant improvement in sensitivity at TeV energies, a wider energy coverage from tens of GeV to hundreds of TeV and a much better angular and energy resolution with respect to the currently running facilities. The next generation gamma-ray observatory, the Cherenkov Telescope Array Observatory (CTAO), is the answer to this need. In this talk I will present this upcoming observatory from its design to the construction, and its potential science exploitation. CTAO will allow the entire astronomical community to explore a new discovery space that will likely lead to paradigm-changing breakthroughs. In particular, CTA has an unprecedented sensitivity to short (sub-minute) timescale phenomena, placing it as a key instrument in the future of multi-messenger and multi-wavelength time domain astronomy. Abstract: The ASTRI (Astrofisica con Specchi a Tecnologia Replicante Italiana) Mini-Array will be composed of nine imaging atmospheric Cherenkov telescopes at the Observatorio del Teide site. The array will be best suited for astrophysical observations in the 0.3-200 TeV range with an angular resolution of few arc-minutes and an energy resolution of 10-15\%. A core-science programme in the first four years will be devoted to a limited number of key targets, addressing the most important open scientific questions in the very-high energy domain. At the same time, thanks to a wide field of view of about 10 degrees, ASTRI Mini-Array will observe many additional field sources, which will constitute the basis for the long-term observatory programme that will eventually cover all the accessible sky. In this paper, we review different astrophysical Galactic environments, e.g. pulsar wind nebulae, supernova remnants, and gamma-ray binaries, and show the results from a set of ASTRI Mini-Array simulations of some of these field sources made to highlight the expected performance of the array (even at large offset angles) and the important additional observatory science that will complement the core-science program., Comment: Accepted for publication in Journal of High-Energy Astrophysics

Abstract: The ASTRI (Astrofisica con Specchi a Tecnologia Replicante Italiana) Project led by the Italian National Institute for Astrophysics (INAF) is developing and will deploy at the Observatorio del Teide a mini-array (ASTRI Mini-Array) composed of nine telescopes similar to the small-size dual-mirror Schwarzschild-Couder telescope (ASTRI-Horn) currently operating on the slopes of Mt. Etna in Sicily. The ASTRI Mini-Array will surpass the current Cherenkov telescope array differential sensitivity above a few tera-electronvolt (TeV), extending the energy band well above hundreds of TeV. This will allow us to explore a new window of the electromagnetic spectrum, by convolving the sensitivity performance with excellent angular and energy resolution figures. Abstract: Pulsar wind nebulae (PWNe) represent the largest class of sources that upcoming {\gamma}-ray surveys will detect. Therefore, accurate modelling of their global emission properties is one of the most urgent problems in high-energy astrophysics. Correctly characterizing these dominant objects is a needed step to allow {\gamma}-ray surveys to detect fainter sources, investigate the signatures of cosmic-ray propagation and estimate the diffuse emission in the Galaxy. In this paper we present an observationally motivated construction of the Galactic PWNe population. We made use of a modified one-zone model to evolve for a long period of time the entire population. The model provides, for every source, at any age, a simplified description of the dynamical and spectral evolution. The long term effects of the reverberation phase on the spectral evolution are described, for the first time, based on physically motivated prescriptions for the evolution of the nebular radius supported by numerical studies. This effort tries to solve one of the most critical aspects of one-zone modeling, namely the typical overcompression of the nebula during the reverberation phase, resulting in a strong modification of its spectral properties at all frequencies. We compare the emission properties of our synthetic Pulsar Wind Nebulae population with the most updated catalogues of TeV Galactic sources. We find that the firmly identified and candidate PWNe sum up to about 50% of the expected objects in this class above threshold for detection. Finally, we estimate that CTA will increase the number of TeV detected PWNe by a factor$\geq3$., Comment: 15 pages, 8 figures, 2 tables. 18. Foundations of Cosmic Ray Astrophysics

Abstract: The Enrico Fermi Schools, a cultural initiative promoted by the Italian Physical Society (SIF), were initiated in 1953 in a period that marked the beginnings of what is now called cosmic ray astrophysics. Indeed, the very first edition of the School focused on the contribution of cosmic ray physics to the laws of elementary particle physics. Almost seventy years later, another School has been focusing on the foundations of cosmic ray astrophysics, and this book contains the lectures presented as Course 208 of the prestigious International School of Physics Enrico Fermi, Foundations of Cosmic Ray Astrophysics, held in Varenna, Italy from 23 to 29 June 2022. Topics covered in the book range from particle acceleration to cosmic ray transport, and from radiation processes to feedback of cosmic rays in galaxy formation. In particular, a thorough description of the physics of cosmic ray transport in turbulent magnetic fields, and of the production of magnetic perturbations in the presence of cosmic ray gradients is provided, both these phenomena being essential in describing particle acceleration at shocks and the transport of cosmic rays within the galaxy. The transport of cosmic rays in different environments is also discussed. This area of research has received an extraordinary boost in recent times, thanks to the now unprecedented accuracy of observations, and to the possibility of testing new ideas and complex scenarios offered by recent developments in numerical simulation, and the book will be of interest to all those working in the field of high energy astrophysics.

Abstract: Introduction: many studies show an higher prevalence of OSA in males than female in adulthood.Woman show non specific symptoms ( tiredness, initial insomnia, morning headache) with multiple comorbidities. Sleep disorders in women are underdiagnosed. Detecting "atypical" symptons is important for identiphyng OSA.Unrecognized OSA in women can have an important impact on mortality. Aim of the study: to analyze the clinical aspects of OSA with subjective and objective methods in two groups of patients, distinguished by age and sex. Materials and methods: 1160 patients performed basal poloygraphy:961 eligible for the study,580 M, 381 F, dvided into two age group:adults(l65 years),and elderly (g65 years). Parameters subjective (ESS, PSQI,Berlin test) and objective (anthropometric measurements, polygraphy data) was peformed. Exclusion criteria: no informed consent. Results: the statistical analysis shows the presence of gender differences in the presentation and severity of the disease:OSA is more severe in males, who exhibit significant snoring, in women this symptom is less evident.The usual test are insufficient to identify OSA in women. In aging, estrogenic protection due to menopause fails, the AHI increases significantly compared to adulthood.While the circumference of the neck seems more related to the severity of OSA in both sexes in the geriatirc group. Anatomical susceptibiity to OSA appears to worsen whit aging and there appears to be preferential deposition of fat around the pharynx with aging regardless of systemic fat. Conclusion: Female with OSA are older, obese and have different symptoms than male.Specific questionnaires should be created and used to optimize diagnosis.

Abstract: SUMMARYThe annual incidence of listeriosis in Italy is lower (0·19–0·27 per 100 000 inhabitants per year) than in Europe (0·34–0·52 per 100 000 inhabitants per year). Since the observed incidence of listeriosis may be biased downward for underdiagnosis or under-reporting, this work aims to estimate the real incidence of listeriosis during a 9-year period in the Lombardy region, Italy. Data on listeriosis cases were collected from national mandatory notification system (MAINF) and Laboratory-based Surveillance System (LabSS). The two sources were cross-matched and capture–recapture method was applied to estimate the number of undetected cases and the real incidence of invasive listeriosis. Five hundred and eighty invasive listeriosis cases were detected by the two sources between 2006 and 2014: 50·2% were identified only via MAINF, 16·7% were recorded only via LabSS, overlaps occurred in 192 cases (33·1%). The mean annual incidence detected only by MAINF was 0·56 per 100 000 inhabitants, which rose to 0·67 per 100 000 considering also the cases detected by LabSS. The capture–recapture method allowed to estimate an incidence of 0·84 per 100 000. The high incidence of listeriosis may be due to improved sensitivity of the surveillance system, but also reflect a real increase, associated with an increased population at risk.

Abstract: Listeria monocytogenes causes invasive syndromes with high fatality rates in specific population groups. Cheeses have been commonly implicated in outbreaks worldwide. Gorgonzola is a cheese only produced in Northwestern Italy (it is the third Italian cheese in terms of production and export) and L. monocytogenes is frequently isolated from the production chain. The aims of this study were to assess the distribution of L. monocytogenes Virulence Types (VTs) in isolates collected in Gorgonzola processing plants and to determine the presence of Epidemic Clones (ECs). Fifty-Six L. monocytogenes strains collected between 2004 and 2016 from cheese and environmental samples were subtyped with Multi-Virulence-Locus Sequence Typing (MVLST) and compared to previously typed strains. Most isolates (n=50) belonged to two new VTs (VT113 and VT114). The remaining isolates belonged to previously identified VTs: VT14-ECVIII (milk chocolate outbreak, 1994, USA) and VT80 (ricotta salata outbreak, 2012, USA). VT14, VT80 and VT113 were shared with isolates from apparently sporadic human cases in the same geographical area and temporal period (Piedmont and Lombardy, 2005-2016). The overall L. monocytogenes population appears to be homogeneous and may be characteristic of Gorgonzola production. Nevertheless, the detection in cheese and environmental samples of VTs observed in clinical isolates or outbreak related strains (VT80, VT14) contributed to better describe the current scenario and pointed out the need for increased surveillance.

Abstract: BackgroundReproductive success in seed plants depends on a healthy fruit and seed set. Normal seed development in the angiosperms requires the production of functional female gametophytes. This is particularly evident in seedless cultivars where defects during megagametophyte’s developmental processes have been observed through cytohistological analysis. Several protocols for embryo sac histological analyses in grapevine are reported in literature, mainly based on resin- or paraffin-embedding approaches. However their description is not always fully exhaustive and sometimes they consist of long and laborious steps. The use of different stains is also documented, some of them, such as hematoxylin, requiring long oxidation periods of the dye-solution before using it (from 2 to 6 months) and/or with a differentiation step not easy to handle. Paraffin-embedding associated to examination with light microscope is the simplest methodology, and with less requirements in terms of expertise and costs, achieving a satisfactory resolution for basic histological observations. Safranin O and fast green FCF is an easy staining combination that has been applied in embryological studies of several plant species.ResultsHere we describe in detail a paraffin-embedding method for the examination of grapevine ovules at different phenological stages. The histological sample preparation process takes 1 day and a half. Sections of 5 µm thickness can be obtained and good contrast is achieved with the safranin O and fast green FCF staining combination. The method allows the observation of megasporogenesis and megagametogenesis events in the different phenological stages examined.ConclusionsThe histological sample preparation process proposed here can be used as a routine procedure to obtain embedded ovaries or microscope slides that would require further steps for examination. We suggest the tested staining combination as a simple and viable technique for basic screenings about the presence in grapevine of a normally and fully developed ovule with embryo sac cells, which is therefore potentially functional.

Abstract: Overview: Abilities Care Experts™ (ACE™) is an educational training program for nursing staff who care for individuals with dementia in skilled nursing facilities (SNF). The objective of the program is to provide the nursing staff with knowledge, and methods to engage residents’ remaining abilities in order to maximize occupational participation using a stage-specific dementia care approach. ACE™ training has yet to be evaluated for effectiveness in a controlled study design. Objectives: This study sought to evaluate the effectiveness of the ACE™ training. Design: The study employed a quantitative, cross-sectional, quasi-experimental design. Participants: A total of 29 CNAs, 13 ACE™ trained and 16 non-ACE™ trained, from two Ensign SNFs. Measurements: Participants completed a survey battery measuring key factors associated with quality of care: 1) knowledge 2) attitudes 3) self-efficacy and 4) job satisfaction. Results: Compared to non-ACE™ trained participants, ACE™ trained participants scored higher on each outcome measure. ACE™ participants demonstrated significantly higher levels of self-perception of dementia knowledge (t (27) =3.44, p=.002), knowledge of dementia care approaches (t (26) =3.57, p=.001), and self-efficacy (t (25) =2.40, p=.024). Conclusion: This study provides initial evidence that ACE™ can improve key factors associated with quality of care.

Abstract: Polypyridine-based ruthenium complexes are among the most interesting photoactive molecular systems, in virtue of a number of outstanding (photo)physicochemical properties. To exploit such properties in solid-state applications, such as molecular electronics, optoelectronics, and photovoltaics, tailored experimental strategies are needed for the anchoring of such complexes on surfaces. At the same time, these strategies need to be investigated and validated through precise monitoring of the surface composition. In this contribution, the authors report on the application of time-of-flight secondary ion mass spectrometry (ToF-SIMS) for studying the anchoring onto transparent (semi)conducting fluorine tin-doped oxide of the polypyridinic photoactive RuDT2. This molecule is characterized by an Ru(tpy)2 core and two free terpyridinic units. The authors’ approach involves the use of a surface priming strategy based on the chemistry of zirconium phosphates and phosphonates (ZP-priming) to provide a platform for direct anchoring of RuDT2 molecules at the oxide surface. Onto patterned ZP-fluorine-doped tin oxide (FTO), the authors report selective anchoring of RuDT2 only onto ZP domains, while no binding occurs onto bare FTO areas. The latter were shown to remain available for further functionalization using functional molecules containing phosphonic moieties, and in the authors’ case, a phosphonic derivative of rhodamine B (RhB-P) was used. ToF-SIMS imaging was intensively used to monitor each reaction step and confirm the successful laterally resolved and selective adsorption of RuDT2 and RhB-P onto ZP and bare FTO domains, respectively.

Abstract: The fabrication of stable, highly conductive molecular nano-junctions is one of the main research goals in the field of molecular electronics. In this paper we report on the self-assembly and functional characterisation of highly conductive molecular wires, based on mixed-metal polynuclear complexes, at the surface of a transparent conductive oxide. The adopted synthetic approach involves metal-coordination reactions on oxide surfaces, pre-functionalised with a monolayer of terpyridine moieties that are used as anchoring sites for the integration of ditopic, redox-active ruthenium-bisterpyridine molecules through iron(ii) centres. By the stepwise iteration of the iron-coordination reaction, molecular wires of the desired length can be prepared, which alternate iron and ruthenium centres in the wire backbone. The stepwise assembly of the wires at the transparent conductive oxide surface was characterised by means of UV-Vis spectroscopy and, at the nanoscale, by means of ToF-SIMS measurements. The electrical characteristics of the wires were obtained by the liquid-metal eutectic-gain nano-junction technique, with results that show good electron transport capabilities along the wires. The demonstrated feasibility of the integration of these metal-polypyridinic, redox-active, conductive wires at the surface of a transparent and conductive oxide, and the evidence for good electrical conduction indicates prospective applications in the field of nanoscale molecular optoelectronics.

Abstract: Background In recent years Europe has experienced a dramatic increase in migration flows. Nevertheless, limited data is available about the occurrence of neglected parasitic diseases among migrant population. The purpose of the present study was to evaluate the prevalence of intestinal and urinary parasites in newly arrived asylum seekers. Methods A total of 364 newly arrived migrants hosted at the Asylum Seekers Centre of Castelnuovo di Porto (Italy) were screened during 8 months period for intestinal and urinary parasites. Each enrolled subject was interviewed using a standardized questionnaire, with focus on socio-demographical data and risk factors of parasitic infections. Results Stool analysis showed a prevalence of intestinal parasites of 20.6%. The travel route did not affect the prevalence of intestinal parasites (p = 0.096), while a significant negative correlation was found between the length of travel and the prevalence of parasite infection (p = 0.019). No statistically significant correlation between gastrointestinal symptoms and the presence of intestinal parasites was detected. Conclusion The prevalence of intestinal parasitosis reported in asylum seekers does not necessarily reflect the prevalence of the parasitosis in the motherland. An anamnestic and syndromic approach may not be sufficient to highlight the problem of intestinal parasitic infestations in a screening setting.

Abstract: Staphylococcus aureus is a known major cause of foodborne illnesses, and raw milk and dairy products are often contaminated by enterotoxigenic and antimicrobial-resistant S. aureus strains. In the present study, 35 S. aureus strains were isolated from 383 raw milk samples collected from various dairy herds in the province of Milan (northern Italy). The isolates were characterized based on their antimicrobial susceptibility patterns and the presence of genes encoding staphylococcal enterotoxins (sea, seb, sec, sed, and see). About half (45.7%) of the strains were enterotoxigenic, and 37.1% were resistant to at least one of the antimicrobial drugs tested. Seven (20%) of 35 isolates were identified as methicillin-resistant S. aureus (MRSA), and SCCmec typing performed with a multiplex PCR assay revealed the presence of gene cassettes IV and V, typical of community-acquired MRSA, and I and II, characteristic of health care-associated MRSA. The MRSA strains were evaluated for the presence of the Panton-Valentine leukocidin gene, but this gene was not found. The results of the present study revealed the presence of toxin-producing S. aureus and MRSA strains in raw milk. MRSA and enterotoxigenic S. aureus in dairy farms are an important risk factor for the spread of staphylococcal infections; therefore, further studies are needed to find strategies for monitoring and controlling the presence of S. aureus, especially MRSA, in dairy products.

UV/Vis measurements indicate the formation of 1:1 complexes. 1H-, 31P-NMR titrations and T-ROESY experiments confirm that oximes bind the organophosphate via hydrogen bond.

Abstract: In developed countries, pregnancy-related listeriosis accounts for 20–43% of total invasive listeriosis. This work describes the first pregnancy-related listeriosis survey in Italy based on two data sources, that is, mandatory notification system and regional laboratory-based network. Out of 610 listeriosis cases reported over a 10-year period, 40 were pregnancy-related (6.6%). Among these, 29 pregnancy-related isolates were available and have been analysed with serotyping, Pulsed-Field Gel Electrophoresis, and Multi-Virulence-Locus Sequence Typing. No maternal fatality was recorded, but 11 (29.7%) pregnancies resulted in a foetal death, a miscarriage, or a birth of a foetus dying immediately after birth. The average incidence of pregnancy-related listeriosis was 4.3 cases per 100000 births, and the proportion of pregnancy-associated listeriosis among ethnic minorities was significantly higher compared to the general population (30.0% versus 3.5%,P<0.01).L. monocytogenesisolates belonged to serotypes 1/2a, 1/2b, and 4b, with the latter significantly more prevalent among pregnancy-related isolates. Twenty different pulsotypes were distinguished and 16 out of the 29 isolates were classified into seven clusters. A total of 16 virulence types (VTs) were identified. Five VTs accounted for 45% of the total cases and coincided with those of previously described Epidemic Clones (ECs) ofL. monocytogenes.

Abstract: Background Molecular subtyping and enhanced surveillance in Lombardy region identified a cluster of possibly related listeriosis cases from 2006 to 2010. This cluster grouped 31 isolates that belonged to serotype 1/2a and Sequence Type 38 (ST38) as defined by Multilocus Sequence Typing (MLST). Methods Our study expanded the previous investigation to include cases from 2011 to 2014 and used Multi-Virulence-Locus Sequence Typing (MVLST) on all ST38 isolates to better understand their epidemiology and possibly identify a common source outbreak. Results Out of 306 L. monocytogenes clinical isolates collected, 43 (14.1%) belonged to ST38 with cases occurring in nine out of twelve Lombardy provinces. The ST38 isolates were split by MVLST into two Virulence Types (VTs): VT80 (n = 12) and VT104 (n = 31). VT104 cases were concentrated between 2009 and 2011 in two provinces, Bergamo and Milan. An epidemiologic investigation was performed and in one case, a matching VT104 isolate was retrieved from a soft cheese sample from a patient’s refrigerator. Conclusions Our findings revealed a major listeriosis outbreak in Northern Italy linked to soft cheese in 2009–2011, which went undetected by local health authorities. Our study shows that integrating subtyping methods with conventional epidemiology can help identify the source of L. monocytogenes outbreak clones. Abstract: Salmonella enterica serotype Napoli (S. Napoli) is currently emerging in Europe and particularly in Italy, where in 2014 it caused a large outbreak associated with elevated rates of bacteremia. However, no study has yet investigated its invasive ability and phylogenetic classification. Here, we show that between 2010 and 2014, S. Napoli was the first cause of invasive salmonellosis affecting 40 cases out of 687 (invasive index: 5.8%), which is significantly higher than the invasive index of all the other nontyphoidal serotypes (2.0%, p 0.05). Genomic and phylogenetic analyses of an invasive isolate revealed that S. Napoli belongs to Typhi subclade in clade A, Paratyphi A being the most related serotype and carrying almost identical pattern of typhoid-associated genes. This work presents evidence of invasive capacity of S. Napoli and argues for reconsideration of its nontyphoidal category.

Abstract: A multischool outbreak of salmonellosis caused by Salmonella enterica serovar Napoli was investigated in the province of Milan from October to November 2014, following an increase in school absenteeism coinciding with two positive cases. Epidemiological studies detected 47 cases in four primary schools: 46 children and 1 adult woman (51.4% males and 48.6% females, median age 8.9). From these, 14 cases (29.8%) were severe and resulted in hospitalization, including 6 children (12.8%) who developed an invasive salmonellosis. The epidemic curve revealed an abnormally long incubation period, peaking 1 week after the first confirmed case. Twenty-five available isolates were typed by pulsed-field gel electrophoresis showing an identical pattern. The isolate belongs to ST474, an ST composed exclusively of Salmonella Napoli human strains isolated in France and Italy. Antibiotic resistance analysis showed resistance to aminoglycosides, correlating with the presence of the aminoglycoside resistance gene aadA25 in its genome. Trace-back investigations strongly suggested contaminated ham as the most likely food vehicle, which was delivered by a common food center on 21 October. Nevertheless, this ingredient could not be retrospectively investigated since it was no longer available at the repository. This represents the largest Salmonella Napoli outbreak ever reported in Italy and provides a unique scenario for studying the outcome of salmonellosis caused by this emerging and potentially invasive nontyphoidal serotype.

Abstract: Awareness of previous hepatic encephalopathy (HE) and compliance with treatment can probably reduce HE recurrence. The aim of this study was to assess the degree of awareness of previous HE and its treatment in a group of cirrhotic patients and their caregivers. Thirty-five cirrhotic patients with a history of HE and their caregivers (n = 31) were enrolled. Patients underwent evaluation of HE (clinical, psychometry and electroencephalography), quality of life (SF36 questionnaire), and awareness of HE/treatment on an ad hoc questionnaire (QAE). Caregivers underwent the QAE plus the Caregiver Burden Inventory. On the day of study, 7 patients were unimpaired, 8 had minimal and 20 low-grade overt HE. Of the patients, 37 % were aware of previous HE, 6 % of being on treatment and 6 % understood treatment effects. Of the caregivers, 48 % were aware of previous HE, 6 % of their relative being on treatment and 6 % understood treatment effects. Significant correlations were observed between neuropsychiatric status/linear HE indices and both the patients' quality of life and the caregivers' burden. In conclusion, HE awareness was poor in both patients and caregivers, most likely in relation to insufficient/inadequate provision of information.

Abstract: Frozen shoulder is a clinical condition characterized by a relatively sudden onset, variable degrees of pain, restricted range of motion, and normal radiographic imaging techniques. Although the disease occurs in a small segment of the population, its management presents the clinician with an opportunity to use all of his or her skills to alleviate the pain and restore function of the shoulder. This review article discusses contemporary ideas on etiology, diagnosis, and noninvasive treatment including manual medicine techniques, surgical options, and prognosis.

48. Studies on Environmental Effects of Underwater Chemical Munitions in the Southern Adriatic Sea (Mediterranean Sea)

Abstract: The present paper shows the multidisciplinary approach used to assess the ecotoxicity of chemical munitions lying on the seabed in the Southern Adriatic Sea where aerial bombs charged with mustard agent and organoarsenic chemical warfare agents (CWAs) have been dumped.Sampling activities and laboratory analyses have been carried out on two sentinel species, the blackbelly rosefish, Helicolenus dactylopterus, and the European conger, Conger conger, collected in a CWA dumping site 35 nm from the coast of Apulia, Italy, and from a reference site. Fish were analyzed through an ecotoxicological approach, integrating chemical analysis and biological responses.Degradation products of the blister agents bis-(2-chloroethyl)sulphide, commonly known as mustard agent or yperite (also called sulfur or sulfur mustard), were found in sediment samples collected nearby aerial bombs. Although no evidence of these compounds was detected in tissues of either fish, levels of heavy metals (arsenic and mercury), potentially released by rusted chemical weapons, were significantly higher in sediment and fish from the CWA site compared to the reference site. Severe external and internal lesions were observed in fish captured in the CWA site, with congers displaying small to large skin ulcers along the body. Health assessment index (HAI) values, as well as spleen melano-macrophages centres and CYP1A ethoxyresorufin-O-deethylase activity were higher in fish from the CWA site, indicating a chronic state of illness and environment degradation.The working procedures and analyses performed during these surveys could be suitable for future biomonitoring studies in other CWA dumping sites.

49. The Impact of Perioperative Catastrophes on Anesthesiologists

Abstract: Most anesthesiologists will experience at least one perioperative catastrophe over the course of their careers. Very little, however, is known about the emotional impact of these events and their effects on both immediate and long-term ability to provide care. In this study, we examined the incidence of perioperative catastrophes and the impact of these outcomes on American anesthesiologists.We sent a self-administered postal survey to 1200 randomly selected members of the American Society of Anesthesiologists. Participants were sent an advance letter, up to 2 copies of the survey, up to 2 reminder postcards, and a small cash incentive. Six hundred fifty-nine physicians (56%) completed the survey.Eighty-four percent of respondents had been involved in at least one unanticipated death or serious injury of a perioperative patient over the course of his/her career. Queried about the emotional impact of a "most memorable" perioperative catastrophe,70% experienced guilt, anxiety, and reliving of the event with 88% requiring time to recover emotionally from the event and 19% acknowledging having never fully recovered. Twelve percent considered a career change. Sixty-seven percent of respondents believed that their ability to provide patient care was compromised in the first 4 hours subsequent to the event, but only 7% were given time off.A perioperative catastrophe may have a profound and lasting emotional impact on the anesthesiologist involved and may affect his or her ability to provide patient care in the aftermath of such events.

Abstract: A series of new compounds containing a benzimidazole, benzothiazole, or benzoxazole nucleus linked to an arylpiperazine by different thioalkyl chains was prepared. They were tested in radioligand binding experiments to evaluate their affinity for 5-HT 1A and 5-HT 2A serotonergic, alpha 1 adrenergic, D1, and D2 dopaminergic receptors. Many of tested compounds showed an interesting binding profile; in particular, 36 displayed very high 5-HT 1A receptor affinity and selectivity over all the other investigated receptors. Selected compounds, evaluated in functional assays, showed antagonistic or partial agonistic activity at 5-HT 1A receptor. An extensive conformational research using both NMR and modeling techniques indicated that extended conformations predominated in vacuum, in solution and during interactions with 5-HT 1A receptor. Finally, the elaborated binding mode of selected compounds at 5-HT 1A receptor was used to explain the influence of spacer length on ligands affinity.
