T-cell replete hematopoietic stem cell transplantation (HSCT) from a haploidentical donor followed by high doses of cyclophosphamide has been demonstrated to provide the best chances of a cure for many children in need of an allograft but who lack both a sibling and an unrelated donor. In this study we retrospectively compared the outcome of pediatric patients undergoing T-replete haploidentical HSCT (Haplo) for acute leukemia with those undergoing transplantation from unrelated HLA-matched donor (MUD) and HLA mismatched unrelated donor (MMUD) from 2012 to 2017 at our Center. Both univariable and multivariable analyses showed similar 5-year overall survival rates for MUD, MMUD, and Haplo patients: 71% (95% CI 56-86), 72% (95% CI 55-90), and 75% (95% CI 54-94), respectively (p = 0.97). Haplo patients showed reduced event-free survival rates compared to MUD and MMUD patients: 30% (95% CI 12-49) versus 70% (95% CI 55-84) versus 53% (95% CI 35-73), respectively (p = 0.007), but these data were not confirmed by a multivariable analysis. Non-relapse mortality (NRM) and relapse incidence (RI) were similar for the three groups. Therefore, our data confirm that Haplo is a suitable clinical option for pediatric patients needing HSCT when lacking both an MUD and an MMUD donor.

Prognosis of relapsed leukemia patients after second allogeneic hematopoietic stem cell transplantation (HSCT2) is historically considered very poor. We report the outcome of 18 pediatric patients after failure of HSCT2. The 2-year overall survival was 26% (95% confidence interval [CI], 6-47). The lymphoid malignancies were associated with better survival (40% [95% CI, 12-68]) than myeloid malignancies (0%, P=0.002), together with time to relapse after the HSCT2 (≥5 mo: 44% [95% CI, 12-76] vs. 0% for patients who relapsed within 5 mo from HSCT2, P=0.005), other factors such as sex, donor type, conditioning regimen, and graft versus host disease prophylaxis did not have statistical significance. When the multivariate analysis was carried out, 2 independent protective factors were identified: the lymphoid malignancies and the graft versus host disease 0 to I after HSCT2. When we look at the treatments, patients receiving blinatumomab after relapse got benefit in terms of overall survival and, more importantly, with a long-term control of acute lymphoblastic leukemia.

So far, we do not have any published database regarding HCT patients during the present pandemic. We herein describe the first case of COVID-19 presenting in the early hematopoietic cell transplantation (HCT) setting.

openaire: EC/H2020/633053/EU//EUROfusion Gross and net erosion of tungsten (W) and other plasma-facing materials in the divertor region have been investigated in deuterium (D) and helium (He) plasmas during dedicated experiments in L- and H-mode on ASDEX Upgrade and after full-length experimental campaigns on the WEST tokamak. Net erosion was determined via post-exposure analyses of plasma-exposed samples and full-size wall components, and we conclude that the same approach is applicable to gross erosion if marker structures with sub-millimeter dimensions are used to eliminate the contribution of prompt re-deposition. In H-mode plasmas, gross erosion during ELMs may exceed the situation in inter-ELM conditions by 1-2 orders of magnitude while net erosion is typically higher by a factor of 2-3. The largest impact on net erosion is attributed to the electron temperature while the role of the impurity mixtures is weaker, even though both on ASDEX Upgrade and WEST significant amounts of impurities are present in the edge plasmas. Impurities, on the other hand, will lead to the formation of thick co-deposited layers. We have also noted that with increasing surface roughness, net erosion is strongly suppressed and the growth of co-deposited layers is enhanced. In He plasmas, gross erosion is increased compared to D due to the higher mass and charge states of the plasma particles, resulting from larger energies due to sheath acceleration, but strong impurity fluxes can result in apparent net deposition in the divertor. Our results from ASDEX Upgrade and WEST are comparable and indicate typical net-erosion rates of 0.1-0.4 nm s(-1), excluding the immediate vicinity of the strike-point regions.

Background PGF is historically associated with high morbidity and mortality after allogeneic hematopoietic stem cell transplantation (allo-HSCT). Methods In this study, we report our multicenter experience on stem cell boost (SCB) for PGF, or incomplete donor engraftment, in 16 pediatric patients. Donors were HLA-matched siblings (n = 4), unrelated donors (n = 11), or haploidentical family members (n = 1). Ten patients had two-lineage cytopenia, 5 had one-lineage cytopenia, and 1 had poor immunological reconstitution together with a low percentage of donor cell engraftment. A median of 6.6x106 selected CD34+/Kg was infused after 194 days from allo-HSCT (48-607). Results In 4 out of 5 patients, one-lineage cytopenia was resolved, while among the 10 patients with two-lineage cytopenia, 4 resolved both cytopenia, 5 resolved one-lineage, and one did not respond. All patients reverted their mixed chimera to full donor chimera. OS was 56%, transplant-related mortality (TRM) 32%, and RI 12%. The main causes of failure were related to infections with 4 out of 7 deaths caused by this. Conclusions SCB may rescue over 50% of patients with PGF after allo-HSCT. An earlier treatment may reduce the infectious complications and improve survival.

BACKGROUND Hematopoietic stem cell transplantation (HSCT) is curative in patients with primary immunodeficiencies. However, pre-HSCT conditioning entails unacceptably high risks if the liver is compromised. The presence of a recurrent opportunistic infection affecting the biliary tree and determining liver cirrhosis with portal hypertension posed particular decisional difficulties in a 7-year-old child with X-linked CD40-ligand deficiency. We aim at adding to the scanty experience available on such rare cases, as successful management with sequential liver transplantation (LT) and HSCT has been reported in detail only in 1 young adult to date. METHODS A closely sequential strategy, with a surgical complication-free LT, followed by reduced-intensity conditioning, allowed HSCT to be performed only one month after LT, preventing Cryptosporidium parvum recolonization of the liver graft. RESULTS Combined sequential LT and HSCT resolved the cirrhotic evolution and corrected the immunodeficiency so that the infection responsible for the progressive sclerosing cholangitis did not recur. CONCLUSIONS Hopefully, this report of the successful resolution of a potentially fatal combination of immunodeficiency and chronic opportunistic infection with end-stage organ damage in a child will encourage others to adapt a sequential transplant approach to this highly complex pathology. However, caution is to be exercised to carefully balance the risks intrinsic to transplant surgery and immunosuppression in primary immunodeficiencies.

Several guidelines have been published about management of chronic GvHD (cGvHD), but the clinical practice still remains demanding. The Gruppo Italiano Trapianto di Midollo Osseo (GITMO) has planned a prospective observational study on cGvHD, supported by a dedicated software, including the updated recommendations. In view of this study, two surveys have been conducted, focusing the management of cGvHD and ancillary therapy in cGvHD, to address the current 'real life' situation. The two surveys were sent to all 57 GITMO centers, performing allografting in Italy; the response rate was 57% and 66% of the interviewed centers, respectively. The first survey showed a great disparity especially regarding steroid-refractory cGvHD, although extracorporeal photo-apheresis resulted as the most indicated treatment in this setting. Another challenging issue was the strategy for tapering steroid: our survey showed a great variance, and this disagreement could be a real bias in evaluating outcomes in prospective studies. As for the second survey, the results suggest that the ancillary treatments are not standardized in many centers. All responding centers reported a strong need to standardize management of cGvHD and to participate in prospective trials. Before starting observational and/or interventional studies, a detailed knowledge of current practice should be encouraged.

Allogeneic hematopoietic stem cell transplantation (HSCT) from an unrelated HLA-mismatched donor (MMUD) is one of the alternatives where an HLA-matched donor is not found. Patients were divided into 4 groups: 43 pediatric with ATG and bone marrow as stem cell source (BM, A), 17 pediatric with ATG and peripheral blood stem cells (PBSC, B), 39 adult patients with ATG and PBSC (C) and 41 adults with PT-Cy-MMF-FK506 and PBSC (D) as GvHD prophylaxis. Variation in clinical practice affects veno-occlusive disease management, mainly in patients who undergo allogeneic hematopoietic stem cell transplantation. Disputes about diagnostic criteria, treatment, and prophylaxis, due to the lack of high-quality data, are at the base of this variability. With the aim of limiting inconsistency in clinical care, thus improving both patient outcomes and data collection reliability, the Italian Society of Stem cell transplant (Gruppo Italiano Trapianto Midollo Osseo e Terapia Cellulare) launched a collaborative effort to formulate recommendations based on integration of available evidence and expert's consensus. A systematic method, according to US National Institute of Health guidelines and Italian National System for Guidelines, was used. Twenty-nine recommendations were approved with a strong (20) or weak (9) level of agreement, while 26 were rejected. In particular, the panel pointed out the need to achieve an early diagnosis, encouraging the adoption of European Society for Blood and Marrow Transplantation criteria and the prompt use of ultrasonography. Moreover, our experts strongly recommended in favor of prophylactic use of ursodeoxycholic acid. As soon as a veno-occlusive disease diagnosis is established, treatment with defibrotide should be started for at least 21 days. A number of areas of uncertainty, particularly concerning risk stratification and use of diagnostic tools such as elastography has been identified and discussed.

aGvHD remains a major obstacle to successful HSCT. We report our experience on steroid-refractory aGvHD III and IV from 1989 to 2017. Ninety patients with aGvHD III or IV were stratified according to the HSCT year: 1989-1998, 1999-2007, and 2008-2017 and to aGvHD extension (GvHD III vs IV) and finally the probability of OS, RI, and TRM was calculated accordingly. aGvHD III patients had a substantial improvement over time: day 100 OS raised from 64% (95% CI 39-89) in the first cohort to 100% in the latest (P = .022), and it was mainly due to a reduction of TRM (it was 28% [95% CI 12-65] in the first cohort to 0% in the latest (P = .01). The aGvHD IV patients did not present a significant improvement. Day 100 OS was 42% (95% CI 16-68) in the first group and 54% (95% CI 25-83) in the year 2008-2017 (P = NS), and the day-100 TRM was very similar (it was 57% [95% CI 36-90] in the first cohort and 45% [95% CI 23-89] in the latest (P = NS). We report significant improvements in OS and TRM in patients diagnosed with grade III aGvHD. Patients with the most severe aGvHD appear to have no or fewer benefits on long-term outcomes.

We have measured the thermal expansion of Ni nanowires (NWs) electrodeposited into self-organized nanoporous amorphous aluminum oxide (AAO) membranes without an Al substrate using X-ray diffraction between 110 K and 350 K. The results indicate an average thermal expansion of the Ni NWs - along the wire axis - of α ¯ NiNW = − 1.6 ± 1.5 × 10 − 6 K − 1. Assuming a bulk-like thermal expansion of the isolated Ni NWs, this result indicates that AAO also has a negative thermal expansion. We estimate the thermal expansion of nanoporous AAO to be α AAO = − 5 ± 1 × 10 − 6 K − 1. We show that data obtained previously on the thermal expansion of metallic NWs grown in the nanoporous AAO may be interpreted as originating from a negative thermal expansion of the matrix. At the next World Radiocommunication Conference 2019 of the International Telecommunication Union, Agenda Item 1.13 will address the identification and the possible additional allocation of radio-frequency spectrum to serve the future development of the International Mobile Telecommunications (IMT) for 2020 and beyond, mainly focused on systems supporting the fifth generation of cellular mobile communications. The frequency range of interest goes from 24.25 to 86 GHz, which fully covers all millimeter bands used or planned by the European Space Agency’s space missions for high data rate transmissions. This paper shows the potential impact of IMT deployments in terms of received radio frequency interference by ESA’s telecommunication links in frequency bands allocated to the Earth Exploration-Satellite Service and to the Space Research Service. Particular focus is given to the ESA’s tracking network and to the earth stations located in New Norcia (Australia), Cebreros (Spain), Malargue Sur (Argentina) and Kiruna (Sweden). Results are shown in terms of required separation distances, i.e., the minimum distance between the earth station and the IMT station ensuring that the protection criteria for the earth station are met by the emissions of an IMT base station or user equipment. Results point out the need of having separation distances up to 100 km depending on the station and frequency band under consideration.

Solar Orbiter is an ESA mission in cooperation with NASA scheduled for launch in February 2020. The Solar Orbiter spacecraft will enter, after a multi-year cruise phase, into an elliptical orbit around the sun with a perihelion of around 0.3 AU and an increasing inclination of up to 35° to return images of the solar polar regions and to probe the plasma of the inner heliosphere.As all deep space missions, Solar Orbiter has a severely power limited data downlink, which means that optimizing the science data return of the mission within the given constraints is of paramount importance.The Solar Orbiter mission operations centre has consequently optimized the concept, technique and duration of the required ranging transmissions to improve the science data return while meeting the orbit determination requirements for flying the mission through multiple planetary gravity assists.Solar Orbiter TT&C subsystem is derived from the successful Bepi Colombo design that implemented for the first time the CCSDS recommended Pseudo Noise (PN) regenerative ranging with residual carrier telemetry modulation schemes like NRZ-L/PSK/PM or SP-L/PM. In order to provide the required mission science data return and, due to the bandwidth limitation of the SP-L/PM modulation, GMSK (Gaussian Minimum Shift Keying) modulation was included in the Solar Orbiter design allowing increased telemetry data rates up to 1 Mbps while NRZ-L/PSK/PM and SP-L/PM were kept for lower data rates.Successful in-flight utilization of GMSK modulation with the Herschel and Planck spacecraft in 2009, Gaia in 2013 and ExoMars TGO in 2016, has increased the interest to investigate the possibility of utilizing GMSK modulation simultaneously with PN regenerative ranging. As such ESA started to investigate this technique for Lagrange and Deep Space missions as part of the CCSDS standardization activities. The development activities started in 2012 and the first tests with a ground station modem breadboard [7] to demonstrate the concept feasibility were performed in 2014 at the ESOC reference station in Darmstadt.Being GMSK a suppressed carrier modulation, it does not allow to use at the same time the ESA standard phase modulated ranging signals (based on a residual carrier) so that dedicated ranging sessions were needed in the missions flown so far. Alternating sessions with residual carrier modulation of low rate telemetry plus ESA standard ranging and sessions with suppressed carrier GMSK modulation of high rate telemetry had to be performed in each communication pass in order to allow gathering of the required ranging data for orbit determination while meeting the scientific data volume requirements.To avoid such inefficient mode switching, the use of simultaneous GMSK telemetry modulation with PN regenerative ranging was studied in CCSDS and a recommend standard was published. Solar Orbiter adopted and implemented it both on board and on ground.The Bepi Colombo transponder also implemented the generation of Differential Delta One-way Ranging (DDOR) signals used to improve the navigation during gravity assist manoeuvres (GAMs) and during fly-bys. Solar Orbiter will also rely on Earth and Venus GAMs during its 10 years extended mission, so DDOR signals were also inherited in the design. The additional inclusion of in flight programmable modulation indexes brought to the creation of a semaphores modulation scheme that will be used to provide fundamental spacecraft health status information in case it would enter in survival mode during which only limited communication capabilities are possible.This paper will present the implementation of these new features and report on the performance results obtained during the Radio Frequency compatibility tests between the RF suitcase including the on-board TT&C engineering models and the ESTRACK ground station at ESOC.

Purpose: Reiki is a growing complementary therapy in pediatric oncology that needs evidence to become more credible among the health community. A within-subject design experiment was conducted to pilot testing the feasibility and efficacy of Reiki to provide pain relief among pediatric patients undergoing hematopoietic stem cell transplantation (HSCT). Method: Pediatric patients undergoing HSCT during the inpatient phase in the Stem Cell Transplantation Unit were eligible to participate to the pilot study. Short and medium effects were assessed investigating the increase or decrease of patient’s pain during three specific time periods (“delta”) of the day: morning of the Reiki session versus assessment before Reiki session (within subjects control period), assessment before Reiki session versus assessment after Reiki session (within subjects experimental period) and assessment after Reiki session versus morning the day after Reiki session (within subject follow-up period). The long-term effects were verified comparing the pain evolution in the day of the Reiki session with the following rest day. Results: The effect of 88 Reiki therapy sessions in nine patients (Mage = 12; Female = 61%) was analyzed following a short, medium, and long-term perspective. Repeated-measures analysis of variance revealed a significant difference among the three periods ( F = 17,17 p < .0001): A decrease of the pain occurred in the experimental period in short and medium term, while in the follow-up period, the pain level remained stable. Conclusions: This study demonstrates the feasibility of using Reiki therapy in pediatric cancer patients undergoing HSCT. Furthermore, these findings evidence that trained pediatric oncology nurses can insert Reiki into their clinical practice as a valid instrument for diminishing suffering from cancer in childhood.

Combined direct antineoplastic activity and the long-lasting immunological effects of allogeneic hematopoietic cell transplant (HCT) can cure many hematological malignancies, but broad adoption requires non-relapse mortality (NRM) rates and graft-versus-host disease (GVHD) control. Recently, posttransplant cyclophosphamide (PTCy) given after a bone marrow transplant significantly reduced GVHD-incidence, while PTCy given with tacrolimus/mofetil mycophenolate (T/MMF) showed activity following allogeneic peripheral blood stem cell transplantation (alloPBSCT). Here, we report the experience of a larger cohort (85 consecutive patients) and expanded follow-up period (03/2011–12/2019) with high-risk hematological malignancies who received alloPBSCT from Human-Leukocyte-Antigens HLA-matched unrelated/related donors. GVHD-prophylaxis was PTCy 50 mg/kg (days+3 and +4) combined with T/MMF (day+5 forward). All patients stopped MMF on day+28 with day+110 = median tacrolimus discontinuation. Cumulative incidences were 12% for acute and 7% for chronic GVHD- and no GVHD-attributed deaths. For surviving patients, the 12, 24, and 36-month probabilities of being off immunosuppression were 92, 96, and 96%, respectively. After a 36-month median follow-up, NRM was 4%, median event-free survival (EFS) and overall survival (OS) had yet to occur. One- and two-year chronic GVHD-EFS results were 57% (95% CI, 46–68%) and 53% (95% CI, 45–61%), respectively, with limited late infections and long-term organ toxicities. Disease relapse caused the most treatment failures (38% at 2 years), but low transplant toxicity allowed many patients (14/37, 38%) to receive donor lymphocyte infusions as a post-relapse strategy. We confirmed that PTCy+T/MMF treatment effectively prevented acute and chronic GVHD and limited NRM to unprecedented low rates without loss of disease control efficacy in an expanded patient cohort. Post-transplant high-dose cyclophosphamide (PTCy) is a novel approach to prevent graft-versus-host disease (GVHD) and rejection in patients given haploidentical hematopoietic stem cell transplantation (HSCT). Thirty-three patients with high-risk hematologic malignancies and lacking a match-related or -unrelated donor were treated with PTCy haploidentical HSCT in 5 Italian AIEOP centers. Nineteen patients had a nonmyeloablative preparative regimen (57%), and 14 patients received a full myeloablative conditioning regimen (43%). No patients received serotherapy; GVHD prophylaxis was based on PTCy (50 mg/kg on days +3 and +4) combined with mycophenolate plus tacrolimus or cyclosporine A. Neutrophil and platelet engraftment was achieved on days +17 (range, 14 to 37) and +27 (range, 16 to 71). One patient had autologous reconstitution for anti-HLA antibodies. Acute GVHD grades II to IV and III to IV and chronic GVHD developed in 22% (95% CI, 11 to 42), 3% (95% CI, 0 to 21), and 4% (95% CI, 0 to 27) of cases, respectively. The 1-year overall survival rate was 72% (95% CI, 56 to 88), progression-free survival rate was 61% (95% CI, 43 to 80), cumulative incidence of relapse was 24% (95% CI, 13 to 44), and transplant-related mortality was 9% (95% CI, 3 to 26). The univariate analysis for risk of relapse incidence showed how 3 significant variables, mother as donor (P = .02), donor gender as female (P = .04), and patient gender as female (P = .02), were significantly associated with a lower risk of relapse. Disease progression was the main cause of death. PTCy is a safe procedure also for children and adolescents who have already received several lines of chemotherapy. Among the different diseases, a trend for better 1-year rates of overall survival was obtained for nonacute leukemia patients.

Fertility after childhood haemopoietic stem cell transplant (HSCT) is a major concern. Conditioning regimens before HSCT present a high risk (>80%) of ovarian failure. Since 2000, we have proposed cryopreservation of ovarian tissue to female patients undergoing HSCT at our centre, to preserve future fertility. After clinical and haematological evaluation, the patients underwent ovarian tissue collection by laparoscopy. The tissue was analysed by histologic examination to detect any tumour contamination and then frozen following the slow freezing procedure and cryopreserved in liquid nitrogen. From August 2000 to September 2013, 47 patients planned to receive HSCT, underwent ovarian tissue cryopreservation. The median age at diagnosis was 11.1 years and at the time of procedure it was 13 years, respectively. Twenty-four patients were not pubertal at the time of storage, whereas 23 patients had already experienced menarche. The median time between laparoscopy and HSCT was 25 days. Twenty-six out of 28 evaluable patients (93%) developed hypergonadotropic hypogonadism at a median time of 23.3 months after HSCT. One patient required autologous orthotopic transplantation that resulted in one live birth. Results show a very high rate of iatrogenic hypergonadotropic hypogonadism, highlighting the need for fertility preservation in these patients.

Here, we report a patient with Niemann-Pick disease type B, with early severe onset of disease and pulmonary involvement, treated with hematopoietic stem cell transplant (HSCT) from a bone marrow matched unrelated donor. We confirm that HSCT is feasible and potentially beneficial for patients with severe phenotype. Noteworthy, we discussed the potential usefulness of the activity of peripheral chitotriosidase for the longitudinal evaluation of HSCT success and effectiveness.

Allogeneic hematopoietic cell transplant (HCT) remains the only curative therapy for many hematologic malignancies but it is limited by high nonrelapse mortality (NRM), primarily from unpredictable control of graft-versus-host disease (GVHD). Recently, post-transplant cyclophosphamide demonstrated improved GVHD control in allogeneic bone marrow HCT. Here we explore cyclophosphamide in allogeneic peripheral blood stem cell transplantation (alloPBSCT). Patients with high-risk hematologic malignancies received alloPBSCT from HLA-matched unrelated/related donors. GVHD prophylaxis included combination post-HCT cyclophosphamide 50 mg/kg (days +3 and +4) and tacrolimus/mofetil mycophenolate (T/MMF) (day +5 forward). The primary objective was the cumulative incidence of acute and chronic GVHD. Between March 2011 and May 2015, 35 consecutive patients received the proposed regimen. MMF was stopped in all patients at day +28; the median discontinuation of tacrolimus was day +113. Acute and chronic GVHD cumulative incidences were 17% and 7%, respectively, with no grade IV GVHD events, only 2 patients requiring chronic GVHD immunosuppression control, and no deaths from GVHD. Two-year NRM, overall survival, event-free survival, and chronic GVHD event-free survival rates were 3%, 77%, 54%, and 49%, respectively. The graft-versus-tumor effect was maintained as 5 of 15 patients (33%) who received HCT with evidence of disease experienced further disease response. A post-transplant cyclophosphamide + T/MMF combination strategy effectively prevented acute and chronic GVHD after alloPBSCT from HLA-matched donors and achieved an unprecedented low NRM without losing efficacy in disease control or impaired development of the graft-versus-tumor effect. We present the results of structural and magnetic characterization of Permalloy (Fe20Ni80) films deposited by sputtering on self-organized nanoporous alumina previously treated with phosphoric acid to vary the pore diameters. SEM and AFM images of the top film surfaces show a triangular array of pores with diameters similar to the untreated porous alumina. However, the underlying pore enlargement is evidenced by the magnetic study. Indeed magnetization measurements reveal a decrease in the easy-plane anisotropy energy with elapsing time of chemical etching. Consistent with these results, ferromagnetic resonance measurements perpendicular to the film indicate a systematic reduction of the resonance field which can be directly related to an increase in the pore diameter. The effect of lateral confinement (due to the pores) in the spin-wave resonance is evidenced in multiple absorption lines when the applied field is in the film plane and perpendicular to it. This contrasts with the results for the continuous test film.

This paper discusses a new implementation of a Gaussian minimum-shift keying (GMSK) modulator and demodulator on the European Space Agency (ESA)'s common deep-space receiver-the Intermediate Frequency Modem System (IFMS), which is a software radio based platform. The GMSK demodulator is needed for ESA's deep-space and near-Earth missions, starting with the Herschel-Planck satellites in 2008. The implementation requirements and hardware restrictions from the IFMS lead to the need for a significant simplification versus the optimum demodulation approach. In part, this can be achieved by using a demodulator based on the Laurent decomposition, yet further simplifications and changes to obtain a feasible implementation were necessary. The presented GMSK demodulator was directly implemented on the existing IFMS receiver without requiring any hardware changes. Measurements with the demodulator showed only a marginal technical degradation in the order of 0.1-0.3 dB for the chosen approach. Furthermore, for testing purposes, a GMSK modulator was implemented on the same platform.

The European Space Agency (ESA) is today autonomously flying three interplanetary missions: Rosetta traveling to the comet Churyumov-Gerasimenko and the two orbiters Mars Express and Venus Express. The capability of supporting these and future deep-space missions is the consequence of a farsighted decision taken in 1996 to expand the ESA network of 15-m tracking antennas into the deep-space domain. The ambitious plan to provide around-the-clock coverage to all ESA interplanetary missions is almost completed: two deep-space antennas, located in New Norcia (Australia) and Cebreros (Spain), have been in operation since 2002 and 2005, respectively, while a third antenna is planned for 2011. This paper presents the two existing antennas starting from the underlying system requirements originated from the Rosetta mission, which was the most demanding in terms of required performance. The selected architecture is then described, followed by a detailed discussion about the critical performances that play a major role in deep-space support and the associated design issues.

We present magnetic resonance results obtained at L, X, and Q bands (1.2, 9.4 and 34 GHz, respectively) on ferromagnetic nanowires with a hysteresis cycle characterized by a remanent magnetization M r / M s ∼ 0.92 and a coercive field H c = 1.0 kOe . The hysteretic response of the ferromagnetic resonance spectra is discussed in terms of independent contributions of the nanowires aligned along and opposite to the applied field. We will discuss the implications of this study on the magnetic resonance in nanoparticles and other systems with large anisotropy.

Space agencies are currently considering replacing conventional ranging systems with regenerative schemes for future demanding interplanetary missions. The Consultative Committee for Space Data Systems (CCSDS) in charge of standardization among Space Agencies is studying both Pseudo-Noise (PN) and Pseudo-Noise-Like (PNL) sequences and intends to generate a recommended standard (Blue Book) by 2007. The present paper treats the PNL schemes under consideration by CCSDS, with emphasis on the scrambled weighted-voting Stiffler PNL ranging-sequence scheme, which is proposed herein and which appears to be superior to all other PN and PNL ranging schemes. Hematopoietic stem cell transplantation is a therapeutic strategy for several oncohematological diseases. It increases survival rates but leads to a high incidence of related effects. The objective of this paper was to examine the existing literature on physical exercise interventions among pediatric HSCT recipients to explore the most often utilized rehabilitative assessment and treatment tools. Studies published from 2002 to April 1, 2015 were selected: 10 studies were included. A previous literary review has shown that rehabilitation programs have a positive impact on quality of life. Our analysis identified some significant outcome variables and shared intervention areas.

Our study was aimed to evaluate the feasibility and safety of intraspinal cord implantation of autologous mesenchymal stem cells (MSCs) in a few well-monitored amyotrophic lateral sclerosis (ALS) patients.Seven patients affected by definite ALS were enrolled in the study and two patients were treated for compassionate use and monitored for at least 3 years. Bone marrow was collected from the posterior iliac crest according to the standard procedure and MSCs were expanded ex vivo according to Pittenger's protocol. The cells were suspended in 2 ml autologous cerebrospinal fluid and transplanted into the spinal cord by a micrometric pump injector.The in vitro expanded MSCs did not show any bacterial o fungal contamination, hemopoietic cell contamination, chromosomic alterations and early cellular senescence. No patient manifested major adverse events such as respiratory failure or death. Minor adverse events were intercostal pain irradiation and leg sensory dysesthesia, both reversible after a mean period of 6 weeks. No modification of the spinal cord volume or other signs of abnormal cell proliferation were observed. A significant slowing down of the linear decline of the forced vital capacity was evident in four patients 36 months after MSCs transplantation.Our results demonstrate that direct injection of autologous expanded MSCs into the spinal cord of ALS patients is safe, with no significant acute or late toxicity, and well tolerated. The clinical results seem to be encouraging.

The impact of GH deficiency and rhGH replacement therapy on IGF-I, IGFBP-3 and ALS levels has been widely studied. There is less information available on IGF-II levels, the component of the ternary complex poorly dependent on GH. We investigate the components of IGFs system in 36 GHD adults (28M, 8F, age 45 +/- 14 yrs) before and after 12 months of rhGH therapy (mean dose 0.3 +/- 0.1 mg/day). One-hundred healthy sex- and age-matched subjects were studied for comparison. At baseline, GHD patients showed IGF-I and IGF-II levels and IGFs to IGFBP-3 molar ratios that were lower than controls. During therapy, IGF-I levels increased (p < 0.01) to normal range. IGF-II levels, though higher than at baseline (p < 0.01), remained lower than in controls (p < 0.01). ALS and IGFBP-3 significantly increased (p < 0.001). These modifications resulted in normalization in IGF-I to IGFBP-3 ratio, while no change in IGF-II to IGFBP-3 ratio was observed. In conclusion, the increase of serum IGF-II levels during rhGH treatment in GHD patients probably reflects the increase in the other components of ternary complex (ALS and IGFBP-3). However, serum IGF-II levels as well as IGF-II to IGFBP-3 ratio, although increased, were definitely lower than in controls. This last result, given the increasing evidences of a direct implication of IGF-II in cancer, may further confirm the safety of rhGH replacement in adults with severe GHD as diagnosed by appropriate stimulation tests.

Present study reports on magnetostatic interactions in highly-ordered arrays of Ni nanowires embedded in nanoporous alumina membranes. We use two techniques supplying complementary information: ferromagnetic resonance, FMR, studies from which we derive information of the whole array of nanowires, and magnetic force microscopy, MFM, that informs us about the magnetic state of individual nanowires. From FMR study of the angular dependence of resonance field and itsline-width it is concluded that the magnetostatic interaction plays an important role to decrease the effective anisotropy field of individual nanowires. This is confirmed by analysis of MFM images at remanence and its comparison with vibrating sample magnetometer measurements.

BACKGROUND Outcome data were analyzed for 27 patients who were affected with recurrent or newly diagnosed high-risk brain tumors and who underwent high-dose chemotherapy with triethylenethiophosphoramide (thiotepa) and etoposide in addition to autologous stem cell transplantation between May 1992 and September 2002. METHODS Fifteen males and 12 females (median age, 11 years) were included in the study. Twelve patients had newly diagnosed high-risk brain tumors, and 15 patients had recurrent brain tumors. The conditioning regimen consisted of thiotepa 900 mg/m2 and etoposide 1500 mg/m2 over 3 days starting on Day −5. Stem cell rescue was performed using bone marrow (BM) in 8 patients, peripheral blood stem cells (PBSCs) in 18 patients, and BM and PBSCs in 1 patient. RESULTS For the BM group, neutrophil (PMN) engraftment was achieved on Day +14 (median value), whereas platelet (PLT) engraftment was achieved on Day +68 (median value). One patient did not achieve PLT engraftment. For the PBSC group, the PMN engraftment was achieved on Day +10.0 (median value), and the PLT engraftment was achieved on Day +15.5 (median value). Transplantation-related toxicity (evaluated using the Bearman score) included Grade 2–3 mucositis in 16 patients, Grade 1 kidney toxicity in 6 patients, Grade 1 liver toxicity in 6 patients, and Grade 2 liver toxicity in 1 patient. Transplantation-related mortality was observed in 1 patient (3.6%), who died of Candida pneumonia. The 3-year overall survival (OS) rate was 44.6%, and the 3-year event-free survival (EFS) rate was 31%. There was a statistically significant difference in OS and EFS rates for patients who underwent ASCT and achieved complete remission compared with patients who had measurable disease. CONCLUSIONS The results of the current study suggest that high-dose chemotherapy followed by ASCT may be beneficial for patients who achieve complete remission before ASCT, whereas for other patients, new strategies are required.
