Osteoporosis is characterized by skeletal degeneration with low bone mass and destruction of microarchitecture of bone tissue which is attributed to various factors including inflammation. Women are more likely to develop osteoporosis than men due to reduction in estrogen during menopause which leads to decline in bone-formation and increase in bone-resorption activity. Estrogen is able to suppress production of proinflammatory cytokines such as IL-1, IL-6, IL-7, and TNF-α. This is why these cytokines are elevated in postmenopausal women. Studies have shown that estrogen reduction is able to stimulate focal inflammation in bone. Labisia pumila (LP) which is known to exert phytoestrogenic effect can be used as an alternative to ERT which can produce positive effects on bone without causing side effects. LP contains antioxidant as well as exerting anti-inflammatory effect which can act as free radical scavenger, thus inhibiting TNF-α production and COX-2 expression which leads to decline in RANKL expression, resulting in reduction in osteoclast activity which consequently reduces bone loss. Hence, it is the phytoestrogenic, anti-inflammatory, and antioxidative properties that make LP an effective agent against osteoporosis.

Background: COVID-19 pandemic has tremendously caused significant health impacts leading to high load of ICU critical patients. Multiple Intensive Care Unit (ICU) admission framework and triaging score have been adapted worldwide for admission of COVID-19 patients. The aim of this article is to review the COVID-19 ICU admission policy in five (5) countries, identify challenges and provide recommendations. Materials and Methods: Literature review was carried out through searching available literature from online databases such as EBSCO, PubMed, Research Gate, Google Scholar and Google search engine. The selected information used in this review is taken from any journal, article, report, thesis, national report, news update regarding the topic. Result: The ICU admission policy in 5 countries, namely South Africa, USA, UK, Hong Kong and Malaysia were reviewed. Different countries apply different guidelines to triage, prioritize and admit severe COVID-19 patients to ICU such as Color-coded Priority Scale in South Africa, Critical Care Triage Tool in USA, COVID-19 Critical Care Guideline in UK, COVID- 19 Critical Care Triage Tool in Hong Kong and ICU Admission Prioritization Criteria in Malaysia. The ICU admission decision-making is supported with multiple ICU scorings such as Sequential Organ Failure Assessment (SOFA), Clinical Frailty score and National Early Warning score 2 (NEWS2). Conclusion: The COVID-19 ICU admission policies in these countries are well established and have been practised appropriately despite challenges faced by the ICU practitioners. All five selected national guidelines in this paper correspond to the WHO framework on the ICU admission policy. Ethical issues and legal supports on the ICU practitioners in ICU admission decision-making requires great attention form government and relevant policy stakeholders. [ABSTRACT FROM AUTHOR]

Materials and Methods : Retrospective review of a HIPAA-compliant institutional review-board approved database identified 40 patients with chemo-refractory colorectal liver metastases treated with radioembolization who underwent tumor genotyping for hotspot mutations in 6 key genes in the MAPK/PI3K pathways (KRAS, NRAS, BRAF, MEK1, PIK3CA, and AKT1). Mutation status as well as clinical, tumor, and treatment variables were recorded. These factors were evaluated in relation to time to local progression (TTLP), which was calculated from time of radioembolization to first radiographic evidence of local progression. Predictors of outcome were identified using a proportional hazards model for both univariate and multivariate analysis with death as a competing risk. Results : Sixteen patients (40%) had no mutations in either pathway, eighteen patients (45%) had mutations in the MAPK pathway, ten patients (25%) had mutations in the PI3K pathway and four patients (10%) had mutations in both pathways. The cumulative incidence of progression at 6 and 12 months was 33% and 55% for the PI3K mutated group compared with 76% and 92% in the PI3K wild type group. Mutation in the PI3K pathway was a significant predictor of longer TTLP in both univariate (p=0.031, sHR 0.31, 95% CI: 0.11-0.90) and multivariate (p=0.015, sHR=0.27, 95% CI: 0.096-0.77) analysis. MAPK pathway alterations were not associated with TTLP. Conclusions : PI3K pathway mutation predicts longer time to local progression after radioembolization of colorectal liver metastases.

Le necessità di una revisione delle circoscrizioni giudiziarie è da tempo un'esigenza concreta . I cambiamenti radicali intervenuti nel contesto socio-economico in Italia negli ultimi centocinquant'anni, da quando cioè è stata impostata la geografia giudiziaria tuttora vigente1, hanno reso questo impianto anacronistico, inadeguato e certamente troppo costoso (Carnevali 2014a). Il quadro si è poi aggravato a seguito della crisi economico-finanziaria, che ha influenzato le politiche di riforma dei principali settori di policy, compresa la giustizia, nella direzione della razionalizzazione organizzativa e territoriale, ai fini del contenimento della spesa. In questo capitolo, si presenterà l'intero percorso a partire dalla fase di formulazione della policy, con particolare attenzione a come la issue è stata tematizzata e alle caratteristiche del processo di policy design. Successivamente, sarà illustrata l'implementazione della politica e i suoi effetti. In particolare, saranno messi in evidenza gli obiettivi dichiarati e quanto realmente raggiunto, evidenziando i criteri utilizzati nel processo di riforma. Inoltre, saranno indicati i risultati e le prospettive di sviluppo. Nelle considerazioni conclusive verranno precisate le criticità e le opportunità che questa policy ha messo in evidenza nell'ambito delle politiche del servizio giustizia.

8. La sarcoïdose cutanée en Tunisie : à propos d’une série hospitalière de 75 cas

Introduction La sarcoidose est une granulomatose systemique chronique benigne d’etiologie indeterminee dont les manifestations cutanees peuvent apparaitre a tous les stades de la maladie. L’objectif de ce travail etait d’etudier les manifestations cutanees de cette maladie en Tunisie. Patients et methodes Il s’agissait d’une etude descriptive, retrospective, colligeant 75 cas de sarcoidose cutanee diagnostiques entre 2001 et 2014 a l’hopital Charles Nicolle de Tunis. Nous avons inclus les patients dont le diagnostic de sarcoidose a ete confirme sur l’histologie associee a une confrontation anatomo-clinique. Resultats L’âge moyen au moment du diagnostic etait 48,7 ans [4–96 ans]. Le delai moyen de consultation etait de 18 mois [1 mois–10 ans]. Le sex-ratio (F/H) etait de 4,5. Les manifestations cutanees etaient a type de petits sarcoides dans 21 cas (28 %), gros sarcoides dans 6 cas (8 %), modification de cicatrices anciennes dans 4 cas (5 %), nodules dermohypodermiques dans 4 cas (5 %), angiolupoides dans 4 cas (5 %), lupus pernio dans 13 cas (17 %), macrocheilite dans 5 cas (6,5 %), plaques annulaires dans 5 cas (6,5 %), plaques psoriasiformes dans 4 cas (5 %). Une atteinte du cuir chevelu etait notee dans 6 cas (8 %) dont 4 a type d’alopecie cicatricielle. L’erytheme noueux a ete revelateur dans uniquement 5 cas (6,5 %). Les manifestations cutanees etaient inaugurales dans 63 cas (85 %). L’intradermoreaction a la tuberculine etait negative dans 48 cas sur 50 (96 %), le dosage de l’enzyme de conversion de l’angiotensine etait augmente dans 11 cas sur 19 (57 %). Le bilan de systematisation montrait une atteinte pulmonaire dans 23 cas (32,8 %), une atteinte ORL dans 6 cas (4 cas d’atteinte nasale et un cas d’atteinte jugale), une atteinte ophtalmologique (sequelle d’uveite anterieure) dans 4 cas, une atteinte hepatique dans 6 cas : cytolyse (3 cas) et cholestase anicterique (3 cas), renale (nephropathie interstitielle) dans 3 cas. Les modalites therapeutiques ont inclus les dermocorticoides, l’allopurinol, les antipaludeens de synthese et la corticotherapie generale en cas d’atteinte systemique severe. L’evolution a ete marquee par une amelioration partielle et la recidive etait la regle. Discussion La sarcoidose est une granulomatose cutaneo-systemique. Elle survient habituellement chez les sujets entre 20 et 40 ans avec une predominance feminine (84 % des cas dans notre serie). Les manifestations cutanees les plus rapportees dans la litterature sont les sarcoides a petits et a gros nodules le lupus pernio et l’erytheme noueux qui ont ete note aussi dans notre serie. Par ailleurs, l’atteinte de cuir chevelu a ete rarement rapportee dans la litterature. Dans notre serie nous avons retrouve 6 cas d’atteinte de cuir chevelu confirmes par l’histologie. Conclusion Les manifestations cutanees de la sarcoidose sont polymorphes peuvent etre revelatrices. Il est donc important de les connaitre et de les biopsier au moindre doute.

10. Catalytic Dewaxing for Lube Oil Production

The selective cracking of long-chain normal paraffin's of medium neutral raffinate, derived from a lube oil-phenol extraction unit, by the catalytic dewaxing technique over H-ZSM-5 and NiMo-H-mordenite catalysts was studied. The runs were conducted to produce lube oils with acceptable cold flow properties. The influences of zeolite types, metals loading, and operating reactor temperatures (290°C–475°C) can have a great effect on cracking high pour point n-paraffins into lower ones, and hence a reduction in pour points. An increase in temperature (between 290°C and 375°C) increased wax conversion (percent dewaxing) on H-ZSM-5 compared with NiMo-H-mordenite catalysts due to its higher cracking activity. As a result, large amounts of C1-C4 gases and C5-170°C naphtha were produced. The low pour point lube oils produced from catalytic dewaxing over H-ZSM-5 compared with NiMo-H-mordenite catalyst indicates that the former was more selective for removing wax components than the later. 12. La maladie dermatophytique : deux nouvelles observations

Introduction La maladie dermatophytique (MD) est une forme rare d’infection fongique generalisee et chronique. Elle affecte la peau et les phaneres associant parfois des localisations secondaires dermohypodermiques, ganglionnaires voire viscerales. Elle est essentiellement decrite aux pays du Maghreb ou elle est liee a une forte consanguinite. Observation Il s’agit de deux sœurs ZA et ZM âgees respectivement de 18 ans et 7 ans, issues d’un mariage consanguin de 1 er degre. L’ainee ZA est suivie depuis l’âge de 8 ans pour une dermatophytose generalisee. Le debut de la symptomatologie etait marque par l’apparition d’une dermatophytie circinee du visage rapidement etendue au reste du corps. L’examen physique montrait des plaques erythemato-squameuses au niveau du tronc, du dos, du visage, du cou et des membres et s’etendant a tout le cuir chevelu avec rarefaction des cheveux. Un onyxis des doigts et des orteils est associe a une keratodermie plantaire. Des adenopathies cervicales bilaterales ont ete retrouvees. L’examen mycologique isolait Trichophyton Rubrum au niveau de tous les sites preleves. La biopsie ganglionnaire montrait une adenite chronique reactionnelle. L’exploration de l’immunite aussi bien humorale que cellulaire etait normale. Un traitement sequentiel alternant griseofulvine et terbinafine etait instaure. Une recidive etait notee a chaque tentative d’arret du traitement. Pour ZM, le debut de la symptomatologie remontait a l’âge de 2 ans marque par l’installation de dermatophytie etendue du visage, du tronc, et des membres, associee a une teigne du cuir chevelu et un onyxis des doigts et des orteils. Des adenopathies cervicales bilaterales etaient retrouvees. Les prelevements mycologiques permettaient d’isoler un T. Rubrum au niveau de la peau, du cuir chevelu et des ongles. L’exploration de l’immunite humorale et cellulaire etait egalement sans anomalie. La patiente a ete mise sous griseofulvine au long cours permettant de controler la maladie. Discussion Nos observations resument les principales caracteristiques de la MD : l’importance des formes familiales, un debut pediatrique, avec frequence des teignes et l’importance de l’atteinte profonde essentiellement ganglionnaire. Le diagnostic peut etre precocement evoque devant la decouverte d’une teigne a T. Rubrum ou d’un onyxis dermatophytique pediatrique, et devant la recidive frequente a l’arret du traitement. En l’absence d’une prise en charge precoce les lesions peuvent gagner les organes profonds, avec notamment une atteinte osseuse ou cerebrale. Dans ces cas, la survie est serieusement compromise. En Tunisie, la MD atteint plus de filles que de garcons. Une mutation homozygote du gene CARD9 a ete identifiee pouvant expliquer une reponse immunitaire defectueuse contre les dermatophytes. Les differents traitements antifongiques utilise n’ont permis qu’un controle partiel de la maladie. Conclusion La MD est une maladie grave pouvant engager le pronostic vital. L’etude genetique de cette affection ouvre de nouvelles possibilites diagnostiques et probablement therapeutiques ainsi qu’un conseil genetique.

Hunter (2006) states that the image of a shopping centre is one of the factors that may increase the frequency of visits. In addition, the anticipated emotion that the customers experience during shopping is also a variable that influences the frequency of visits. The customers that have emotional relationship with a shopping centre have a higher desire to visit the related shopping centre. The customers’ higher desire to visit the shopping centre will influence the increasing intention to visit a shopping centre and will positively contribute to the frequency (the number) of customers’ visits to a shopping centre. The research was conducted in Yogyakarta, namely to the customers who visited Plaza Ambarrukmo Yogyakarta. The objectives that the researcher would like to investigate were the role of anticipated emotion in mediating the influence of shopping centre image to the desire of visiting the shopping centre, the influence of a desire to visit a shopping centre to the intention of visiting a shopping centre, the influence of an intention to visit a shopping centre to the frequency of visiting a shopping centre, and the difference of customers’ evaluation toward the shopping centre’s image, the anticipated emotion, the intention to visit a shopping centre and the frequency of visiting a shopping centre between men and women. The results of the research conducted to the customers of Plaza Ambarrukmo Yogyakarta Shopping Centre provides a hard evidence that the shopping centre’s image, the anticipated emotion, the desire of visiting a shopping centre and the intention of visiting a shopping centre influence positively and significantly the increase of customers’ visits to Plaza Ambarrukmo Yogyakarta. Anticipated emotion is a mediation variable from the relationship between the shopping centre image and the intention of visiting a shopping centre.

Studies have shown that estrogen reduction is able to stimulate focal inflammation in bone.Labisia pumila(LP) which is known to exert phytoestrogenic effect can be used as an alternative to ERT which can produce positive effects on bone without causing side effects. LP contains antioxidant as well as exerting anti-inflammatory effect which can act as free radical scavenger, thus inhibiting TNF-α production and COX-2 expression which leads to decline in RANKL expression, resulting in reduction in osteoclast activity which consequently reduces bone loss. Hence, it is the phytoestrogenic, anti-inflammatory, and antioxidative properties that make LP an effective agent against osteoporosis.

17. Mathematical analysis of stochastic epidemic model of MERS-corona & application of ergodic theory.

- Abstract
The "Middle East Respiratory" (MERS-Cov) is among the world's dangerous diseases that still exist. Presently it is a threat to Arab countries, but it is a horrible prediction that it may propagate like COVID-19. In this article, a stochastic version of the epidemic model, MERS-Cov, is presented. Initially, a mathematical form is given to the dynamics of the disease while incorporating some unpredictable factors. The study of the underlying model shows the existence of positive global solution. Formulating appropriate Lyapunov functionals, the paper will also explore parametric conditions which will lead to the extinction of the disease from a community. Moreover, to reveal that the infection will persist, ergodic stationary distribution will be carried out. It will also be shown that a threshold quantity exists, which will determine some essential parameters for exploring other dynamical aspects of the main model. With the addition of some examples, the underlying stochastic model of MERS-Cov will be studied graphically for more illustration., (© 2022 International Association for Mathematics and Computers in Simulation (IMACS). - Abstract
Lunasin is a novel promising health-beneficial peptide derived from soybean. However, the application of lunasin is limited by its high cost. In this study, we developed a successful protocol for expression of a dimer formation protein containing 4 tandem repeated lunasin analogs (lunasin-4) in Pichia pastoris. The expression level at the optimal condition (initial pH 7.0, 1.0% final methanol concentration and induction for 72 h at 26 °C) was 0.24 mg/mL cell-free broth. Lunasin analog, obtained from purified lunasin-4 protein through enterokinase digestion and ultrafiltration, significantly decreased (p < 0.05) the release of nitric oxide (NO), tumor necrosis factor-α (TNF-α) and interleukin-6 (IL-6) in lipopolysaccharide (LPS)-stimulated RAW264.7 macrophages in a dose-dependent manner. In addition, intracellular signaling array analysis demonstrated down-regulated levels of phosphorylated Akt, mechanistic target of rapamycin (mTOR) and p70 s6 kinase (p70s6k) and an up-regulated level of glycogen synthase kinase-3β (GSK-3β) after lunasin analog treatment. These results suggest that lunasin analog exerted anti-inflammatory activities in LPS-stimulated RAW264.7 cells partly via inhibiting the activation of Akt/mTOR/p70s6k signaling pathway. In conclusion, this study provides a potential strategy for recombinant production of bioactive lunasin in industry., (Copyright © 2018 The Society for Biotechnology, Japan. - Abstract
Embolisation of head and neck hypervascular tumours and arterioveinous malformations (AVMs) is now a well-established therapeutic procedure. The purpose of this study was to analyse the technique and to evaluate the safety and value of preoperative embolization of hypervascular lesions. We retrospectively reviewed the records of 5 patients, agged between 14 and 47 years, with hypervascular tumours and AVMs in the head and neck. Lesions were distributed as follow: nasal angiofibroma (n = I), nasal angioleiomyoma (n = I), nasopharyngeal angiofibroma (n = I), aneurysmal bone cyst in the posterior element of the second cervical vertebra (n = I) and AVM of the inferior lip (n = I). Angiography and embolization were carried out at the same time. Complete exclusion of lesions was obtained without any complications. Complementary surgical resection was performed with not notable blood loss.
