Objective: to study the microbiota of the vagina, myomatous nodes, their bed in patients with fibroids. Materials and methods: a comprehensive examination of 83 patients aged 26 to 50 years with diagnosed interstitial, interstitial-subserous uterine myoma was carried out. By age, all women are divided into 3 groups. Standard clinical and clinical laboratory studies, colposcopy, microbiological examination of the detachable posterior vaginal fornix, biopsy specimens of myomatous nodes and their bed, as well as morphological examination of biopsy specimens of myomatous nodes and their bed were performed.Results: in the structure of clinical manifestations of uterine fibroids, pain syndrome was the leading one (p0.05). According to the results of a bacteriological study of the detachable posterior vaginal fornix in patients in the 2nd group, a decrease in the frequency of detection and the number of lactobacilli was revealed compared to the 1st group, and in women in the 3rd group, these microorganisms were absent. Among the anaerobic microorganisms in the 1st and 2nd groups, Eubacterium spp. dominated, in the 3rd — Peptostreptococcus spp. In patients of group 3, the frequency of detection of Bacteroides spp. was significantly increased (pConclusions: In women with uterine myoma of different age groups, multidirectional changes in the vaginal microbiota were revealed. In most cases, the myomatous node (92.8%) and its bed (82.3%) are not sterile with the dominance of anaerobic microbiota taxa. Identified significant correlations in the loci «vagina – myomatous node – myomatous node bed» indicate their relationship. The detection of various taxa of microorganisms in the myomatous node and its bed, according to morphological studies, is not associated with the presence of infectious and inflammatory processes in the tissues.

Helicobacter pylori is the most common human infection. H. pylori is known to cause chronic gastritis and gastric and duodenal ulcers. However, the H. pylori-host interaction is not confined to the gastric mucosa, the mechanism responsible for the adaptation of this bacterium to the living conditions in the human body, can cause systemic immune imbalance, provoking various autoimmune reactions. The paper considers the investigations demonstrating the possible role of H. pylori as a trigger or aggravating factor in rheumatic diseases, such as rheumatoid arthritis (RA), Sjogren's syndrome and disease, systemic lupus erythematosus, and progressive systemic sclerosis. The relationship between H. pylori and autoimmune diseases is confirmed by the higher frequency of more aggressive strains of this microorganism in rheumatic diseases. There are also clinical trials showing the positive impact of H. pylori eradication on RA activity.

Musculoskeletal diseases (MSDs) are a topical medical, social, and economic problem. According to the 2015 data, MSDs in the structure of primary morbidity were 3.9% and those in the structure of mortality were 0.2% in the Russian Federation. Official mortality statistics is formed only by one (underlying) cause of death. The study of multiple causes of death will enable the understanding of its mechanisms, the necessity of making changes in the management tactics for patients and in the standards of medical care, as well as the possibility of correcting the mortality reduction plans. Objective: to investigate the reliability of statistics on MSD mortality and the ways of its reduction, by analyzing the underlying and multiple causes of death. Materials and methods. This investigation used a database on medical death certificates of the residents of the Tula Region in 2015. Mortality rates were calculated by conventional statistical methods. Death rates for the Russian Federation and the Tula Region were taken from the book «Medico-demographic indicators of the Russian Federation in 2015» by the Ministry of Health of Russia and C52 tables by the Federal State Statistics Service. The 2012–2014 European mortality databases were used for international comparisons. Results and discussion. The mortality rates were analyzed using the underlying and multiple causes of death. Expert evaluation showed that the 2015 mortality rates from MSDs were underestimated by approximately 11%. The MSD mortality rates in the Tula Region were 2.4 and 2.7 times higher than those in the entire Russian Federation and in Europe, respectively. Defects were found in drawing up medical death certificates. The reliability of mortality statistics was shown to be related to monitoring, physician training, and introduction of an automated system. Emphasis is laid on the correction of treatment tactics for patients with MSD.

In the semiotic tradition text is considered a sign with its own content. This content is shaped by three meanings within three spaces of sign: semantic, syntactic and pragmatic. It is crucial that text is heterogeneous from the point of view of meaning organization. Three spaces or three spheres of experience integrated within text - existential, rational and communicative - focus upon themselves the narrative, typological and paralogical meanings of text. These meanings constitute the true 'pattern' of text. The world of text is the one created, arranged and thought over in great details. The first layer is the level of the plot of existence. And since text is, by definition, an intertextual determinacy, in communication this level of meaning acts as the initial, or zero, meaning in the process of understanding text. However, understanding content only begins at this point, con¬tinuing through the typological level and, further on, through interpretational practices, finally reaching the paralogical subtleties of understanding. Text is a reality oriented at being understood. And it is the very struc¬ture of meaning of text that shapes the technologies of understanding. One can and must be taught to understand. The paper addresses the concept of zero meaning as the initial, existential layer of meanings that form the subjectness of text.

This work presents the geological structure of the Neogen uranium-bearing ore sedimentary strata that filled the paleovalleys of the Koretkonde deposit. As a result of work we carried out, productive deposits were stratigraphically divided, the patterns of the localization of uranium mineralization were studied, and lithological and mineralogical ore-controlling factors were revealed. The main mineral phases of uranium were identified with scanning-electron microscopy and microprobe studies. In the future, a more detailed study of the deposits is planned in order to reveal methods for improving the efficiency of their exploration and organize the search for uranium deposits of such a type within the Vitim uranium-bearing ore region to expand the mineral resource base of Russia.
