Special educators must be prepared to meet the diverse needs of their students, yet they have few opportunities to collaborate with others in similar roles or receive targeted professional development. Therefore, practices that intentionally sustain special educators are needed. Such experiences have existed in the professional development world through learning communities and case-based learning. The core components of both models are featured in a relatively new method for supporting practitioners. In this article, the authors provide an overview of the ECHO (Extension for Community Healthcare Outcomes) Model® as a mechanism to support special educators. The ECHO Model® addresses many of the shortcomings in professional development and the special education workforce (i.e., isolated teachers with limited opportunities to collaborate) by strategically building a community of practice.

Mixed-reality simulation is a technology-based tool used to augment preservice teacher education in special education; as such, researchers have conducted several studies to assess mixed-reality simulation for its effectiveness and efficacy. Though these researchers have concluded that mixed-reality simulation can be an effective tool, there is limited information regarding a synthesis of the study characteristics (e.g., participants' demographic information, amount of time, skills taught using simulation, and participants' perceptions). Understanding these components is critical to the continued success and growth of using mixed-reality simulation in special education coursework. We identified and synthesized eight mixed-reality simulation studies with a total of 256 participants from 2005 to 2021. Together, the eight studies suggest that mixed-reality simulation is a valuable practice-based experience for preservice special educators. Implications for practice and future research are discussed.

Abstract Introduction Typically, oncology is not a structured part of the curriculum in Brazilian medical schools. Furthermore, sarcomas, which are uncommon tumors, are seldom covered in depth. A lack of comprehensive education on sarcomas might result in medical professionals being ill-equipped to care for patients with this condition. Objectives To assess medical students' understanding and awareness of sarcomas and the specific principles related to these tumors. Materials and methods A quantitative, cross-sectional study was conducted using a questionnaire, applied to medical students, focusing on the epidemiology, pathophysiology, and treatments of bone and soft tissue sarcomas. In all tests, the significance level adopted was 5%. The SPSS version 25.0 software was used. Results Of the 825 questionnaires distributed, 325 were returned. Educational sessions on sarcomas did not appear to significantly improve the student's knowledge. Only 29.5% of students identified the lack of pain as an indicator of potential malignancy in soft tissue sarcomas, while 73.8% correctly recognized pain as a symptom of bone sarcomas. Limb amputation as the optimal surgical method for patient recovery was incorrectly reported by 39.1% of the sample. Conclusion A great part of the surveyed population does not have adequate knowledge about the basic concepts associated with limb sarcomas. The minority of them are satisfied with the knowledge gained during their medical education about these tumors. Inadequate medical academic training may initially lead to the wrong clinical management of patients with bone and soft tissue tumor lesions. An educational effort is needed to enhance oncology education for medical students, especially concerning sarcomas.

Abstract Compared to most ATP-site kinase inhibitors, small molecules that target an allosteric pocket have the potential for improved selectivity due to the often observed lower structural similarity at these distal sites. Despite their promise, relatively few examples of structurally confirmed, high-affinity allosteric kinase inhibitors exist. Cyclin-dependent kinase 2 (CDK2) is a target for many therapeutic indications, including non-hormonal contraception. However, an inhibitor against this kinase with exquisite selectivity has not reached the market because of the structural similarity between CDKs. In this paper, we describe the development and mechanism of action of type III inhibitors that bind CDK2 with nanomolar affinity. Notably, these anthranilic acid inhibitors exhibit a strong negative cooperative relationship with cyclin binding, which remains an underexplored mechanism for CDK2 inhibition. Furthermore, the binding profile of these compounds in both biophysical and cellular assays demonstrate the promise of this series for further development into a therapeutic selective for CDK2 over highly similar kinases like CDK1. The potential of these inhibitors as contraceptive agents is seen by incubation with spermatocyte chromosome spreads from mouse testicular explants, where they recapitulate Cdk2 -/- and Spdya -/- phenotypes.

Assessing the linkage between breeding and non-breeding areas has important implications for understanding the fundamental biology of and conserving animal species. This is a challenging task for marine species, and in sea turtles a combination of stable isotope analysis (SIA) and satellite telemetry has been increasingly used. The Northwest Atlantic (NWA) loggerhead (Caretta caretta) Regional Management Unit, one of the largest sea turtle populations in the world, provides an excellent opportunity to investigate key biological patterns as well as methodological aspects related to the use of stable isotopes to infer spatial distribution of turtles in foraging areas. We provide the first comprehensive assessment of the annual distribution of NWA adult female loggerheads among foraging areas and investigate the efficacy of various analytical approaches as well as the effect of sample size in these types of studies. A total of 5168 individual females were sampled from seven Management Units (MUs) between 2013-2018. We provide the first estimate of the proportion of females originating from each MU that uses each foraging area and show how this proportion varies over time. We also estimate the relative importance (in terms of number of turtles) of each foraging area to the overall loggerhead breeding aggregation nesting in Florida and in the NWA for each year of the study. The foraging area used by reproductively active females differs considerably across MUs. One of these, the Subtropical NWA, is by far the most important foraging area in terms of both number of individuals and genetic diversity, and therefore this region may be considered as a conservation priority. Through simulations, we show that limited sizes of sample groups (unknowns; training; priors) may result in false geographic differentiation and consequently mislead interpretations. We provide thresholds and methodological recommendations for future studies. This study establishes a fundamental baseline for monitoring the annual contribution of foraging area to a terrestrial-based breeding aggregation of a marine animal in a cost-effective way. This type of monitoring allows for early detection of changes in foraging distributions—a possible effect of climate change on marine ecosystems or of area-specific anthropogenic threats.

Purpose: This analysis investigates whether research productivity during medical school predicts future research productivity during radiation oncology residency. Methods and Materials: At our institution, there have been 20 medical students who graduated between 2005 and 2015 and subsequently completed their residency training in radiation oncology. We built a database of all PubMed-indexed publications in which these former students were the first author or coauthor. Mean publication rates with 95% confidence intervals (95% CI) were computed. The paired t test and McNemar-Bowker test of symmetry were used to examine differences in first-author and coauthor publications between the medical school and residency periods. An ordinal logistic regression model was employed to measure the odds ratio of publishing during residency versus during medical school. A Spearman correlation coefficient was calculated for the relationship between the number of publications during medical school and the number during residency. Results: A total of 14 and 60 first-author publications (46 and 117 coauthor publications) were identified for 20 individuals during medical school and residency, respectively. There was an average of 0.7 (95% CI, 0.17-1.23) first-author publications during medical school and 3.08 (95% CI, 1.56-4.44) first-author publications during residency (P = .003). Only 15% (3/20) had ≥2 publications during medical school, and 50% (10/20) had ≥2 publications during residency (P = .012). The Spearman correlation coefficient between research publications before and during residency was .457 (P = .043). The mean number of coauthor publications during medical school and residency was 2.3 (95% CI, 0.92-3.68) and 5.85 (95% CI, 3.50-8.20), respectively (P = .004). Conclusions: Based on this retrospective analysis from our institution, student research productivity during medical school, as defined by the number of first-author publications, does correlate with future research productivity during radiation oncology residency.

Abstract Background Clostridiodies difficile infection (CDI) has been characterized by the Center for Disease Control and Prevention (CDC) as an urgent public health threat and a major concern in hospital, outpatient and extended-care facilities worldwide. Methods A retrospective cohort study of patients aged ≥ 18 hospitalized with CDI in New York State (NYS) between January 1, 2014–December 31, 2016. Data were extracted from NY Statewide Planning and Research Cooperative (SPARCS) and propensity score matching was performed to achieve comparability of the CDI (exposure) and non-CDI (non-exposure) groups. Of the 3,714,486 hospitalizations, 28,874 incidence CDI cases were successfully matched to 28,874 non-exposures. Results The matched pairs comparison demonstrated that CDI cases were more likely to be readmitted to the hospital at 30 (28.26% vs. 19.46%), 60 (37.65% vs. 26.02%), 90 (42.93% vs. 30.43) and 120 days (46.47% vs. 33.74), had greater mortality rates at 7 (3.68% vs. 2.0%) and 180 days (20.54% vs. 11.96%), with significant increases in length of stay and total hospital charges (p

The vertical slots of fish passes represent bottlenecks that must be passed by every fish migrating upstream. The hydraulics in fish passes are well investigated but less is known about the small scale behaviour of fish while passing the vertical slot. Understanding the species-specific swimming behaviour during the passage could allow for creation of future fish passes with hydraulics adapted to the swimming requirements of desired target species. We recorded the swimming trajectories of three fish species as point coordinates per video frame using cameras. Then, two common machine learning algorithms were used to identify species characteristic swimming patterns in the trajectories. A Random Forest model trained on 21 trajectory features revealed that water discharge, the spatial trajectory position, and the trajectory length were most distinct trajectory features among species. The model identified the species with a mean F1 score of 0.86 ± 0.08 SD for round goby [Neogobius melanostomus (Pallas, 1814)], 0.81 ± 0.12 SD for gudgeon (Gobio L.), and 0.58 ± 0.20 SD for bullhead (Cottus gobio L.). A Convolutional Neural Network achieved a mean F1 score of 0.89 ± 0.03 SD for round goby, 0.76 ± 0.05 SD for gudgeon, and 0.67 ± 0.02 SD for bullhead if exclusively trained on the point coordinates of the swimming trajectories. These results demonstrate that fish species exhibit distinct swimming patterns when passing through a vertical slot, and how these patterns can be used for species identification using machine learning algorithms. Because round goby achieved the highest F1 scores, we conclude that round goby showed the most characteristic swimming trajectories among the species tested. Future fish passage research should account for the individual swimming patterns of the fish in these bottleneck flow fields and on adapting the flow to the individual swimming patterns of the target fish. Flow conditions being supportive for swimming patterns of the desired fish could have the potential to improve the river connectivity and thereby support the aquatic biodiversity.

In the face of modern challenges, analyzing sea turtle nesting trends is critical to better understand impacts to these vulnerable species. The introduction of hard-armoring structures (e.g., seawalls, rock revetments) on sea turtle nesting beaches poses a threat to nesting leatherback (Dermochelys coriacea), loggerhead (Caretta caretta), and green (Chelonia mydas) sea turtles due to habitat loss and turtle interactions with the physical structure. Despite much of Florida’s coastline being protected by some form of hard-armoring technology, research on the impacts of these structures to sea turtles is limited to loggerheads. Our objectives were to (1) examine nest density, nesting success, washout rates, and hatching and emergence success at hard-armoring sites in comparison to a control area and (2) characterize impacts of obstructions encountered by sea turtles nesting in northern Palm Beach County, Florida. Our results indicate that the hard-armoring site showed significantly lower nest density for green turtles and nesting success for loggerheads and green turtles in comparison to a control area. Additionally, nesting success for loggerheads and green turtles that encountered hard-armoring structures was significantly lower in comparison to those that encountered no obstructions or other obstructions (e.g., beach furniture, walkovers, escarpments, etc.). These results suggest that hard-armoring structures negatively impact sea turtle nesting behavior, which could result in loss of energy or other physiological derangements. Green turtles showed the most significant differences between the two sites, likely a result of their typical nest site selection favoring the upper portions of the beach, crawling further distances from the high-water line than loggerheads or leatherbacks. Before additional hard-armoring structures are permitted and installed, governing agencies should first consider more natural methods of protecting shorelines (e.g., dune restoration).

A material termed “carbonaceous sulfur hydride” has recently been reported to be a high-pressure room temperature superconductor [Snider et al., Nature 586, 373 (2020)]. We have previously pointed out that certain anomalies observed in the published data for the ac magnetic susceptibility of this material would be cleared up once the measured raw data were made available [J. E. Hirsch, arXiv:2110.12854v1 (2021) and J. E. Hirsch, Physica C 590, 1353964 (2021) (temporarily removed)]. The measured raw data, as well as numerical values of the data presented in figures in the aforementioned paper by Snider et al., have recently been posted on the arXiv [R. P. Dias and A. Salamat, arXiv:2111.15017v1 (2021) and R. P. Dias and A. Salamat, arXiv:2111.15017v2 (2021)]. Here, we report the results of our analysis of these raw data and published data and our conclusion that the raw data are incompatible with the published data. Implications of these results for the claim that the material is a room temperature superconductor are discussed.

Objective: Scoping review of published literature to establish clinical characteristics and audiologic outcomes in patients diagnosed with Susac’s Syndrome(SS) who have undergone cochlear implantation (CI). Data sources: All published studies of CI in SS and contribution of two of our own patients who have not been reported previously. Methods: A comprehensive search of MEDLINE (via PubMed) was carried out in March 2020 using the following keywords and related entry terms: Susac’s Syndrome, Cochlear Implantation. Results: Our search identified a total of five case reports of CI in SS. With the addition of our two patients reported here, we analyzed characteristics and outcomes in seven patients. Mean age at implantation was 30 years old (range 19–46), with six women and one man implanted. Mean time from onset of hearing loss to implantation was 17 months (range three months to four years). Best reported postoperative speech understanding was reported via different metrics, with six of seven patients achieving open set speech scores of 90% or better, and one subject performing at 68%. Vestibular symptoms were present preoperatively in four of seven patients (57%), with vestibular testing reported in two patients, and showing vestibulopathy in one patient. No complications were reported following cochlear implantation. Conclusion: Cochlear implantation is a viable option for hearing rehabilitation in patients with SS, with levels of attainment of open set speech comparable to other populations of CI candidates.
