There is also evidence that the common apolipoprotein E (APOE) polymorphism may affect the response to treatment with donepezil in Alzheimer's disease. We investigated the association between response to donepezil and the rs1080985 single nucleotide polymorphism, the minor allele (G) of which was previously reported to be associated with a poor response to this drug in patients with Alzheimer's disease. The common APOE polymorphism was also assessed for its relevance to the outcome of this treatment. Methods: Analysis of CYP2D6 and APOE polymorphisms was undertaken in 88 naive Caucasian patients with Alzheimer's disease. All patients received treatment with donepezil for at least 10 months, and the response to treatment was then assessed according to the National Institute for Health and Clinical Excellence criteria. Results: No significant differences were observed in distribution of the CYP2D6 rs1080985 single nucleotide polymorphism or common APOE polymorphism between responders (68.2%) and nonresponders (31.8%) to treatment with donepezil. Conclusion: Our results suggest that neither the CYP2D6 nor the APOE polymorphism influences the response to treatment with donepezil in a Polish population with Alzheimer’s disease. 7. Anatomy of the occipital lobe using lateral and posterior approaches: a neuroanatomical study with a neurosurgical perspective on intraoperative brain mapping.

Abstract

Background: A major concern of occipital lobe surgery is the risk of visual field deficits. Extending anatomical occipital lobectomy to the functional requires awake conditions because the anterior resection border comprises language-, motor- and visuospatial function-related areas within the temporal and parietal lobes. This study investigated the lateral and posterior perspectives of the occipital lobe anatomy when approaching intraaxial occipital lobe lesions. Materials and methods: Ten adult cadaveric cerebral hemispheres were dissected after being prepared following the concept described by Klingler for the first time. Results: The occipital lobe was located posteriorly to the parietotemporal line. Within the occipital lobe, the occipital horn of the lateral ventricle represented the only anatomical landmark. Laterally, optic radiation was identified as a part of the sagittal stratum. None of the intraoperatively identifiable tracts was found medial to the occipital horn. Language- and motor-related areas were identified anteriorly and should be actively identified when lobectomy based on function is planned. Subcortically, from a posterior perspective, the anterolateral border constituted the arcuate fascicle/superior longitudinal fascicle complex and was anteromedial to the thalamocortical tract. Remaining posterior to the line connecting the preoccipital notch with the superior Rolandic point avoided the cortical and white matter tracts related to language, motor and visuospatial function. Conclusions: Knowledge of occipital lobe anatomy and surrounding structures is essential to preoperatively assess the risk of the procedure and proper consultation of a patient in terms of the extent of resection, primarily concerning visual field deficits. Abstract

OBJECTIVE: We sought to assess whether genetic risk factors for atrial fibrillation (AF) can explain cardioembolic stroke risk. METHODS: We evaluated genetic correlations between a previous genetic study of AF and AF in the presence of cardioembolic stroke using genome-wide genotypes from the Stroke Genetics Network (N = 3,190 AF cases, 3,000 cardioembolic stroke cases, and 28,026 referents). We tested whether a previously validated AF polygenic risk score (PRS) associated with cardioembolic and other stroke subtypes after accounting for AF clinical risk factors. RESULTS: We observed a strong correlation between previously reported genetic risk for AF, AF in the presence of stroke, and cardioembolic stroke (Pearson r = 0.77 and 0.76, respectively, across SNPs with p < 4.4 × 10-4 in the previous AF meta-analysis). An AF PRS, adjusted for clinical AF risk factors, was associated with cardioembolic stroke (odds ratio [OR] per SD = 1.40, p = 1.45 × 10-48), explaining ∼20% of the heritable component of cardioembolic stroke risk. The AF PRS was also associated with stroke of undetermined cause (OR per SD = 1.07, p = 0.004), but no other primary stroke subtypes (all p > 0.1). CONCLUSIONS: Genetic risk of AF is associated with cardioembolic stroke, independent of clinical risk factors. Studies are warranted to determine whether AF genetic risk can serve as a biomarker for strokes caused by AF.

Abstract

Objective: For 3,670 stroke patients from the United Kingdom, United States, Australia, Belgium, and Italy, we performed a genome-wide meta-analysis of white matter hyperintensity volumes (WMHV) on data imputed to the 1000 Genomes reference dataset to provide insights into disease mechanisms. Methods: We first sought to identify genetic associations with white matter hyperintensities in a stroke population, and then examined whether genetic loci previously linked to WMHV in community populations are also associated in stroke patients. Having established that genetic associations are shared between the 2 populations, we performed a meta-analysis testing which associations with WMHV in stroke-free populations are associated overall when combined with stroke populations. Results: There were no associations at genome-wide significance with WMHV in stroke patients. All previously reported genome-wide significant associations with WMHV in community populations shared direction of effect in stroke patients. Abstract

Liver cirrhosis is often accompanied by a spectrum of cognitive deficits, labelled hepatic encephalopathy (HE). The precise specification of cognitive impairment associated with HE has not been yet elucidated. The aim of this study was an attempt to examine cortical function in cirrhotic patients using EEG event-related potentials during a demanding task involving selective attention. We compared group of 30 patients with liver cirrhosis without minimal or overt HE with education-, age- and sex-matched 29 non-cirrhotic controls. Both groups performed an attentional blink (AB) task, which requires detecting and identifying two target characters in a longer series of rapidly and sequentially presented characters. EEG signals from 32 electrodes were measured and then analyzed in the paradigm of event-related potentials (ERP). Though the groups did not differ in the detection rate of the target stimuli, ERP waveforms revealed two group differences of component amplitudes. The first difference was related to the waveform amplitude within the 200-400 ms after first target in the right frontal region (frontocentral N2 component). Moreover, in patient group this amplitude positively correlated with the blood plasma level of alkaline phosphatase and gamma-glutamyl transpeptidase. The second amplitude difference was observed in the midline parieto-occipital regions within the 400-600 ms after the first target (P3b component). The AB task and ERP analysis allowed to find differences in cortical functioning in cirrhotic patients even without overt cognitive deficits. Our finding demonstrates that liver dysfunction can influence cortical processing associated with detecting and categorizing stimulus change.

Abstract

International audience; Ca. 500 Ma orthogneisses and bimodal suites are widespread along the northern part of the Bohemian Massif (central European Variscides) and are interpreted to document intense magmatism during a continental break-up episode along the northern periphery of Gondwana. Based on geological setting, and geochemical and isotopic evidence, these felsic igneous rocks record the generation of: (1) magmas of pure or predominantly crustal derivation, represented by minor extrusives and much more voluminous orthogneisses similar to S-type granitoids; (2) subordinate magmas of exclusively mantle origin (ranging from within-plate alkali trachytes to oceanic plagiogranites) corresponding to felsic derivatives of associated basalts; and (3) magmas of hybrid origin, produced either as a result of large degrees of contamination of mantle-derived magmas ascending through the crust, or alternatively, generated by partial melting of mixed sources, such as interlayered sediments and mafic rocks or graywackes containing a juvenile component. The high-temperature dehydration melting process responsible for the generation of the most abundant rock-types necessitated the advection of mantle heat, in a context of continental lithosphere extension, as documented by broadly coeval basaltic magmatism at the scale of the igneous province. The large volumes of felsic magmas generated during the 500-Ma anorogenic event are interpreted to result from the combination of a hot extensional tectonic regime with the widespread availability in the lower crust of fertile lithologies, such as metagraywackes. This in turn reflects the largely undifferentiated nature of the crustal segment accreted some 50–100 m.y. earlier during the Cadomian orogeny.
