The prolonged duration of chronic low back pain (cLBP) inevitably leads to changes in the cognitive, attentional, sensory and emotional processing brain regions. Currently, it remains unclear how these alterations are manifested in the interplay between brain functional and structural networks. This study aimed to predict the Oswestry Disability Index (ODI) in cLBP patients using multimodal brain magnetic resonance imaging (MRI) data and identified the most significant features within the multimodal networks to aid in distinguishing patients from healthy controls (HCs). We constructed dynamic functional connectivity (dFC) and structural connectivity (SC) networks for all participants (n = 112) and employed the Connectome-based Predictive Modeling (CPM) approach to predict ODI scores, utilizing various feature selection thresholds to identify the most significant network change features in dFC and SC outcomes. Subsequently, we utilized these significant features for optimal classifier selection and the integration of multimodal features. The results revealed enhanced connectivity among the frontoparietal network (FPN), somatomotor network (SMN) and thalamus in cLBP patients compared to HCs. The thalamus transmits pain-related sensations and emotions to the cortical areas through the dorsolateral prefrontal cortex (dlPFC) and primary somatosensory cortex (SI), leading to alterations in whole-brain network functionality and structure. Regarding the model selection for the classifier, we found that Support Vector Machine (SVM) best fit these significant network features. The combined model based on dFC and SC features significantly improved classification performance between cLBP patients and HCs (AUC=0.9772). Finally, the results from an external validation set support our hypotheses and provide insights into the potential applicability of the model in real-world scenarios. Our discovery of enhanced connectivity between the thalamus and both the dlPFC (FPN) and SI (SMN) provides a valuable supplement to prior research on cLBP.

Background: Amnestic mild cognitive impairment (aMCI) is considered to be a transitional stage between Alzheimer's disease (AD) and normal cognitive state because it has the same clinical symptoms as AD but with lower severity. Studies have confirmed that patients with aMCI are more likely to develop to AD. Although studies on resting state functional connectivity have revealed the abnormal organization of brain networks, the dynamic changes of the functional connectivity across the scans have been ignored.Objective: Dynamic functional connectivity is a novel method to reveal the temporal variation of brain networks. This paper aimed to investigate the dynamic characteristics of brain functional connectivity in the early and late phases of aMCI.Methods: Based on the "triple network" model, we used the sliding time window approach to construct dynamical functional networks and then analyzed the dynamic characteristics of the functional connectivity across the entire scan.Results: The results showed that patients with aMCI had longer dwell times in weaker network connection than in the strong network. The transitions between different states become more frequent, and the stability of the patient's brain core network deteriorates. This study also found the correlation between the altered dynamic properties of the core functional networks and the patient's clinical Mini-Mental State Examination assessment scale sores.Conclusion: This study revealed that the characteristics of dynamic functional networks constructed by the core cognitive networks varied in distinct ways at different stages of aMCI, which could provide a new idea for exploring the neuro-mechanisms of neurological disorders. However, conventional D-FCN is essentially low-order network, which only reflects the pairwise interaction pattern between brain regions and thus overlooking the high-order interactions among multiple brain regions. In addition, D-FCN is innate with temporal sensitivity issue, i.e., D-FCN is sensitive to the chronological order of its subnetworks. To deal with the above issues, we propose a novel high-order functional connectivity network framework based on the central moment feature of D-FCN. Specifically, we firstly adopt a central moment approach to extract multiple central moment feature matrices from D-FCN. Furthermore, we regard the matrices as the profiles to build multiple high-order functional connectivity networks which further capture the higher level and more complex interaction relationships among multiple brain regions. Finally, we use the voting strategy to combine the high-order networks with D-FCN for autism spectrum disorder diagnosis. Experimental results show that the combination of multiple functional connectivity networks achieves accuracy of 88.06%, and the best single network achieves accuracy of 79.5%.
