Amaç: Bu çalışmanın amacı meme kanseri moleküler subgrup sıklığını belirlemek ve meme kanseri tanısı ile takip ettiğimiz hastalarda otoimmun tiroid hastalığı (OİTH) ile meme kanseri prognostik ve prediktif faktörleri arasındaki ilişkiyi araştırmaktır.Gereç ve Yöntem: Retrospektif tanımlayıcı bir araştırma olarak planlanan bu çalışmaya 01 Ocak 2012–31 Aralık 2015 tarihleri arasında endokrinoloji ve tıbbi onkoloji polikliniklerine başvurmuş, meme kanseri tanısı ile takip edilen 101 hasta dahil edildi, moleküler sınıflandırmaya göre subgruplara ayrıldı. Yüksek serum anti-TPO düzeyi olanlar OİTH pozitif kabul edildi. OİTH’nin eşlik ettiği meme kanseri hastalarında prognostik ve prediktif parametreler (tümör evresi, tümör boyutu, aksiller lenf nodu tutulumu, histolojik derece, lenfovaskuler invazyonu, perinöral invazyonu, hormon reseptör durumu, HER2 ekspresyonu) otoimmün tiroid hastalığının eşlik etmediği meme kanseri hastaları ile karşılaştırıldı.Bulgular: Çalışmaya dahil edilen meme kanseri hastalarında tiroid otoimmunite (yüksek serum anti-TPO düzeyi) prevalansı %23,8 (n=24) olarak saptandı. OİTH pozitif meme kanseri hastalarında aksiller lenf nodu tutulumu (%37,5’e karşı %61; p=0,043) OİTH negatif meme kanseri hastalarına göre daha düşüktü. Diğer parametreler açısından iki grup arasında anlamlı fark izlenmedi.Sonuç: OİTH’nin eşlik ettiği meme kanseri hastalarında önemli bir prognostik parametre olan aksiller lenf nodu tutulum oranı daha düşük bulunmuştur. Bu sonuç tiroid otoimmunitesinin meme kanseri açısından olumlu bir prognostik faktör olduğu görüşünü desteklemektedir. Bunun hangi mekanizma üzerinden etkili olduğu ise yeni çalışmalarla araştırılmalıdır., Aim: The aim of this study is to determine the frequency of breast cancer molecular subgroups and to investigate the relationship between autoimmune thyroid disease (AITD) and breast cancer prognostic and predictive factors in patients followed up with a diagnosis of breast cancer.Material and Method: This study, which was planned as a retrospective descriptive study, included 101 patients who applied to the endocrinology and medical oncology outpatient clinics between January 01, 2012 and December 31, 2015, followed up with the diagnosis of breast cancer, and were divided into subgroups according to molecular classification. Those with high serum anti-TPO levels were considered AITD positive. Prognostic and predictive parameters (tumor stage, tumor size, axillary lymph node involvement, histological grade, lymphovascular invasion, perineural invasion, hormone receptor status, HER2 expression) in breast cancer patients with AITD were compared with breast cancer patients without autoimmune thyroid disease.Results: The prevalence of thyroid autoimmunity (high serum anti-TPO level) in breast cancer patients included in the study was 23.8% (n=24). Axillary lymph node involvement (37.5% vs. 61%; p=0.043) was lower in AITD-positive breast cancer patients than in AITD-negative breast cancer patients. No significant difference was observed between the two groups in terms of other parameters.Conclusion: Axillary lymph node involvement, which is an important prognostic parameter, was found to be lower in breast cancer patients with AITD. This result supports the view that thyroid autoimmunity is a positive prognostic factor for breast cancer. The mechanism by which this is effective should be investigated with new studies.

Endocrine disruptors are naturally occurring or man-made substances that at certain doses can mimic or interfere with the endocrine systems. Bisphenol-A is an endocrine disruptor due to its estrogenic properties and effects on the estrogen receptor. We are exposed to Bisphenol-A, which is especially used in the plastics industry, in our daily lives without even being aware of it. The effects of Bisphenol-A on human health have been on the agenda recently, and worrying negative effects have been discovered. As in many countries, even if there is a restriction on its use in our country, this is not considered possible in practice. In this review, it is aimed to draw attention to the negative effects of Bisphenol-A on human health such as endocrine system insulin resistance, obesity, polycystic ovary syndrome, cardiovascular diseases, breast cancer, abortion and premature birth., Endokrin bozucular, belirli dozlarda endokrin sistemlerini taklit edebilen veya bunlara müdahale edebilen, doğal olarak meydana gelen veya suni maddelerdir. Östrojenik özelliği ve östrojen reseptörü üzerinden etki etmesi nedeniyle, bir endokrin bozucu kimyasal olan özellikle plastik sanayinde kullanılan Bisfenol-A’ya günlük yaşantımızda farkında bile olmadan maruz kalmaktayız. Bisfenol-A’nın insan sağlığı üzerindeki etkileri son günlerde gündemde olup kaygı verici düzeyde olumsuz etkiler keşfedilmiştir. Birçok ülkede olduğu gibi ülkemizde de kullanımına sınırlama getirilse bile bu pratikte pek de mümkün görülmemektedir. Bu derlemede Bisfenol-A’nın özellikle endokrin sistem insülin direnci, obezite, polikistik over sendromu, kardiyovasküler hastalıklar, meme kanseri, abortus ve erken doğum gibi insan sağlığı üzerindeki olumsuz etkileri üzerine dikkat çekmek amaçlanmıştır.

Hipertiroidizm, tiroid bezi tarafından aşırı tiroid hormonu üretimi ile olan ilişkili hipermetabolik bir durumdur. Tiroid hormonlarının kemik ve kalsiyum metabolizması üzerine önemli etkileri vardır. Yaklaşık beş hastadan birinde hipertiroidiye bağlı hafif ila orta derecede hiperkalsemi bildirilmiştir. Hipertiroidiye sekonder hiperkalsemiye azalmış ya da subnormal parathormon seviyeleri eşlik eder. Hipertiroidizm ile hiperparatiroidizm birlikteliği nadir olarak görülmektedir. Bununla birlikte, hipertiroidi hastalarında ötiroid duruma ulaştıktan sonra hiperkalseminin devam etmesi durumunda paratiroid bezlerinin ayrıntılı olarak değerlendirilmesi gerekir.Burada, hiperkalsemisi olan, Graves hastalığı ve primer hiperparatiroidizm tanısı alan, kombine tiroid ve paratiroid cerrahisi ile başarılı bir şekilde tedavi edilen 30 yaşında bir erkek hasta olgusunu sunuyoruz., Hyperthyroidism is a hypermetabolic condition associated with excessive production of thyroid hormones by the thyroid gland. Thyroid hormones play an important role in bone and calcium metabolism. Mild to moderate hypercalcemia due to hyperthyroidism has been reported in approximately one in five patients. Hypercalcemia due to hyperthyroidism is associated with decreased or subnormal parathyroid hormone levels. The co-occurrence of hyperthyroidism and primary hyperparathyroidism is rare. However, in patients with hyperthyroidism, it is necessary to evaluate the parathyroid glands if hypercalcemia persists after an euthyroid state is reached.Here, we report the case of a 30-year-old man with hypercalcemia who had Graves' disease and primary hyperparathyroidism and was successfully treated with combined thyroid and parathyroid surgery.

Amaç: Karaciğer, kolesterol sentezinde ve katabolizmasında çok önemli rol oynar. Non-alkolik yağlı karaciğer hastalığı (NAYKH), gelişmiş ülkelerde kronik karaciğer hastalığının en sık sebebidir ve sıklıkla diabetes mellitus, hipertansiyon, dislipidemi ve obezite gibi metabolik hastalıklarla komorbidite gösterir. Bu çalışmanın amacı Ailevi Hiperkolesterolemide (AH) NAYKH görülme sıklığını araştırmaktır.Gereç ve Yöntemler: 2017-2018 yılları arasında Erzurum Bölge Eğitim ve Araştırma Hastanesi Endokrinoloji Polikliniğine başvurmuş olan AH’li kişiler tek merkezli bu vaka kontrol çalışmasına alındı. AH’li toplam 30 kişi (17 Kadın 13 erkek), 39 kişiden oluşan (30 kadın, 9 erkek) kontrol grubu ile karşılaştırıldı. İki grup sayı ve cinsiyet yönünden denkti. Aile öyküsü kaydedildi. Tanı kan tetkikleri ve karaciğer ultrasonografisi ile kondu.Bulgular: İki grup arasında yaş ve cinsiyet açısından anlamlı fark görülmedi. NAYKH, kontrol grubundaki 39 kişiden 12’sinde saptanırken, AH grubunda 30 kişiden 16’sında görüldü (p = 0,058). AH ve kontrol gruplarının sırasıyla toplam kolesterol (333,27 ± 58,46) (219,23 ± 82,84) (mg / dl) (p = 0,000), LDL (249,93 ± 45,43) (127,11 ± 46,52) (mg / dl) (p = 0,000) ve CRP (3,37 ± 4,16) (1,43 ± 1,96) (mg / L) (p = 0,014) düzeyleri arasında anlamlı fark bulundu.Sonuçlar: AH ve kontrol grupları arasında NAYKH açısından anlamlı bir fark yoktur. Ultrasonografide NAYKH saptanmaması, AH’ye yaklaşımdaki ciddiyeti azaltmamalıdır., Aim: The liver plays a crucial role in the synthesis and catabolism of cholesterol. Non-alcoholic fatty liver disease (NAFLD) is the most common cause of chronic liver disease in the developed world and commonly associated with metabolic comorbidities such as diabetes mellitus, hypertension, dyslipidemia, and obesity. The aim of this study was to investigate the frequency of NAFLD in Familial Hypercholesterolemia (FH).Material and Method: Between 2017 and 2018, individuals with FH who had been referred to the Department of Endocrinology at Erzurum Regional Training and Research Hospital were admitted to this single center case-control study. Total 30 individuals (17 female and 13 male) who have FH were compared to a control group included 39 participants (30 female and 9 male). The two groups were thought to be well matched in terms of sample size and gender distribution. Family history was registered. Diagnosis is made by blood tests and ultrasound imaging of liver.Results: The two groups showed no significant differences in terms of age and gender. NAFLD was seen at 12 of 39 individuals in the control group. It was seen at 16 of 30 persons in the FH group (p = 0.058). Total cholesterol (mg / dl) (333.27 ± 58.46) (219.23 ± 82.84) (p = 0.000), LDL (mg / dl) (249.93 ± 45.43) (127.11 ± 46.52) (p = 0.000) and CRP (mg / L) (3.37 ± 4.16) (1.43 ± 1.96) (p = 0.014) levels were significantly different between the FH and control groups respectively.Conclusion: There isn’t a significant difference between control and FH groups in terms of NAFLD. The absence of NAFLD on ultrasonography, should not reduce the severity and importance of FH, and the treatments should be made to prevent complications such as atherosclerosis.

The most endocrinologists in routine practice are used to Bethesda classification for evaluation of thyroid nodule in fine needle aspiration biopsy (FNA). It is accepted that FNA biopsy is an accurate diagnostic and gold standard test. However, false-positive diagnosis may sometimes occur as a patient with a ‘malign’ lesion is found incorrectly rather than an actually benign lesion on histological examination. Hashimoto’s thyroiditis probably is the most common cause of false-positive cytology. The aims of present study evaluate retrospectively postoperatively biopsy results in patients with Hashimoto disease have a thyroidectomy. Results of totally 29 patients with Hashimoto thyroiditis achieved from our hospital records. All of the patients had undergone totally or subtotals thyroidectomy. We are-evaluated retrospectively biopsy results postoperatively. We compared their preoperative FNA results. We detected 24.1% papillary thyroid cancer in seven patients (mean age 45.5±11.9 year), 44.8% nodular goitre in 13 patients (mean age 45.8±4.9 year), 13.7% adenomatous nodule in four patients (mean age 43.5±14.4 year), 3.4% hurtle cell adenoma in one patient with 56 years old. But, 14% in four patients with Hashimoto thyroiditis (mean age 41.5±13.1 year) were redundantly operated due to atypia of undetermined significance in FNA. Although Hashimoto disease is a benign diagnosis, its misclassification as atypia of undetermined significance in FNA accounts for some false-positive errors. The cytopathologists can frequently be interpreted as atypia of undetermined significance instead of Hashimoto’s thyroiditis. Therefore, result of atypia of undetermined significance in FNA may lead unnecessarily concern among with the endocrinologists. More importantly, our results demonstrated that high papillary thyroid cancer rate was found postoperatively in suspicious nodules evolved background Hashimoto thyroiditis.

Introduction:The intralesional injection ofrecombinant human epidermal growth factor (EGF-IL) is a new approach forreducing amputations. In this study we aimed to show efficacy of EGF-IL withcases.Material and Method: EGF-IL 75 μg application to 8 diabetic foot ulcers(6 males, 2 females; mean age: 60.3±9.3 years) was evaluated. Patients hadhistory of prior standard treatments.Results: In alleight patients’ completely healing was observed diabetic foot ulcers after theintralesional EGF treatment.Conclusion: In our experience our patients’wounds wish priorly have healing problems, improved with intralesional EGF.This treatment is expensive and caused of some allergic reactions. Bu çalışmada EGF-IL'nin olgularla etkinliğini göstermeyiamaçladık.Gereç ve Yöntem: Sekiz diyabetik ayak ülserine (6erkek, 2 kadın; ortalama yaş: 60.3 ± 9.3 yıl) EGF-IL 75 μg uygulandı. Hastalardaha önce standart tedaviler geçmişi vardı.Bulgular: Sekiz hastanın tamamında,intralezyonel EGF tedavisinden sonra diyabetik ayak ülserleri iyileşti.Sonuç: Tecrübelerimize göre,hastalarımızın yaraları önceden intralezyonal EGF ile iyileşen iyileşmeproblemlerine sahip olmak istemektedir. Bu tedavi pahalıdır ve bazı alerjikreaksiyonlara neden olur. Malignitelerle olan ilişki uzun süredirbilinmemektedir.

While hyperparathyroidism is wellknown among the clinicians, such side effects as renal complications and osteoporosis are closely monitored.Besides various conducted studies havefound out several side effects such as hypertension, diastolic dysfunction,endothelial dysfunction, heart valve calcification, arythmia, cardiachypertropy. Even some studies have pointed to increase in cardiac mortality iscorrelated with hyperparathyroidizm. İn this review, it is intended to raise aconcern about cardiac complications occuring in primary hyperparathyroidism., Hiperparatiroidi klinisyenlertarafından iyi bilinen bir hastalık olmakla birlikte öncelikle renal komplikasyonlar ve osteoporoz gibi yan etkiler yakın takip edilmektedir. Bununla birlikteyapılan çeşitli çalışmalarda hiperparatiroidiye bağlı hipertansiyon, diastolikdisfonksiyon, endotel disfonksiyonu, kalp kapak kalsifkasyonu, aritmi, kardiyakhipertrofi gibi yan etkiler belirtilmiştir. Hatta bazı çalışmalar kardiyakmortalite artışıyla ilişkili bulunmuştur. Bu derlemede primerhiperparatiroidide rastlanan kardiyak sorunlara dikkat çekmek amaçlanmıştır.

Ailesel hipokalsiürik hiperkalsemi, ılımlı hiperkalsemi, idrarda kalsiyum atılımının az olması ile giden ve kalsiyum duyarlı reseptörlerde mutasyonlardan kaynaklanan benign bir hastalıktır. 2010 Ocak ile 2015 Haziran ayları arasında İç hastalıkları polikliniğimize başvuran altı hasta klinik ve biyokimyasal parametrelerle değerlendirilmiştir. Olgularımız, 21 ile 86 yaş arasında, dördü kadın olmak üzere toplam altı hasta idi. Beş olgumuzun çocukları ve bir olgumuzun ebeveynleri poliklinik ortamında değerlendirildi. Olguların değerlendirmesinde ılımlı hiperkalsemi, sınırda yüksek parathormon düzeyleri ve idrarla günlük kalsiyum atılımının belirgin düşüklüğü nedeniyle ailesel hipokalsiürik hiperkalsemi olabileceği düşünüldü. Beş erişkin hastanın çocuklarında ve bir genç hastamızın da annesinde kalsiyum metabolizmasında benzer laboratuvar bulguları saptanması ve hiperkalsemi yapan diğer nedenlerin dışlanması üzerine tanı doğrulandı. Familial hypocalciuric hypercalcemia is a benign disease caused by mutations in the calcium-sensitive receptors, with moderate hypercalcemia and of calcium excretion in the urine leading to less. Six patients was evaluated in our internal medicine polyclinic with clinic and biochemistry parameters between 2010 January and 2015 June. Our cases were six patients including four women between 21 and 86 old. We evaluated in our polyclinic five cases childs and one cases parents. The results of evaluation of the patients were thought to be familial hypocalciuric hypercalcemia because of moderate hypercalcemia, borderline high parathormone levels and marked impairment of urinary calcium excretion. The diagnosis was confirme exclusion of other causes of hypercalcemia and the childrens of five patients and mother of one patients detected in calcium metabolism in the laboratory findings are similar to.

The prevalence of obesity, defined as increased amount of body fat, varies according to race, age, and sex. Obesity prevalence in children and adolescents increases worldwide. The most important reason for the increase is, along with developing technology, limitation of activity, and alterations in nutritional habits and food preferences of the children. Although there is imbalance of calorie intake and utilization in the etiologyof obesity, genetic factors as in monogenic obesity, hundreds of environmental factors known as endocrine destructors, changes in intestinal microbiota, and hormonal factors are also accused. Body mass index (BMI), a simple, practical, cheap, and safe method, is used in the diagnosis of obesity in children. Obesity occurring in early period causes serious complications such as, type 2 diabetes mellitus, non-alcoholic hepatosteatosis, hypertension, hyperlipidemia, and cardiovascular diseases. Erken dönemde ortaya çıkan obezite tip 2 diabetes mellitus, non alkolik hepatosteatoz, hipertansiyon, hiperlipidemi ve kardiyovasküler hastalıklar gibi ciddi komplikasyonlara neden olmaktadır. The diagnosis of acromegaly is based on demonstration of excess growth hormone (GH) and Insulin-Like Growth Factor-1 (IGF-1) secretion. IGF-1 is the most reliable biochemical indicator of activity of acromegaly. However, there are some pitfalls in the interpretation of change of plasma IGF-1 levels. We aim to report a case with acromegaly that has low IGF-1 levels and elevated GH levels associated with poorly controlled type 2 diabetes mellitus and malnutrition. A 38-years-old woman was admitted to emergency departmant due to hyperglycemia, weakness, cough, dyspnea, high fever. She has been complaining for enlargement of her hands and feet for ten years and she was cachectic for a long time. During oral glucose tolerance test (OGTT), serum growth hormone levels were found to be higher than normal range according to the matching age and sex subjects but IGF-1 and IGFBP-3 levels were measured lower than the reference range. A macroadenoma of 3x2.5 cm diameter was determined in magnetic resonance imaging of the pituitary gland. As a conclusion, determining elevated IGF-1 levels are very important for the diagnosis and activity of acromegaly, but careful interpretation of IGF-1 levels is necessary in type 2 diabetic patients with acromegaly.Key Words: Acromegaly, IGF-1, malnutrition, type 2 diabetes mellitus., Akromegali tanısı aşırı GH ve Insulin-Like Growth Factor-1 (IGF-1) sekresyonunun gösterilmesine dayanır. Biz kötü kontrollü tip 2 diabetes mellitus ve malnütrisyon ile ilişkili olarak düşük IGF-1 ve artmış GH düzeyleri olan bir akromegalili olguyu sunmayı amaçladık. Otuz sekiz yaşındaki kadın hasta hiperglisemi, halsizlik, öksürük, dispne, yüksek ateş yakınmaları yüzünden acil servise baş vurdu. On yıldır ellerinde ve ayaklarında büyüme olmasından yakınmaktaydı ve uzun zamandır kaşektikti. OGTT sırasında serum growth hormon düzeyleri yaş ve cinsiyete göre olan normal sınırlardan yüksek bulundu. Fakat IGF 1 and IGFBP-3 normal referans aralığından düşük ölçüldü. Hipofizin manyetik rezonans görüntülemesinde 3x2.5 cm çaplı bir pituiter makroadenom saptandı. Sonuç olarak, artmış IGF-1 düzeyleri akromegali tanı ve aktivitesinde çok önemlidir, ancak tip 2 diyabetli akromegalilerde IGF-1’ in dikkatli yorumlanması gereklidir.Anahtar Kelimeler: Akromegali, IGF-1, malnutrisyon, tip 2 diabetes mellitus
