Abstract

Background: In MAPT (Multidomain Alzheimer Preventive Trial), a cognitive effect of multidomain intervention (MI) was showed in non-demented subjects with positive amyloid PET. However, screening eligible patients for multidomain intervention by PET is difficult to generalize in real-world settings. Methods: MAPT study was a 3-year, randomized, placebo-controlled trial followed by a 2-year observational and optional extension. All participants were non-demented and randomly assigned (1:1:1:1) to the MI plus omega 3, MI plus placebo, omega 3 alone, or placebo alone group. The objectives were to assess the cognitive effect of MAPT interventions (omega 3 supplementation, MI, combined intervention) in non-demented subjects according to amyloid blood status at 12, 36, and 60 months. In this subgroup analysis (n = 483), amyloid status was defined by plasma Aβ42/40 ratio (cutoff ≤ 0.0107). The primary outcome measure was the change in cognitive composite score after a 1, 3, and 5-year clinical follow-up. Results: The intention-to-treat (ITT) population included 483 subjects (161 positive and 322 negative amyloid participants based on plasma Aβ42/40 ratio). In the positive amyloid ITT population, we showed a positive effect of MI plus omega 3 on the change in composite cognitive score in 12 (raw p =.0350, 0.01917, 95% CI = [0.0136 to 0.3699]) and 36 months (raw p =.0357, 0.2818, 95% CI = [0.0190 to 0.5446]). After correction of multiple comparisons and adjustments, these differences were not significant (adjusted p =.1144 and.0690). In the per-protocol positive amyloid group (n = 154), we observed a significant difference between the combined intervention and placebo groups at 12 (p =.0313, 0.2424, 0.0571 to 0.4276) and 36 months (p =.0195, 0.3747, 0.1055 to 0.6439) persisting after adjustment. In the ITT and per-protocol analyses, no cognitive effect was observed in the positive and negative amyloid group at 60-month visit. Conclusions: These findings suggest a benefit of MI plus omega 3 in positive blood amyloid subjects. Abstract

Late‐life aging is often associated with appetite reduction and weight loss. Physical activity (PA) may prevent these processes, but the molecular mechanisms involved remain elusive. The present study investigated the putative mediating aspect of growth differentiation factor 15 (GDF‐15), a stress signalling protein involved in aging, exercise and appetite control, on the association between PA and late‐life‐associated weight loss. One thousand eighty‐three healthy adults (63.8% women) aged 70 years and over who participated in the Multidomain Alzheimer Preventive Trial were included. Bodyweight (kg) and PA levels (square root of metabolic equivalent of task‐min/week) were assessed repeatedly from baseline to the 3‐year visit, whereas plasma GDF‐15 (pg/mL) was measured at the 1‐year visit. Multiple linear regressions were performed to test the association between first‐year mean PA level, 1‐year visit GDF‐15 concentration and subsequent bodyweight changes. Mediation analyses were used to investigate whether GDF‐15 mediated the association between first‐year mean PA levels and consecutive bodyweight changes. Multiple regression analyses demonstrated that higher first‐year mean PA levels significantly predicted lower GDF‐15 and bodyweight at 1 year (B = −2.22; SE = 0.79; P= 0.005). In addition, higher 1‐year visit GDF‐15 levels were associated with faster subsequent bodyweight loss (Time × GDF‐15 interaction B = −0.0004; SE = 0.0001; P= 0.003). Mediation analyses confirmed that GDF‐15 mediated the association between first‐year mean PA levels and subsequent bodyweight changes (mediated effect ab = 0.0018; bootstrap SE = 0.001; P< 0.05) and revealed that mean PA had no direct effect on subsequent bodyweight changes (c′ = 0.006; SE = 0.008; P> 0.05). This study suggests that GDF‐15 may be one of the molecules mediating the link between PA and late‐life weight loss, but mechanistic studies are necessary to further support the present findings.

23. Searching for a relevant definition of sarcopenia: results from the cross-sectional EPIDOS study

Abstract

Background The diversity of definitions proposed for sarcopenia has been rarely tested in the same population, and so far, their clinical utilities for predicting physical difficulties could not be clearly understood. Our objective is to report the prevalence of sarcopenia and the characteristics of sarcopenic community-dwelling older women according to the different definitions of sarcopenia currently proposed. We also assessed these definitions for their incremental predictive value over currently standard predictors for some self-reported difficulties in physical function and knee extension strength. Methods Cross-sectional analysis included data from 3025 non-disabled women aged 75 years or older without previous history of hip fracture from the inclusion visit of the EPIDémiologie de l'OStéoporose study. A total body composition evaluation was available for 2725 women. Sarcopenia was defined using six different definitions of sarcopenia based on different muscle mass, gait speed, and grip strength cut-offs. Self-reported difficulties in physical function and knee extension strength were collected. Logistic regression and multiple linear regression models were built for each physical dysfunction, and the predictive capacity of sarcopenia (one model for each definition) was studied using the C-statistic, the net reclassification index, or adjusted R2. Results The estimated prevalence of sarcopenia ranged from 3.3–20.0%. Only 85 participants (3.1%) were identified having sarcopenia according to all definitions. All definitions were, to some degree, associated with self-reported difficulties in physical function and knee extension strength, but none improved the predictive ability of the self-reported difficulties in physical function. Conversely, all definitions accounted for a small but significant amount of explained variation for predicting knee extension strength. Conclusions Prevalence of sarcopenia varies widely depending on the definition adopted. Based on this research, the current definitions for sarcopenia does not substantially increment the predictive value of clinical characteristics of patients to predict self-reported physical difficulties and knee extension strength.

Abstract

Midwives in assisted reproductive technology (ART) centers: what is their role in France, in 2012? The recent promulgation of decree 2012-885 of 17 July 2012 has asked the role of midwives within assisted reproductive technology centers (ART). The thesis is about a questionnaire-based study for midwives of these centers throughout the French territory. They were questioned about their activities, their future prospects and their feeling about the various ethical issues raised by the recent revision of the bioethics laws in July 2011. Midwives' activities are eclectic, whether technical or administrative, medical and informative. Midwives provide most often support and monitoring of couples. Furthermore, the ultrasound monitoring of ovulation allows them to provide assistance and advices for couples in their ART pathway. These roles are essential for midwives who continue to acquire new skills in this area. Most of them express the wish to keep developing this side of their profession.; Les sages-femmes dans les centres d'assistance médicale à la procréation : quelle place occupent-elles, en France, de nos jours, en 2012 ? La promulgation récente du décret 2012-885 du 17 juillet 2012 a amené à s'interroger sur la place occupée par les sages-femmes au sein des centres d'assistance médicale à la procréation (AMP). L'étude a été menée au moyen d'un questionnaire destiné aux sages-femmes de ces centres, sur l'ensemble du territoire français. Celles-ci étaient interrogées sur leurs activités, leurs perspectives d'avenir et leur ressenti quant aux différents enjeux éthiques actuels soulevés par la récente révision des lois de bioéthique de juillet 2011. Les activités des sages-femmes sont éclectiques, qu'elles soient techniques et médicales ou encore administratives et informatives. Les sages-femmes assurent le plus souvent l'accompagnement, le soutien et le suivi des couples. De plus, la réalisation des échographies de monitorage de l'ovulation leur permet d'assurer une prise en charge quasi globale des couples dans leur désir et parcours d'AMP. Ces rôles sont primordiaux pour les sages-femmes qui ne cessent d'acquérir de nouvelles compétences dans ce domaine et qui souhaitent majoritairement continuer à faire évoluer ce versant de leur métier.

29. La prévention de la dépendance : facteurs associés à l'observance d'une intervention multidomaine

Abstract

Entre 2000 et 2040, la population âgée fragile sera multipliée par 2,5 et touchera 10 millions de sujets âgés de 65 ans et plus. Les études épidémiologiques récentes ont illustré que la dépendance est fréquente et multidimensionnelle (impliquant des déficits fonctionnels, cognitifs et sensoriels). Récemment, des éléments tels que l'effet protecteur de l'activité physique ou encore de la nutrition (en action isolée) sur le déclin cognitif et fonctionnel sont bien connus cependant peu d'études se sont intéressées à l'impact d'une intervention multidomaine. L'efficacité de ces interventions est actuellement en cours d'étude dans des études randomisées et contrôlées (MAPT, Do-health, Mid-frail, Finger, preVida). Ainsi, il semble important d'étudier l'observance et les facteurs prédicteurs (la motivation et les obstacles pour ces personnes âgées fragiles) de la participation à un programme de prévention à long terme. Pour déterminer la population cible à risque de perte d'autonomie chez les personnes âgées, l'étude de la définition la plus pertinente de la sarcopénie est nécessaire. La conception des essais liés à des facteurs nutritionnels pertinents (la vitamine D et les compléments nutritionnels oraux(CNO)) est également discutée actuellement., Between 2000 and 2040, the elderly frail population will be multiplied by 2,5 to affect 10 million people aged 65 years and over. Previous epidemiologic studies have highlighted that functional limitations are frequent and multidimensional (associated with functional, cognitive and sensorial decline). Currently, the evidence for the protective role of physical activity or nutrition (action isolated) on cognitive and functional decline are well known but few studies have examined the impact of multidomain interventions. The effectiveness of such interventions must be demonstrated in the current randomized trials. It seems important to study rates of adherence and attrition factors (motivation and barriers for these frail elderly) to participate in a prevention program of long-term. To determine the target population to prevent disability in elderly, consideration of the most relevant definition of sarcopenia is necessary. The design of trials related to the relevant nutritional factors (vitamin D and the Oral Nutritional Supplement (ONS)) is also currently discussed.

Abstract

Omega-3 (n-3) and 6 (n-6) polyunsaturated fatty acids (PUFAs) have been associated with reduced cognitive decline in observational studies. Hence, we examined the cross-sectional associations between cortical β-amyloid (Aβ) and erythrocyte membrane PUFAs in 61 non-demented elderly individuals reporting subjective memory complaints from the Multidomain Alzheimer Preventive Trial placebo arm. Cortical-to-cerebellar standard uptake value ratios were obtained using [18F] florbetapir positron emission tomography. Fatty acids were measured in erythrocyte membranes by gas chromatography. Associations were explored using adjusted multiple linear regression models and were considered significant at p ≤ 0.005 after correction for multiple testing (10 comparisons). We found no significant associations between cortical Ab and erythrocyte membrane PUFAs. The associations closest to significance after adjustment were those between Ab and erythrocyte membrane arachidonic acid (without apolipoprotein E status adjustment: B-coefficient, 0.03; CI, 0.01, 0.05; p = 0.02. Including Apolipoprotein E adjustment: B-coefficient, 0.03; CI, 0.00, 0.06; p = 0.04) and Ab and erythrocyte membrane linoleic acid (without apolipoprotein E status adjustment: B-coefficient, 0.02; CI, 0.04, 0.00; p = 0.02. Including Apolipoprotein E adjustment: B-coefficient, 0.02; CI, 0.04, 0.00; p = 0.09). Furthermore, the association between Aβ and erythrocyte membrane arachidonic acid seemed to be specific to Apolipoprotein E ε4 non-carriers (B-coefficient 0.03, CI: 0.00, 0.06, p = 0.03, n = 36). In contrast, no association was found between Aβ and erythrocyte membrane linoleic acid in Apolipoprotein E ε4 stratified analysis. Investigating the relationships between Aβ and PUFAs longitudinally would provide further evidence as to whether fatty acids, particularly arachidonic acid and linoleic acid, might modulate cognition through Aβ-dependent mechanisms. Abstract

Objectives To examine the associations between amyloid- β brain deposition and physical activity ( PA) in elderly adults without dementia and to investigate whether the association has a dose-response relationship. Design Cross-sectional study. Setting French community-dwelling people. Participants Elderly adults with normal or mildly impaired cognition (mean age 74.7 ± 4.2; 60.4% female) with available information on current self-reported PA and amyloid- β brain deposition measured using positron emission tomography ( PET) using the PET-ligand florbetapir F 18 (n = 268). Measurements A standardized uptake value ratio ( SUVR) was obtained for each subject. Participants were divided according to amyloid plaque cortical retention defined according to a SUVR cutoff of 1.10 ( SUVR+ vs SUVR−). Results Bivariate and multivariate analyses showed that PA was not significantly associated with SUVR. SUVR+ and SUVR− participants did not differ in terms of volume (continuous PA variables) and levels (categorical PA variables) of PA. PA was not correlated with SUVR in apolipoprotein E ε4 carriers or noncarriers. PA was not associated with cognitive function. Conclusion Although PA protects against dementia, there is no solid evidence that this protection involves a reduction in amyloid- β brain deposition. Further studies are needed to determine whether PA (ideally measured at several time-points using objective measures) is involved in the pathophysiology of Alzheimer's disease. Abstract

Background: Therapeutic patient education is expanding in the field of Alzheimer's disease (AD). Objective: To evaluate the impact of a therapeutic educational program, on AD-affected patients and their caregivers, living in the community, on the patient's quality of life. Methods: Non experimental before and after study. Patient/caregiver dyads were recruited in the geriatric department of the Toulouse University Hospital. The intervention consisted of an educational program, designed for both patients and caregivers. It included two individual sessions (at baseline (M0) and two months later (M2)) and four group sessions for caregivers only, one per week between M0 and M2. The primary outcome was the patient's quality of life at two months, hetero-evaluated by the caregiver. We compared the QoL-AD score between M0 and M2 with a paired Student's test. The secondary outcomes were patient's autonomy (activities of daily living) and caregiver's burden (Zarit Burden interview). Results: 29 patient/caregiver dyads were recruited. The QoL-AD score was 24.6 ± 5.1 at M0 versus 27.2 ± 6.0 at M2 (p = 0.038). This difference is statistically significant. There was no difference in the secondary outcomes. Conclusion: This study revealed a significant positive impact of a therapeutic educational program on patients' quality of life. Our results led us to design a randomized controlled trial called the THERAD study (THERapeutic education in Alzheimer's disease). It started in January 2013, and the results will be available in 2015. Abstract

L’importance de la vitamine D dans le métabolisme osseux est établie de longue date mais il existe actuellement un regain d’intérêt pour cette vitamine en raison de son rôle éventuel dans un certain nombre d’événements cliniques comme les chutes, les fractures, la force, l’équilibre, les troubles cognitifs, et la survenue de certains cancers et de certaines maladies inflammatoires au cours du vieillissement. La plupart des données de la littérature rapportent une insuffisance en vitamine D définie par une concentration en 25 (OH) D inférieure à 20 à 30 ng/ml et une carence définie par une concentration inférieure à 10 ng/ml. La prévalence de l’insuffisance ou de la carence en vitamine D est donc importante dans la population de personnes âgées ; elle reste cependant variable d’une étude à l’autre (de 16 à 78%) probablement du fait de l’utilisation de définitions différentes pour identifier les sujets en insuffisance ou carencés, de l’âge de la population étudiée et du sexe (analyses réalisées conjointement chez les hommes et les femmes, ou par sexe). Seule la double supplémentation calcium + vitamine D paraît efficace dans la prévention des fractures ostéoporotiques chez les sujets âgés. Les recommandations actuelles dans le traitement de l’ostéoporose suggèrent une supplémentation calcique de 1200 mg à 1500 mg/j associée à une supplémentation en vitamine D3 de 800 UI/j (Grade 2B). Il semble que les produits alimentaires fortifiés (lait, jus d’orange, pain) permettraient de garantir un apport minimum supplémentaire en vitamine D et en calcium., ROLE PLAYED BY VITAMIN D IN THE NUTRITION OF OLDER SUBJECTS The importance of vitamin D in bone metabolism has been established for a long time but currently there is a renewed interest for this vitamin because of its possible role during the ageing process in a certain number of clinical events such as falls, fractures, strength, balance, cognitive disorders, and in the appearance of certain cancers and certain inflammatory diseases. Most literature data reports an inadequacy in vitamin D defined by a concentration in 25 (OH) D lower than 20 in 30 ng / ml and a deficiency defined by a concentration lower than 10 ng / ml. Prevalence of the inadequacy or the deficiency in vitamin D is thus important in the population of older people ; it remains however variable from one study to the other (from 16 to 78%) probably because of the use of different definitions to identify the subjects in inadequacy or deficiency, in the age of the studied population and the sex (analyses realized collectively with men and women, or by sex). Solely the double supplementation calcium + vitamin D seems effective in the prevention of osteoporotic breaks in older subjects. The current recommendations in the treatment of osteoporosis suggest a calcic supplementation of 1,200 mg in 1,500 mg/j associated with a supplementation in vitamin D3 of 800 UI/j (Rank 2B). It seems that fortified health food (milk, orange juice, and bread) could guarantee an additional minimum contribution in vitamin D and in calcium.
