In this article, the authors focus in particular on section 56 of the Social Security Law (LFSS) for 2018, which provides for the possibility for the Health product economic Comity (CEPS) to set a unified tariff for drugs on the list in addition to generic and biosimilar drugs. By imposing the same reimbursement basis for generics as for originators or biosimilar and their reference medicinal product, the LFSS aims to remove the incentive for health establishments to choose to list a more expensive medicinal product in the hope of benefiting from a possible profit-sharing margin. The liability tariff then becomes a selling price limit. In addition, section 56 of the LFSS for 2018 repeals the system of flat-rate reduction of the hospitalization tariff when a drug on the list in addition is used for the purpose, in particular, of simplifying the conditions under which these products are covered. The authors indicate that the extension of the single tariff to all comparable specialties in terms of indications or therapeutic aim is likely to have a significant impact provided that it is known on which criterion comparability should be established., Dans cet article, les auteurs s’intéressent en particulier à l’article 56 de la LFSS pour 2018 qui prévoit la possibilité pour le CEPS de fixer un tarif unifié pour les médicaments de la liste en sus, pour les médicaments génériques et biosimilaires. En imposant la même base de remboursement pour les génériques que les princeps ou que les biosimilaires et leur médicament de référence, la LFSS vise à supprimer l’incitation qu’auraient les établissements de santé à choisir le référencement d’un médicament plus cher dans l’espoir du bénéfice d’une éventuelle marge d’intéressement. Le tarif de responsabilité devient alors un prix limite de vente. Par ailleurs, l’article 56 de la LFSS pour 2018 abroge le dispositif de minoration forfaitaire du tarif d’hospitalisation lorsqu’un médicament de la liste en sus est utilisé dans le but, notamment, de simplifier les conditions de prise en charge de ces produits. Les auteurs indiquent que l’extension du tarif unique à toutes les spécialités comparables en terme d’indications ou de visée thérapeutique est susceptible d’avoir un impact important sous réserve de savoir sur quel critère il convient d’établir la comparabilité.

This article traces the history of care especially with religious hospices that have scattered in Europe in the Middle Ages until the creation of French Palliative Care Support Company in the 1980’s. Then it defines the palliative cares as “the care and support that must be provided whenever a pathological condition threatens life, whether death occurs or can be prevented”. Then it recalls the legal framework in particular with: the law n° 99-477 of June 9th 1999 ensuring the right of access to the palliative cares; the law n° 2002-303 of March 4th 2002 about the patients’ rights and the Health system quality; and the law n° 2016-87 of March 4th 2002 creating new rights in favor of the patients and people at the end of life, and the different national plans implemented., Cet article retrace l’histoire des soins notamment avec les hospices religieux qui ont essaimé en Europe au Moyen-Âge jusqu’à la création de la Société Française d’Accompagnement de Soins Palliatifs dans les années 80. Ils définissent ensuite les soins palliatifs comme « les soins et l’accompagnement qui doivent être mis en œuvre toutes les fois qu’une atteinte pathologique menace l’existence, que la mort survienne ou puisse être évitée. » puis en rappellent le cadre légal notamment avec la Loi n° 99-477 du 9 juin 1999 visant à garantir le droit à l’accès aux soins palliatifs, la Loi n° 2002-303 du 04 mars 2002 relative aux droits des malades et la qualité du système de Santé et la Loi n° 2016-87 du 2 février 2016 créant de nouveaux droits en faveur des malades et des personnes en fin de vie, ainsi que les différents plans nationaux mis en œuvre. L’organisation des soins palliatifs et de la fin de vie est ensuite abordée.

Although gesture recognition has been studied for several decades, much research stays in the realm of indoors laboratory experiments. In this thesis, we address the problem of designing a truly usable, real- world gesture recognition system, focusing mainly on the real-time control of an outdoors robot for use by military soldiers. The main contribution of this thesis is the development of a real-time gesture recognition pipeline, which can be taught in a few minutes with: very sparse input ("small data"); freely user-invented gestures; resilience to user mistakes during training; and low computation requirements. This is achieved thanks to two key innovations: first, a stream-enabled, DTW-inspired technique to compute distances between time series; and second, an efficient stream history analysis procedure to automatically determine model hyperparameters without user intervention. Additionally, a custom, hardened data glove was built and used to demonstrate successful gesture recognition and real-time robot control. We finally show this work's flexibility by furthermore using it beyond robot control to drive other kinds of controllable systems.; Alors que les systèmes de reconnaissance gestuelle actuels privilégient souvent un usage intérieur, nous nous intéressons à la conception d'un système dont l'utilisation est possible en environnement extérieur et en mobilité. Notre objectif est le contrôle temps-réel d'un robot mobile dont l'usage est destiné aux fantassins débarqués. La contribution principale de cette thèse est le développement d'une chaîne de reconnaissance gestuelle temps réel, qui peut être entraînée en quelques minutes avec: un faible nombre d'exemples ("small data"); des gestes choisis par l'utilisateur; une résilience aux gestes mal réalisés; ainsi qu'une faible empreinte CPU. Ceci est possible grâce à deux innovations clés: d'une part, une technique pour calculer des distances entre séries temporelles en flux, basée sur DTW; d'autre part, une rétro-analyse efficace du flux d'apprentissage afin de déterminer les hyperparamètres du modèle sans intervention de l'utilisateur. D'autre part, nous avons construit notre propre gant de données et nous l'utilisons pour confirmer expérimentalement que la solution de reconnaissance gestuelle permet le contrôle temps réel d'un robot en mobilité. Enfin, nous montrons la flexibilité de notre technique en ce sens qu'elle permet de contrôler non seulement des robots, mais aussi des systèmes de natures différentes.

This study investigates the activity and stability of a Prussian blue analogue (PBA) as an inexpensive anode catalyst for Polymer Electrolyte Membrane Water Electrolysis (PEMWE). While some PBAs have recently been reported to catalyze the oxygen evolution reaction (OER) in acidic electrolytes, the present study focuses on their integration in a PEMWE device. Cobalt hexacyanoferrate nanoparticles were interfaced with an electrically conductive support that withstands the PEMWE anodic conditions, namely Sb-doped SnO2. The OER activity of the composite materials was first verified in liquid electrolytes and then in PEMWE. A promising current density of 50–100 mA cm- 2 was reached at 2 V cell voltage. The PBA/Sb– SnO2 anode was stable up to 1.9 V, but showed more and more instability at higher potentials. Increasing leaching rates of Sn and Sb observed above 1.9 V suggest that the material instability above 1.9 V can mainly be assigned to Sb-doped SnO2 conductive support. These results are overall promising for the use of PBAs as catalytic sites at the anode of PEMWE. The study also identifies the need for more active PBAs in order to reach a higher current density at a cell voltage of 1.6–1.9 V, a potential range necessary for an acceptable energy efficiency of the PEMWE.

International audience; Current robotics advances offer consequent potential for assistance, not only for individuals, but also for professionals operating on the ground, such as firemen or military. However, such operators must stay focused on their mission; hence they expect a non-invasive, intuitive control system, disrupting them as least as possible. Furthermore, the inherent mobility arising from such missions (walking, running, jumping) must not be an obstacle to gesture recognition. Here, we present a glove-based gesture detection algorithm, whose learning phase is instantaneous among a large span of possible gestures. It has been evaluated on a seven-word dictionary dedicated for the purpose of robot control, with satisfying recognition results.; Les progrès actuels de la robotique offrent des opportunités considérables , pas seulement pour les particuliers mais également les professionnels en mission, tels que des pompiers ou militaires. Cependant, ce type d'opérateurs doit rester focalisé sur sa mission et attend donc un moyen de contrôle qui soit peu intrusif et tr es intuitif. De plus, il est indispensable de livrer un système qui prend en compte la mobilité intrinsèque à ce type d'activité (marche, course, saut). Nous présentons ici un système de reconnaissance gestuelle à base de gant de données , à l'apprentissage instantané sur une large gamme de gestes possibles. Nous l'avons évalué sur sept gestes dédiés au contrôle d'un robot mobile, avec des résultats tout à fait satisfaisants.
