6. The evolution of lung function in newborn screened preschool children with cystic fibrosis

Abstract

Most morbidity in cystic fibrosis (CF) is due to progressive pulmonary disease. Recently, small molecule therapies targeting the basic defect of the cystic fibrosis transmembrane conductance regulator protein have been developed, and newborn screening (NBS) for CF allows intervention before irreversible lung damage occurs. Unless the evolution of pulmonary function in young children is known, the optimal age of starting new therapies or enrolling CF NBS children into interventional trials is unclear. The London Cystic Fibrosis Collaboration has studied CF NBS infants longitudinally from diagnosis, as well as a contemporaneous group of matched healthy controls, and reported lung function was normal in the NBS CF group at two years of age. This thesis reports the preschool follow-up of the same infants, and describes the evolution of lung function to six years of age. The primary hypothesis was that lung function would remain within the normal range. Secondary aims were to compare preschool pulmonary function to that measured in children diagnosed with CF a decade earlier, and to describe which measures in NBS infants could predict preschool lung function. 67 preschool children with CF and 41 healthy controls underwent multiple breath washout (MBW), specific airway resistance and spirometry measurements. Lung Clearance Index (LCI), measured by MBW, was abnormal in CF NBS preschool children, but was better than children diagnosed with CF a decade earlier. The most significant predictor of abnormal preschool lung function was LCI at two years of age. Contrary to the hypothesis, compared to contemporaneous controls, lung function deteriorates at preschool follow-up in CF NBS children managed with standard UK care. As two year LCI was a significant predictor of later decline, implying that the root causes of decline were before this age, disease modifying therapies and interventional trials should be targeted before two years of age.

18. A scoping review of the literature reporting stress and burnout in specialist nurses: making the case for inflammatory bowel disease specialist nurses

Abstract

Introduction Indicators of stress and burnout were emerging amongst Inflammatory Bowel Disease Clinical Nurse Specialists (IBD-CNSs) prior to the COVID-19 pandemic, which pushed an already stretched workforce beyond capacity. Whilst there is documented evidence of stress and burnout amongst other specialist nurse groups, to our knowledge there is none addressing the situaton of IBD-CNSs. As a precursor to a qualitative study, we aim to deliver a scoping review to address the question: What is the available qualitative evidence of stress and burnout amongst Inflammatory Bowel Disease Clinial Nurse Specialists? Methods and analysis This scoping review will follow the framework recommended by Levac and colleagues (2010). We will search relevant electronic databases (EBSCO host, including Cinahl Plus with full text, Medline, PsycArticles; Web of Science, including Science Citation Index Expanded, Social Science Citation Index, and Arts and Humanities Citation index; Scopus; Pubmed, and OVID), and handsearch reference lists of located studies to identify articles for inclusion. All abstracts and full-text articles will be independently reviewed and screened by two reviewers, and then the final selection will be verified with the team. Search criteria: published since 2010, in English language, full text, primary research. Any study which reports qualitative data and meets the inclusion criteria, will be retained. Following first screening, the full team will independently review each item against the selection criteria, reaching consensus on the final studies for inclusion in the review. Data charting (demographic, study characteristics and key aspects which address the review question) will be completed by the full team, using an iteratively developed data charting tool. Key aspects drawn from the selected studies will be described, grouped and synthesised using a standard qualitative thematic analysis. Outcomes We expect to demonstrate minimal qualitative evidence of stress and burnout across specialist nurses generally, and particularly across IBD-CNSs. The evidence from this review will support a planned qualitative study into the experiences of stress and burnout in IBD-CNSs in the UK, which is expected to produce evidence to inform workforce planning, career development, and clinical support policies for IBD-CNSs. We anticipate one open access publication from this review.

Introduction: Patients with gastrointestinal (GI) cancers have an increased risk of serious complications and death from SARS-CoV-2 infection. The immunogenicity of vaccines in patients with GI cancers receiving anti-cancer therapies is unclear. We conducted a prospective study to evaluate the prevalence of neutralizing antibodies in a cohort of GI cancer patients receiving chemotherapy following SARS-CoV-2 vaccination. Materials and methods: Between September 2020 and April 2021, patients with cancer undergoing chemotherapy were enrolled. At baseline (day 0), days 28, 56, and 84, we assessed serum antibodies to SARS-CoV-2 spike (anti-S) and anti-nucleocapsid (anti-NP) and concomitantly assessed virus neutralization using a pseudovirus neutralization assay. Patients received either the Pfizer/BioNTech BNT162b2, or the Oxford/AstraZeneca ChAdOx1 vaccine. Results: All 152 patients enrolled had a prior diagnosis of cancer; colorectal (n = 80, 52.6%), oesophagogastric (n = 38, 25.0%), and hepato pancreatic biliary (n = 22, 12.5%). Nearly all were receiving systemic anti-cancer therapy (99.3%). Of the 51 patients who did not receive a vaccination prior to, or during the study, 5 patients had detectable anti-NP antibodies. Ninety-nine patients received at least one dose of vaccine prior to, or during the study. Within 19 days following the first dose of vaccine, 30.0% had anti-S detected in serum which increased to 70.2% at days 20-39. In the 19 days following a second dose, anti-S positivity was 84.2% (32/38). However, pseudovirus neutralization titers (pVNT80) decreased from days 20 to 39. Conclusion: Despite the immunosuppressive effects of chemotherapy, 2 doses of SARS-CoV-2 vaccines are able to elicit a protective immune response in patients' ongoing treatment for gastrointestinal cancers. Decreases in pseudoviral neutralization were observed after 20-39 days, re-affirming the current recommendation for vaccine booster doses.

Abstract

Previous studies suggested an association between upper airway pneumococcal colonization density and pneumococcal pneumonia, but data in children are limited. Using data from the Pneumonia Etiology Research for Child Health (PERCH) study, we assessed this potential association.PERCH is a case-control study in 7 countries: Bangladesh, The Gambia, Kenya, Mali, South Africa, Thailand, and Zambia. Cases were children aged 1-59 months hospitalized with World Health Organization-defined severe or very severe pneumonia. Controls were randomly selected from the community. Microbiologically confirmed pneumococcal pneumonia (MCPP) was confirmed by detection of pneumococcus in a relevant normally sterile body fluid. Colonization density was calculated with quantitative polymerase chain reaction analysis of nasopharyngeal/oropharyngeal specimens.Median colonization density among 56 cases with MCPP (MCPP cases; 17.28 × 106 copies/mL) exceeded that of cases without MCPP (non-MCPP cases; 0.75 × 106) and controls (0.60 × 106) (each P.001). The optimal density for discriminating MCPP cases from controls using the Youden index was6.9 log10 copies/mL; overall, the sensitivity was 64% and the specificity 92%, with variable performance by site. The threshold was lower (≥4.4 log10 copies/mL) when MCPP cases were distinguished from controls who received antibiotics before specimen collection. Among the 4035 non-MCPP cases, 500 (12%) had pneumococcal colonization density6.9 log10 copies/mL; above this cutoff was associated with alveolar consolidation at chest radiography, very severe pneumonia, oxygen saturation92%, C-reactive protein ≥40 mg/L, and lack of antibiotic pretreatment (all P.001).Pneumococcal colonization density6.9 log10 copies/mL was strongly associated with MCPP and could be used to improve estimates of pneumococcal pneumonia prevalence in childhood pneumonia studies. Our findings do not support its use for individual diagnosis in a clinical setting.

Abstract

Primary ciliary dyskinesia (PCD) is a rare heterogenous condition that causes progressive suppurative lung disease, chronic rhinosinusitis, chronic otitis media, infertility and abnormal situs. 'Better Experimental Approaches to Treat Primary Ciliary Dyskinesia' (BEAT-PCD) is a network of scientists and clinicians coordinating research from basic science through to clinical care with the intention of developing treatments and diagnostics that lead to improved long-term outcomes for patients. BEAT-PCD activities are supported by EU funded COST Action (BM1407). The second BEAT-PCD conference, and third PCD training school were held jointly in April 2017 in Valencia, Spain. Presentations and workshops focussed on advancing the knowledge and skills relating to PCD in: basic science, epidemiology, diagnostic testing, clinical management and clinical trials. The multidisciplinary conference provided an interactive platform for exchanging ideas through a program of lectures, poster presentations, breakout sessions and workshops. Three working groups met to plan consensus statements. Progress with BEAT-PCD projects was shared and new collaborations were fostered. In this report, we summarize the meeting, highlighting developments made during the meeting.
