2. Long-term placement of lumen apposing metal stent for EUS guided duodeno and jejuno-jejunal anastomosis for direct access to excluded jejunal limb

- Abstract
Background: Management of biliary disorders in patients with altered anatomy may be challenging. EUS-guided gastrointestinal anastomosis using lumen apposing metal stent (LAMS) have been introduced to allow endoscopic retrograde cholangiography (ERC) in such cases. However the appropriate stent indwelling time is still uncertain. We report long-term LAMS deployment after duodeno-jejunal or jejuno-jejunal anastomosis (EUS-DJA) to allow endoscopic re-interventions in case of recurrences. Methods: 11 consecutive patients underwent to EUS-DJA with long standing LAMS between January 2017 and December 2018. ERC treatment was carried out over a 12 months period with multiple endoscopic sessions across DJA. Results: Technical success was 91% (10/11 pts) for EUS-DJA and 100% for ERC. Four patients presented stricture recurrence after an average of 489±31.7 days from the end of ERC treatment. A novel ERC across LAMS anastomosis was feasible in all cases. At average of 781 days±253 all LAMS are still in place with no evidence of any adverse event. Conclusion: Long-term LAMS placement after EUSDJA may be feasible and safe in order to maintain a direct access to the excluded limb.

En régime per ascensum, des solutions calcaires incrustantes circulent dans des colonnes expérimentales de sable. Dans ces conditions, le phénomène de précipitation se localise dans certaines couches de sable. Une loi de diffusion du gaz carbonique en milieu poreux insaturé d'eau est utilisée pour rendre compte de ce phénomène. Le flux d'évaporation qui contrôle les débits d'ascension capillaire decroît dans le temps par paliers successifs. Ce phénomène discontinu est en partie responsable de l'étagement des niveaux d'accumulation distincts observés. Experiments were performed on supersaturated calcium-carbonate solutions flowing through sand columns. Under such conditions, crystallization of calcium carbonate takes place at specific levels in the sand column. A diffusion law for gaseous carbon dioxide through unsaturated porous media was determined to account for this phenomenon. During the entire experiment, the evaporation rates controlling upward capillary flow decreased discontinuously in successive steps. This discontinuons phenomenon was partially responsible for the discrete limy layers that were observed.

Context. Until recently, camera networks designed for monitoring fireballs worldwide were not fully automated, implying that in case of a meteorite fall, the recovery campaign was rarely immediate. This was an important limiting factor as the most fragile – hence precious – meteorites must be recovered rapidly to avoid their alteration. Aims. The Fireball Recovery and InterPlanetary Observation Network (FRIPON) scientific project was designed to overcome this limitation. This network comprises a fully automated camera and radio network deployed over a significant fraction of western Europe and a small fraction of Canada. As of today, it consists of 150 cameras and 25 European radio receivers and covers an area of about 1.5 × 106 km2. Methods. The FRIPON network, fully operational since 2018, has been monitoring meteoroid entries since 2016, thereby allowing the characterization of their dynamical and physical properties. In addition, the level of automation of the network makes it possible to trigger a meteorite recovery campaign only a few hours after it reaches the surface of the Earth. Recovery campaigns are only organized for meteorites with final masses estimated of at least 500 g, which is about one event per year in France. No recovery campaign is organized in the case of smaller final masses on the order of 50 to 100 g, which happens about three times a year; instead, the information is delivered to the local media so that it can reach the inhabitants living in the vicinity of the fall. Results. Nearly 4000 meteoroids have been detected so far and characterized by FRIPON. The distribution of their orbits appears to be bimodal, with a cometary population and a main belt population. Sporadic meteors amount to about 55% of all meteors. A first estimate of the absolute meteoroid flux (mag < –5; meteoroid size ≥~1 cm) amounts to 1250/yr/106 km

Introduction: Treatment of pancreato-biliary disorders after gastric bypass is challenging due to altered anatomy. Several techniques have been proposed to overcome this condition; however, none has emerged as the gold standard treatment. Furthermore, a decision-making algorithm evaluating when and why apply one technique over another is still lacking. Objectives: To describe a novel trans-gastric approach to allow endoscopic retrograde cholangiopancreatography (ERCP) in Roux-en-Y gastric bypass (RYGB) anatomy soon after prior laparoscopic cholecystectomy (LC) and to propose a decision-making algorithm for selection of the most suitable technique according a tailored approach. Setting: Private hospital. Methods: Between January and March 2020, patients with Roux-en-Y gastric bypass anatomy referred to our tertiary center to undergo ERCP after recent laparoscopic cholecystectomy were retrospectively evaluated. A 20 french (Fr) gastrostomy was performed during cholecystectomy. A single-stage ERCP was carried out by means of temporary trans-gastric stent deployment over a 20 Fr gastrostomy. Results: A total of 5 patients (mean age 41; mean body mass index 48.3) were enrolled. ERCP was performed after an average of 2 days from surgery. Technical and clinical success was achieved in 100%. No adverse events occurred. Spontaneous closure of the gastrostomy after its bedside removal was observed in all cases. Conclusions: Our approach allows to perform a single-stage ERCP in RYGB patients, early after LC, with no need of any other re-interventions. Any surgeon facing unexpected biliary disorders, during LC, can easily perform a 20 Fr gastrostomy thus allowing the patient to undergo early ERCP without any delay.

Background Due to the large number of laparoscopic sleeve gastrectomy (LSG) performed over the last decade, the management of the leak following LSG has been increasingly reported. The role of covered Self Expandable Metal Stents (cSEMS) for the treatment of the leak is still controversial because of the poor tolerance and high risk of complications. Objectives The aim of the present study was to analyze the foregut wall perforation and aorta injuries, a very rare but potentially fatal complication, related to the treatment of the leak following LSG using cSEMS. Setting Private hospital, France. Methods An audit was conducted in 2 French tertiary bariatric endoscopic centers focusing on aortic injuries after cSEMS use for leak. We examined and classified the initial procedure, leak characteristics, primary endoscopic treatment, and outcome of endoscopic complication for each eligible case. Results A total of 5 patients were identified with foregut wall perforation and aorta injuries. All stents were deployed for staple line leak following LSG. The recorded mortality in case of esophageal-aortic injuries related to cSEMS use was 80%. Conclusion cSEMS are potentially effective tools for the management of foregut leaks in bariatric surgery. The biggest challenges with this approach are stent migration and poor quality of life. Caution is required due to the risk of fatal complications such as foregut wall perforation and aortic injury.

Context. Until recently, camera networks designed for monitoring fireballs worldwide were not fully automated, implying that in case of a meteorite fall, the recovery campaign was rarely immediate. This was an important limiting factor as the most fragile - hence precious - meteorites must be recovered rapidly to avoid their alteration. Aims. The Fireball Recovery and InterPlanetary Observation Network (FRIPON) scientific project was designed to overcome this limitation. This network comprises a fully automated camera and radio network deployed over a significant fraction of western Europe and a small fraction of Canada. As of today, it consists of 150 cameras and 25 European radio receivers and covers an area of about 1.5 × 10km. Methods. The FRIPON network, fully operational since 2018, has been monitoring meteoroid entries since 2016, thereby allowing the characterization of their dynamical and physical properties. In addition, the level of automation of the network makes it possible to trigger a meteorite recovery campaign only a few hours after it reaches the surface of the Earth. Recovery campaigns are only organized for meteorites with final masses estimated of at least 500 g, which is about one event per year in France. No recovery campaign is organized in the case of smaller final masses on the order of 50 to 100 g, which happens about three times a year; instead, the information is delivered to the local media so that it can reach the inhabitants living in the vicinity of the fall. Results. Nearly 4000 meteoroids have been detected so far and characterized by FRIPON. The distribution of their orbits appears to be bimodal, with a cometary population and a main belt population. Sporadic meteors amount to about 55% of all meteors. A first estimate of the absolute meteoroid flux (mag < -5; meteoroid size ≥∼1 cm) amounts to 1250/yr/10km. This value is compatible with previous estimates. Finally, the first meteorite was recovered in Italy (Cavezzo, January 2020) thanks to the PRISMA network, a component of the FRIPON science project., FRIPON was initiated by funding from ANR (grant N.13- BS05-0009-03), carried by the Paris Observatory, Muséum National d’Histoire Naturelle, Paris-Saclay University and Institut Pythéas (LAM-CEREGE). Background Endoscopy plays a pivotal role in the management of adverse events (AE) following bariatric surgery. Leaks, fistulae, and post-operative collection after sleeve gastrectomy (SG) may occur in up to 10% of cases. Objectives To evaluate the efficacy and safety of endoscopic internal drainage (EID) for the management of leak, fistula, and collection following SG. Setting Retrospective, observational, single center study on patients referred from several bariatric surgery departments to an endoscopic referral center. Methods EID was used as first-line treatment for the management of leaks, fistulae, and collections. Leaks and fistulae were treated with double pigtail stent (DPS) deployment in order to guarantee internal drainage and second intention cavity obliteration. Collections were treated with endoscropic ultrasound (EUS)–guided deployment of DPS or lumen apposing metal stents. Results A total of 617 patients (83.3% female; mean age, 43.1 yr) were enrolled in the study for leak (n = 300, 48.6%), fistula (n = 285, 46.2%), and collection (n = 32, 5.2%). Median follow-up was 19.5 months. Overall clinical success was 84.7% whereas 15.3% of cases required revisional surgery after EID failure. Clinical success according to type of AE was 89.5%, 78.5%, and 90% for leak, fistula, and collection, respectively. A total of 10 of 547 (1.8%) presented a recurrence during follow-up. A total of 28 (4.5%) AE related to the endoscopic treatment occurred. At univariate logistic regression predictors of failure were: fistula (OR 2.012), combined endoscopic approach (OR 2.319), need for emergency surgery (OR 1.755), and previous endoscopic treatment (OR 4.818). Conclusion Early EID for the management of leak, fistula, and post-operative collection after SG seems a safe and effective first-line approach with good long-term results.

Background: Submucosal tumors (SMTs) of the gastrointestinal tract are a rare pathological entity comprising a wide variety of neoplastic and non-neoplastic lesions. Even if most SMTs are benign tumors (e.g., leiomyomas), a smaller portion may have a malignant potential (e.g., gastrointestinal stromal tumor (GIST)). Preoperative diagnosis of SMT in bariatric patients may arise challenging clinical dilemmas. Long-term surveillance may be difficult after bariatric surgery. Moreover, according to SMT location, its presence may interfere with planned surgery. Submucosal tunneling endoscopic resection (STER) has emerged as an effective approach for minimally invasive en bloc excision of SMTs. This is the first case series of STER for SMTs before bariatric surgery. Methods: Seven female patients underwent STER for removal of SMTs before bariatric surgery. All lesions were incidentally diagnosed at preoperative endoscopy. STER procedural steps comprised mucosal incision, submucosal tunneling, lesion enucleation, and closure of mucosal defect. Results: En bloc removal of SMT was achieved in all cases. Mean procedural time was of 45 min (SD 18.6). No adverse event occurred. Mean size of the lesions was 20.6 mm (SD 5.8). Histological diagnoses were 5 leyomiomas, 1 lipoma, and 1 low grade GIST. Bariatric procedure was performed after a mean period of 4.1 months (SD 1.6) from endoscopic resection. Conclusion: STER is a safe and effective treatment for the management of SMT even in bariatric patients awaiting surgery. Preoperative endoscopic resection of SMTs has the advantages of reducing the need for surveillance and removing lesions that could interfere with planned surgery. STER did not altered accomplishment of bariatric procedures.

COVID-19 (Coronavirus disease 2019) caused by SARS-CoV-2 has become a global pandemic. Obesity is a risk factor for severe disease, and the practice of endoscopy poses special challenges and risks of SARS-CoV-2 transmission to patients and providers given the evolving role of the gastrointestinal tract in viral transmission and aerosol generation during endoscopic procedures. It is therefore necessary to distinguish between urgent interventions that cannot be postponed despite the risks during the pandemic and, in contrast, purely elective interventions that could be deferred in order to minimize transmission risks during a time of infection surge and limited access. Semi-urgent bariatric procedures have an intermediate position. Since the chronological course of the pandemic is still unpredictable, these interventions were defined according to whether or not they should be performed within a nominal 8-week period. In this position statement, the IFSO Endoscopy Committee offers guidance on navigating bariatric endoscopic procedures in patients with obesity during the COVID-19 pandemic, in the hope of mitigating the risk of SARS-CoV-2 transmission to vulnerable patients and healthcare workers. These recommendations may evolve as the pandemic progresses.

Background: Rapid weight loss after bariatric surgery is associated with a high prevalence of gallstone formation. In laparoscopic Roux-en-Y gastric bypass (RYGBP), the bypassed segment is not readily available for endoscopic or radiographic examination. We propose a laparoscopic Janeway gastrostomy for secondary access to excluded structures in bariatric centers with no mandatory technical equipment in endoscopic retrograde cholangiopancreatography (ERCP), double-balloon ERCP or spiral enteroscopy., Method: This was a single-institution retrospective review of a prospectively collected database of patients with a history of laparoscopic RYGBP who underwent laparoscopic Janeway gastrostomy for duodenal and biliary access. The operative indications, technical aspects, endoscopic findings, outcomes, and complications were investigated., Results: Five patients with a history of RYGBP underwent laparoscopic Janeway gastrostomy for exploration of the bypassed segment. All of them had biliary pathology, and all underwent successful ERCP and papillotomy. The gastrostomies were closed secondarily. The mean duration of hospitalization was 12 days. No complications developed. All procedures were performed laparoscopically., Conclusion: If access to excluded structures and simultaneous ERCP was not possible, temporary laparoscopic Janeway gastrostomy could be the last option alternative for a staged ERCP to gain access to the bypassed structures. It is a feasible and safe solution for the exploration and treatment of patients with a history of RYGBP in bariatric centers that have no endoscopists with expertise in ERCP., (Copyright © 2017 Elsevier Masson SAS. - Abstract
Although celioscopic cholecystectomy provides acknowledged advantages, lesions of the common bile duct appear to be more frequent with this technique than with laparotomy. Two cases of annular stenosis of the common bile duct after celioscopic cholecystectomy are presented. Evidence of their ischemic origin is available, and they lead to reflexions on the use of monopolar coagulation. In both cases, the stenosis was repaired surgically, with resection and anastomosis in one case and with a hepaticojejunal anastomosis in the other. Laying an endoprosthesis as used in case 2 seems to be interesting as a temporary solution to prepare for surgery.

- Abstract
Acalculous cholecystitis and sclerosing cholangitis due to Cryptosporidium sp, and cytomegalovirus have been described in patients with the acquired immunodeficiency syndrome (AIDS). However, in about 40% of cases of AIDS-related biliary disease, no opportunistic pathogen is identified. The current case report describes the first case, to the best of the authors' knowledge, of AIDS-related sclerosing cholangitis associated with microsporidiosis. Enterocytozoon bieneusi was detected in the duodenum and bile by means of light microscopy and confirmed by electron microscopy. Microsporidian infection should be suspected in patients with AIDS-related sclerosing cholangitis as well as in cases of diarrhea in which none of the usual pathogens are found.

- Abstract
The authors report a case of surgical ligation of the common bile duct and discuss the retrograde and transhepatic cholangiographic features. The stenosis was more extensive than suggested by the cause of the obstruction. The transhepatic insertion of a balloon catheter demonstrated the ligature by showing a zone of concentric constriction. The ligature was unable to be ruptured by means of an angioplasty catheter, which would nevertheless appear to be a dangerous technique.

Sixty-four gallstone patients aged 75 or more (mean age 83 +/- 5.1 years) were divided prospectively into two groups. They were mostly high-risk patients (average number of major risk factors, 2.2 +/- 0.9). Thirty-three showed one or several signs of lithiasis of the common bile duct and were treated with endoscopic sphincterotomy (ES) (31 successful cases, two technical problems) followed by early cholecystectomy (33 cases). Choledocholithiasis was present in 26 cases and stones were extracted in 25 cases. Two patients (6 per cent) died. ES caused no complications. Thirty-one other patients showed no sign of choledocholithiasis and were treated by cholecystectomy with operative cholangiography. Choledocholithiasis was found in two of these patients and treated by extraction and external drainage. Five of these patients (16 per cent) died. In 30 cases acute cholecystitis was found at operation, 15 in each group. ES is therefore an efficient procedure in high-risk patients, which facilitates operation, especially in cases of acute cholecystitis, and it is recommended in all cases of complicated biliary lithiasis. Early cholecystectomy is justified for these patients by the high frequency of associated acute cholecystitis.
