Amaç: Bu çalışmanın amacı kanser hastalarında koronavirus hastalığı 2019 (coronavirus disease 2019, COVID-19) bilgisayarlı tomografi (BT), klinik ve laboratuvar bulgularının değerlendirilmesi ve polimeraz zincir reaksiyonu (polymerase chain reaction, PCR) pozitif ve negatif hastaların bulgularının karşılaştırılmasıdır.Gereç ve Yöntemler: Çalışmaya PCR testi pozitif olan 23, klinik ve radyolojik bulgularla COVID-19 tanısı almış 22 kanser hastası alındı. Hastaların BT görüntüleri iki radyolog tarafından eşzamanlı değerlendirildi. Komorbid hastalık varlığı, semptomlar ve laboratuvar değerleri değerlendirildi.Bulgular: En sık BT tutulum paterni %88,9 (n=40) ile periferaldi. Bilateral akciğer tutulumu oranı %57,8 (n=26) idi. En sık saptanan bulgu buzlu cam dansiteleri idi (n=38, %84,5). Bunların %35,5 (n=16)’ine konsolidasyon da eşlik etmekteydi. Vakaların %62,2 (n=28)’sinde multifokal tutulum mevcuttu. En sık tutulan loblar alt loblar idi. Diğer nispeten sık bulgular septal kalınlaşma, subplevral çizgilenme ve hava bronkogramı idi. Hastaların ortanca nötrofil, lenfosit, D-dimer, prokalsitonin, C-reaktif protein ve laktat dehidrogenaz değerleri sırasıyla 2000 mm3, 1200 mm3, 1990 ng/mL, 30.7 µg/L 15.8 mg/dl, 161 IU/L idi.Sonuç: Multifokal ve bilateral tutulum ile buzlucam dansiteleri en sık bulgulardı. Ancak genelde daha az saptanan septal kalınlaşmanın yüksek saptanması kanser hastalarında bulguların daha ciddi olabileceğini düşündürmektedir. PCR negatif vakalarda inflamatuvar markerların birçoğu daha yüksekti. Çok merkezde daha fazla hasta ile yapılacak çalışmalar kanser hastalarındaki bulguları genel popülasyonla daha iyi karşılaştırmayı sağlayacaktır., Aim: The aim of this study was to evaluate the computed tomography (CT), clinical and laboratory findings of coronavirus disease 2019 (COVID-19) in cancer patients and to compare the findings between polymerase chain reaction (PCR) positive and negative patients.Material and Methods: Twenty-three cancer patients with positive PCR tests and 22 diagnosed as COVID-19 with clinical and radiological findings were included in the study. CT images of the patients were evaluated simultaneously by two radiologists. Presence of comorbid diseases, symptoms and laboratory values were evaluated.Results: The most common CT involvement pattern was peripheral with 88.9% (n=40). Bilateral lung involvement rate was 57.8% (n=26). The most common finding was ground glass opacities (n=38, 84.5%). 35.6% (n=16) of these were accompanied by consolidation. Multifocal involvement was present in 62.2% (n=28) of the cases. The most frequently involved lobes were lower lobes. Other relatively common findings were septal thickening, subpleural streaking, and air bronchogram. The median neutrophil, lymphocyte, D-dimer, procalcitonin, C-reactive protein and lactate dehydrogenase values of the patients were 2000 mm3, 1200 mm3, 1990 ng/mL, 30.7 mcg/L 15.8 mg/dl, 161 IU/L, respectively.Conclusion: Multifocal and bilateral involvement, and ground glass opacities were the most common findings. However, higher rates of septal thickening, which is generally less common, suggest that the findings may be more severe in cancer patients. Most of the inflammatory markers were higher in PCR negative cases. Studies with more patients in multiple centers will provide better comparison of the findings in cancer patients with the general population.

Purpose: Assessment of HER-2 has become of great importance following the advent of HER-2 antibody treatment. Use of trastuzumab therapy depends on the assessment of HER-2 via standard immunohistochemistry (IHC) assays for protein overexpression or via fluorescence in situ hybridization (FISH) for gene amplification. The aim of this study was to determine the rate of discordance between FISH and IHC in our hospital. The rate of discordance varies from hospital to hospital, and due to certain staff, technical, and biological reasons, IHC and FISH may not be concordant in every patient. It is important to determine the rate of discordance between these 2 methods and attempt to lower it. In addition to the rate of discordance, we also investigated the reasons for the discordance and what affect it has on the progression of the disease and treatment. Material and Methods: A total of 79 patients with breast cancer were assessed for HER-2 protein expression via IHC and HER-2 gene amplification via FISH. Results: Among these 79 patients, 47 (59.5%) were FISH (-) and 32 (40.5%) were FISH (+), whereas FISH was negative in 8 (10.4%) patients that were HER-2 (3+) and FISH was positive in 9 (11.4%) patients that were HER-2 (1+). The discordance rate was 21.5%. Conclusion: The clinical implication of this discordance rate and it's affect on the choice of appropriate therapy are of critical importance. As such, it would be beneficial for each hospital to determine its own discordance rate., Amaç: HER-2"nin değerlendirilmesi, HER-2 antikor tedavisinin takibinde büyük öneme sahiptir. Transtuzumab tedavisinde HER-2"nin değerlendirilmesi protein aşırı üretiminin standart immünohistokimyasal yöntemler ile veya gen amplifikasyonunun FISH yöntemi ile belirlenmesine bağlıdır. Bu çalışmanın amacı hastanemizde kullanılan bu iki yöntem arasındaki diskordans oranını incelemektir. Diskordans oranı hastaneler arasında farklılık göstermektedir, bu farklılık çalışanlara, teknik nedenlere ve biyolojik nedenlere bağlı olabilmektedir. Dolayısıyla bu iki metod arasındaki diskordans oranını belirlemek ve bu oranı minimize önemlidir. Bunlara ek olarak, diskordansın sebeplerini, hastalığın ve tedavinin progresyonu üzerine nasıl etki ettiğini araştırdık. Materyal ve Metod: Meme kanseri tanısı alan toplam 79 hastada HER-2 protein ekspresyonu immunohistokimyasal, HER-2 gen amplifikasyonu ise FISH yöntemleriyle incelendi. Bulgular: İncelenen 79 hastadan 47 sinde (%59.5) FISH (-) 32 sinde (%40.5) FISH (+), öte yandan FISH negatif olan 8 (%10.4) hasta için HER-2 (3+) ve FISH pozitif 9 hasta (%11.4) içinde HER-2 (1+) bulundu. Diskordans oranı %21.5 tir. Sonuç: Diskordans oranının klinik implikasyonu uygun tedavi prosedürünü seçmek için önemlidir, bu yaklaşım hastanelerin kendi diskordans oranlarını belirlemeleri için faydalı olabilir.

Purpose: Behçet"s disease (BD) is a vasculitis of unknown origin. In autoimmune and vasculitic disorders the risk of malignancies has been extensively studied. In BD patients more than hundred cases associated with malignancies have been published. However, the direct relationship with BD has not been proved. We aimed to to investigate the incidence of malignancies in BD patients Material and Methods: Here in we assessed 105 cases retrospectively between 2005-2010 with BD diagnosed according to International Study Criteria (ISG) . The patient"s data were reviewed for development of histopathologically confirmed malignancy Results: One of our 105 patients with BD was found to have solid tumor in the rate of 0.9 %. The remaining 104 patients with BD were in follow up without any malignancy in this period. The malignancy that we reported was invasive right breast carcinoma in a 55-year-old woman with BD. A radically modified right mastectomy and axillary lymphadenectomy were performed and postoperative doxorubicin, cyclophosphamide therapy for four cycles and hormonal therapy with tamoxifen and leuprolide acetate was administered as adjuvant therapy. This patient had a history of 16 years colchicine usage as medication for BD. Conclusion:The rheumatologic diseases can predispose malignancy, the autoimmune nature of BD or the immunosupressive medicines could be possible causes of this carcinoma. However, in our study only one patient had malignancy. The another point of view; management and also prevention of cancer with BD is important entity., Amaç: Behçet Hastalığı vaskülitik bir hastalıktır. Behçet Hastalığı ile kanser birlikteliği çok sık değildir. Biz çalışmamızda Behçet Hastalığı kanser ilşkisini araştırdık. Hastalar ve Yöntem: 2005-2010 yılları arasında takip edilmiş 105 Behçet Hastası ça lışmaya alındı. Histopatolojik olarak konfirme edilmiş malignensisi olan hastalar belirlendi. Bulgular: Bu takip süresinde klavuzlara uygun olarak düzenli taramaları yapılan hastalardan sadece birinde malignensi gözlendi. Bu hasta 55 yaşında meme kanseri tanısı aldı ve 16 yıl kolşisin kullanımı mevcuttu. Meme kanserine yönelik olarak modifiye radikal mastektomi ve 4 kür doksorubisin; siklofosfamid kombinasyonu uygulandıktan sonra tedaviye Tamoksifen ve LHRH anologu ile devam edildi. Sonuç: Bazı romatolojik hastalıklar kanser gelişimine predispozisyon yaratabilir. Hastalığın natürü ve kullanılan immunsupresif ilaçlar bundan sorumlu olabilir. Bizim çalışmamızda direk böyle bir ilişki gösterilemedi. Ama diğer bir yandan önemle vurgulanması gereken Behçet Hastalığı ile malignensi birlikteliğinde her 2 hastalığın birlikte dikkatlice yönetimidir.

Kanser hastalarında tromboz çok sayıda nedene bağlı olarak gelişebilmektedir. Kanserhastalarında tromboz genel populasyona göre çok daha fazladır. Kemoterapi ilişkili arteryaltromoz çok nadiren rapor edilmiştir. Kanser hastalığı hiperkoagulibiteye neden olan birdurumdur. Sisplatin bu yan etkiden en çok sorumlu tutulan ajandır. Sisplatinin bu vaskuler olayınasıl tetiklediği bilinmemektedir. Endotelyal hasarlanma temel rol oynayan faktör olarakgözlenmektedir. Diğer sorumlu tutulan hipotezler plazmada sisplatinin indüklediği vonWillebrand faktör yüksekliği, pıhtılaşma kaskadının aktivasyonu, tromboksan prostosiklinhomostatik bozukluğu ve fibroblast stimulasyonudur. Bu vaka dolayısıyla kapesitabin, sisplatin,trastuzumab kombinasyonunun nadir yan etkisi sunulmuştur, The increased risk of thrombosis in patients with active cancer has multiple causes. Althoughvenous thrombosis is more common in patients with cancer than in the general population,chemotherapy-induced arterial thrombosis rarely has been reported. Patients with cancer are ina hypercoagulable state. Cisplatin seems the most responsible from this adverse event. Themechanism by which cisplatin triggers vascular events is unknown, but endothelial damageseems to play a major role. Other proposed hypotheses are cisplatin-induced elevation of plasmalevels of von Willebrand factor, alteration of clotting cascade, thrombaxane–prostacyclinhomeostatic disturbances, and stimulation of fibroblasts.
