21. Predict the Effects of Dolutegravir (DTG) Plus Lamivudine (3TC) on Immunological Responses in People Living with HIV (PLWHIV)

Background: Immune recovery in people living with HIV (PLWHIV) is a residual aspect of antiretroviral treatment (ART) in most patients, but in a non-negligible proportion of them, the CD4+ lymphocytes count, or CD4/CD8 ratio remains suboptimal. Methods: We performed a model of the immune response after 24 weeks of switching to a 2DR with DTG plus 3TC in a retrospective multicenter cohort of undetectable and experienced patients using significant predictor variables associated with the parameters or situations defined as success and failure. Clinical variables studied were CD4+ and CD8+ lymphocyte count, percentage of CD4, and CD4/CD8 ratio. These parameters were assessed at baseline and 24 weeks after the switch. Based on the evolution of each variable, four categories of immune response and four categories of non-immune response were defined. Immune response was defined as CD4+ count > 500 cells/mm3, %CD4 > 30%, CD8+ count < 1000 cells/mm3 and CD4/CD8 ratio ≥ 0.9. Non-response is just the opposite. Results: In our different models of immunological response, the presence of stage of AIDS (p = 0.035, p = 0.065) and current age over 50 years (p = 0.045) are postulated as statistically significative limiting factors in achieving an improvement in CD4, %CD4, CD8, and CD4/CD8 ratio. Late HIV diagnosis (p = 0.156), without statistical significance, enhanced late the previous variables. In contrast, conditions where patients start with CD4 > 500 cells/mm3 (p = 0.054); CD4 > 30% (p = 0.054, p = 0.084); CD8 < 1000 cells/mm3 (p = 0.018), and CD4/CD8 ≥ 0.9 (p = 0.013, p = 0.09) are detected as stimulating or conducive to DTG plus 3TC treatment success. Conclusion: These models represent a proof of concept that could become a valuable tool for clinicians to predict the effects of DTG plus 3TC on immunological responses prior to the switch in undetectable pre-treated PLWHIV with immune dysfunction. - Abstract
Spain has a new Law on International Cooperation for Sustainable Development and Global Solidarity. This legislation gives the desired commitment to allocate 0.7% of Gross National Income to Official Development Assistance by 2030 the status of a domestic law. The article analyses this regulation in terms of legal reflections and in comparison, with the previous law. First, the origin of the provision is developed, and its legal approach is considered. The British precedent and the precedents in Spain are reviewed, and then the draft bill, its respective parliamentary debate and the prospects for its application are addressed, about the implications that this new regulation could generate, both in the geopolitical aspect by fulfilling commitments assumed in both United Nations and European Union forums, as well as in the aspect of justiciability., España cuenta con la nueva Ley de cooperación internacional para el desarrollo sostenible y la solidaridad global. Este ordenamiento jurídico le da el rango de norma interna al anhelado compromiso del destinar el 0,7% de la Renta Nacional Bruta a la Ayuda Oficial al Desarrollo en el horizonte 2030. El artículo analiza esta disposición en la línea de las reflexiones jurídicas y en comparación en relación con la anterior Ley. Primeramente, se desarrolla el origen de la disposición y se plantea su enfoque jurídico. Se revisa el precedente británico y los antecedentes en España, para después abordar el anteproyecto de Ley, su respectivo debate parlamentario y las perspectivas vislumbradas en su aplicación respecto de las implicaciones que esta nueva norma podría generar, tanto en el aspecto geopolítico al cumplimentar compromisos asumidos tanto en foros de Naciones Unidas como de la Unión Europea, así como en el aspecto de la justiciabilidad., L’Espagne s’est dotée d’une nouvelle Loi sur la Coopération Internationale pour le Développement durable et la Solidarité Mondiale. Ce système juridique confère à l’engagement souhaité d’allouer 0,7 % du revenu national brut à l’aide publique au développement pour 2030, le statut de loi nationale. L’article analyse cette disposition sous l’angle de la réflexion juridique et en comparaison avec la loi précédente. Tout d’abord, l’origine de la disposition est développée et son approche juridique est soulevée. Le précédent britannique et les précédents espagnols sont examinés, puis le projet de loi, son débat parlementaire respectif et les perspectives de son application sont abordés, au regard des implications que cette nouvelle réglementation pourrait générer, tant sur le plan géopolitique en remplissant les engagements pris dans les forums des Nations Unies et de l’Union européenne, que sur le plan de la justiciabilité.

- Abstract
[Background] Fosfomycin is a potentially attractive option as step-down therapy for bacteraemic urinary tract infections (BUTI), but available data are scarce. Our objective was to compare the effectiveness and safety of fosfomycin trometamol and other oral drugs as step-down therapy in patients with BUTI due to MDR Escherichia coli (MDR-Ec)., [Methods] Participants in the FOREST trial (comparing IV fosfomycin with ceftriaxone or meropenem for BUTI caused by MDR-Ec in 22 Spanish hospitals from June 2014 to December 2018) who were stepped-down to oral fosfomycin (3 g q48h) or other drugs were included. The primary endpoint was clinical and microbiological cure (CMC) 5–7 days after finalization of treatment. A multivariate analysis was performed using logistic regression to estimate the association of oral step-down with fosfomycin with CMC adjusted for confounders., [Results] Overall, 61 patients switched to oral fosfomycin trometamol and 47 to other drugs (cefuroxime axetil, 28; amoxicillin/clavulanic acid and trimethoprim/sulfamethoxazole, 7 each; ciprofloxacin, 5) were included. CMC was reached by 48/61 patients (78.7%) treated with fosfomycin trometamol and 38/47 (80.9%) with other drugs (difference, −2.2; 95% CI: −17.5 to 13.1; P = 0.38). Subgroup analyses provided similar results. Relapses occurred in 9/61 (15.0%) and 2/47 (4.3%) of patients, respectively (P = 0.03). The adjusted OR for CMC was 1.11 (95% CI: 0.42–3.29, P = 0.75). No relevant differences in adverse events were seen., [Conclusions] Fosfomycin trometamol might be a reasonable option as step-down therapy in patients with BUTI due to MDR-Ec but the higher rate of relapses would need further assessment.

- Abstract
[EN] Background. Antibiotic resistance in Gram-negative bacilli poses a serious problem for public health. In hospitals, in addition to high mortality rates, the emergence and spread of resistance to practically all antibiotics restricts therapeutic options against serious and frequent infections. Objectives. The aim of this work is to present the views of a group of experts on the following aspects regarding resistance to antimicrobial agents in Gram-negative bacilli: 1) the current epidemiology in Spain, 2) how it is related to local clinical practice and 3) new therapies in this area, based on currently available evidence. Methodology. After reviewing the most noteworthy evidence, the most relevant data on these three aspects were presented at a national meeting to 99 experts in infectious diseases, clinical microbiology, internal medicine, intensive care medicine, anaesthesiology and hospital pharmacy. Results and conclusions. Subsequent local debates among these experts led to conclusions in this matter, including the opinion that the approval of new antibiotics makes it necessary to train the specialists involved in order to optimise how they use them and improve health outcomes; microbiology laboratories in hospitals must be available throughout a continuous timetable; all antibiotics must be available when needed and it is necessary to learn to use them correctly; and the Antimicrobial Stewardship Programs (ASP) play a key role in quickly allocating the new antibiotics within the guidelines and ensure appropriate use of them., [ES] Contexto. La resistencia a los antibióticos en bacilos gramnegativos representa un grave problema de salud pública. En el hospital, además de unas elevadas tasas de mortalidad, la aparición y propagación de resistencias a la práctica totalidad de los antibióticos limita las opciones terapéuticas frente a infecciones graves y frecuentes. Objetivos. Este trabajo tiene por objetivo dar a conocer la visión de un grupo de expertos en los siguientes aspectos respecto a la resistencia a agentes antimicrobianos en bacilos gramnegativos: 1) la epidemiología actual en España, 2) su relación con la práctica clínica local y 3) las novedades terapéuticas en este ámbito, fundamentada en la evidencia actualmente disponible. Metodología. Tras la revisión de la evidencia más destacada, los datos más relevantes de estos 3 aspectos fueron presentados en una reunión nacional ante 99 expertos en enfermedades infecciosas, microbiología clínica, medicina interna, medicina intensiva, anestesiología y farmacia hospitalaria. Resultados y conclusiones. De debates locales posteriores entre estos expertos se extrajeron conclusiones al respecto entre las que se destacan que la aprobación de nuevos antibióticos hace necesaria la formación de los especialistas implicados para optimizar su uso y mejorar los resultados en salud; los laboratorios de Microbiología de los hospitales deben estar disponibles en horario continuado; todos los antibióticos deben estar disponibles para cuando sean necesarios y se debe aprender a usarlos de forma correcta; y los Programas de Optimización del Uso de Antimicrobianos (PROA) desempeñan una labor clave en ubicar de forma ágil los nuevos antibióticos en las guías y asegurar un uso apropiado de los mismos.

In contrast, conditions where patients start with CD4 > 500 cells/mm3 (p = 0.054); CD4 > 30% (p = 0.054, p = 0.084); CD8 < 1000 cells/mm3 (p = 0.018), and CD4/CD8 ≥ 0.9 (p = 0.013, p = 0.09) are detected as stimulating or conducive to DTG plus 3TC treatment success. Conclusion: These models represent a proof of concept that could become a valuable tool for clinicians to predict the effects of DTG plus 3TC on immunological responses prior to the switch in undetectable pre-treated PLWHIV with immune dysfunction. The main predictors for immunological failure were late HIV diagnosis, stage of AIDS, and current age over 50 years. In contrast, starting with a normalized immune status was detected as stimulating or conducive to DTG plus 3TC treatment success.

- Abstract
[Importance] The consumption of broad-spectrum drugs has increased as a consequence of the spread of multidrug-resistant (MDR) Escherichia coli. Finding alternatives for these infections is critical, for which some neglected drugs may be an option., [Objective] To determine whether fosfomycin is noninferior to ceftriaxone or meropenem in the targeted treatment of bacteremic urinary tract infections (bUTIs) due to MDR E coli., [Design, Setting, and Participants] This multicenter, randomized, pragmatic, open clinical trial was conducted at 22 Spanish hospitals from June 2014 to December 2018. Eligible participants were adult patients with bacteremic urinary tract infections due to MDR E coli; 161 of 1578 screened patients were randomized and followed up for 60 days. Data were analyzed in May 2021., [Interventions] Patients were randomized 1 to 1 to receive intravenous fosfomycin disodium at 4 g every 6 hours (70 participants) or a comparator (ceftriaxone or meropenem if resistant; 73 participants) with the option to switch to oral fosfomycin trometamol for the fosfomycin group or an active oral drug or parenteral ertapenem for the comparator group after 4 days., [Main Outcomes and Measures] The primary outcome was clinical and microbiological cure (CMC) 5 to 7 days after finalization of treatment; a noninferiority margin of 7% was considered., [Results] Among 143 patients in the modified intention-to-treat population (median [IQR] age, 72 [62-81] years; 73 [51.0%] women), 48 of 70 patients (68.6%) treated with fosfomycin and 57 of 73 patients (78.1%) treated with comparators reached CMC (risk difference, −9.4 percentage points; 1-sided 95% CI, −21.5 to ∞ percentage points; P = .10). While clinical or microbiological failure occurred among 10 patients (14.3%) treated with fosfomycin and 14 patients (19.7%) treated with comparators (risk difference, −5.4 percentage points; 1-sided 95% CI, −∞ to 4.9; percentage points; P = .19), an increased rate of adverse event–related discontinuations occurred with fosfomycin vs comparators (6 discontinuations [8.5%] vs 0 discontinuations; P = .006). In an exploratory analysis among a subset of 38 patients who underwent rectal colonization studies, patients treated with fosfomycin acquired a new ceftriaxone-resistant or meropenem-resistant gram-negative bacteria at a decreased rate compared with patients treated with comparators (0 of 21 patients vs 4 of 17 patients [23.5%]; 1-sided P = .01)., [Conclusions and Relevance] This study found that fosfomycin did not demonstrate noninferiority to comparators as targeted treatment of bUTI from MDR E coli; this was due to an increased rate of adverse event–related discontinuations. This finding suggests that fosfomycin may be considered for selected patients with these infections.

- Abstract
Although the COVID-19 disease has developed into a worldwide pandemic, its pathophysiology remains to be fully understood. Insulin-degrading enzyme (IDE), a zinc-metalloprotease with a high affinity for insulin, has been found in the interactomes of multiple SARS-CoV-2 proteins. However, the relevance of IDE in the innate and adaptative immune responses elicited by circulating peripheral blood mononuclear cells is unknown. Here, we show that IDE is highly expressed on the surface of circulating monocytes, T-cells (both CD4+ and CD4−), and, to a lower extent, in B-cells from healthy controls. Notably, IDE’s surface expression was upregulated on monocytes from COVID-19 patients at diagnosis, and it was increased in more severe patients. However, IDE’s surface expression was downregulated (relative to healthy controls) 3 months after hospital discharge in all the studied immune subsets, with this effect being more pronounced in males than in females, and thus it was sex-dependent. Additionally, IDE levels in monocytes, CD4+ T-cells, and CD4− T-cells were inversely correlated with circulating insulin levels in COVID-19 patients (both at diagnosis and after hospital discharge). Of note, high glucose and insulin levels downregulated IDE surface expression by ~30% in the monocytes isolated from healthy donors, without affecting its expression in CD4+ T-cells and CD4− T-cells. In conclusion, our studies reveal the sex- and metabolism-dependent regulation of IDE in monocytes, suggesting that its regulation might be important for the recruitment of immune cells to the site of infection, as well as for glucometabolic control, in COVID-19 patients.

The proposal in this article is about cases and examples of practices linked to 2030 Agenda, and it aims to answer the question of whether it could be considered that 2030 Agenda hase been socialized in such a way beyond national state actors. To this end, the previous efforts of the United Nations will be analyzed, the four Decades for Development and Millennium Development Goals to conclude that while the Latter succeeded in positioning the issue of development on the global agenda, they did not adequately permeated in actors other than the state actors and international organizations involved and in this case, undoubtedly, 2030 Agenda has played a better role. In this regard, the article analyses the specific case of Recommendation 26/2019 issued by the State Commission on Human Rights of the State of Jalisco (Mexico), in September of 2019, on the issue of the healthy environment of the inhabitants of the municipality of San Pedro Tlaquepaque. This recommendation stands out due to consider much of its content in the 2030 Agenda and applicable Sustainable Development Goals and for mandating to the municipal government to train all city council staff on the 2030 Agenda for Sustainable Development, both issues, coadjuvant in the referred socialization La propuesta de este artículo se circunscribe a casos y ejemplos de prácticas vinculadas a la Agenda 2030, particularmente pretende dar respuesta a la pregunta de si podría considerarse que ésta se ha socializado de tal manera que haya traspasado el ámbito de los actores estatales nacionales. Para ello se analizarán los esfuerzos previos de Naciones Unidas como lo fueron los Decenios para el Desarrollo y los Objetivos de Desarrollo del Milenio para concluir que, si bien éstos últimos lograron posicionar el tema del desarrollo en la agenda global, no permearon adecuadamente en otros actores que no fueran los actores estatales naciones y los organismos internacionales involucrados, cuestión en que la Agenda 2030 ha hecho, indudablemente, un mejor papel. En este sentido, se examina el caso específico de la Recomendación 26/2019 emitida por la Comisión Estatal de Derechos Humanos del Estado de Jalisco (CEDHJ), de fecha 10 de septiembre de 2019 y relativa al tema del ambiente sano de los habitantes del municipio de San Pedro Tlaquepaque. Lo destacable de esta recomendación es que, además de fundamentar gran parte de su contenido en la Agenda 2030 y en específicos Objetivos de Desarrollo Sostenible (ODS), se instruye al gobierno municipal para que capacite a todo el personal del ayuntamiento en materia de la Agenda 2030 para el Desarrollo Sostenible, ambas cuestiones, coadyuvantes en la referida socialización.

The present work analyzes the public datasets that provide a detailed usage information of the bike-sharing system BiciMAD. This analysis involves the automation of data gathering and preprocessing, an statistical analysis of the datasets (including descriptive statistics and clustering), and the development of an interactive dashboard for the analysis of complex networks defined by the service stations and the user trips between them. The generated code is publicly available and can be helpful for future work.
