6. Impact of simulation on multidisciplinary NICU teamwork during delivery and transport of extremely preterm infants.

Abstract

BACKGROUND AND OBJECTIVES: Multidisciplinary teamwork during delivery room (DR) resuscitation and timely transport to the neonatal intensive care unit (NICU) can reduce morbidity and mortality for infants born Extremely Preterm (EP). We aimed to assess the impact of a multidisciplinary high-fidelity simulation curriculum on teamwork during resuscitation and transport of EP infants. METHODS: In a prospective study conducted at a Level III academic center, seven teams (each consisting of one NICU fellow, two NICU nurses, and one respiratory therapist) performed three high-fidelity simulation scenarios. Videotaped scenarios were graded by three independent raters using the Clinical Teamwork Scale (CTS). Times of completion of key resuscitation and transport tasks were recorded. Pre- and post- intervention surveys were obtained. RESULTS: Overall, time of completion of key resuscitation and transport tasks decreased, with significant decreases in the time to attach the pulse oximeter, transfer of the infant to the transport isolette, and exit the DR. There was no significant difference in CTS scores from Scenario 1 to 3. Scenarios led by first-year fellows showed a trend towards improvement in all CTS categories. A comparison of teamwork scores pre- and post-simulation curriculum during direct observation of high-risk deliveries in real time revealed a significant increase in each CTS category. CONCLUSION: A high-fidelity teamwork-based simulation curriculum decreased time to complete key clinical tasks in the resuscitation and transport of EP infants, with a trend towards increased teamwork in scenarios led by junior fellows. 8. Ferroelectric phase transitions induced by a strain gradient

Abstract

In perovskite oxide ferroelectrics, gradients of lattice strain are known to induce nanoscale topological structures, leading to novel or enhanced functionality. Here, we experimentally detect and theoretically analyze thickness distribution of structural properties in epitaxial Pb0.5 Sr0.5 TiO3 films grown on (001) SrTiO3 substrates. We show that the relaxation of substrate-imposed stress produces a strain gradient, which leads to the formation of distinct ferroelectric phases as a function of distance from the film-substrate interface within the same film. Charge carriers trapped at phase boundaries stabilize the induced phases and manifest themselves under electric field. Crosstalk between the phases, where polarization may rotate in one phase and invert in the other one, opens perspectives for advanced ferroelectric thin film devices.

Abstract

This paper reports on the ongoing IntelCities (Intelligent Cities - IST no. 507860) integrated project, which commenced in January 2004 that seeks to integrate e-governance and e-urban planning. IntelCities is still at a formative stage and the paper seeks to explain the background, the research questions and preliminary design of prototyping studies to be carried out in the cities of Marseille, Siena, Helsinki, Rome, Leicester, Dresden, Berlin and Manchester. These prototypes are to be linked together to demonstrate an integrated open system city platform. The main business opportunities for IST companies and in terms of increased efficiency in e-urban planning are explored. The main outcomes are anticipated, which include a new public asset in terms of a city-wide intelligent information system.

Abstract

Many companies in different industrial sectors are exploring alternative work schedules to deal with diverse problems associated with shiftwork. The use of extended workday schedules (regular shift lengths exceeding 8 h with compressed workweeks) is attracting growing interest in many industries that use continuous operations. To address concerns regarding possible fatigue effects on safety and work performance associated with such schedules, the U.S. Bureau of Mines conducted a two-phase study at an underground metal mine in western Canada. Data were collected before and after a group of workers employed at the mine changed from an 8- to a 12-h schedule. Results indicate nearly unanimous acceptance and improved sleep quality associated with the new schedule. In general, fatigue-sensitive behavioral and physiological performance measures show either no change or improvement with 12-h shifts. We conclude that the extended workday schedule should be retained but periodically reevaluated. [ABSTRACT FROM AUTHOR]

Abstract

Melanosomes isolated from human, horse and Harding-Passey mouse melanomas, from human hair and from bovine eye pigment tissues were hydrolyzed with 6N HCl 24hours at 110 °C. An electron microscopic comparison of hydrolyzed and untreated melanosomes showed that the hydrolyzed melanosomes retained their size, shape, and, to a great extent, their ultrastructural features. The results 1/suggest that melanin constitutes the rigid framework of the particles providing them with unusual resistibility; 2/raise the question of the nature of their lysosomal degradation/reported by some authors/ if extreme acid hydrolysis has remained without any effect. Melanosomes are special subcellular particles of pigment cells ( Seiji et al., 1961 ) originating by apposition of polymerizing melanin precursors on primary protein polymer — either on premelanosome matrix protein exhibiting tyrosinase activity or on matrix protein under tyrosinase catalysis. The synthetized melanoprotein complex is a tough substance, which represents the major melanosome component. Unlike the biosynthesis, the melanosome degradation has remained obscure. Electron microscopy revealed that in vivo melanosomes are disintegrated in lysosomes ( Mishima, 1966; Kawamura et al., 1967; Sato et al.,1969; Olson et al., 1970; Wolff and Honigsmann, 1972; Wolff, 1974). The in vitro model experiments confirmed that the protein moiety of melanosomes could be hydrolyzed by lysosomal enzymes ( Ohtaki, 1970; Ohtaki and Seiji, 1971; Saito and Seiji, 1973 ), however, the melanin moiety appeared to be very resistant to lysosomal digestion. In this study the isolated melanosomes were, therefore, exposed to extreme hydrolytic treatment — acid hydrolysis and its influence was studied by means of electron microscopy.
