Animal studies have suggested an improved cognitive performance but the link with analgesic and antipyretic modes of action is incomplete. This study aims at exploring cognitive tests in healthy volunteers in the context of antinociception and temperature regulation. A double-blind randomized controlled study (NCT01390467) was carried out from May 30, 2011 to July 12, 2011. Methods: Forty healthy volunteers were included and analyzed. Nociceptive thresholds, core temperature (body temperature), and a battery of cognitive tests were recorded before and after oral APAP (2 g) or placebo: Information sampling task for predecisional processing, Stockings of Cambridge for spatial memory, reaction time, delayed matching of sample, and pattern recognition memory tests. We used behavioral measures and functional magnetic resonance imaging to investigate the response to experimental thermal stimuli with APAP or placebo administration. Region-of-interest analysis revealed that activity in response to noxious stimulation diminished with APAP compared to placebo in prefrontal cortices, insula, thalami, anterior cingulate cortex, and periaqueductal gray matter.Conclusion: These findings suggest an inhibitory effect of APAP on spinothalamic tracts leading to a decreased activation of higher structures, and a top-down influence on descending inhibition. Further binding and connectivity studies are needed to evaluate how APAP modulates pain, especially in the context of repeated administration to patients with pain. When fast relief is required and/or oral or iv routes are not available because of the patient's condition, the transmucosal route may be an alternative. Methodology: A new transmucosal/buccal (b) pharmaceutical form of APAP dissolved in 50% wt alcohol is compared with other routes of administration. Abstract

BACKGROUND: Many countries will not reach elimination targets for lymphatic filariasis in 2020 using the two-drug treatment regimen (diethylcarbamazine citrate [DEC] and albendazole [DA]). A cluster-randomized, community-based safety study performed in Fiji, Haiti, India, Indonesia and Papua New Guinea tested the safety and efficacy of a new regimen of ivermectin, DEC and albendazole (IDA). METHODOLOGY/PRINCIPAL FINDINGS: To assess acceptability of IDA and DA, a mixed methods study was embedded within this community-based safety study. The study objective was to assess the acceptability of IDA versus DA. Community surveys were performed in each country with randomly selected participants (>14 years) from the safety study participant list in both DA and IDA arms. In depth interviews (IDI) and focus group discussions (FGD) assessed acceptability-related themes. In 1919 individuals, distribution of sex, microfilariae (Mf) presence and circulating filarial antigenemia (CFA), adverse events (AE) and age were similar across arms. A composite acceptability score summed the values from nine indicators (range 9-36). The median (22.5) score indicated threshold of acceptability. There was no difference in scores for IDA and DA regimens. Mean acceptability scores across both treatment arms were: Fiji 33.7 (95% CI: 33.1-34.3); Papua New Guinea 32.9 (95% CI: 31.9-33.8); Indonesia 30.6 (95% CI: 29.8-31.3); Haiti 28.6 (95% CI: 27.8-29.4); India 26.8 (95% CI: 25.6-28) (P<0.001). AE, Mf or CFA were not associated with acceptability. Qualitative research (27 FGD; 42 IDI) highlighted professionalism and appreciation for AE support. No major concerns were detected about number of tablets. Increased uptake of LF treatment by individuals who had never complied with MDA was observed. Abstract

BACKGROUND: The Global Programme to Eliminate Lymphatic Filariasis (GPELF) provides antifilarial medications to hundreds of millions of people annually to treat filarial infections and prevent elephantiasis. Recent trials have shown that a single-dose, triple-drug treatment (ivermectin with diethylcarbamazine and albendazole [IDA]) is superior to a two-drug combination (diethylcarbamazine plus albendazole [DA]) that is widely used in LF elimination programs. This study was performed to assess the safety of IDA and DA in a variety of endemic settings. METHODS AND FINDINGS: Large community studies were conducted in five countries between October 2016 and November 2017. Two studies were performed in areas with no prior mass drug administration (MDA) for filariasis (Papua New Guinea and Indonesia), and three studies were performed in areas with persistent LF despite extensive prior MDA (India, Haiti, and Fiji). Participants were treated with a single oral dose of IDA (ivermectin, 200 μg/kg; diethylcarbamazine, 6 mg/kg; plus albendazole, a fixed dose of 400 mg) or with DA alone. Treatment assignment in each study site was randomized by locality of residence. Treatment was offered to residents who were ≥5 years of age and not pregnant. Adverse events (AEs) were assessed by medical teams with active follow-up for 2 days and passive follow-up for an additional 5 days. A total of 26,836 persons were enrolled (13,535 females and 13,300 males). A total of 12,280 participants were treated with DA, and 14,556 were treated with IDA. On day 1 or 2 after treatment, 97.4% of participants were assessed for AEs. The frequency of all AEs was similar after IDA and DA treatment (12% versus 12.1%, adjusted odds ratio for IDA versus DA 1.15, 95% CI 0.87-1.52, P = 0.316); 10.9% of participants experienced mild (grade 1) AEs, 1% experienced moderate (grade 2) AEs, and 0.1% experienced severe (grade 3) AEs. 30. Dosing pole recommendations for lymphatic filariasis elimination: A height-weight quantile regression modeling approach

Abstract

BACKGROUND: The World Health Organization (WHO) currently recommends height or age-based dosing as alternatives to weight-based dosing for mass drug administration lymphatic filariasis (LF) elimination programs. The goals of our study were to compare these alternative dosing strategies to weight-based dosing and to develop and evaluate new height-based dosing pole scenarios. METHODOLOGY/PRINCIPAL FINDINGS: Age, height and weight data were collected from >26,000 individuals in five countries during a cluster randomized LF clinical trial. Weight-based dosing for diethylcarbamazine (DEC; 6 mg/kg) and ivermectin (IVM; 200 ug/kg) with tablet numbers derived from a table of weight intervals was treated as the "gold standard" for this study. Following WHO recommended age-based dosing of DEC and height-based dosing of IVM would have resulted in 32% and 27% of individuals receiving treatment doses below those recommended by weight-based dosing for DEC and IVM, respectively. Underdosing would have been especially common in adult males, who tend to have the highest LF prevalence in many endemic areas. We used a 3-step modeling approach to develop and evaluate new dosing pole cutoffs. First, we analyzed the clinical trial data using quantile regression to predict weight from height. We then used weight predictions to develop new dosing pole cutoff values. Finally, we compared different dosing pole cutoffs and age and height-based WHO dosing recommendations to weight-based dosing. We considered hundreds of scenarios including country- and sex-specific dosing poles. A simple dosing pole with a 6-tablet maximum for both DEC and IVM reduced the underdosing rate by 30% and 21%, respectively, and was nearly as effective as more complex pole combinations for reducing underdosing. CONCLUSIONS/SIGNIFICANCE: Using a novel modeling approach, we developed a simple dosing pole that would markedly reduce underdosing for DEC and IVM in MDA programs compared to current WHO recommended height o

Abstract

Curcumin exerts a range of biological activities of potential interest in cardiovascular prevention. However, the vascular protective effect of curcumin is still poorly investigated in humans and the cellular and molecular mechanisms by which this compound could modulate the vascular endothelial integrity and function remain to be established. In a randomized control trial, we showed that after an acute intake of curcumin and a subgroup analysis of our study population, curcumin induced a positive effect of on endothelial function only in women and in subjects at lower cardiovascular risk. This observation indicates that curcumin intake could be of better interest to prevent endothelial dysfunction rather than to restore the more established dysfunction, ultimately suggesting that curcumin could be a nutraceutical of special interest to maintain endothelial function. In addition, at the cellular level, we showed that the pre-exposure of endothelial cells to curcumin prior to their activation reduced monocyte adhesion and their trans-endothelial migration in both static and shear stress conditions mimicking the blood flow. Our findings also demonstrated the ability of curcumin, in physiologically relevant conditions, to reduce monocyte TEM through a multimodal regulation of the endothelial cell dynamics with a potential benefit on the vascular endothelial function barrier. In conclusion, taken together these results provide new clinical and mechanistic insights supporting the capacity of curcumin to promote vascular integrity and function. This compound exhibits potent antioxidant, anti-inflammatory, immunomodulatory or anti-angiogenic properties. Several animal studies reported hypocholesterolemic, antioxidant, anti-atherogenic and antiplatelet activities of this compound, however the cardiovascular protective effect of curcumin was poorly investigated in humans. The aim of this study was to determine the effect of an acute intake of curcumin on vascular endothelial function in healthy smokers and its nutrigenomic impact in circulating immune cells.Methods: In a randomized, double-blind, cross-over study on 18 smokers (9 men and 9 women), 50–70 yrs old, the effect of curcumin (5 g) on gene expression in peripheral blood mononuclear cell (PBMC) and endothelial function was evaluated. Effect on expression of 93 genes involved in inflammation, cell adhesion, cholesterol absorption/efflux was assessed using TaqMan-Low-Density-Array (TLDA) and endothelial function was evaluated by Flow Mediated Dilatation (FMD). Measurements were done before and 2-hours after curcumin or placebo intake. Results: We failed to show significant difference between curcumin and placebo for FMD partly due to inter-individual variability. However, analysis of data separately for men and women revealed a significant effect of curcumin for FMD in women, but not in men. Similarly, no change in gene expression between curcumin and placebo was observed when data were analysed for all volunteers. However, analysis of gene expression data separately for men and women revealed 9 differentially expressed genes in men and 8 in women with 3 genes in common (FABP3, CXCL3, CCL20). Interestingly, the expression of the 9 and 8 genes was identified as up-regulated in men and down-regulated in women, respectively.Conclusion: Although results did not account for a beneficial effect of curcumin in the whole studied population, gender comparison analysis suggests a difference in nutrigenomic and vascular response to curcumin between men and women.

Abstract

Context: In 2013, reimbursements for Proton-Pump Inhibitors (PPIs) amounted to nearly 530 million euros in France. PPI usage was subject to official recommendations in France in 2007 and 2009. Six years later, however, they are being prescribed primarily for off-label uses. This study sought to further light the prevalence of noncompliant PPI prescriptions at our institution. Patients and methods: Transversal, descriptive, observational 1-day study including all patients receiving at least one PPI, in all conventional hospitalization and intensive care wards at a university hospital centre. The study excluded day- and week-stay hospitalization wards, the emergency department, as well as the short-term hospitalization and care ward. Besides demographic data, comorbidity was assessed based on the Charlson index. Medications that could potentially interact with PPIs were recorded. Results: In total, 26 wards participated and 519 patients were assessed, 198 of which (38%), aged 67 ± 13 years on average, were receiving a PPI treatment, were including 113 men (57%). The average Charlson score was 1.7 ± 2. Amongst these 198 patients, 50 (25%; IC95%: [19-32%]) were taking PPIs in compliance with official recommendations for best clinical practice, and 126 (63%) were additionally undergoing at least one treatment known to cause drug interactions with PPIs. For all included wards, expenditures for PPIs amounted to 31.57 Euros for the study day. Conclusion: Over one out of three hospitalized patients (38%) were receiving a PPI (23% had a PPI at arrival). Whilst over half of prescriptions exhibited potential drug interactions, only 25% of them complied with good clinical practice recommendations. The significant number of such prescriptions may be explained by their low cost, the image of good tolerance they enjoy, a lack of information regarding side-effects and drug interactions, fear of ceasing PPI administration with ensuing peptic ulcers, as well as unwillingness to question a prescription lacking proper scientific basis. Prescribers must, therefore, be better informed.

Abstract

Background Short-chain fructooligosaccharides (sc FOS) have beneficial effects in subjects with minor digestive complaints, but the potential mechanisms involved have not been elucidated. The aim of the study was to evaluate changes in rectal sensitivity related to the clinical effects of sc FOS in a selected group of patients with irritable bowel syndrome ( IBS) and rectal hypersensitivity. Methods In 79 IBS patients (defined by Rome III criteria) with rectal hypersensitivity (defined as discomfort threshold ≤44 g) a parallel, placebo-controlled, randomized, and double-blind study was performed to assess the effects of dietary supplementation (5 g d−1) with sc FOS vs placebo for 4 weeks on rectal sensitivity (primary outcome: tolerance to increasing wall tension applied by a tensostat), clinical outcomes ( IBS, anxiety/depression and quality of life scores) and composition of fecal microbiota. Key Results Rectal discomfort threshold, and IBS and quality of life scores, significantly improved during treatment, but in a similar manner in both sc FOS and placebo groups; a post-hoc analysis showed that the effect of sc FOS on rectal sensitivity was more pronounced in constipation-predominant- IBS patients ( P=.051 vs placebo). Contrary with placebo, sc FOS significantly reduced anxiety scores and increased fecal Bifidobacteria ( P<.05 for both) without modifying other bacterial groups. Conclusions & Interfences The effect of sc FOS on anxiety may be related to modulation of the gut microbiota; demonstration of effects of sc FOS on rectal sensitivity may require higher doses and may depend on the IBS subgroup.
