Abstract

Background The use of prothrombin complex concentrates and the role of plasma concentration of anticoagulants in the management of bleeding in patients treated with direct oral anticoagulants are still debated. Our aim was to describe management strategies and outcomes of severe bleeding events in patients treated with direct oral anticoagulants. Methods We performed a prospective cohort study of 732 patients treated with dabigatran, rivaroxaban, or apixaban hospitalized for severe bleeding, included prospectively in the registry from June 2013 to November 2015. Results Bleeding was gastrointestinal or intracranial in 37% (212 of 732) and 24% (141 of 732) of the cases, respectively. Creatinine clearance was lower than 60 ml/min in 61% (449 of 732) of the cases. The plasma concentration of direct oral anticoagulants was determined in 62% (452 of 732) of the cases and was lower than 50 ng/ml or higher than 400 ng/ml in 9.2% (41 of 452) and in 6.6% (30 of 452) of the cases, respectively. Activated or nonactivated prothrombin complex concentrates were administered in 38% of the cases (281 of 732). Mortality by day 30 was 14% (95% CI, 11 to 16). Conclusions Management of severe bleeding in patients treated with direct oral anticoagulants appears to be complex. The use of prothrombin complex concentrates differs depending on bleeding sites and direct oral anticoagulant plasma concentrations. Mortality differs according to bleeding sites and was similar to previous estimates.

Abstract

Among vertebrates, herpetofauna has the highest proportion of declining species. Detection of environmental DNA (eDNA) is a promising method towards significantly increasing large-scale herpetological conservation efforts. However, the integration of eDNA results within a management framework requires an evaluation of the efficiency of the method in large natural environments and the calibration of eDNA surveys with the quantitative monitoring tools currently used by conservation biologists. Towards this end, we first developed species-specific primers to detect the wood turtle ( Glyptemys insculpta) a species at risk in Canada, by quantitative PCR (qPCR). The rate of eDNA detection obtained by qPCR was also compared to the relative abundance of this species in nine rivers obtained by standardized visual surveys in the Province of Québec (Canada). Second, we developed multi-species primers to detect North American amphibian and reptile species using eDNA metabarcoding analysis. An occurrence index based on the distribution range and habitat type was compared with the eDNA metabarcoding dataset from samples collected in seven lakes and five rivers. Our results empirically support the effectiveness of eDNA metabarcoding to characterize herpetological species distributions. Moreover, detection rates provided similar results to standardized visual surveys currently used to develop conservation strategies for the wood turtle. We conclude that eDNA detection rates may provide an effective semiquantitative survey tool, provided that assay calibration and standardization is performed. [ABSTRACT FROM AUTHOR]

14. Écologie thermique et sélection d'habitats chez la tortue des bois (Glyptemys insculpta) à la limite nord de sa distribution

Abstract

Il est généralement accepté que les reptiles peuvent réguler leur température corporelle (T[indice inférieur c]) au moyen d'ajustements comportementaux, afin d'optimiser leurs performances physiologiques, écologiques et reproductives. Nous proposons donc que la T[indice inférieur c] supérieure (T[indice inférieur sup] ; 95e percentile) atteinte par les animaux nourris, plutôt que la T[indice inférieur c] moyenne, devrait correspondre à la T[indice inférieur opt]. Dans un premier temps, nous avons mesuré en continu la T[indice inférieur c] de 8 mâles et 8 jeunes tortues des bois (Glyptemys insculpta) soumis à différents traitements en laboratoire. Les tortues étaient placées individuellement dans des gradients thermiques secs, puis en deux groupes dans des enclos offrant l'accès à l'eau et à des radiations infrarouges pour la thermorégulation. Le régime d'alimentation (privation vs ad libitum ) influença la T[indice inférieur c] moyenne des jeunes (23,1[degrés celsius] vs 26,6[degrés celsius), mais non celle des mâles (25,0[degrés celsius]) dans les gradients thermiques. Dans les enclos, le régime d'alimentation n'a pas influencé la T[indice inférieur c] moyenne des jeunes (22,6[degrés celsius), ni des mâles (25,3[degrés celsius]), lorsqu'ils avaient accès à 10 h de radiations infrarouges par jour. Par contre, le régime ad libitum a augmenté la T[indice inférieur c] moyenne des jeunes (24,9[degrés celsius]), mais non celle des mâles (24,2[degrés celsius]), lorsque le temps de thermorégulation était restreint à 5 h par jour. Bien que les distributions de T[indice inférieur c] des tortues, lors des régimes ad libitum, aient glissé vers des T[indice inférieur c] plus élevées, la T[indice inférieur sup] était constante (30[degrés celsius]) à travers les traitements. Cela créa des distributions négativement asymétriques, avec des fréquences croissantes pour les T[indice inférieur c] approchant la T[indice inférieur sup] et une chute abrupte des fréquences au-delà de la T[

Abstract

International audience; INTRODUCTION: Although Pseudomonas aeruginosa is a leading pathogen responsible for ventilator-associated pneumonia (VAP), the excess in mortality associated with multi-resistance in patients with P. aeruginosa VAP (PA-VAP), taking into account confounders such as treatment adequacy and prior length of stay in the ICU, has not yet been adequately estimated. METHODS: A total of 223 episodes of PA-VAP recorded into the Outcomerea database were evaluated. Patients with ureido/carboxy-resistant P. aeruginosa (PRPA) were compared with those with ureido/carboxy-sensitive P. aeruginosa (PSPA) after matching on duration of ICU stay at VAP onset and adjustment for confounders. RESULTS: Factors associated with onset of PRPA-VAP were as follows: admission to the ICU with septic shock, broad-spectrum antimicrobials at admission, prior use of ureido/carboxypenicillin, and colonization with PRPA before infection. Adequate antimicrobial therapy was more often delayed in the PRPA group. The crude ICU mortality rate and the hospital mortality rate were not different between the PRPA and the PSPA groups. In multivariate analysis, after controlling for time in the ICU before VAP diagnosis, neither ICU death (odds ratio (OR) = 0.73; 95% confidence interval (CI): 0.32 to 1.69; P = 0.46) nor hospital death (OR = 0.87; 95% CI: 0.38 to 1.99; P = 0.74) were increased in the presence of PRPA infection. This result remained unchanged in the subgroup of 87 patients who received adequate antimicrobial treatment on the day of VAP diagnosis. CONCLUSIONS: After adjustment, and despite the more frequent delay in the initiation of an adequate antimicrobial therapy in these patients, resistance to ureido/carboxypenicillin was not associated with ICU or hospital death in patients with PA-VAP.
