Abstract

La "Casa de las Flores" es un interesante proyecto de arquitectura urbana residencial -bloques de departamentos de alquiler- del arquitecto bilbaíno Secundino Zuazo Ugalde (tit. 1912, m. en 1970). Realizado entre 1930 y 1932, es uno de los tres conjuntos más representativos del Movimiento Moderno en Madrid, y fue declarado Monumento Nacional en 1981. En uno de los pisos de este conjunto residió el Premio Nobel de literatura Pablo Neruda.This is a remarkably housing project -apartments for rent- of Secundino Suazo. The building –National Monument since 1981- is representative of the Modern Movement of Madrid as well as a fine example of urban architecture. Pablo Neruda, the Chilean Nobel Prize in Literature, was a resident there.

Abstract

Background. Cytomegalovirus (CMV) infection remains the most common clinically significant infection after allogeneic hematopoietic stem cell transplantation (allo-HCT) and is associated with considerable morbidity and mortality. Objectives: The present study was designed to describe and compare the incidence of untreated CMV reactivation (uCMVr), clinically significant infection (cs-CMVi) and disease (CMVd), as well as CMV-related hospitalization and outcome of allo-HCT patients, either treated with letermovir (LET) primary prophylaxis or managed with preemptive therapy (PET). Methods: This is a prospective observational cohort study of adult CMV seropositive allo-HCT patients who either received primary prophylaxis with LET within the first 100 days after HCT or were managed with PET. Results: The study population comprised 105 patients (28 in the LET group and 77 in the PET group). Compared to the PET group, patients in the LET group received more allo-HCT from alternative donors (54.5% vs. 82.14%, P=0.012). More than half of the patients in both groups were classified as high risk for CMVd. In the LET vs. PET group, cs-CMVi and CMVd developed respectively in 0 vs. 50 (64.94%), P=<0.0001, and 0 vs. 6 (7.79%), P=0.18. In the LET group, uCMVr occurred in 5 (17.8%) and were all considered blips. Hospital admissions related to cs-CMVi or CMVd in the PET group vs. LET group were 47 (61.04%) vs. 0, respectively, P=<0.0001. No differences were observed in 100-day mortality. Conclusions: LET primary prophylaxis proved effective in preventing cs-CMVi and CMVd and reducing hospitalizations in allo-HCT adults. Abstract

Mozambique's economic and political transformations since the late 1980s, including the transition from socialism to multi-party democracy, formally completed by the 1990 Constitution, and the end of the 1992 civil war, marked a new era in the path of education, especially Higher Education in Mozambique (HEM). Increasing State investment in this sector and the liberalization of the market, which has allowed the intervention of private actors, has rapidly increased the supply of higher education through the opening of new institutions and curricula. However, despite the expansion of the system and the increase in supply, gender inequalities are persistent throughout the years. If on the one hand, girls continue to be less present compared to boys in the system, on the other hand, their choices of orientation turn more towards the sectors of the Humanities and Social Sciences (HSS), regarded as less cost-effective compared to Science and technology (ST), dominated by boys, and where girls are almost absent. We wonder what is at the origin of these gender inequalities in the choice of courses between girls and boys in HEM. More specifically, we seek to analyze the role played by socio-cultural origin of students in the production of these inequalities, as well as the role of the educational institution in the differentiated orientation of girls and boys. In addition, we wonder about the role of the representations that girls and boys have of work according to gender stereotypes linked to gendered division of work, associated with subjective perceptions of the chances of school success on a gender basis. Using a combination of methods that includes fieldwork in two secondary and two higher education institutions, both private and public, consisting of the implementation of a student questionnaire, the realization of semi-structured interviews with educational leaders and observations in the field, we seek to study the mechanisms that are at the root of these disparities in differentiated and unequal choices between girls and boys, analyzing, in a particular way, the role of the social origin, of the school institution and the role of representations that girls and boys make of trades. The results suggest that the practices of the institutions, especially through the support programs available to students, play an important role in the decision-making process. In addition, this role is especially important when the student's social origin is more modest and where there is a lack of key information about university rules. In this case, girls seem to be much more constrained than boys in these situations, limiting their choices to less ambitious options. The socio-economic condition is important for the choices, as it determines the choice of formations considered difficult or those which have a longer duration., Les transformations économiques et politiques que le Mozambique a connu depuis la fin des années 1980, notamment la transition du socialisme à la démocratie multipartiste, formellement achevée par la Constitution de 1990 et la fin de la guerre civile en 1992, ont marqué une nouvelle ère dans l’histoire des politiques éducatives, en particulier celle de l'enseignement supérieur au Mozambique (ESM). Les investissements croissants de l'État sur ce secteur et la libéralisation du marché qui a permis l'intervention d'acteurs privés, ont augmenté rapidement l'offre de l'enseignement supérieur à travers l'ouverture de nouvelles institutions et de nouveaux cursus. Cependant, malgré l'expansion du système et l'augmentation de l'offre, les inégalités de sexe restent persistantes. Si d'un côté, les filles continuent d'être moins présentes par rapport aux garçons dans le système, d'un autre côté, leurs choix d'orientation se tournent davantage vers les filières des Sciences Humaines et Sociales (SHS), considérées comme moins rentables par rapport aux filières des Sciences et Technologies (ST) dominées par les garçons, et où les filles sont quasi absentes. On se questionne sur qu’est-ce qui est à l’origine de ces inégalités sexuées dans les choix de filières entre filles et garçons dans l’ESM ? On cherche, de façon plus spécifique, à analyser le rôle joué par l’origine socioculturelle des étudiant.e.s dans la production de ces inégalités, ainsi que le rôle de l’institution scolaire dans le cheminement différencié des étudiant.e.s. En plus, on se questionne sur le rôle des représentations que les filles et les garçons se font des métiers d’après les stéréotypes de sexe liés à la division sexuée du travail, associées aux perceptions subjectives des chances de réussite fondées sur une base sexuée. À partir d'une combinaison de méthodes qui inclut un travail de terrain dans deux établissements d'enseignement secondaire et deux d'enseignement supérieur, privées et publiques, consistant en la mise en œuvre d'un questionnaire aux étudiant.e.s ainsi que la réalisation d'entretiens semi-directifs auprès de responsables pédagogiques et d’observations de terrain, nous cherchons à étudier les mécanismes qui sont à l'origine de ces disparités dans les choix différenciés et inégaux entre filles et garçons, en analysant, de façon particulière, le rôle de l'origine sociale, de l'institution scolaire et le rôle des représentations que les filles et les garçons se font des métiers. Les résultats suggèrent que les pratiques des institutions, notamment au travers des programmes d’aide et de soutien à l’orientation mis à disposition des étudiant.e.s, jouent un rôle important dans le processus de prise de décision d’orientation. En plus, ce rôle s’avère surtout important, lorsque le contexte social d’origine de l’étudiant.e est plus modeste car il y a un manque d’informations clés sur les filières, qui est souvent aussi lié à un rapport éloigné de la famille vis-à-vis de l’école. Dans ce cas, les filles semblent être beaucoup plus contraintes que les garçons dans ces mêmes situations, limitant leurs choix à des options moins ambitieuses. La condition socioéconomique se révèle alors décisive dans les choix, dans la mesure où elle conditionne le renoncement à des formations considérées difficiles ou celles qui nécessitent une durée d’étude plus longue.

Abstract

Violence against women and girls in the public space in Mozambique is poorly addressed and documented, but present in the daily lives of women. This article is based on an exploratory study that seeks to raise awareness of violence against women and girls in public space in Mozambique. The study was conducted in two densely populated municipal districts of the city of Maputo, namely KaMaxaquene and KaLhamankulu. The assessed sample represents 0.11% of the two districts. The participants were women (35%), girls (40%), boys (12.5%) and men (12.5%). A descriptive study of the data was performed with Statistical Package for the Social Sciences 17. Both women and girls (60%) reported having experienced violence in their lives. Streets and alleys are prone to violence due to poor public illumination and lack of effective/visible police presence. Unemployment, drug abuse and social norms are the most influencing factors for violence.

Abstract

We aimed at analyzing the outcome of allogeneic stem cell transplant (ASCT) in adult patients with acute lymphoblastic leukemia (ALL), comparing Haploidentical (Haplo) with HLA-matched (sibling and unrelated) donors. Between 2008 and 2017, we collected data from 236 patients (median age 31 years; range 16–64; 90% HCT-CI 0–1) who underwent unmanipulated ASCT in first complete remission and subsequent remissions in 15 Argentinian centers. Donors were HLA-matched (n= 175; 74%) and Haplo (n= 61; 26%). Two-year overall survival (OS) was 55% (95% CI 47–63) for the HLA-matched group and 49% (95% CI 34–62) for the Haplo group (p= 0.351). For OS, crude HR, adjusted HR for covariates (HR 1.24; 95% CI 0.77–1.99; p= 0.363) and HR including a propensity score in the model (HR 1.22; 95% CI 0.71–2.08; p= 0.414) showed no impact of donor category on the OS. No difference was found in terms of nonrelapse mortality, relapse, leukemia-free survival, and grade 3–4 acute graft-versus-host disease (GVHD); 2-year incidence of chronic GVHD was higher in HLA-matched vs Haplo group (p= 0.028). Patients with ALL who underwent ASCT were young subjects with low HCT-CI. In this setting, a Haplo donor represents an alternative widely available in the absence of an HLA-matched donor. Relapse remains a challenge for all donor categories.

Abstract

Introduction: POEMS syndrome is a rare paraneoplastic syndrome caused by an underlying plasma cell disorder. The acronym refers to the following features: polyradiculoneuropathy, organomegaly, endocrinopathy, monoclonal paraproteinemia, and skin changes. Methods: The study was conducted at 24 hematological centers across 8 Latin-American countries. The study included a total of 46 patients {median age was 52 years (interquartile range [IQR]: 42–61.5), 30 males and 16 females} fulfilling the POEMS syndrome criteria diagnosed over a period of 12 years (January 1, 2011, through July 31, 2023). Epidemiological and clinical data were collected in an ad hoc database sent to the members of GELAMM, as well as the Kolmogorov-Smirnov test and Kaplan-Meier estimates. Results: All patients had polyneuropathy and monoclonal gammopathy; 89% had bone marrow plasma cell infiltration, 33% had sclerotic bone lesions. Only 10 patients underwent vascular endothelial growth factor (VEGF) testing in plasma samples. The paraproteinemia was IgG λ in 32% and IgA λ in 30%. 59% patients presented with cutaneous changes, mainly hyperpigmentation, 54% had organomegaly, and 74% endocrinopathy. The median interval from symptom onset to diagnosis was 7.7 months (IQR: 4.0–12.6). 69% of patients received a single line of treatment. The median follow-up period was 25 months (IQR: 9.37–52.0) and the 2-year overall survival rate was 100%. All patients who underwent transplantation (43%) are alive, with a median follow-up of 45.62 months (IQR: 15.46–70). Conclusion: This study investigates POEMS syndrome in Latin America and presents an initial overview of the disease in the region. VEGF usage is recommended for accurate diagnosis, but only 7 hematology centers in the region used it. Survival rate in Latin America is comparable with those observed internationally. Abstract

Purpose: Multiple myeloma (MM) is a highly heterogeneous, incurable disease most frequently diagnosed in the elderly. Therefore, data on clinical characteristics and outcomes in the very young population are scarce., Patients and Methods: We analyzed clinical characteristics, response to treatment, and survival in 103 patients with newly diagnosed MM age 40 years or younger compared with 256 patients age 41-50 years and 957 patients age 51 years or older., Results: There were no statistical differences in sex, isotype, International Scoring System, renal involvement, hypercalcemia, anemia, dialysis, bony lesions, extramedullary disease, and lactate dehydrogenase (LDH). The most used regimen in young patients was cyclophosphamide, bortezomib, dexamethasone, followed by cyclophosphamide, thalidomide, dexamethasone and bortezomib, thalidomide, dexamethasone. Of the patients age 40 years or younger, only 53% received autologous stem-cell transplant (ASCT) and 71.1% received maintenance. There were no differences in overall survival (OS) in the three patient cohorts. In the multivariate analysis, only high LDH, high cytogenetic risk, and ASCT were statistically associated with survival., Conclusion: In conclusion, younger patients with MM in Latin America have similar clinical characteristics, responses, and OS compared with the elderly.

Abstract

The coronavirus disease 2019 (COVID-19) pandemic requires rapid medical responses. The risk of venous and arterial thromboembolism increases in critically ill patients with SARS-CoV-2 infection. There is a hypercoagulable state that includes elevated levels of D-dimer, with an increased risk of organ failure and increased mortality. The abnormalities described in hemostasis should be considered for therapeutic decision making. We analyzed the available scientific evidence for the therapeutic approach of coagulopathy in the course of the disease with the objective of designing realistic therapeutic recommendations aimed at reducing morbidity and mortality in patients with COVID-19.

Abstract

We present a 24 year old immunocompetent male who developed a spontaneous rupture of the spleen (SRE) during an acute cytomegalovirus (CMV) infection. The only previous clinical feature was the presence of flu-like symptoms two weeks before the SRE. The diagnosis was confirmed by the presence of IgM antibodies to CMV in the serum and a positive CMV-PCR in the splenic biopsy after splenectomy. The patient recovered completely after surgery. Spontaneous splenic rupture is an uncommon event associated with primary cytomegalovirus infection, and this is the first case reported in our country.
