7. Clinical value of alpha-fetoprotein: alpha-fetoprotein-L3 isoform ratio and des-γ-carboxyprothrombin in early diagnosis of HBV-related hepatocellular carcinoma

Abstract

Objective To evaluate the value of different combinations of alpha-fetoprotein （AFP）， alpha-fetoprotein-L3 isoform ratio （AFP-L3%） and des-γ-carboxyprothrombin （DCP） for early diagnosis of hepatitis B virus （HBV）-related hepatocellular carcinoma （HCC）. Methods A total of 167 patients were enrolled in this study， including 100 patients with HCC and 67 patients with infection-related liver cirrhosis （LC）. The levels of serum AFP， AFP-L3% and DCP were detected by chemiluminescence enzyme immunoassay. The receiver operating characteristic （ROC） curve was delineated for subsequent analysis. Results The levels of AFP， AFP-L3% and DCP in HCC patients were significantly higher than those in LC patients （all P < 0.05）. There was no significant difference in AFP and AFP-L3% levels between stage 0 and stage A patients in HCC group （both P > 0.05）. DCP levels in stage 0 patients were significantly lower than those in stage A patients in HCC group （P < 0.05）. The sensitivity and specificity of AFP， AFP-L3% and DCP in the diagnosis of early HCC were 51.5%， 92.5%， 28.3%， and 95.5%， 60.0%， 84.7%， respectively. Analysis of the area under the ROC curve （AUC） results showed that the AUC values of AFP， AFP-L3% and DCP for the diagnosis of early HCC were 0.760， 0.619 and 0.749， respectively. The AUC value of combined detection of AFP and DCP was 0.810， which yielded the highest diagnostic efficiency. Conclusion AFP has important diagnostic value in the diagnosis of early HCC. The combination of AFP and DCP can improve the diagnostic efficiency of HBV-related early HCC.

15. Increased cTnI Predicts Early Death in Patients with Severe Fever with Thrombocytopenia: A Multicenter Study in North China.

Abstract

Background: Myocardial injury is common in severe fever with thrombocytopenia syndrome (SFTS) patients. Currently, research on the prognostic value of cardiac troponin I (cTnI) for predicting the mortality of SFTS patients, especially death within 7 days is limited. Methods: Between May 2011 and October 2022, clinical and laboratory data on admission of consecutive SFTS cases were collected from six medical centres in China. The clinical endpoint was in-hospital all-cause death within seven days. Risk factors of myocardial injury and death were analysed using multivariable regression models. Prognostic models were established using Cox regression and performance of indicators was evaluated in terms of calibration, discrimination. Results: A total of 1379 laboratory-confirmed patients were enrolled, in which 686 subjects were included for analysis. The median age was 66 years, with 48.1% of male. Eighty-seven patients died within seven days and 396 patients diagnosed with myocardial injury during hospitalization. Non-survivors had significant higher levels of cardiac indices than survivors, including cTnI, aspartic transaminase (AST) and lactate dehydrogenase (LDH). Elevated levels of cTnI (HR = 1.058, 95% CI:1.032– 1.085), AST (HR = 1.191, 95% CI:1.150– 1.234) and LDH (HR = 1.019, 95% CI:1.009– 1.029) predicted risk of early in-hospital mortality. cTnI model performed best, with area under curve of 0.850 (0.774– 0.926) and concordance index of 0.842, respectively. Statistical differences were found between high and low levels of cTnI for mortality (P< 0.001) using 0.35 ng/mL as the optimal cut-off. Conclusion: The risk of early in-hospital death can be predicted by cTnI. Clinical doctors should remind vigilant concerning the elevation of cardiac enzyme as soon as possible. Abstract

East Asia has experienced strong warming since the 1960s accompanied by an increased frequency of heat waves and shrinking glaciers over the Tibetan Plateau and the Tien Shan. Here, we place the recent warmth in a long-term perspective by presenting a new spatially resolved warm-season (May-September) temperature reconstruction for the period 1–2000 CE using 59 multiproxy records from a wide range of East Asian regions. Our Bayesian Hierarchical Model (BHM) based reconstructions generally agree with earlier shorter regional temperature reconstructions but are more stable due to additional temperature sensitive proxies. We find a rather warm period during the first two centuries CE, followed by a multi-century long cooling period and again a warm interval covering the 900–1200 CE period (Medieval Climate Anomaly, MCA). The interval from 1450 to 1850 CE (Little Ice Age, LIA) was characterized by cooler conditions and the last 150 years are characterized by a continuous warming until recent times. Our results also suggest that the 1990s were likely the warmest decade in at least 1200 years.
