In cryptography, we use different methods to hide information and make sure it's safe when shared. This keeps hackers from getting at it. The RSA cryptosystem is a way to protect messages that uses two different keys. In this paper, a new method is suggested. It includes changing messages into hexadecimal values and then turning them into decimals. Public and private keys are generated based on the input of message's length, adding an increase of complexity to enhance the security of the cryptosystem. The proposed algorithm uses two different keys to encrypt and decrypt each character, this makes the cryptosystem increasing the difficultly for attackers trying to hack it. A comparison is made between the proposed algorithm and the original RSA, using NIST tests and measuring the running time of key generation, encoding, and decoding operations. The results show that the new algorithm provides a secure transmitting of data. The proposed algorithm enhances security over the standard RSA algorithm by using hexadecimal conversion, multiple keys, dual key encryption per one-character, increased randomness, and a more advanced cryptography method, offering improved resistance against attacks and protecting data. Abstract Background Although many people have concerns about their body image, weight, and eating behaviors these issues are not usually discussed in a productive manner with medical providers. Thus, we examined nursing and medical students’ willingness to discuss patients’ weight, body image, and eating disorders and reasons why they may do so. Method One hundred and eighty-three nursing and medical students (M age = 25.06, SD = 5.43) participated in this study. Participants completed open-ended questions pertaining to their willingness to discuss body image, eating, and weight-related issues with future patients. We further queried students’ perspective on body mass index (BMI) as a measure of weight status and sought to determine if participants’ own weight, weight concerns, appearance evaluation, body appreciation, and experiences of stigma were associated with their willingness to discuss weight-related issues with prospective patients. Results Coding of qualitative data indicated that nursing and medical students were “sometimes” willing to discuss prospective patients’ weight, body image, and eating disorders, especially if a health concern was evident. Nursing students seemed somewhat more willing to discuss weight issues than medical students and willingness to discuss one of these issues (e.g., body image) was positively associated with willingness discuss the others. Plans for future discussions of body image and weight were marginally associated with personal experiences of weight stigma. The majority of participants indicated that BMI was not a valid measure of health. Conclusions Taken together, findings suggest that future providers’ conversations with patients about these sensitive topics are less likely to be associated with their own experiences and more with the relevance of these topics to specific patients.

الأهداف: تهدف هذه الدراسة إلى بيان عناصر المنافع، واستعراض ما لها من أبعاد يتطرق لها احتمالات عديدة، من خلال تتبع المسائل والقضايا المالية أو الجنائية أو التعبدية الدائرة حولها بشكل أساسي أو ثانوي، مع الأخذ بالاعتبار مراحل تطور المنافع في العصر الحديث. المنهجية: اتبعت الدراسة المناهج العلمية المناسبة والمتمثلة، بالمنهج الاستقرائي لما تناوله العلماء من آراء وأفكار حول كيفية الوصول بالمنفعة إلى حالة المعلومية المناسبة للقضايا والمسائل المرتبطة بها، كما استخدم المنهج الوصفي عند عرض عناصر المنفعة وما يندرج تحتها من تفريعات، وذلك بعد الاستعانة بالمنهج التحليلي لتفسير سبب انتظام هذه العناصر تحت أقسام معيّنة واعتبارات خاصة تُسهل معرفة ما يُساهم في إنتاج المنفعة ويؤدي إلى اختلافها. النتائج: توصلت الدراسة إلى نتائج أهمّها أنّ وجود المنفعة في الخارج يعتمد على ثلاثة عناصر أساسية، متمثلة بالموارد والأنشطة التفاعلية الواقعة بينها والظروف المحيطة بها، وكل واحد من هذه العناصر يندرج تحته احتمالات لا نهائية، وفيما يتعلق بالموارد فهي تضم المصادر المشاركة في النشاط التفاعلي، سواء كانت من جهة المقدّم للمنفعة أو من جهة المستفيد منها، وهي تشمل كل ما له بعد مادي ومعنوي كالموارد البشرية والثروات النباتية والحيوانية وجميع الأعيان المنقولة وغير المنقولة، وتشمل أيضاً ما له بعد معنوي فقط كالمعلومات والبرامج والتطبيقات الإلكترونية. الخلاصة: خلصت هذه الدراسة إلى أنّ السبب الأساسي لتنوع المنافع واختلافها؛ هو اعتماد وجودها في الخارج على عناصر لها أبعاد مادية أو معنوية متفاوتة بالأوصاف والمقادير، وبالوقف على ما يُمثل هذه العناصر وما يُبين تأثير اختلافها على المنافع، فإن هذه الدراسة جاءت لتوصي بالبحث في الأحكام الشرعية الخاصة بعناصر المنفعة المساهمة في إنتاجها وتوفيرها.

Abstract Background Eales’ disease is an idiopathic peripheral retinal vasculopathy characterized by retinal phlebitis, ischemia, retinal neovascularization, and recurrent vitreous hemorrhages. But CRAO is an unusual presentation. Case presentation A 27-year-old healthy female nurse of Indian descent presented with sudden vision loss in her right eye upon awakening. Central retinal artery occlusion (CRAO), combined with mild central retinal vein occlusion (CRVO), was diagnosed. During the second of three consecutive sessions of hyperbaric oxygen treatments, her vision rapidly improved. One week later, she developed peripheral phlebitis in the same eye. Infectious, inflammatory, and hematologic etiologies were excluded. The systemic evaluation was normal except for a positive Mantoux tuberculin skin test. Following systemic steroidal treatment, she experienced gradual improvement of her vasculitis. Two weeks later, mild retinal phlebitis appeared in her left eye. Eales’ disease was diagnosed after the exclusion of other diseases. Conclusion This is an unusual Eales’ disease case, which presented as combined CRAO with mild CRVO. The association of CRAO and Eales’ disease is reported here for the first time, to our best knowledge.

Abstract Background We aimed to explore the role of parental health beliefs in parent seeking of eye examinations for their children, using the Health Belief Model. Methods In this quantitative correlational survey study, 100 parents who presented to Barzilai University Medical Center in July 2021 to perform an eye examination to their child completed a questionnaire. Results Only 29.6% of the parents knew that a vision screening is performed in first grade, and 10% of the parents were unsure about where to find local eye care for their kids. Moreover, 19% of the parents indicated that they were concerned that their child would be prescribed glasses unnecessarily, and 10% believed that wearing glasses would weaken their child’s eyes. Data security is vital for multimedia communication. A number of cryptographic algorithms have been developed for the secure transmission of text and image data. Very few contributions have been made in the area of video encryption because of the large input data size and time constraints. However, due to the massive increase in digital media transfer within networks, the security of video data has become one of the most important features of network reliability. Block encryption techniques and 1D-chaotic maps have been previously used for the process of video encryption. Although the results obtained by using 1D-chaotic maps were quite satisfactory, the approach had many limitations as these maps have less dynamic behavior. To overcome these drawbacks, this article proposes an Intertwining Logistic Map (ILM)-Cosine transformation-based video encryption technique. The first step involved segmenting the input video into multiple frames based on the frames per second (FPS) value and the length of the video. Next, each frame was selected, and the correlation among the pixels was reduced by a process called permutation/scrambling. In addition, each frame was rotated by 90° in the anticlockwise direction to induce more randomness into the encryption process. Furthermore, by using an approach called the random order substitution technique, changes were made in each of the images, row-wise and column-wise. Finally, all the encrypted frames were jumbled according to a frame selection key and were joined to generate an encrypted video, which was the output delivered to the user. The efficiency of this method was tested based on the state of various parameters like Entropy, Unified Average Change in Intensity (UACI), and correlation coefficient (CC). The presented approach also decrypts the encrypted video, and the decryption quality was checked using parameters such as mean square error (MSE) and peak signal-to-noise ratio (PSNR).

Objective: To assess knowledge, attitude and practices regarding commonly encountered first aid scenarios among non-medical undergraduate students of Karachi University (KU). Methods: A cross-sectional study was conducted in KU. Institutional Review Board approval was taken from Jinnah Sindh Medical University. At KU, permission was taken from concerned authorities for data collection, sample comprised of 386 students. Students were selected on convenience and availability from various departments of Arts, Business, Social Sciences, Islamic Studies and Pharmacy. Written consent was obtained from the students. Data was analyzed on SPSS 20. Results: Out of 386 participants, the majority of students (67.1%) were willing to attend sessions on first aid. Behçet disease (BD) is a multisystemic disease that commonly involves the eyes. Although it affects patients in all age groups, data on ocular disease by age of onset are limited. This retrospective, multicenter study aimed to compare epidemiology, systemic and ocular manifestations, treatments and outcomes between three age groups: juvenile (n = 25, 14.3%), adult-onset (n = 120, 68.6%) and late-onset (n = 30, 17.1%). Most patients in all groups were male. Systemic manifestations were similar in all groups. Systemic co-morbidities were more common in late-onset patients. Bilateral panuveitis was the most common ocular manifestation in all patients. Non-occlusive retinal vasculitis, peripheral vessel occlusions, cataract and elevated intraocular pressure were found more commonly among juvenile-onset eyes. Anterior uveitis and macular ischemia were most common among late-onset eyes, while branch retinal vein occlusion was most common in adult and late-onset eyes. All patients were treated with corticosteroids. Methotrexate, immunomodulatory combinations and biologic treatments were more commonly used for juvenile-onset patients. All groups had a similar visual outcome. Our study showed that patients with ocular BD have varied ocular manifestations and require different treatments according to age of disease onset, but visual outcome is similar.

A number of intricate deep learning architectures for effective End-to-End (E2E) speech recognition systems have emerged due to recent advancements in algorithms and technical resources. The proposed work develops an ASR system for the publicly accessible dataset on Gujarati language. The approach provided in this research combines features like Mel frequency Cepstral Coefficients (MFCC) and Constant Q Cepstral Coefficients (CQCC) at front-end feature extraction methodologies. Enhanced spell corrector with BERT-based algorithm and Gated Recurrent Units (GRU) based DeepSpeech2 architecture are used to implement the back end portion of the proposed ASR system. The proposed study shown that combining the MFCC features and CQCC features extracted from speech with the GRU-based DeepSpeech2 model and the upgraded or enhanced spell corrector improves the Word Error Rate (WER) by 17.46% when compared to the model without post processing.
