Abstract

Catheter ablation has been wildly used to treat atrial fibrillation (AF) and has achieved a better efficacy for paroxysmal AF (PAF) but not for persistent AF (PerAF). Furthermore, a few data on the efficacy and safety of catheter ablation for PerAF were reported. This study aimed to investigate long-term efficacy of radiofrequency catheter ablation (RFCA) for PerAF and explore predictors of late recurrence of atrial fibrillation (LRAF). A total of 92 consecutive patients with PerAF (64 males, aged 56.42 ± 11.24 years) were enrolled in this study and accepted circumferential pulmonary vein isolation (CPVI) alone or CPVI combined additional ablation. Maintenance rate of sinus rhythm (SR) was 40.2% after a single procedure with median follow-up of 15 months and 52.2% after mean 1.3 ± 0.6 procedures with median follow-up of 26 months. Long-term SR maintenance rate was no statistical difference between patients with CPVI alone and with CPVI combined additional ablation (48.6% vs 35.1%, log rank test, P = .152). Patients with AF duration < 24 months had a higher long-term SR maintenance rate than those with AF duration ≥ 24 months (55.6% vs 30.4%, log rank test, P = .022). Abstract

Background Focused ultrasound ablation research commences in cardiology, and we seek to compare efficacy of focused ultrasound circumferential pulmonary veins ablation (CPVa) and BOX ablation (BOXa) in acute atrial fibrillation (AF) model. Method Twenty mongrel dogs were divided into either CVPa or BOXa groups. CPV or BOX focused ultrasonic ablation (type: CZB, High Intensity Focused Ultrasound (HIFU), China) was performed in corresponding group when acute AF models established. Left atrial effective refractory period (LAERP), inducibility, and induced AF lasting time were evaluated pre- and post-ablation in both groups. The histological observation was performed after ablation procedures. Results The LAERP after AF model establishment was significantly shorter than that before establishment (102±10 ms vs 140±10 ms, p Conclusions In experimental AF model, AF could be prevented by epicardial focused ultrasound ablation. The BOX ablation might be more effective than CPV ablation.

Abstract

Background Understanding the recurrence of atrial fibrillation is important for selecting patients who are undergoing catheter ablation. Several studies respectively evaluated the risk factor of the recurrence of atrial fibrillation postablation. The aim of this study is to explore comprehensive risk factors of AF recurrence in the setting of clinical practice. Objective To investigate the factors predicting the recurrence of atrial fibrillation (AF) after catheter ablation. Methods 186 patients (55.12±12.06 years, 123 male) including 161 paroxysmal AF and 25 non-paroxysmal AF who underwent catheter ablation were studied. Clinical datum before and during ablation were recorded, and systematic follow-up was conducted after ablation. Univariate and multivariate analysis were carried out to determine the factors predicting late recurrence of AF (LRAF) which means AF recurrence after 3 months. Results There were 47 (25.27%) patients experienced LRAF. Multivariate Logistic regression analysis was carried out to the parameters that p Conclusion The diagnosis of overweight/obesity, and metabolic syndrome preablation, procedural failure and ERAF are independent predictors of LRAF in this group of patients.
