Abstract

Introduction: The objective was to conduct a systematic review of economic assessment of interventional strategies, in the prevention of frailty in elderly of 60 years and over living at home. Methods: The keywords were searched in databases such as Pubmed, ScienceDirect, Google Scholar and Embase. Articles published in English and French between 31/10/2010 and 31/12/2021 were included. The CHEERS statement reading grid was used to assess the quality of the studies in terms of economic assessments. Results: The search had identified nine relevant research studies, including seven randomised controlled trials and two quasi-experimental studies. Of these studies, we classified them into three programs: five studies on frailty screening, three studies on falls prevention and one study on the analysis of drugs and treatments prescribed and delivered. According to the cost-effectiveness plan of these programs, four studies had no conclusion on economic results, two studies had a dominant strategy, less expensive and more efficient, and three studies had a dominated strategy, more expensive and not efficient. Only 40% of the studies were of good quality. Conclusion: Only two multidimensional and interdisciplinary intervention strategies were highly efficient, cost-effective and improved the quality of life of elderly. Economic results were more mixed. Many methodological weaknesses were present in these studies.

International audience; Introduction et but de l’étudeL’obésité est un problème mondial de santé publique. Le comportement alimentaire des actifs français évolue avec la société et la tachyphagie peut favoriser la prise pondérale. La tachyphagie peut jouer un rôle sur la prise pondérale. Le but de notre travail était d’étudier les modalités de la pause du déjeuner et le lien avec l’obésité chez des travailleurs actifs.Matériel et méthodesCette étude prospective a été menée de janvier à mai 2016 dans 8 services de médecine. À l’aide d’un auto-questionnaire anonyme des données déclaratives concernant les informations générales dont le poids et la taille avec calcul de l’indice de masse corporelle (IMC) et les modalités de la pause du midi dont le temps de prise du repas ont été recueillies. L’analyse statistique comprenait les tests t de Student, du Chi2 et la régression logistique.Résultats et analyse statistique Quatre-cent-quinze actifs d’un âge moyen de 41,1 ± 12,6 ans ont été inclus. L’IMC moyen était de 23,8 ± 4,6 kg/m2. La prévalence d’obésité était de 15,9 % contre 8,7 % au début du travail actuel (p = 0,02). La profession actuelle était exercée depuis 12,9 ± 12,4 ans. Une tachyphagie (temps de repas < 15 min) était retrouvée dans 20,3 % des cas. En analyse multivariée, le fait de manger seul, et de manger en restauration rapide étaient positivement associés à la tachyphagie (OR = 4,59 [IC95 % : 2,44–9,08], p < 0,0001 et OR = 2,03 [IC95 % : 1,10–3,70], p = 0,023, respectivement) mais aussi à l’obésité (OR = 2,14 [IC95 % : 1,21–3,79], p = 0,008 et OR = 2,40 [IC95 % : 1,25–4,52], p = 0,009, respectivement). En revanche, il n’y avait pas d’association significative entre la tachyphagie et l’obésité.ConclusionNotre étude fournit des informations importantes sur les modalités de la prise du déjeuner chez des actifs. En effet, la tachyphagie était retrouvée dans 20 % des cas. Il est indispensable de rechercher les habitudes alimentaires des actifs au moment de leur pause repas, tout particulièrement chez les patients en excès pondéral. Le médecin généraliste et les services de médecine du travail sont donc des acteurs prépondérants du dépistage des troubles nutritionnels des actifs.

The aim of our work was to study the modalities of the lunch break and the link with obesity inFrench workersMethods:This prospective study was conducted from January to May 2016 in 5 departments of Occupational Medicine of the French region of Haute-ienne. Abstract

ABSTRACT Aggressive behaviour is the most disturbing and distressing behaviour displayed by elderly people. The prevalence of aggressive behaviour is around 50% among psychogeriatric patients. Objective: This study sought to analyze the psychometric properties and diagnostic accuracy of the French version of the Rating Scale for Aggressive Behaviour in the Elderly (F-RAGE). Methods: The F-RAGE was administered to 79 patients hospitalized in a geriatric psychiatry department. A psychiatrist, who was blind to the subjects' RAGE scores, performed the diagnosis for aggressivity based on global clinical impression. The F-RAGE and MMSE were applied by a trained researcher blind to subjects' clinical diagnoses while the Cohen-Mansfield Agitation Inventory and Neuropsychiatric Inventory were administered by medical and nursing staff. Internal consistency, reliability, cut-off points, sensitivity and specificity for F-RAGE were estimated. Results: F-RAGE showed satisfactory validity and reliability measurements. Regarding reliability, Cronbach's a coefficient was satisfactory with a value of 0.758. For diagnostic accuracy, a cut-off point of 8 points (sensitivity=74.19%; specificity=97.98%) and area under curve of 0.960 were estimated to distinguish between aggressive patients and control subjects. Discussion: F-RAGE showed acceptable psychometric properties, supported by evidence of validity and reliability for its use in the diagnosis of aggressive behaviour in elderly.
