Abstract

Background Therapeutic patient education (TPE) is recommended for children with atopic dermatitis (AD), but no consensus has been reached on the optimal tailoring of delivery. While repeated multidisciplinary group education sessions have shown effectiveness, the benefits of one-on-one educational interventions led by nurses for children with AD have not yet been assessed. Objectives To assess the benefits of additional, well-structured, 1-h nurse-led individual TPE interventions in children with AD and their families compared with standard care alone. Methods Children with moderate-to-severe AD and their parents were randomized to receive a 1-h nurse-led education session in addition to standard care vs. standard care alone. The primary outcome was the area under the curve (AUC) of the SCORing of Atopic Dermatitis index (SCORAD) from baseline to week 24 (lower AUC values represent better long-term control of the disease). Results In our study, 176 patients were randomized across 11 centres, and 153 were included in the full analysis set. The mean (SD) age was 4.47 (4.57) years. By week 24, there were no significant differences in the AUCs of the SCORAD between the two groups (P = 0.3). Secondary outcomes including patient-reported severity and quality of life [AUCs of the patient-oriented SCORAD (PO-SCORAD) and Infants' Dermatitis Quality of Life Index (IDLQI), Children's Dermatitis Quality of Life Index (CDLQI) and Family Dermatitis Quality of Life Index (FDLQI)] were not significantly different between the two groups. The only significant change observed in the intervention group, when compared with the one receiving standard care, was a decrease in topical steroid phobia, as assessed by the topical corticosteroid phobia (TOPICOP) score. Prespecified subgroup analyses showed that disease severity in the intervention group was significantly lower throughout the study, compared with the standard-care group when participants had moderate AD at baseline (n = 47); while participants with severe AD at baseline (n = 106) did not show benefit from the intervention. Participants showed no additional benefit from the intervention regardless of age group. Conclusions This study did not show any additional effectiveness, in long-term severity control, of a 1-h nurse-led TPE intervention in children with AD treated with standard care, compared with those treated with standard care alone. However, it should be noted that the intervention reduced the fear of using topical steroids and may be beneficial for patients in the subgroup with moderate AD. [ABSTRACT FROM AUTHOR]

Abstract

Objectives SLE is a systemic autoimmune disease characterized by heterogeneous manifestations and severity, with frequent lung involvement. Among pulmonary function tests, the measure of the diffusing capacity of the lungs for carbon monoxide (DLCO) is a noninvasive and sensitive tool assessing pulmonary microcirculation. Asymptomatic and isolated DLCO alteration has frequently been reported in SLE, but its clinical relevance has not been established. Methods This retrospective study focused on 232 SLE patients fulfilling the 2019 EULAR/ACR classification criteria for SLE. Data were collected from the patient's medical record, including demographic, clinical and immunological characteristics, while DLCO was measured when performing pulmonary function tests as part of routine patient follow-up. Results At the end of follow-up, DLCO alteration (<70% of predicted value) was measured at least once in 154 patients (66.4%), and was associated with a history of smoking as well as interstitial lung disease, but was also associated with renal and neurological involvement. History of smoking, detection of anti-nucleosome autoantibodies and clinical lymphadenopathy at diagnosis were independent predictors of DLCO alteration, while early cutaneous involvement with photosensitivity was a protective factor. DLCO alteration, at baseline or any time during follow-up, was predictive of admission in intensive care unit and/or of all-cause death, both mainly due to severe disease flares and premature cardiovascular complications. Conclusion This study suggests a link between DLCO alteration and disease damage, potentially related to SLE vasculopathy, and a prognostic value of DLCO on death or intensive care unit admission in SLE. Abstract

International audience; BACKGROUND: On the basis of safety data on patients with inflammatory rheumatism or inflammatory bowel disease, treatment with Janus kinase (JAK) inhibitors has been linked to the occurrence of major adverse cardiovascular events (MACEs). However, these inflammatory diseases are pro-atherogenic; in contrast, patients with atopic dermatitis (AD) do not usually have a high cardiovascular (CV) comorbidity burden. METHODS: We systematically searched the PubMed, Embase, Cochrane Library and Google Scholar databases from their inception to September 2nd, 2022. Cohort studies, randomized controlled trials and pooled safety analyses providing CV safety data on patients taking JAK inhibitors for AD were selected. We included patients aged 12 and over. We built a "controlled-period" cohort (9,309 patients: 6,000 exposed to JAK inhibitors and 3,309 exposed to comparators) and an "all JAK inhibitors" cohort (9,118 patients exposed to a JAK inhibitor in any of the included studies). The primary outcome was a composite of acute coronary syndrome (ACS), ischemic stroke, and CV death. The broader secondary MACE outcome encompassed ACS, stroke (whether ischemic or hemorrhagic), transient ischemic attack, and CV death. The frequency of primary and secondary MACEs was assessed in both cohorts. A fixed-effects meta-analysis using Peto’s method was used to calculate the odds ratio (OR) [95% confidence interval (CI)] for MACEs in the "controlled-period" cohort. Risk of bias evaluation was made using the Cochrane Risk of bias tool version 2. Certainty of evidence used the Grading of Recommendations Assessment, Development and Evaluation (GRADE) approach. RESULTS: Eight percent of the initially identified records met the selection criteria, corresponding to 23 records included in the "all JAK inhibitors" cohort". The patients had been exposed to baricitinib, upadacitinib, abrocitinib, ivarmacitinib, placebo or dupilumab. Four primary events (3 with JAK inhibitors and 1 with placebo) and 5 secondary events (4 with JAK inhibitors and 1 with placebo) occurred among 9,309 patients in the "controlled-period" cohort (MACE frequency = 0.04 % and 0.05%, respectively). Eight primary events and 13 secondary events occurred among 9,118 patients in the "all JAK inhibitors" cohort (MACE frequency = 0.08 and 0.14 %, respectively). Odds ratio for primary MACEs in patients with AD treated with JAK inhibitors compared to placebo or dupilumab was 1.35 (95% CI 0.15-12.21; I2, 12%, very low certainty of evidence). CONCLUSION: Our review highlighted rare cases of MACE among JAK inhibitors users for AD. Janus kinase inhibitors may have little to no effect on the occurrence of MACEs in patients with AD compared to comparators but the evidence is very uncertain. Real-life, long-term, population-level safety studies are needed.

Abstract

International audience; OBJECTIVES:To evaluate the diagnostic value of hand ultrasound (US) in systemic sclerosis (SSc) and to explore its relevance within a combined diagnostic approach.METHODS:224 patients with suspected SSc were consecutively included. They all had US evaluation assessing the presence of fibrotic tenosynovitis (fibrotic TS) and ulnar artery occlusion (UAO). The final diagnosis of SSc was based on the clinical evaluation of a board of experts independently of any pre-established classification criteria.RESULTS:166 patients were finally diagnosed as SSc according to the experts as reference standard. 62 SSc and 8 non-SSc patients had UAO (uni or bilateral) (p=0.001). 23 SSc patients and 1 non-SSc patient had US fibrotic TS (p=0.007). Https://doi.org/10.1093/bjd/ljac028 Dear Editor, Slow-flow vascular malformations (VMs), particularly venous VMs, might get complicated by intramalformative thrombosis linked to localized intravascular coagulopathy (LIC).[[1], [3]] Several studies have shown the effectiveness of sirolimus on slow-flow VMs but its effect on coagulation has been poorly studied.[3] Moreover, vascular endothelial growth factor (VEGF) A and C are known for their proangiogenic and lymphangiogenic activities, respectively, and sirolimus may reduce their levels.[4] In this study, we investigated blood levels of VEGF-A and -C, tissue factor (TF) and coagulation markers in children with slow-flow VMs before and after sirolimus treatment. Overall, 82% of patients with venous VMs ( I n i = 18) and 63% with combined VMs ( I n i = 12) had increased D-dimer; two had low fibrinogen. Two studies involving 24 and 62 children with venous VMs reported elevated D-dimer in 33% and 39% of patients, respectively.[2],[7] The proportion of patients with increased D-dimer in venous VMs was higher in our study (82%). Abstract

Background: Parents of children with atopic dermatitis (AD) report reduced quality of life and higher stress level, which could increase risk of psychiatric and pain disorders, and medication use. Methods: By use of Danish national registries, we identified family members of all first‐born Danish children born between 1 January 1995 and 31 December 2013 with a hospital diagnosis of AD, matched them 1:10 with family members of children without AD, and followed the cohorts over time. Results: Mothers of children with hospital‐managed AD had higher risk of filling a prescription for medications for depression, anxiety, pain and sleep problems, and of consulting a psychologist, but most associations disappeared after full adjustment. Siblings had higher risk of receiving a diagnosis for adjustment disorder, and fathers showed increased risk of filling a prescription for pain medication and of divorce, in crude but not adjusted models. Conclusions: The increased risk of study endpoints seen in mothers of children with hospital‐managed AD was not explained by pediatric AD alone. Rather, the total burden in these families including parent and child morbidity and socioeconomic resources seems to explain these observations. Abstract

Background: There is currently little information on switching biologics in pediatric psoriasis. Objective: To evaluate the real‐world clinical practice and safety of switching biologics in the "Biological Treatments for Pediatric Psoriasis" (BiPe) cohort. Methods: Data for all 134 patients included in the BiPe cohort were analyzed. A further evaluation of the subpopulation of patients who switched from a first‐line biologic to a second‐line biologic was then conducted. Drug survival rates were also compared between biologics given as first‐line or second‐line agents. Results: Overall, 29 patients (female: 55%; mean age: 16.6 ± 3.0 years) switched between two biologics. Etanercept (ETN) was the first‐line biologic used in 23 patients: 16 (69.6%) switched to adalimumab (ADA) and seven (30.4%) to ustekinumab (UST). Six patients received first‐line ADA and switched to UST. Loss of efficacy (62.1%), primary inefficacy (20.7%), and parental choice (6.9%) were the main reasons for switching biologics. One (3.4%) of the switches was performed because of adverse events or intolerance. For UST and ADA, the 18‐month drug survival rate did not differ according to whether the agent was given as a first‐line or second‐line biologic (UST: P =.24; ADA: P =.68). No significant differences in drug survival rates were observed between the three different switches (ADA to UST, ETN to ADA, and ETN to UST). Conclusion: Our study provided key insights into the real‐life clinical practice of switching biologics in pediatric psoriasis patients. However, more information and guidance on switching biologics in pediatric psoriasis are needed to improve real‐life practice and outcomes. [ABSTRACT FROM AUTHOR]
