The high GWP potential of construction requires a holistic approach such as circular economy. Currently, common joining, construction and planning practices result in heterogeneous assemblies of different components that are difficult to deconstruct. Furthermore, there is currently little information and data on building components used and the climatic impacts on them. In this context and with the intention of recording long-term (circular) processes in construction, the Piezo-Klett basic research project (FFG no. 879459) funded by the Austrian Research Promotion Agency (FFG) deals with the energy supply of active sensor technology in construction by combining the hook and loop fastener with piezoelectric components. The aim is to open new perspectives on sustainable energy production systems by transforming buildings into energy carriers and generators, analogous to a “battery”. To this purpose, the result presented in this conference paper is a description of the constructive structure (climatic impacts, construction, piezo technology) of a “Piezoelectric Textile Facade” as well as test results on piezo tapes. This opens new possibilities in the context of the application of hook-and-loop fasteners, the energy supply of active sensor technologies as well as in the field of data acquisition and data management.

Introduction: Neurophysiological tests allow accurate assessment of the function of the peripheral nervous system. Detection of neurophysiological changes allows us to understand the neurological clinical symptoms and signs of patients with type 1 and type 2 diabetes and the possibility for their symptomatic treatment. Aim: Evaluate the effect of diabetes mellitus on the “cutaneous silent period” in detecting diabetic polyneuropathy. Material and Methods: The study included 150 subjects, 90 suffering from diabetes, divided into three groups of 30, depending on the disease duration, and a control group of 60 respondents not suffering from diabetes or other polyneuropathies. The control group are referred for EMG analysis on another basis (cervical radiculopathy, brachialgia, etc.). Group 1 consisted of 30 subjects with diabetes mellitus type 2 and duration of illness up to 5 years. Group 2 consisted of 30 subjects with type 2 diabetes mellitus 2 and illness duration from 5 to 10 years. Group 3 consisted of 30 patients with type 1 diabetes mellitus. The study groups consisted of patients referred for EMNG analysis to the EMG office of the Clinical Center of Sarajevo University, Neurology Clinic and the Neurophysiology Laboratory in Ljubljana, from July 1, 2011 to May 1, 2016. All patients were examined neurologically and electroneurographic analysis was performed. Results: A statistically significant difference was found in the incidence of pathologic CSP with respect to the study groups, χ2 = 26.153; p=0.001. Pathologic CSP was more common in group 1 and group 2 of subjects (56.17%) compared to group 3 and control subjects, where it occurred in 13.3% of the cases. Conclusion: The pathological cutaneous period of silence was more frequent in subjects of group 1 and group 2, that is, in subjects with DM type 2, compared to subjects with DM type 1.

Highlights • Spinal dural arteriovenous fistula is an uncommon cause of longitudinal transverse myelitis. • Spinal dural arteriovenous fistula can be easily misdiagnosed. • It usually presents with venous congestive myelopathy symptoms and misdiagnosis is common. • Prescence of flow voids on MRI should raise the suspicion of underlying fistula. • Intravenous steroid and lumbar puncture may be associated with acute neurological deterioration., Introduction and importance Spinal dural arteriovenous fistula (SDAVF) is an uncommon cause of longitudinal extensive transverse myelitis (LETM). It usually presents with vague congestive myelopathy symptoms and diagnosis is usually difficult on initial presentation. Common daily neurological interventions can aggravate the underlying pathophysiology leading to undesirable acute neurological deterioration. Intravenous steroids administration and lumbar (LP) puncture as a diagnostic tool are amongst the most commonly reported aggravating interventions. This rare case presentation highlights this association with its negative impact on the patient outcome in misdiagnosed cases. Case presentation The authors present a sixty-eight-year-old male with paraplegia following steroid administration and LP for presumed inflammatory/autoimmune LETM in the setting of misdiagnosed SDAVF. The absence of flow voids on the conventional T2-weighted magnetic resonance image (MRI) lead to misdiagnosis. He had satisfactory neurological recovery few hours after surgical disconnection. Clinical discussion SDAVF is known to cause congestive myelopathy symptoms. Spinal angiogram is the gold standard for diagnosis. Although the exact mechanism is not fully understood, misdiagnosed cases like our case can develop severe neurological deterioration with steroid administration and lumbar puncture. Conclusion Although SDAVF is an uncommon cause of LETM, Clinicians should carefully exclude it before proceeding to steroid administration or performing LP as they can lead to devastating neurological deterioration.

Aim: To examine the morphologic variations of occipital sulci patterns in patients with schizophrenia and migraine haeadache regarding gender and laterality as well as damage of visual pathway in patients with schizophrenia using magnetic resonance imagining (MRI) and visual evoked potentials (VEPs). Methods: This study included 80 patients and brain scans and visual evoked potential responses recorded over the occipital cortex were performed to analyze the occipital region of both hemispheres. Average total volumes of both hemispheres and average values of amplitude of the healthy population for the comparasion were used. Results: There was statistically significant difference between subjects considering gender (p=0.012). Parameters of parieto-occipital fissure (p=0.0314) showed statistically significant positive correlation with P100 amplitude (p=0.05), body of the calcarine sulcus (p=0.0213) had significant positive correlation with P100 amplitude (p=0.04), retro calcarine sulcus (p=0.0516) and P100 amplitude (p=0.03) showed statistically significant difference only of left hemisphere in male patients with schizophrenia with shallower depth of the sulcus and P100 amplitude reduction. Conclusion: Schizophrenia is associated with the volume changes of the occipital lobe. Comparative analysis of morphologic differences in the gray matter of occipital lobe using MRI and VEPs revealed changes especially of the left hemisphere (shallower depth of sulcus and P100 amplitude reduction) only among a group of male patients.

Conclusion The consistency of the findings reveals distinct multiple brain regions, which show changes in the gray matter of patients with chronic forms of schizophrenia. The neurocognitive deficits of schizophrenia show highly consistent cross-sectional relationships to each type of functional outcome.

Introduction: Compulsory electromyoneurography (EMNG) analysis of all neurophysiological parameters, including the most sensitive parameter for early detection of diabetic polyneuropathy (cutaneous silent periods), in patients without subjective symptoms, and EMNG analysis demonstrates the existence of incipient signs for polynomial neuropathy due to which timely therapeutic approach is needed to prevent complications of diabetic disease and prevent irreversible changes in peripheral nerves. Aim: Examine the influence of type diabetes mellitus, therapeutic modality, and gender of patients on neurophysiological parameters obtained by EMNG analysis. Methods: The study included 90 patients with diabetes who were divided into three groups of 30, depending on the duration of the disease. Group 1 consisted of 30 respondents with type 2 diabetes mellitus and up to 5 years of disease duration. Group 2 consisted of 30 respondents with type 2 diabetes mellitus type and 5 to 10 years of disease duration. Group 3 consisted of 30 respondents with Type 1 diabetes mellitus. An electron-neurography analysis of peripheral nerve in the extremities was performed. Results: Group 1 (50%) and group 2 (56.17%) respondents had statistically higher incidence of tingling than those in Group 3 (13.3%), p=0.004. Tingling was not statistically significantly different in relation to the examined groups (p=0.314). Reflexes were statistically the most preserved in Group 3 (86.7%), p = 0.001. Measurement of motor conductivity values at median nerve had a significant difference in all parameters (distal latency, amplitude, mean conduction velocity (MCV) and latency in the group with DM type 1, compared to respondents with DM type 2. The same significant difference between all parameters was found when testing peroneus nerve. When measuring motor velocity conductivity in ulnar nerve, there was no significant difference in amplitude, while DM1 type 1 patients had significant differences in values: distal latency and MCV p

Background Perioperative hypothermia has been reported to increase the occurrence of cardiovascular complications. By increasing the activity of sympathetic nervous system, perioperative hypothermia also has the potential to increase cardiac injury and dysfunction associated with subarachnoid hemorrhage. Methods The Intraoperative Hypothermia for Aneurysm Surgery Trial randomized patients undergoing cerebral aneurysm surgery to intraoperative hypothermia (n = 499, 33.3 degrees +/- 0.8 degrees C) or normothermia (n = 501, 36.7 degrees +/- 0.5 degrees C). Cardiovascular events (hypotension, arrhythmias, vasopressor use, myocardial infarction, and others) were prospectively followed until 3-month follow-up and were compared in hypothermic and normothermic patients. A subset of 62 patients (hypothermia, n = 33; normothermia, n = 29) also had preoperative and postoperative (within 24 h) measurement of cardiac troponin-I and echocardiography to explore the association between perioperative hypothermia and subarachnoid hemorrhage-associated myocardial injury and left ventricular function. Results There was no difference between hypothermic and normothermic patients in the occurrence of any single cardiovascular event or in composite cardiovascular events. There was no difference in mortality (6%) between groups, and there was only a single primary cardiovascular death (normothermia). There was no difference between hypothermic and normothermic patients in postoperative versus preoperative left ventricular regional wall motion or ejection fraction. Compared with preoperative values, hypothermic patients had no postoperative increase in cardiac troponin-I (median change 0.00 microg/l), whereas normothermic patients had a small postoperative increase (median change + 0.01 microg/l, P = 0.038). Conclusion In patients undergoing cerebral aneurysm surgery, perioperative hypothermia was not associated with an increased occurrence of cardiovascular events.

Background Although hypothermia and barbiturates improve neurologic outcomes in animal temporary focal ischemia models, the clinical efficacy of these interventions during temporary occlusion of the cerebral vasculature during intracranial aneurysm surgery (temporary clipping) is not established. Methods A post hoc analysis of patients from the Intraoperative Hypothermia for Aneurysm Surgery Trial who underwent temporary clipping was performed. Univariate and multivariate logistic regression methods were used to test for associations between hypothermia, supplemental protective drug, and short- (24-h) and long-term (3-month) neurologic outcomes. An odds ratio more than 1 denotes better outcome. Results Patients undergoing temporary clipping (n = 441) were assigned to intraoperative hypothermia (33.3 degrees +/- 0.8 degrees C, n = 208) or normothermia (36.7 degrees +/- 0.5 degrees C, n = 233), with 178 patients also receiving supplemental protective drug (thiopental or etomidate) during temporary clipping. Three months after surgery, 278 patients (63%) had good outcome (Glasgow Outcome Score = 1). Neither hypothermia (P = 0.847; odds ratio = 1.043, 95% CI = 0.678-1.606) nor supplemental protective drug (P = 0.835; odds ratio = 1.048, 95% CI = 0.674-1.631) were associated with 3-month Glasgow Outcome Score. The effect of supplemental protective drug did not significantly vary with temperature. The effects of hypothermia and protective drug did not significantly vary with temporary clip duration. Similar findings were made for 24-h neurologic status and 3-month Neuropsychological Composite Score. Conclusion In the Intraoperative Hypothermia for Aneurysm Surgery Trial, neither systemic hypothermia nor supplemental protective drug affected short- or long-term neurologic outcomes of patients undergoing temporary clipping.

This thesis deals with the topic of fiscal policy in the USA during the administration of Bill Clinton and origins of budget surpluses at the end of the 1990s. First I describe fiscal and economic situation during the administration of presidents Reagan and Bush. Then I focus on period of the Clinton administration and key budget acts. Using budget analysis I determine main changes in budget outlays and receipts. I come to conclusion that favourable economic situation led by development of informational technology and other positive factors had an essential impact on stabilization of American federal budgets. However, the Clinton administration managed to take advantage of these circumstances and through fiscal policy significantly contributed to creation of the budget surpluses. In the end I present few recommendations for policymakers.

Pljevlja settlement was founded in the second half of the 15th Century in a location where significant trade routes passed through. This settlement was apart of the Bosnian kingdom and partially was the property of Herceg Stipan Kosaca, a local notable. The settlement region of Pljevlja was incorpareted into the administrative organization of the Ottoman Empire in 1465 at the latest. Pljevlja, to be named Taşluca later on, was quite densely inhabitated by the indigenous inhabitants, called "Bosnjani". According to the first survey register of Bosnian Sanjack dated 1468/1469, which included the records of Pljevlja, there were 72 households and 23 single male population in this settlement. In the course of the 16th century, Pljevlja, i.e., Taşluca, experienced a constant population increase, but this situation slowed down in 1580's due to the appearance of plaque, and, therefore, Pljevlja' population underwent a decrease as was the case for the Herzegovinian Sanjak in general. Although the foundation date of the town of Pljevlja can be traced back to 1477, it was only in 1570s that the foundation process was fully completed. The primary role in this regard was played by the Sanjak Beg of Herzegovina, Hüseyin Beg, who built a mosque and many other buildings that gave the Islamic character of the city Pljevlja in 1570s. Thanks to these developments and its becoming the residence of the Herzegovinian Sanjak Beg, Pljevlja had the opportunity to continue its demographic development., Bu çalışmada amaç, 15. yüzyılın ikinci yarısı ve 16. yüzyıl Osmanlı tahrir defterlerine dayanarak Pljevlja'nın Plyevlya 1 demografik gelişimini ve söz konusu dönemde nüfusun dinsel ve üretimsel yapısını ortaya koymaktır2. Listeria monocytogenes is a small, aerobic or facultative anaerobic, non-sporulating gram positive bacillus that can be isolated from soil, vegetation or animal reservoirs. There are six species of Listeria, and only L. monocytogenes is pathogenic for humans. Human disease occurs mainly in immunocompromised people, neonates and in pregnancy, while the cases in immunocompetent people are rare. CNS manifestations of the disease can be in form of meningitis, encephalitis, and also cerebritis and abscess since L. monocytogenes shows tropism for brain and brain stem as well for the meninges. In this case we presented 55 year old male patient with etiologically confirmed listerial meningoencephalitis, transferred from regional hospital tothe Clinic for Infectious Diseases with diagnosis of acute meningoencephalitis. Disease started 4 days before the admission. Prior to this the patient was completely healthy. In his history he denied any preexisting disease. At admittance he was febrile, with altered consciousness, disoriented, showing ocular deviation, dystaxia, and with completely positive meningeal signs. Neurologist diagnosis was rhombencephalitis. CSF analysis showed mildly opalescent liquor with pleocytosis 546/mm3 and polymorphonuclear cell predominance70%. CSF culture showed positive isolate of L. monocytogenes. Initial therapy was: Penicillin G and Chloramphenicol, together with all other supportive and symptomatic therapy. After initial therapy and based on antibiogram, ampicillin was administered for4 weeks, followed by imipenemum for 10 days. Control CSF analysis showed pleocytosis and increased protein level and the patient was discharged as recovered with diagnosis of acute meningoencephalitis

OBJECTIVE: We examined the incidence of perioperative fever and its relationship to outcome among patients enrolled in the Intraoperative Hypothermia for Aneurysm Surgery Trial. METHODS: One thousand patients with initial World Federation of Neurological Surgeons grades of I to III undergoing clipping of intracranial aneurysms after subarachnoid hemorrhage were randomized to intraoperative normothermia (36 degrees C-37 degrees C) or hypothermia (32.5 degrees C-33.5 degrees C). Fever (> or =38.5 degrees C) and other complications (including infections) occurring between admission and discharge (or death) were recorded. Functional and neuropsychologic outcomes were assessed 3 months postoperatively. The primary outcome variable for the trial was dichotomized Glasgow Outcome Scale (good outcome versus all others). RESULTS: Fever was reported in 41% of patients. In 97% of these, fever occurred in the postoperative period. The median time from surgery to first fever was 3 days. All measures of outcome were worse in patients who developed fever, even in those without infections or who were World Federation of Neurological Surgeons grade I. Logistic regression analyses were performed to adjust for differences in preoperative factors (e.g., age, Fisher grade, initial neurological status). This demonstrated that fever continued to be significantly associated with most outcome measures, even when infection was added to the model. An alternative stepwise model selection process including all fever-related measures from the preoperative and intraoperative period (e.g., hydrocephalus, duration of surgery, intraoperative blood loss) resulted in the loss of significance for dichotomized Glasgow Outcome Scale, but significant associations between fever and several other outcome measures remained. After adding postoperative delayed ischemic neurological deficits to the model, only worsened National Institutes of Health Stroke Scale score, Barthel Activities of Daily Living index, and discharge destination (home versus other) remained independently associated with fever. CONCLUSION: These findings suggest that fever is associated with worsened outcome in surgical subarachnoid hemorrhage patients, although, because the association between fever and the primary outcome measure for the trial is dependent on the covariates used in the analysis (particularly operative events and delayed ischemic neurological deficits), we cannot rule out the possibility that fever is a marker for other events. Only a formal trial of fever treatment or prevention can address this issue.

Q-fever is antropozoonosis caused by Coxiella burnetii. The microorganism usually transmitted to humans through the inhalation of infected dust from sub clinical infected mammals. The most common reservoirs for Coxiella burnetii are large numbers domestic and wild animals. The illness can occur by farmers or slaughterhouses, or veterinarian and laboratories workers. In this report we presented case of endocarditic caused by Coxiella burnetii. The patient is driver 43 years old, who lived in the countryside Nahorevo. He hospitalized in May, 2002. He had symptoms of hepatitis, with elevation of transaminases in sera. Endocarditic was diagnosed by echosonografy. The patient is controlled by infectologist, cardiologist by cardio-surgeon, but since disease is in progress and operation treatment will be probably necessary.

In siege and blocked Sarajevo, in 40 months, in primary care unit and than in Emergency Department 42,075 injured and ill persons have got the first aid. Out of that, 37% were injures 16% acute surgical conditions, 23% internal urgent conditions (since October 1994). There were 24% non-urgent cases from the surgical field. In this period of time 107,453 patients were treated hospitably, with an average of 2.108 hospital beds. In 1996, the primary care was given in 21,708 urgent cases, or 60% of 36,312 hospitably treated persons in total. There were 29% injuries, 24% of acute surgical cases, and 47% of internal medicine cases. Urgent centre received 53% individual paints who came by themselves without any medical aid; 25% from health centres where they have bog a primary care, and 22% came after received first aid of primary care in the Emergency department. By applying war surgical doctrine and general medical doctrine, with appropriate services for prompt diagnostics and treatment, with qualified specialists and consultancy services, a good quality of adequate treatment and care has been achieved, the mortality has been reduced, the hospitalisation has been rationalised, and treatment length has been reduced. Analysis demonstrates the reasons for existence of such medical centres in large hospitals. We have recognised the problems we are facing with, and which have to be solved progressively, as well as those concerning the functioning health services in Sarajevo Canton, and Federation of BH. On the basis of all these knowledge's we believe that a well organised urgent medicine implemented in practice has great importance in the overall treatment. Basic objectives for establishing the urgent medicine as a system are: The first priority is to organise a modern system of urgent medicine, based on the principles of general medical doctrine and complementing with health care system. Urgent medicine programme should be incorporated in the process of medical education of the medical staff, giving a priority to the activities in practice. Provide modern equipment and adequate working space. Develop already developed co-operation with institutions and professionals from Europe and USA. Improve the low level of the first aid training of the population through the first aid courses.

Introduction: Incidence mumps infection has declined since the introduction of the inevitable MRP vaccination during the stage of childhood. In Bosnia and Herzegovina (BIH), from a period of February 2011 until today, there is an evident inclination of the recorded cases of the mumps infection. Orchitis usually occurs in between 3 to 10 days after the parotitis and is found with the post-puberty population. Aim: The aim of the study is to confirm the changes that occur during an early stage of the spermiogram and hormonal status, and after treated mumps orchitis, the patients treated in the Clinic for the Infectious Diseases. Patients and Methods: Retrospectively, the analysis of 54 historical diseases was undertaken. During the research stage, the patients after being discharged have made an inquiry to our clinical consultancy units with final results of the spermiogram and hormone-FSH, LH and testosteron. The data analysis was processed with the SPSS program for Windows. Results: the average length of the hospitalization period was in between 8 (medium) days, and average age M (mean) = 21, 9±5,4 years. The speriogram of the treated patients was undertaken one month after the acute phase of the disease and has shown the following results: azoospermia with 14 patients (25,9%), oligospermia with 30 patients (55,6%) and normospermia with 10 patients (18,5%). During the hormonal status with 11 patients (20,4%), the results have shown the inclination of the value in FSH hormons, with 11 patients (20,4%), have shown the lesser value of the testosterons. Conclusion: The current study suggests that post-orhitis atrofia is expected within a period of 2-3 months after the infection, and thus, the monitoring on the patients’ treatment would continue. The male infertility as a result of mumps ocrhitis is controversial and continues to be the thematic issue as well as the effect of orchitis on testicular endocrine function.

The authors present their experience on, 1168 operated patients with hepato-pulmonary hydatid disease. In a long work with hydatid disease the authors refused many earlier methods, as mistake. The authors think that great radical surgery in the treatment of the hydatid disease is not justified. For success in treatment of hydatid disease it's necessary to operate on the parasite and it's consequences, to keep the function of the organ to prevent contamination of the environment, and to make conditions for fast reduction of peri-cystic cavity.
