Introduction : Les métastases gastriques d’un mélanome choroïdien sont exceptionnelles. La symptomatologie clinique est souvent non spécifique responsable d’un retard diagnostique. Seule une chirurgie radicale lorsqu’elle est possible permet une chance de survie. Observation : Patiente âgée de 60 ans, traitée pour mélanome choroïdien en 2007, admise aux urgences pour une hématémèse de faible abondance. La patiente a rapporté également des épigastralgies depuis trois mois et une perte pondérale de 10 kg. La fibroscopie oesogastroduodénale a objectivé de multiples ulcérations fundiques avec un saignement minime. L’histologie a révélé une métastase d’un mélanome. La TDM n’a pas montré d’autres localisations secondaires. Une gastrectomie totale était envisagée. La patiente est cependant décédée à la suite d’une hémorragie foudroyante avant qu’aucun geste d’hémostase n’ait été tenté. Discussion : La symptomatologie clinique de ces métastases est atypique, et les complications sont rarement révélatrices. A la fibroscopie oesgastroduodénale, la présence d’un ou plusieurs nodules noirâtres parfois surmontée d’une ulcération permet d’évoquer le diagnostic. L’himmunohistochimie confirme le diagnostique et principalement devant des lésions achromiques. Le bilan d’extension doit rechercher d’autres localisations secondaires : pulmonaires, hépatiques, péritonéale. Le traitement est chirurgical et ne doit être envisagée que si la résection complète est possible ou en cas de complication. La survie des métastases gastriques des mélanomes est liée au caractère complet de la résection. Conclusion : L’exploration endoscopique doit être envisagée devant toute symptomatologie digestive chez des patients ayant un antécédent de mélanome. Le diagnostic précoce permet la réalisation d’un geste chirurgical radical et donne une chance de survie au patient., Maroc Médical, Vol. 34, No 1 (2012)

- Abstract
Lumbosciatica is a frequent symptom. When it is hyperalgic and/or deficient, it requires urgent exploration and an eventual surgical procedure. In most of the cases, medullary lumbar (instead of medullary) MRI is required looking in the first place at an intervertebral disc herniation. Other etiologies are rare but must be kept in mind. We report a case of a 37-year-old man with left L5 hyperalgic and deficient lumbosciatica. The conclusion of the etiological research led to pseudoaneurysm of the internal iliac artery compressing the ipsilateral sciatic nerve. The patient was operated on with a good clinical course.

14. Traumatic Atlantoaxial Dislocation with an Odontoid Fracture: A Rare and Potentially Fatal Injury.

- Abstract
Traumatic dislocation of the atlanto-axial joint in combination with an odontoid fracture remains a rare entity. Beaucause of its instability, it's alsoo a seriuous injury. A fatal outcome is feared especially in elderly. We report a case of 74-year-old man who presented with neck pain Confusion and spastic tetraparesia after a low energy trauma. Radiographs and computed tomography demonstrated a C1C2 dislocation with odontoid fracture. After an unsuccessful attempt at closed reduction with halo traction, a surgical stabilisation was performed using a posterior approach. - Abstract
Leishmaniosis (Leishmania infantum infection) and piroplasmoses (Theileria equi and Babesia caballi infections) are vector-borne diseases with significant economic and public health impacts. Despite their importance, there is a lack of data concerning these infections in equids from Tunisia. The present study was carried out to estimate the prevalence of L. infantum, T. equi and B. caballi in 104 equids from northern Tunisia. The authors reported for the first time on the seroprevalence of anti-Leishmania antibodies in equids in Tunisia (6.7%). The study reported a high infection prevalence of piroplasms (23.1%), revealed for the first time T. equi and B. caballi infections in Tunisian donkeys, and showed that these animals act as reservoirs for the maintenance and dissemination of piroplasms., (© World Organisation for Animal Health (OIE), 2018)

- Abstract
Pycnodysostosis is a rare hereditary disease, characterized by systemic bone sclerosis. Susceptibility to long bone fractures is characteristic, whereas vertebral fractures are extremely rare. We report a case of a 21-year-old man with a past history of pycnodysostosis and spontaneous leg fractures who was admitted in hospital for a neck pain after a banal fall. Radiological examination revealed C1-C2-C3 posterior arch fractures with a C3-C4 left articular fracture dislocation. A surgical stabilization was decided but refused by the patient. To the best of our knowledge, this is the first publication that reports pycnodysostosis with cervical spine traumatic staged injuries.

- Abstract
Context: Fibroblast-like synoviocytes (FLS) from rheumatoid arthritis (RA) display pathogenic behavior. Various members of the Wnt pathway, especially the canonical Wnt/β-catenin cascade, may contribute to autonomous RA FLS activation. It has been shown that the two Wnt inhibitors: sFRP3 and DKK1 contribute to several critical aspects of joint biology. However, their effects on RA FLS are poorly characterized. The aim of our study was to investigate the effects of sFRP3 and DKK1 on FLS markers, Wnt components, and target oncogenes expression by RA FLS and compare the findings to osteoarthritic (OA) FLS., Materials and Methods: RA and OA FLS were treated with sFRP3 and DKK1 for 6 days. Wnt signaling components (Wnt5a, LRP5 and β-catenin), Wnt target oncogenes (cyclin E1 and WISP1), and FLS markers (fibronectin and MMP3) were analyzed using western blotting and/or qRT-PCR., Results: Our data indicated that sFRP3 down-regulated the key gene β-catenin in RA FLS. sFRP3 decreased fibronectin, a well-known downstream effectors gene of Wnt/β-catenin pathway, and LRP5 expression in both RA and OA FLS. In OA FLS, sFRP3 induced increased expression of Wnt5a and MMP3 but did not affect their levels in RA FLS. - Abstract
Aim: To determine the demographic, clinical, biological and bacteriological profile of acute community acquired hematogenous osteomyelitis (AHO) in children., Methods: Prospective study including children admitted for AHO. We noted the demographic parameters of patients and the clinical, biological and radiological characteristics of the infection. Blood cultures and local specimen in operated children were systematically performed., Results: 70 patients were included. The mean age was 7.7 years. The mean time between onset of symptoms and admission was 3.2 days. Distal tibia was the most frequent localization (18.66%). Fever higher than 38° on admission was found in 92.8% of patients. Creactive protein (CRP) was superior to 20 mg / L in 95.8% and ESR superior to 20 mm in the first hour in 92% of cases. A deep venous thrombosis was found in 7 patients and a pleuropulmonary Staphylococcus infection in 4 patients. A micro organism was isolated in 64.7% of cases. Staphylococcus aureus methicillin susceptible (SAMS) was the predominant germ. Staphylococcus aureus methicillin resistant (SAMR) accounted for 15.7% of staphylococcus aureus infections. The importance of CRP on admission and time to resolution of fever after the start of treatment were significantly higher in SAMR infections. The existence of deep venous thrombosis and a pleuropulmonary Staphylococcus infection and the need for surgical drainage were significantly more frequent in SAMR infections., Conclusion: The existence of a severe form of AHO should lead to a high suspicion of SAMR infection and prompt the prescription of an appropriate antibiotiotherapy.
