1. Characterization of NBI-driven shear Alfv\'en waves in the TJ-II stellarator using Mirnov probes and electrostatic potential fluctuation measurements

Abstract: We present the first experimental measurements of the toroidal mode number of shear Alfv\'en waves in the TJ-II stellarator. A series of experiments were carried out in three different magnetic configurations to investigate counter-NBI driven modes. Co- and counter- electron-cyclotron current drive was used to modify the rotational transform ($\iota/2\pi$) profile leading to the destabilization of a varied set of Alfv\'en eigenmodes with different frequencies and mode numbers. To characterize the spatial structure of the modes we have used two Mirnov probe arrays, one dedicated to the measurement of the poloidal mode number and the other, a recently commissioned helical tri-axial array, dedicated to the measurement of the toroidal mode number. A heavy ion beam probe, operated in radial sweep mode, was employed to characterize the radial location of the modes. We show that the induced changes in $\iota/2\pi$, that are fundamental when it comes to validation studies, cannot be measured experimentally with motional Stark effect so, instead, the shielding current diffusion equation is solved in cylindrical geometry to estimate these changes. We calculate the incompressible shear Alfv\'en continuum for selected cases using \texttt{STELLGAP} and find reasonable consistency with observations. A database with the observed modes has been created, so that it can be used in future work for theory validation purposes.

2. Measurement of zero-frequency fluctuations generated by coupling between Alfven modes in the JET tokamak

Abstract: We report the first experimental detection of a zero-frequency fluctuation that is pumped by an Alfv\`en mode in a magnetically confined plasma. Core-localized bidirectional Alfv\`en modes of frequency inside the toroidicity-induced gap (and its harmonics) exhibit three-wave coupling interactions with a zero-frequency fluctuation. The observation of the zero-frequency fluctuation is consistent with theoretical and numerical predictions of zonal modes pumped by Alfv\'en modes, and is correlated with an increase in the deep core ion temperature, temperature gradient, and confinement factor $H_{89,P}$. Despite the energetic particle transport induced by the Alfv\`en eigenmodes, the generation of a zero-frequency fluctuation that can suppress the turbulence leads to an overall improvement of confinement.

Abstract: Providing stable and clean energy sources is a necessity for the increasing demands of humanity. Energy produced by fusion reactions, in particular in tokamaks, is a promising path towards that goal. However, there is little experience with plasmas under conditions close to those expected in future fusion reactors, because it requires the fusion of Deuterium (D) and Tritium (T), while most of the experiments are currently performed in pure D. After more than 20 years, the Joint European Torus (JET) has carried out new D-T experiments with the aim of exploring the unique characteristics of burning D-T plasmas, such as the presence of highly energetic ions. A new stable, high confinement and impurity-free D-T regime, with strong reduction of energy losses with respect to D, has been found. Multiscale physics mechanisms critically determine the thermal confinement and the fusion power yield. These crucial achievements importantly contribute to the establishment of fusion energy generation as an alternative to fossil fuels.

Abstract: The technological level and human capital are key factors of economic development. The development of human capital and improving the quality of life of the population are the primary tasks for socio‐economic transformations in the country for the next 7–10 years. Taken into account the significant technological changes within the 6th K‐wave (Kondratieff wave), and the prospects for the development of MANBRIC technologies, dis‐ closed in this article, it can be argued that the relevance of human capital will only increase. Aim. To consider the impact of a new technological way on structural changes in human capital in the theory of Kondratieff waves and to identify potential consequences for the development of society. Methods. Method of analysis to understand the aspects and limitations of the theory of technological structures, since it is used by many researchers to explain the causes of long‐wave dynamics; methods of typification and classification of technological challenges. The systematization method made it possible to structure and describe the qualitative characteristics of Kondratieff waves and correlate them with the stages of development of society. The synthesis method was used to describe the consequences of global social changes on the structural components of human capital and the development of society. Results. The article examines the processes of formation and use of human capital in the context of changes in global economic and technological structures. The authors described the components of human capital, identified global technological challenges, correlated global technological challenges with the pe‐ riodization of Kondratieff waves, analyzed and systematized the factors effecting the structural components of human capital within the 6th Kondratieff wave. The described changes and assessment of potential consequences are for the emerging state technological and socio‐economic policies.

7. Dynamic contrast enhancement and wall enhancement index for the quantitative assessment of vascular wall abnormalities in intracranial atherosclerosis: a pilot study

Abstract: Background: Breakthrough neurotechnologies have allowed for new understanding of some brain disorders; however, identification and differential diagnosis of intracranial stenotic and occlusive lesions remains challenging. Magnetic resonance imaging (MRI) with dynamic contrast enhancement (DCE) is a tool that could be used for the quantitative assessment of endothelial permeability and microvascular volume in atherosclerotic plaques (AP). Aim: To assess quantitative parameters of vascular wall abnormalities in AP area and in obviously unchanged wall of intracranial arteries with MRI DCE and high spatial resolution Т1-weighed images before and after contrast injection, with calculation of the wall enhancement index (WEI) by mathematical modelling. Methods: This was a pilot cross-sectional uncontrolled study with consecutive recruitment of 29 patients with atherosclerotic abnormalities of brachiocephalic arteries, including intracranial. The patients’ median age was 66 [57; 72] years; they were mostly men (75.9%, n = 22). For the assessment of any brain abnormalities, MRI (magnetic induction 3 Tesla, Magnetom Prisma, Siemens) was performed in patients with standard sequence (Т2, T2-FLAIR), as well as MRI DCE for the assessment of intracranial arteries, before and after intravenous contrast injection, with high spatial resolution T1-weighed imaging and suppression of the signal from bloodstream and fat, with the calculation of WEI. Results: There were significant differences in WEI in AP and in unchanged wall (0.962 [0.686; 1.387] vs. 0.111 [0.014; 0.206], p 0.001). No significant differences were found between WEI values in internal carotid arteries APs (0.722 [0.573; 1.580]), middle cerebral arteries (0.921 [0.725; 1.183]), and basilar artery (1.343 [1.002; 1.419]) (p = 0.381). We also found significant difference (p = 0.034) in the extravascular extracellular fraction volumes ve (Tofts) in AP located in the basilar artery (0.171 [0.146; 0.325]), internal carotid arteries (0.579 [0.358; 1.000]), and middle cerebral arteries (0.134 [0.101; 0.269]). Conclusion: This is the first description of quantitative parameters characterizing vascular wall abnormalities in intracranial atherosclerosis. Despite its obviously intact state, vascular walls outside the intracranial AP was shown to be abnormal as well.

A new approach to the treatment of axSpA was considered, which consists of depleting autoreactive TRBV9+ T- lymphocytes by introducing a monoclonal antibody – the drug seniprutug. The value of the drug seniprutug in the treatment of the disease was discussed and key aspects for the incorporation of the drug seniprutug into real-world clinical rheumatological practice were agreed.

Abstract: BACKGROUND: Transient Idiopathic Perivascular Inflammation of the Carotid artery (TIPIC) or carotidynia is a clinical and radiological syndrome manifested as ipsilateral neck pain and an ipsilateral perivascular infiltrate according to ultrasound and magnetic resonance imaging. Due to the low awareness of physicians, this pathology is often mistakenly regarded as dissection or atherosclerosis of the carotid arteries, which leads to additional unnecessary diagnosis and treatment. CLINICAL CASE DESCRIPTION: Here, we present a case of idiopathic carotidynia with a discussion of the diagnostic algorithm and management of patients with unilateral neck pain. CONCLUSION: Timely and competently interpreted ultrasound and magnetic resonance imaging studies of arteries is a key link in the diagnosis of carotidynia. The complete regression of symptoms and pathological changes in the arteries without a specific therapy classifies it as a benign variant of non-atherosclerotic arteriopathy.

11. Prevalence and sensitivity of contagious and environmental cow mastitis-causing pathogens to antibiotics in Ukrainian farms

Abstract: Mastitis is considered the most common and problematic disease, resulting in significant economic losses due to reduced milk yields, reduced quantity and quality of milk, treatment costs, and premature culling of animals. One of the traditional methods of treating mastitis in cows is using antibiotics, which leads to the emergence of polyresistant strains of microorganisms, the so-called Superbugs. The emergence of Superbugs, which are not sensitive to most existing antibiotics, is a major concern in veterinary and humane medicine. This study aimed to identify pathogens isolated from the secretion of the mammary gland of cows with mastitis to determine their spread and sensitivity to antibiotics. The samples of secretion from the udder were examined by bacteriological method. The isolates were identified by conventional methods and by the modern method – mass spectrometry (MALDI-TOF MS). The sensitivity of the bacterial isolates to antibiotics was determined by the disc diffusion method (Kirby-Bauer). The results of studies of cow mammary gland secretion samples indicate that 49.2% of the isolates are contagious, and 50.8% are environmental ones. The most common among the isolates of mastitis-causing pathogens were Streptoccocus аgalactiae – 16.9%, Streptococcus uberis – 10.9%, Staphylococcus aureus – 10.7%, E. Coli – 9.6%, Corynebacterium bovis – 7.3%, Staphylococcus haemolyticus – 4.8%, Staphylococcus chromogenes – 3.6%, Streptococcus dysgalactiae – 3.4%. Mastitis is caused by algae and yeast – 1% of all detected pathogens, respectively. According to the results of the determination of the sensitivity of isolates of mastitis-causing pathogens to antibiotics, it was found that most isolates were sensitive to Amoxicillin, Ceftiofur, and Rifampicin, and least of them – to Neomycin, Tylosin, Tilmicosin, Bacitracin.

Abstract: Megaelectron volt (MeV) alpha particles will be the main source of plasma heating in magnetic confinement fusion reactors. Yet, instead of heating fuel ions, most of the energy of alpha particles is transferred to electrons. Furthermore, alpha particles can also excite Alfvenic instabilities, previously considered as detrimental. Contrary to expectations, we demonstrate efficient ion heating in the presence of MeV ions and strong fast-ion driven Alfvenic instabilities in recent experiments on the Joint European Torus (JET). Detailed transport analysis of these experiments with state-of-the-art modeling tools explains the observations. Here we show a novel type of turbulence suppression and improved energy insulation in plasmas with MeV ions and fully developed Alfvenic activities through a complex multi-scale mechanism that generates large-scale zonal flows. This mechanism holds promise for a more economical operation of fusion reactors with dominant alpha particle heating and, ultimately, cheaper fusion electricity

14. Artificial nerve conduit for guiding peripheral nerve growth (cadaveric study)

Abstract: At present, the search for effective ways of restoring peripheral nerves with anatomical damage continues. Autoplasty still remains the gold standard, which, however, is not without its drawbacks. The use of nerve implants for promoting directional axon growth is essential and promising.Objective: to study the biomechanical properties of laboratory samples of an artificial nerve conduit (NGC) made of hybrid biomaterials and to, on cadaveric material, assess the technical feasibility of using them in surgical practice to repair extended peripheral nerve defects.Material and methods. The objects of the study were three electrospun NGC samples: from synthetic material (polycaprolactone, PCL) and hybrid biomaterials (PCL + gelatin or PCL + collagen). The work compared the physical and mechanical properties of NGC: stiffness, plasticity, elasticity, brittleness, resistance to chemical attack, their ability to be impregnated with liquid media, permeability, possibility of making an anastomosis between the implant and the nerve during surgical procedure. Cadaveric material was the object of the study: we used a dissected superficial sensory branch of the human right radial nerve, 2 mm in diameter, isolated on the forearm, about 12 cm in length, because it most corresponded to the diameter of the NGC samples tested. After surgery, the echogenic features of the implants and their anastomoses with the nerve were assessed by ultrasound imaging.Results. It was found that hybrid NGC samples, based on their biomechanical properties, are fundamentally suitable for use in surgical practice, to ensure growth and replacement of a peripheral nerve defect. However, the best composition of a nerve guide can be established after comparative preclinical study of the biocompatible and functional properties of hybrid material samples.Conclusion. The physical and mechanical properties of the investigated NGC samples made of hybrid biomaterials meet the technical requirements for implantable nerve conduits for surgical application.

Abstract: Introduction. Reintervention in patients with spinal disk herniation is shown to significantly decrease likelihood of favorable outcomes in the postoperative period. Thus, it is important to individually assess risk factors for and likelihood of spinal disk herniation recurrence for each patient, and choose a suitable surgical option. Objective: to evaluate changes in the levels of immunoregulatory mediators in the blood serum and extracted spinal disc tissue of allegedly healthy individuals and patients with lumbar disk herniation relapses. Materials and methods. We examined 60 patients. The control group included 19 patients with traumatic spinal cord injuries at the lumbar level. The main group included 41 patients with spinal disk herniation. Twenty-two individuals had primary herniation while 11 patients presented with single clinical and neurological relapses at the pre-operated lumbar level and 8 patients presented with recurrent relapses. Solid-phase enzyme immunoassay detected proinflammatory cytokines (interleukin-6, tumor necrosis factor-), chemokines (interleukin-8, monocyte chemoattractant protein-1), growth factors (vascular endothelial growth factor, transforming growth factor-1), and osteodestruction markers (osteoprogesterin, matrix metalloproteinase-8) in the blood serum and the extracted spinal disc tissue. Results. We found that spinal disk destruction and chronic inflammation developed with both locally and generally elevating levels of proinflammatory cytokines/chemokines, growth factors, and matrix metalloproteinase 8. Conclusion. The results emphasize the significance of local changes in the studied parameters to choose and plan personalized surgical treatment in patients with spinal disk herniation.

18. Clinical and Economic Evaluation of the Blood Purification with Selective Sorption Techniques in ICU Patients

Abstract: The objective was to analyze the clinical and economic feasibility of using selective methods of lipopolysaccharide (LPS) sorption for sepsis treatment.Materials and methods. A clinical and economic model was used to assess the feasibility of using some technologies of selective lipopolysaccharide sorption. The model was developed in accordance with the current industry standard «Clinical and Economic Research» used in Russia. The target population was patients of 18 years old and older with sepsis. The methodology involved the evaluation of available randomized clinical trials, as well as studies involving data from network meta-analyses and systematic reviews of the use of the studied technologies. The calculation of direct medical and indirect non-medical costs was made with an analysis of the impact on the budget during the first year and on the time horizon for five years.Results. The value of direct and indirect costs was determined when using sorption devices based on Efferon LPS, Toraymyxin, Alteco LPS Adsorber, Toxipak columns. It was shown that among the therapeutic alternatives, the use of the Efferon LPS column had the least burden on the budget.Conclusion. The use of selective lipopolysaccharide sorption technologies in the intensive care program for sepsis patients, although accompanied by an increase in direct and indirect financial costs, leads to budget savings in the medium term. Improving the mechanism for compensating the costs of using this technology is a promising way to improve the outcomes of sepsis treatment.

Target group – consumers of smoking tobacco: conventional cigarettes (CC) and modified risk tobacco products (MRTP). Base of calculations – analysis of available time series for: CC and MRTP consumption, life expectancy and healthy life expectancy coefficients, statistics on smoking-related mortality and diseases rates, including data on key nosologies (malignant neoplasms of respiratory system, digestive organs, urinary tract; chronic obstructive pulmonary disease; circulatory diseases; cerebrovascular diseases.Results. We implemented prognoses for all the above mentioned parameters to year 2035, calculated direct medical and indirect costs for demographic and economic loss with attention to budget impact analysis, developed five scenarios based on different CC and MRTP consumption.The model of switching from CC to MRTP consumption proves a significant decline of demographic and economic burden even with rather modest MRTP replacement for CC. With current practices of switching from CC to MRTP remaining, during 2021–2035 summary impact would result in 3.6 mln of years saved, 7.7 mln of healthy years saved, 120 thous. of mortal cases and 345 thous. diseases cases prevented. The economic burden would be 3.3 trillion rubles lower.Conclusion. Smoking cessation is the optimal method to reduce health risks, and state policy for stimulation of smoking quitting is necessary. Along with that, transition from CC to MRTP may be an alternative way to reduce health risks for those smokers with long smoking history and either psychological or physiological causes who cannot quit smoking.Even small in the terms of percent transition from CC to MRTP may result in significant decrease of demographic and economic burden on the national scale.

Abstract: In today’s difficult conditions, the issue of clarifying and improving the basic principles of regulating labor protection relations, including in the field of labor protection, is gaining considerable relevance. All this leads to additional attention being paid to the principles as initial provisions that determine the nature and essence of a certain social phenomenon (in our case – legal regulation of the field of labor protection). The task of the work is an in-depth analysis of the place of principles in the science of labor law and substantiation of directions for improvement of the principle basis of legal regulation of labor protection measures. Various approaches to the application of principles in the field of legal regulation of social labor relations and labor protection are analyzed. It is noted that the principles indicate the essential features of the legal regulation of social and labor relations, that they form the foundation of current and prospective legislation in the field of labor law. It is natural that these principles are specified and acquire the status of legal norms in laws and by-laws. Therefore, it should be about a kind of chain: from the principles to their application in normative legal acts. Some shortcomings in the content of the principles, which require urgent correction, were also noted. The statement that the process of formation and implementation of principles requires urgent improvement is substantiated. The problem of properly enshrining the principles in normative legal acts on labor protection is urgent. First of all, it should be noted the insufficient degree of validity of some of the formally defined principles. The process of improving legal policy in the field of labor protection should be systematic and interconnected: from principles to general and special norms.

Abstract: The first observations and classification of deuterium beam-driven sub-cyclotron frequency range Alfvén eigenmodes (AEs) are presented for the MAST-Upgrade tokamak. Sets of observed eigenmodes are separated in frequency by approximately 200 kHz. We observe a lower frequency separation of ∼10 kHz within each set, and the toroidal mode number n increases with frequency sequentially. The ∼200 kHz step between the sets coincides with the interval between successive curves of the Shear Alfvén continuum according to linear ideal MHD modeling. The sub-cyclotron frequency AEs can be identified as the global AEs (GAEs) localized at the continuum extrema regions. Each set contains the same range of n . These observations are consistent with our GAE modeling. In low plasma current ( I _p ) discharges, we observe only GAEs propagating counter to the plasma current and the beam direction. These GAEs are located near the magnetic axis. We observe counter and co-propagating GAEs simultaneously in higher I _p discharges. The co-propagating GAEs occur because of the formation of second continuum minima. Very flat safety factor profiles in higher I _p MAST-U discharges give rise to these minima. They are located at around half of the plasma radius. The GAEs have properties that are very different to those of compressional AEs previously reported for the MAST tokamak with low magnetic fields (Sharapov et al 2014 Phys. Plasmas 21 082501) before the upgrade.

Abstract: High amplitude energetic particle geodesic acoustic mode (EGAM) oscillations driven by Neutral Beam (NBI) injected in the direction counter to the toroidal plasma current are observed in the TCV tokamak. The modes appear at frequencies close to the geodesic acoustic mode (GAM) frequency, corresponding to the mode radii calculated without plasma elongation corrections. The spatial structure of the EGAM density oscillation is analyzed using multichannel soft x-ray (SXR) and broadband light emission diagnostics. The analysis of spatiotemporal emission data shows a non-rotating structure of the density oscillations. The non-rotating mode discrimination is the additional new capability of the multichannel spatiotemporal SXR data analysis technique in TCV. We present the discrimination method of assessing the standing character of the EGAM wave. The structure of EGAM density oscillations in TCV is consistent with the theoretical GAM poloidal structure, namely a m = 1 standing wave with density oscillations amplitude proportional to sin( θ ), where the poloidal angle θ is measured from the equatorial plane. The poloidal structure of magnetic the field oscillations is analyzed using a poloidal array of magnetic coils. The structure of the EGAM-induced magnetic field oscillations in TCV is a m = 2 standing wave. Time evolution of EGAMs suggests that a nonlinear EGAM chirping is observed. The chirping depends on the EGAM radial location, which varies as the NBI deposition is varied through a vertical shift of the plasma magnetic axis. The chirping disappears at the plasma periphery. A fast periodic radial shift of the EGAMs radial location is also observed to occur during the single chirp.

Abstract: Experiments on ASDEX Upgrade (AUG) in 2021 and 2022 have addressed a number of critical issues for ITER and EU DEMO. A major objective of the AUG programme is to shed light on the underlying physics of confinement, stability, and plasma exhaust in order to allow reliable extrapolation of results obtained on present day machines to these reactor-grade devices. Concerning pedestal physics, the mitigation of edge localised modes (ELMs) using resonant magnetic perturbations (RMPs) was found to be consistent with a reduction of the linear peeling-ballooning stability threshold due to the helical deformation of the plasma. Conversely, ELM suppression by RMPs is ascribed to an increased pedestal transport that keeps the plasma away from this boundary. Candidates for this increased transport are locally enhanced turbulence and a locked magnetic island in the pedestal. The enhanced D-alpha (EDA) and quasi-continuous exhaust (QCE) regimes have been established as promising ELM-free scenarios. Here, the pressure gradient at the foot of the H-mode pedestal is reduced by a quasi-coherent mode, consistent with violation of the high-n ballooning mode stability limit there. This is suggestive that the EDA and QCE regimes have a common underlying physics origin. In the area of transport physics, full radius models for both L- and H-modes have been developed. These models predict energy confinement in AUG better than the commonly used global scaling laws, representing a large step towards the goal of predictive capability. A new momentum transport analysis framework has been developed that provides access to the intrinsic torque in the plasma core. In the field of exhaust, the X-Point Radiator (XPR), a cold and dense plasma region on closed flux surfaces close to the X-point, was described by an analytical model that provides an understanding of its formation as well as its stability, i.e., the conditions under which it transitions into a deleterious MARFE with the potential to result in a disruptive termination. With the XPR close to the divertor target, a new detached divertor concept, the compact radiative divertor, was developed. Here, the exhaust power is radiated before reaching the target, allowing close proximity of the X-point to the target. No limitations by the shallow field line angle due to the large flux expansion were observed, and sufficient compression of neutral density was demonstrated. With respect to the pumping of non-recycling impurities, the divertor enrichment was found to mainly depend on the ionisation energy of the impurity under consideration. In the area of MHD physics, analysis of the hot plasma core motion in sawtooth crashes showed good agreement with nonlinear 2-fluid simulations. This indicates that the fast reconnection observed in these events is adequately described including the pressure gradient and the electron inertia in the parallel Ohm’s law. Concerning disruption physics, a shattered pellet injection system was installed in collaboration with the ITER International Organisation. Thanks to the ability to vary the shard size distribution independently of the injection velocity, as well as its impurity admixture, it was possible to tailor the current quench rate, which is an important requirement for future large devices such as ITER. Progress was also made modelling the force reduction of VDEs induced by massive gas injection on AUG. The H-mode density limit was characterised in terms of safe operational space with a newly developed active feedback control method that allowed the stability boundary to be probed several times within a single discharge without inducing a disruptive termination. Regarding integrated operation scenarios, the role of density peaking in the confinement of the ITER baseline scenario (high plasma current) was clarified. The usual energy confinement scaling ITER98( p,y ) does not capture this effect, but the more recent H20 scaling does, highlighting again the importance of developing adequate physics based models. Advanced tokamak scenarios, aiming at large non-inductive current fraction due to non-standard profiles of the safety factor in combination with high normalised plasma pressure were studied with a focus on their access conditions. A method to guide the approach of the targeted safety factor profiles was developed, and the conditions for achieving good confinement were clarified. Based on this, two types of advanced scenarios (‘hybrid’ and ‘elevated’ q -profile) were established on AUG and characterised concerning their plasma performance.

Abstract: Within the 9th European Framework programme, since 2021 EUROfusion is operating five tokamaks under the auspices of a single Task Force called ‘Tokamak Exploitation’. The goal is to benefit from the complementary capabilities of each machine in a coordinated way and help in developing a scientific output scalable to future largre machines. The programme of this Task Force ensures that ASDEX Upgrade, MAST-U, TCV, WEST and JET (since 2022) work together to achieve the objectives of Missions 1 and 2 of the EUROfusion Roadmap: i) demonstrate plasma scenarios that increase the success margin of ITER and satisfy the requirements of DEMO and, ii) demonstrate an integrated approach that can handle the large power leaving ITER and DEMO plasmas. The Tokamak Exploitation task force has therefore organized experiments on these two missions with the goal to strengthen the physics and operational basis for the ITER baseline scenario and for exploiting the recent plasma exhaust enhancements in all four devices (PEX: Plasma EXhaust) for exploring the solution for handling heat and particle exhaust in ITER and develop the conceptual solutions for DEMO. The ITER Baseline scenario has been developed in a similar way in ASDEX Upgrade, TCV and JET. Key risks for ITER such as disruptions and run-aways have been also investigated in TCV, ASDEX Upgrade and JET. Experiments have explored successfully different divertor configurations (standard, super-X, snowflakes) in MAST-U and TCV and studied tungsten melting in WEST and ASDEX Upgrade. The input from the smaller devices to JET has also been proven successful to set-up novel control schemes on disruption avoidance and detachment.

Abstract: After a long device enhancement phase, scientific operation resumed in 2022. The main new device components are the water cooling of all plasma facing components and the new water-cooled high heat flux divertor units. Water cooling allowed for the first long-pulse operation campaign. A maximum discharge length of 8 min was achieved with a total heating energy of 1.3 GJ. Safe divertor operation was demonstrated in attached and detached mode. Stable detachment is readily achieved in some magnetic configurations but requires impurity seeding in configurations with small magnetic pitch angle within the edge islands. Progress was made in the characterization of transport mechanisms across edge magnetic islands: Measurement of the potential distribution and flow pattern reveals that the islands are associated with a strong poloidal drift, which leads to rapid convection of energy and particles from the last closed flux surface into the scrape-off layer. Using the upgraded plasma heating systems, advanced heating scenarios were developed, which provide improved energy confinement comparable to the scenario, in which the record triple product for stellarators was achieved in the previous operation campaign. However, a magnetic configuration-dependent critical heating power limit of the electron cyclotron resonance heating was observed. Exceeding the respective power limit leads to a degradation of the confinement.

Abstract: In 2021 JET exploited its unique capabilities to operate with T and D–T fuel with an ITER-like Be/W wall (JET-ILW). This second major JET D–T campaign (DTE2), after DTE1 in 1997, represented the culmination of a series of JET enhancements—new fusion diagnostics, new T injection capabilities, refurbishment of the T plant, increased auxiliary heating, in-vessel calibration of 14 MeV neutron yield monitors—as well as significant advances in plasma theory and modelling in the fusion community. DTE2 was complemented by a sequence of isotope physics campaigns encompassing operation in pure tritium at high T-NBI power. Carefully conducted for safe operation with tritium, the new T and D–T experiments used 1 kg of T (vs 100 g in DTE1), yielding the most fusion reactor relevant D–T plasmas to date and expanding our understanding of isotopes and D–T mixture physics. Furthermore, since the JET T and DTE2 campaigns occurred almost 25 years after the last major D–T tokamak experiment, it was also a strategic goal of the European fusion programme to refresh operational experience of a nuclear tokamak to prepare staff for ITER operation. The key physics results of the JET T and DTE2 experiments, carried out within the EUROfusion JET1 work package, are reported in this paper. Progress in the technological exploitation of JET D–T operations, development and validation of nuclear codes, neutronic tools and techniques for ITER operations carried out by EUROfusion (started within the Horizon 2020 Framework Programme and continuing under the Horizon Europe FP) are reported in (Litaudon et al Nucl. Fusion accepted), while JET experience on T and D–T operations is presented in (King et al Nucl. Fusion submitted).

33. An extensive analysis of SOL properties in high-δ plasmas in ASDEX Upgrade

Abstract: A set of dedicated H-mode discharges with constant heating power combining Neutral Beam Injection and Electron Cyclotron Resonance Heating have been executed at the ASDEX Upgrade tokamak using a high triangularity magnetic geometry in order to investigate the impact of filamentary transport to divertor and non-divertor components. The evolution of upstream scrape-off layer (SOL) profiles have been correlated with dedicated separatrix quantities, mostly with the turbulence control parameter $\alpha_{\mathrm{t}}$ (Eich and Manz 2021 Nucl. Fusion 61 086016) describing the turbulence level at the separatrix. With increasing $\alpha_{\mathrm{t}}$ , a broadening of the upstream density profiles in the near-SOL together with the formation of a density shoulder in the far-SOL have been observed. This phenomenon is associated with an enhanced filamentary transport dominating the radial turbulent transport in the far-SOL and confirmed by means of the cooling water calorimetry on non-divertor components. The probe measurements conducted with the ball-pen probe-head mounted on the midplane manipulator and a retarding-field analyzer close to the limiter surface indicate that the key mechanism increasing the radial filamentary transport to the first wall is an increase of the particle flux $\Gamma_\mathrm{r,fil}$ , caused primarily by the packing fraction $f_\mathrm{PF, fil}$ and the filament density $n_\mathrm{e,fil}$ . At the same time, the electron temperature $T_\mathrm{e}$ and ion temperature $T_\mathrm{i}$ measured close to the limiter surface show only small variations above $\alpha_\mathrm{t}$ > 0.5. Both the filamentary heat flux and the gross erosion derived from the first wall probe measurements reach a magnitude that should be considered in the design of future fusion reactors.

Abstract: MAST-U is equipped with on-axis and off-axis neutral beam injectors (NBI), and these external sources of super-Alfvénic deuterium fast-ions provide opportunities for studying a wide range of phenomena relevant to the physics of alpha-particles in burning plasmas. The MeV range D-D fusion product ions are also produced but are not confined. Simulations with the ASCOT code show that up to 20% of fast ions produced by NBI can be lost due to charge exchange (CX) with edge neutrals. Dedicated experiments employing low field side (LFS) gas fuelling show a significant drop in the measured neutron fluxes resulting from beam-plasma reactions, providing additional evidence of CX-induced fast-ion losses, similar to the ASCOT findings. Clear evidence of fast-ion redistribution and loss due to sawteeth (ST), fishbones (FB), long-lived modes (LLM), Toroidal Alfvén Eigenmodes (TAE), Edge Localised Modes (ELM) and neoclassical tearing modes (NTM) has been found in measurements with a Neutron Camera (NCU), a scintillator-based Fast-Ion Loss Detector (FILD), a Solid-State Neutral Particle Analyser (SSNPA) and a Fast-Ion Deuterium- α (FIDA) spectrometer. Unprecedented FILD measurements in the range of 1–2 MHz indicate that fast-ion losses can be also induced by the beam ion cyclotron resonance interaction with compressional or global Alfvén eigenmodes (CAEs or GAEs). These results show the wide variety of scenarios and the unique conditions in which fast ions can be studied in MAST-U, under conditions that are relevant for future devices like STEP or ITER.

Abstract: Recent results from MAST Upgrade are presented, emphasising understanding the capabilities of this new device and deepening understanding of key physics issues for the operation of ITER and the design of future fusion power plants. The impact of MHD instabilities on fast ion confinement have been studied, including the first observation of fast ion losses correlated with Compressional and Global Alfvén Eigenmodes. High-performance plasma scenarios have been developed by tailoring the early plasma current ramp phase to avoid internal reconnection events, resulting in a more monotonic q profile with low central shear. The impact of m / n = 3/2, 2/1 and 1/1 modes on thermal plasma confinement and rotation profiles has been quantified, and scenarios optimised to avoid them have transiently reached values of normalised beta approaching 4.2. In pedestal and ELM physics, a maximum pedestal top temperature of ∼350 eV has been achieved, exceeding the value achieved on MAST at similar heating power. Mitigation of type-I ELMs with n = 1 RMPs has been observed. Studies of plasma exhaust have concentrated on comparing conventional and Super-X divertor configurations, while X-point target, X-divertor and snowflake configurations have been developed and studied in parallel. In L-mode discharges, the separatrix density required to detach the outer divertors is approximately a factor 2 lower in the Super-X than the conventional configuration, in agreement with simulations. Detailed analysis of spectroscopy data from studies of the Super-X configuration reveal the importance of including plasma-molecule interactions and D _2 Fulcher band emission to properly quantify the rates of ionisation, plasma-molecule interactions and volumetric recombination processes governing divertor detachment. In H-mode with conventional and Super-X configurations, the outer divertors are attached in the former and detached in the latter with no impact on core or pedestal confinement.

Abstract: The Doppler-shifted resonance condition for high frequency Alfvénic eigenmodes has been extensively studied on ASDEX Upgrade in the presence of one or a combination of two neutral beam injected (NBI) fast ion populations. In general, only centrally deposited NBI sources drive these modes, while off-axis sources globally stabilize the mode activity. For the case of a single central NBI source, the observed trend is: the highest frequency modes are driven by the lowest energy and lowest pitch angle NBI sources, in line with the expectation from the Doppler-shifted resonance condition. The expected mode frequencies are determined analytically from the two-fluid cold plasma dispersion relation and the most unstable frequency relation, while the mode growth rates are estimated using the fast ion slowing down distribution functions from the ASCOT code. The overall mode frequency trend in a source-to-source variation is tracked, although a systematic overestimate of ∼1 MHz is observed. Possible causes of this overestimate include the finite size of the resonant fast ion drift orbit and non-linear effects such as mode sideband formation. Alternatively, the expected mode frequencies are determined by tracking the growth rate maxima trajectories, this method improves the agreement with the experimentally measured values. A combination of two central mode-driving NBI sources results in the suppression of the mode driven by the lowest energy and the lowest pitch angle NBI source. Computing the analytically expected mode frequency following the method outlined above, again, generally tracks the experimentally observed trend. The mode’s Alfvénic nature allows for a practical application to track the core hydrogen fraction by following the mode frequency changes in response to a varying ion mass density. Such application is demonstrated in a discharge where the average ion mass is varied from ∼2 m _p to ∼1.5 m _p (where m _p is the proton mass) via a hydrogen puff in a deuterium plasma, in the presence of a strong mode activity. The expected mode frequency changes are computed from the existence of the resonance condition, and the values track the measured results with an offset of ∼0.5 MHz. Overall, the results suggest an intriguing possibility to monitor and control the D-T ion fraction in the core of a fusion reactor in real time using a non-invasive diagnostic.

Abstract: Tokamak à configuration variable (TCV), recently celebrating 30 years of near-continual operation, continues in its missions to advance outstanding key physics and operational scenario issues for ITER and the design of future power plants such as DEMO. The main machine heating systems and operational changes are first described. Then follow five sections: plasma scenarios. ITER Base-Line (IBL) discharges, triangularity studies together with X3 heating and N2 seeding. Edge localised mode suppression, with a high radiation region near the X-point is reported with N _2 injection with and without divertor baffles in a snowflake configuration. Negative triangularity (NT) discharges attained record, albeit transient, β _N ∼ 3 with lower turbulence, higher low-Z impurity transport, vertical stability and density limits and core transport better than the IBL. Positive triangularity L-Mode linear and saturated ohmic confinement confinement saturation, often-correlated with intrinsic toroidal rotation reversals, was probed for D, H and He working gases. H-mode confinement and pedestal studies were extended to low collisionality with electron cyclotron heating obtaining steady state electron iternal transport barrier with neutral beam heating (NBH), and NBH driven H-mode configurations with off-axis co-electron cyclotron current drive. Fast particle physics. The physics of disruptions, runaway electrons and fast ions (FIs) was developed using near-full current conversion at disruption with recombination thresholds characterised for impurity species (Ne, Ar, Kr). Different flushing gases (D2, H2) and pathways to trigger a benign disruption were explored. The 55 kV NBH II generated a rich Alfvénic spectrum modulating the FI fas ion loss detector signal. NT configurations showed less toroidal Alfvén excitation activity preferentially affecting higher FI pitch angles. Scrape-off layer and edge physics. gas puff imaging systems characterised turbulent plasma ejection for several advanced divertor configurations, including NT. Combined diagnostic array divertor state analysis in detachment conditions was compared to modelling revealing an importance for molecular processes. Divertor physics. Internal gas baffles diversified to include shorter/longer structures on the high and/or low field side to probe compressive efficiency. Divertor studies concentrated upon mitigating target power, facilitating detachment and increasing the radiated power fraction employing alternative divertor geometries, optimised X-point radiator regimes and long-legged configurations. Smaller-than-expected improvements with total flux expansion were better modelled when including parallel flows. Peak outer target heat flux reduction was achieved (>50%) for high flux-expansion geometries, maintaining core performance ( H _98 > 1). A reduction in target heat loads and facilitated detachment access at lower core densities is reported. Real-time control. TCV’s real-time control upgrades employed MIMO gas injector control of stable, robust, partial detachment and plasma β feedback control avoiding neoclassical tearing modes with plasma confinement changes. Machine-learning enhancements include trajectory tracking disruption proximity and avoidance as well as a first-of-its-kind reinforcement learning-based controller for the plasma equilibrium trained entirely on a free-boundary simulator. Finally, a short description of TCV’s immediate future plans will be given.

Abstract: Background: Sulfonylureas (SU) are often used for second and third line type 2 diabetes mellitus (T2DM) therapy. Currently, there are no unified recommendations governing the choice of drugs for the second line therapy. This requires clarification.Aim: To examine Moscow Region DM register data for assessing ongoing antihyperglycemic therapy and its correspondence to current local and international T2DM treatment recommendations, as well as actual clinical recommendations for the treatment of T2DM patients and COVID-19.Materials and methods: The structure of glucose-lowering therapy with non-insulin drugs (NID) was studied in T2DM patients according to the data T2DM register of Moscow region. The analysis was carried out on 06.02.2021. We analyzed the general group of T2DM patients (237479 people), group 60 years and older (188644 patients), T2DM patients who have had COVID-19. Newly diagnosed T2DM individuals were analyzed for 2020 (5088 people).Results: Glucose-lowering therapy is received by 97.6% of T2DM patients. Of these, 79.07% take NID, insulin + NID - 11.37%. Monotherapy is received by 44.4% of patients, a combination of two drugs - 29.3%, out of three - 5.3% of patients. Metformin is the leader in prescribing NID (69.4% of all T2DM patients and 81.3% in newly diagnosed patients). SU are in second place in T2DM treatment (50% of all patients, 24.1% in newly diagnosed patients), iIDPP-4 is in third place (12.1% and 12.6%, respectively). Among patients of 60 and more years old, 25.99% of patients take gliclazide MR, 14.3% glibenclamide, 7.7% glimepiride. Mortality from COVID-19 depends on the diabetes duration.Conclusion: According to the DM register, half of the T2DM patients and a quarter of T2DM newly diagnosed patients get SU. As a first-line drug, SU is on the second place after metformin and are most often used in double and triple combinations of T2DM therapy. In the older age group on SU, preference is given to gliclazide MR.

45. Medical and social significance and calculation of the economic burden of axial spondyloarthritis in the Russian Federation

Abstract: Objective: to assess the economic burden of ankylosing spondylitis (AS) and non-radiological axial spondyloarthritis (nr-axSpA) in the Russian Federation, as well as the availability and economic prospects for more complete provision of biological disease-modifying antirheumatic drugs (bDMARDs) to patients with AS and nr-axSpA.Patients and methods. The prevalence of AS and nr-axSpA and organizational aspects of providing with bDMARDs to this category of patients were analyzed.A variant model has been developed in Microsoft Excel, allows to calculate the indicators of the economic burden of AS and nr-axSpA in the Russian Federation, taking into account the economic losses associated with disability, as well as the low availability of bDMARDs. During study preparation, data from real clinical practice and the opinions of experts in the field of AS from various regions of the Russian Federation were studied.The economic burden was calculated as the sum of direct and indirect costs per patient and the population. Additionally, the specific economic burden per capita was determined.Results and discussion. According to expert estimates, the prevalence of AS and nr-axSpA today is 105.0 and 33.2 thousand people, respectively.The current economic burden of the AS for 2019 is estimated at 21.9 billion rubles per population, or 395.5 thousand rubles for 1 patient. The ratio of direct and indirect costs was 1:4, i.e. 4.7 billion rubles – direct costs (84.3 thousand rubles per 1 patient) and 17.2 billion rubles – indirect costs (311.2 thousand rubles per 1 patient). The burden per capita – 149 rubles.Nr-axSpA's current economic burden in 2019 reached 3.0 billion rubles, or 182.9 thousand rubles for 1 patient. The ratio of direct and indirect costs is estimated as 2:5, or 0.9 billion rubles – direct costs (53.0 thousand rubles per 1 patient) and 2.2 billion rubles – indirect costs (130.0 thousand rubles per 1 patient). The burden per capita – 21 rubles.Conclusion. The economic burden of AS and nr-axSpA can be reduced by providing patients with bDMARDs in the required amount: 15% of patients with AS and 10% with nr-axSpA. Due to the imperfection of the regulatory framework, patients with nr-axSpA experience serious difficulties in receiving this therapy within the framework of preferential drug coverage and compulsory health insurance. With the provision of bDMARDs for about 15 and 10% of patients with AS and nr-axSpA, respectively, on the horizon of 5 years, a decrease in disability by 75% and temporary disability by 60% is expected. At the same time, the economic burden for 5 years will decrease by about 40% for each nosology.

48. Clinical and economic assessment of the therapy of acute kidney injury in sepsis with continuous combined methods of renal replacement therapy

Abstract: Objective: complex assessment of clinical and economic eﬀectiveness as well as saving of the state budget assets within the therapy of patients with sepsis/septic shock based on diﬀerent technologies of blood purification.Methods. Evaluation of direct and indirect costs of the eight blood purifcation methods. The target population includes adult patients 18+ with septic acute kidney injury. The study was conducted in compliance with acting Standards on Clinical Economic Studies used in the Russian Federation.The study methods included evaluation of existing randomized clinical studies and trials with data from network meta-analyses and systematic reviews of the target technologies. The direct medical and indirect non-medical costs were calculated and analyzed in respect of cost-eﬀectiveness and budget impact analyses.Results. Direct and indirect costs were calculated for the sorption devices oXiris (Baxter International Inc.), Toraymyxin (Toray Medical Co.), Alteco LPS Adsorber (Alteco Medical AB), Toxipak (POKARD NPF), Desepta.LPS (BIOTECH M NPP), CytoSorb (CytoSorbents Inc.), HA330 (Jafron Biomedical Co.), Desepta (BIOTECH M NPP) for two time-horizons: 1-year period and 5-years period with discounting. For both periods, the use of oXiris hemoflter results in the least economic burden for the state budget.Conclusion. The use of blood purifcation in septic patients therapy is associated with considerable direct and indirect fnancial expenses, the amount of which and consequential budget saving depend on the technology of blood purifcation applied. The actual tariﬀ compensation policy evidently demands revising.

50. Antimicrobial susceptibility of mastitis pathogens of dairy cows in Ukraine

Abstract: Mastitis is one of the most common diseases on dairy farms. It causes significant economic damage associated with the cost of treating sick cows, reduced milk yield and quality indicators of dairy products, and the risk of premature culling of animals. Treatment of cows with mastitis on dairy farms is carried out mainly with antimicrobial drugs, which are usually used without a preliminary test to identify the causative agent of the disease and determine its sensitivity to antimicrobial substances, which is an important part of the effectiveness of therapy. Increasing the resistance of bacteria to antimicrobial substances poses a threat not only to the animal but also to humans, as a consumer of dairy products. The availability of data on the sensitivity of mastitis pathogens to antimicrobial drugs makes it possible for veterinary doctors to choose the most effective antibiotic for treating animals with the shortest duration of treatment. The presented results of studies of breast secret samples taken from cows indicate that in 57.5% of cases, contagious pathogens of mastitis were identified. In particular, Streptococcus agalactiae made 24.1%, Staphylococcus aureus – 18.4%, Corynebacterium spp. – 7.2%, Streptococcus dysgalactiae– 5.6%, Streptococcus uberis – 2.2%. Environmental pathogens accounted for 42.5% of the total number of isolated isolates, among which Streptococci represented gram-positive microflora at 11.5 Streptococcuscus spp. (6.2% Streptococcuscus parauberis (4.4% Streptococcuscus Bovis (0.9%) and Staphylococcus spp. – 10.3%. Gram-negative microflora is 20.6%, among which the largest percentage belongs to E. coli – 8.4% and Klebsiella pneumonia – 1.9%. Mastitis caused by yeast accounted for 1.4% of all diagnosed pathogens. Antimicrobial sensitivity was evaluated using the disk diffusion method (Kirby-Bauer). According to the results of determining the sensitivity of mastitis pathogens to antimicrobial substances, it was found that the highest sensitivity of the isolated isolates was to Ceftiofur, Amoxicillin/clavulanic acid, Rifampicin, Amoxicillin, Gentamicin, Ampicillin, Bacitracin, Cephalexin, Cloxacillin, Enrofloxacin, Trimethoprim/sulfamethoxazole, Oxytetracycline, Lincomycin. The least sensitive – to Spiramycin, Tylosin, streptomycin, neomycin, Marbofloxacin, Tilmicosin, and Danofloxacin.
