Introduction: Hematopoietic stem cell transplantation (HSCT) comprises one of the two main treatment regimens for patients with mucopolysaccharidoses (MPS). There is a scarcity of literature concerning the process of growth in children with Mucopolysaccharidosis type I (MPS I) and Mucopolysaccharidosis type I (MPS II) after HSCT. The aim of this manuscript was to evaluate the therapeutic effect of HSCT on the heights of patients with MPS I and MPS II. Material and methods: It was an observational, single-center study on patients with MPS I and II treated with HSCT. Results: 6 MPS patients, including 4 MPS I and 2 MPS II, underwent HSCT at a median age of 2 years. All patients are alive to date, with a median age of 7.7 years (range 5.5–12 years) at the last follow-up. In both (MPS I and MPS II) groups of patients treated with HSCT, the growth rate was higher than in untreated patients and was found to be in line with the population norm. In both MPS I and MPS II patients who were treated with HSCT, normalization of urinary GAG excretion was observed. Additionally, no bands of DS and HS in GAG electrophoresis were visible. Conclusions: Both MPS I and MPS II patients presented height gain after HSCT compared to the curves of untreated patients. The absence of dermatan sulphate after HSCT could lead to normal growth in bone length. [ABSTRACT FROM AUTHOR]

BackgroundRare DNA breakage repair disorders predispose to infection and lymphoreticular malignancies. Hematopoietic cell transplantation (HCT) is curative, but coadministered chemotherapy or radiotherapy is damaging because of systemic radiosensitivity. We collected HCT outcome data for Nijmegen breakage syndrome, DNA ligase IV deficiency, Cernunnos-XRCC4-like factor (Cernunnos-XLF) deficiency, and ataxia-telangiectasia (AT).MethodsData from 38 centers worldwide, including indication, donor, conditioning regimen, graft-versus-host disease, and outcome, were analyzed. Conditioning was classified as myeloablative conditioning (MAC) if it contained radiotherapy or alkylators and reduced-intensity conditioning (RIC) if no alkylators and/or 150 mg/m2 fludarabine or less and 40 mg/kg cyclophosphamide or less were used.ResultsFifty-five new, 14 updated, and 18 previously published patients were analyzed. Median age at HCT was 48 months (range, 1.5-552 months). Twenty-nine patients underwent transplantation for infection, 21 had malignancy, 13 had bone marrow failure, 13 received pre-emptive transplantation, 5 had multiple indications, and 6 had no information. Twenty-two received MAC, 59 received RIC, and 4 were infused; information was unavailable for 2 patients. Seventy-three of 77 patients with DNA ligase IV deficiency, Cernunnos-XLF deficiency, or Nijmegen breakage syndrome received conditioning. Survival was 53 (69%) of 77 and was worse for those receiving MAC than for those receiving RIC (P = .006). Most deaths occurred early after transplantation, suggesting poor tolerance of conditioning. Survival in patients with AT was 25%. Forty-one (49%) of 83 patients experienced acute GvHD, which was less frequent in those receiving RIC compared with those receiving MAC (26/56 [46%] vs 12/21 [57%], P = .45). Median follow-up was 35 months (range, 2-168 months). No secondary malignancies were reported during 15 years of follow-up. Growth and developmental delay remained after HCT; immune-mediated complications resolved.ConclusionRIC HCT resolves DNA repair disorder-associated immunodeficiency. Long-term follow-up is required for secondary malignancy surveillance. Routine HCT for AT is not recommended.

Many activities were initiated by Ukrainian physicians with the support of the international community. Ukrainian refugees received almost unlimited access to treatment in some EU countries, such as Poland. Material and methods: Activities of the 'Help for Ukrainian Hematology Patients' (HUP) initiative are described in this article. Information on HCT and cell therapy in refugee patients in Polish transplant centers was collected by questionnaire between 24 February 2022 and 24 February 2023. Results: Data on 65 patients was available: 22 males and 43 females, with a median age of 35 years. The most frequent diagnoses were Hodgkin's lymphoma (20 patients) and acute lymphoblastic leukemia (17 patients). Allo-HCT was performed in 24 patients, auto-HCT in 36 patients and CAR-T-cells in five patients. Fourteen patients (21.5%) received treatment that was unavailable in Ukraine. The major challenges included social and language issues. Conclusions: The number of initiatives of Ukrainian physicians and HUP led to a significant increase in the number of HCT procedures in Ukraine after the initial drop-down. The almost unlimited access to healthcare in some EU countries has allowed refugees to get treatment that is unavailable in Ukraine. [ABSTRACT FROM AUTHOR]

Background. The high sensitivity of cells of Fanconi anemia (FA) patients to DNA cross-linking agents (clastogens), such as mitomycin C (MMC), was used as a screening tool in Polish children with clinical suspicion of FA. Objectives. The aim of the study was to compare chromosome fragility between 3 groups, namely non-FA, possible mosaic FA and FA patients. Materials and methods. The study included 100 children with hematological manifestations and/or congenital defects characteristic of FA, and 100 healthy controls. Blood samples obtained from participants were analyzed using an MMC-induced chromosomal breakage test. Results. Patients with clinical suspicion of FA were divided into 3 subgroups based on the MMC test results, namely FA, possible mosaic FA and non-FA. Thirteen out of 100 patients had a true FA cellular phenotype. The mean value of MMC-induced chromosome breaks/cell for FA patients was higher than for non-FA patients (6.67 ±3.92 compared to 0.23 ±0.18). In addition, the percentage of cells with spontaneous aberrations was more than 9 times higher in FA patients than in non-FA patients. Conclusions. Our results confirmed that the MMC sensitivity test distinguishes between individuals affected by FA, those with possible somatic mosaicism, and patients with bone marrow failure for other reasons, who were classified as non-FA in the first diagnostic step. However, a definitive differential diagnosis requires follow-up mutation testing and chromosome breakage analysis of skin fibroblasts. [ABSTRACT FROM AUTHOR]

Introduction: Vincristine is used in many therapeutic schemes of oncological treatment in children, and one of its common complications is neurotoxicity. The aim of this study was to assess the incidence of vincristine-induced polyneuropathy in children treated for acute lymphoblastic leukemia. Secondly, we evaluated the clinical spectrum of chemotherapy-induced peripheral neuropathy. Material and methods: The study included 29 patients with newly diagnosed acute lymphoblastic leukemia who were treated according to the ALL IC-BFM 2009 therapeutic program. The study was designed for the induction remission phase, in which patients received unified anticancer treatment. A neurological assessment was performed on days 1, 8, 15, 22, 29 and 36 of treatment, and consisted of an original questionnaire completed by patients or their caregivers, a physical examination, and a neurological examination. Abnormalities were assessed according to the Common Terminology Criteria for Adverse Events, version 5.0 scale. Results: Symptoms of polyneuropathy occurred in 25 patients (86.2%). Motor neuropathy was the most common type of polyneuropathy in the examined group. In half of the cases, the polyneuropathy was severe, rated at 3 or 4 points. The most serious course manifested itself in three patients in severe gastrointestinal ileus with subsequent intestinal perforation. In another case, rated at 4 points, breathing disorders occurred with apnea episodes. Conclusions: Vincristine-induced polyneuropathy is common in children with acute lymphoblastic leukemia in the induction of remission phase, and is severe in half of cases. In the pediatric population, in contrast to adults, vincristine- -induced polyneuropathy motor symptoms are predominant. [ABSTRACT FROM AUTHOR]

Introduction: The devastation of hospitals and the severing of international links has made paediatric haematopoietic stem cell transplantation (HSCT) in Ukraine impossible. The aim of our study was to present the activity of Polish paediatric bone marrow transplant units in saving the lives of child war refugees. Material and methods: The data collected from 6 units on the treatment of child war refugees from Ukraine with HSCT were analysed. Results: From 24 February 2022 up to the end of 2022, 18 children from Ukraine underwent HSCT and one received chimeric antigen receptor T-cell (CAR-T) therapy. This project was possible due to great organisational support from non-governmental organisations (NGOs), international cooperation ("SAFER Ukraine" enterprise), and financial coverage by the Polish national insurance system. Twelve patients were treated in Wrocław, 3 in Lublin, 2 in Bydgoszcz, one in Poznań, and one in Kraków. The group consisted of 10 boys and 9 girls, aged 1.3--17.9 years, 16 of whom suffered from haematological malignancy (acute lymphoblastic leukaemia n = 9, acute myeloblastic leukaemia n = 5, juvenile myelomonocytic leukaemia n = 2) and 3 from non-neoplastic disease (severe aplastic anaemia n = 2 and severe combined immunodeficiency syndrome n = 1). Despite poor preparation, numerous pre-existing comorbidities, a high rate of acute complications, and long hospital stays, the overall results were satisfactory. Only 2 patients (10%) died during the first 100 days after transplantation, both due to severe infection. The total number of allogeneic-HSCTs performed in Poland on Ukrainian children during wartime was not substantially lower than in previous years in their home country. Moreover, this specific war situation allowed the prescribing of CAR-T therapy to one of the first paediatric patients from this country. Conclusions: Saving the lives of seriously ill child war refugees with the use of HSCT and CAR-T therapy is effective provided that all entities responsible for it cooperate in a coordinated manner and support the activities of NGOs. In this article, we would like to present rare diseases, which are immunodeficiency, centromeric instability and facial dysmorphism syndrome (ICF), and myelodysplastic neoplasm (MDS), with possible complications of their course and treatment with particular emphasis on pulmonary alveolar proteinosis (PAP). We are basing that review on our experience that we have gained through the diagnostic and treatment process of our pediatric patient. MDS is a rare disease among children that has a different and uncharacteristic clinical course and genetic background in comparison to MDS occurring in adults. Regarding the uncharacteristic clinical picture, the differential diagnosis can be a challenge for clinicians. In that process also diseases characterized by immunodeficiency should be taken into account, including the ICF. ICF type 2 is an autosomal recessive disease that manifests by agammaglobulinemia or hypogammaglobulinemia, developmental delay, and facial anomalies. Both ICF and MDS can be treated with an allogeneic hematopoietic stem cell transplantation. One of the possible complications in the course and treatment of MDS and ICF can be PAP. Its pathogenesis is based on the accumulation of surfactant in alveoli that leads to pulmonary insufficiency. As hematologic diseases and their treatment are known for their impact on multiple systems, we find the unique value of this article in being the first description of the coexistence of ICF type 2 with PAP. Introduction: Iron deficiency is the most common micronutrient deficiency worldwide. Iron activates the growth and differentiation of immune cells and cytokine actions. Interleukin 6 is a pro-inflammatory cytokine that stimulates the differentiation of B lymphocytes into plasma cells and activates T lymphocytes. Our study aimed to evaluate the correlation between blood levels of iron and interleukin 6 (IL-6) and its impact on infection rates in children. Materials and methods: The study included 36 children. The serum concentration of IL-6, iron, and morphology parameters from a venous blood sample were assessed. An anonymous survey of children’s parents was conducted regarding the frequency of infections before and after the diagnosis of anemia. Results: In the study group, the levels of IL-6 were higher than in the control group and showed greater variability between individual patients. There was a statistically significant, negative correlation between IL-6 and iron levels within the study group (p = 0.012), and a decrease in the number of viral diarrheas after the diagnosis of anemia compared to the state before diagnosis (p = 0.041). There was no statistically significant change in the number of colds or the mean duration of infection (p = 0.144, p = 0.498), nor in the incidence of other infectious diseases and antibiotics intake in the study group before and after iron deficiency anemia diagnosis (p = 0.500, p = 0.219). Conclusions: IL-6 might play a role in iron deficiency anemia. Increased levels of IL-6 were shown not to correspond with visible changes in rates and the course of colds, but did result in a reduction in the frequency of viral diarrhea. Further research on a larger group of carefully selected patients is required to determine the effect of anemia on the immune system. [ABSTRACT FROM AUTHOR]
