Background: An association between ocular adnexal MALT lymphoma (OAL) and Chlamydia psittaci (Cp) infection has been proposed, and recent reports suggest that doxycycline treatment causes tumor regression in patients with Cp-related OAL. The effectiveness of doxycycline treatment in Cp-negative OAL has not been tested. Methods: In a prospective trial, 27 OAL patients (15 newly diagnosed and 12 having experienced relapse) were given a 3-week course of doxycycline therapy. Objective lymphoma response was assessed by computerized tomography scans or magnetic resonance imaging at 1, 3, and 6 months after the conclusion of therapy and every 6 months during follow-up. Cp infection in patients was determined by touchdown enzyme time-release polymerase chain reaction (TETR-PCR). Statistical tests were two-sided. Results: Eleven patients were Cp DNA-positive and 16 were Cp DNA negative. Doxycycline was well tolerated. At a median follow-up of 14 months, lymphoma regression was complete in six patients, and a partial response ([greater than or equal to]50% reduction of all measurable lesions) was observed in seven patients (overall response rate [complete and partial responses] = 48%). Lymphoma regression was observed in both Cp DNA-positive patients (seven of 11 experienced regression) and Cp DNA-negative patients (six of 16 experienced regression) (64% versus 38%; P = .25, Fisher's exact test). The three patients with regional lymphadenopathies and three of the five patients with bilateral disease achieved objective response. In relapsed patients, response was observed both in previously irradiated and nonirradiated patients. The 2-year failure-free survival rate among the doxycycline-treated patients was 66% (95% confidence interval = 54 to 78), and 20 of the 27 patients were progression free. Conclusions: Doxycycline is a fast, safe, and active therapy for Cp DNA-positive OAL that was effective even in patients with multiple failures involving previously irradiated areas or regional lymphadenopathies. The responses observed in PCR-negative OAL may suggest a need for development of more sensitive methods for Cp detection and investigation of the potential role of other doxycycline-sensitive bacteria.

Objective: To determine rates of compliance with doxycycline therapy for patients attending two inner city sexually transmitted diseases (STD) clinics using the MEMS (Medication Event Monitoring System) technology (Aprex Corporation, Fremont, CA). Design: An observational study. Setting: Two STD clinics in Brooklyn, New York and Birmingham, Alabama. Patients: Patients warranting doxycycline as antichlamydial therapy by usual clinical criteria (e.g., documented chlamydial infections, gonococcal urethritis, mucopurulent cervicitis) were enrolled consecutively from both clinics into four separate categories according to gender and the presence or absence of symptoms: symptomatic men (77), asymptomatic men (30), symptomatic women (83), asymptomatic women (33). Intervention: In the clinic area, patients were given their doxycycline in standard 30-dram medication bottles fitted with the MEMS cap, which is capable of recording the date and time of each bottle opening and closing. This information was then retrieved using a software program developed by the manufacturer. Patients were instructed to return the bottle and cap at the completion of therapy. Efforts were made to contact those who did not return their bottles by both telephone and mail. Outcome Measures: Bottle openings as recorded by the MEMS were considered to represent use of medication. Patients were considered strictly compliant with prescription instructions if bottle openings and closings occurred at least twice daily for 6 consecutive days. Noncompliance was defined as initially opening the medication more than 48 hours after leaving the clinic or opening the bottle less than once daily for 5 consecutive days. Usage between these extremes was classified as intermediate. Results: Eighty percent of 223 patients enrolled completed the study by returning their bottles. The rate of strict compliance with prescription instruction was 25%. The rate of noncompliance was 24%. Fifty-one percent used some intermediate amount of medication. There was no statistical difference in compliance by gender, presence or absence of symptoms, or site of enrollment. Conclusions: Few patients administered doxycycline in an STD clinic can be expected to take medication precisely as prescribed. Although most probably take enough to eradicate uncomplicated chlamydial infections, a sizable portion can be expected to use an inadequate amount of medication. This may contribute to persistence of genital chlamydia infections in the community., Many patients given doxycycline therapy for chlamydia may not take their medication as specified. Specialized caps were used on bottles of doxycycline tablets to monitor how frequently 179 patients took their medication. Only 25% of the patients took their medication exactly as specified with 51% taking it fairly regularly. Twenty-four percent of the patients either delayed treatment for at least two days after being given the medication or regularly skipped medication for entire days.

Although several new antibiotics have recently become available, in several clinical instances conventional antibiotics may be equally efficacious at a considerably lower cost. In today's era of cost containment, it is particularly relevant to revisit older, inexpensive antibiotics to reexplore their role in the face of the emergence of resistant microorganisms and competition from newer agents. Doxycycline is one such antibiotic. It is an inexpensive, broad-spectrum antimicrobial agent that remains the drug of first choice for several infections. In addition, it can be used for a variety of other indications. Adverse effects are infrequent and relatively minor. While interactions occur with several medications, none of these interactions has significant adverse consequences.

* Objective: To compare the efficacy and tolerability of minocycline versus doxycycline in the treatment of nongonococcal urethritis and mucopurulent cervicitis. * Design: Randomized, double-blind trial. * Setting: Sexually transmitted disease clinics. * Patients: 151 men and 102 women with nongonococcal urethritis, mucopurulent cervicitis or whose sexual partner had either condition or a positive culture for Chlamydia trachomatis. * Interventions: Minocycline, 100 mg nightly, or doxycycline, 100 mg twice daily, each administered for 7 days. * Measurements: At each visit (days 14 [+ or -] 3, 28 [+ or -] 5, and 49 [+ or -] 7) patients were questioned regarding symptoms, signs, drug compliance, and sexual contact. Cultures for C. trachomatis, Ureaplasma urealyticum, and Mycoplasma hominis were obtained at each visit. * Results: 253 patients were enrolled (133, doxycycline; 120, minocycline). Chlamydia trachomatis was initially isolated from 31% of men and 39% of women. Men with a positive smear had a higher symptom/sign score (P [less than] 0.001) and were more likely to have chlamydia (P = 0.004). Positive endocervical smears were not associated with symptoms or signs (P [greater than] 0.2) but correlated with isolation of chlamydia (P [less than] 0.001). One hundred sixty-two patients (64%) completed the study. The proportion with urethritis or cervicitis did not differ by treatment group at any follow-up visit (P greater than 0.08). Unprotected sexual contact did not affect clinical or microbiological cure rates. Adverse effects occurred more frequently in the doxycycline group (men: 43% versus 26%; P = 0.05; women: 62% versus 35%; P = 0.009). Although the proportion with dizziness did not differ by drug administered (P = 0.1), dizziness was reported more often by women (1 1 % versus 3%). * Conclusions: Minocycline, 100 mg nightly, was as effective as doxycycline, 1 00 mg twice daily, each given for 7 days in the treatment of nongonococcal urethritis and mucopurulent cervicitis. Vomiting and gastrointestinal upset occurred more frequently in the doxycycline group., Minocycline may be as effective as doxycycline for the treatment of nongonococcal urethritis and cervicitis. Nongonococcal urethritis and cervicitis are different types of inflammation caused by sexually transmitted diseases other than gonorrhea. Among 253 male and female patients with nongonococcal urethritis or cervicitis, 120 were treated with 100 milligrams (mg) of minocycline per day and 133 were treated with 100 mg of doxycycline two times per day over a seven-day period. Response to treatment was similar among men and women in either treatment group depending on the type of sexually transmitted disease. Patients treated with minocycline experienced fewer side effects than those treated with doxycycline.

Intrauterine contraceptive devices (IUD) have been associated with pelvic inflammatory disease (PID), bacterial infections in women affecting the reproductive organs. It is thought that PID develops when bacteria are introduced into the uterus during the process of IUD insertion. The effectiveness of preventative antibiotic therapy prior to IUD insertion was studied among 827 women receiving a single oral dose of 200 milligrams of doxycycline, and 828 placebo-treated women attending a family planning clinic in Kenya. Cultures were taken for two organisms responsible for PID infections, gonorrhea and chlamydia, before and after IUD insertion. The rate of PID in the doxycycline-treated group was 1.3 percent compared with 1.9 percent in the placebo-treated group, representing a 31 percent reduction in PID. The greatest reduction in IUD-related PID infections was seen for the bacterial infection caused by chlamydia. There were fewer unscheduled visits (31 percent) made to the health clinic specifically for IUD-related events among the doxycycline-treated women. Although the differences in PID between the two groups were not statistically significant, it is believed that the fewer unscheduled IUD-related visits indicates that doxycycline provides a protective effect against PID. Shigellosis is a bacterial infection of the bowel causing diarrhea, pain and fever. It is a major cause of diarrhea among residents of developing countries, military personnel and travelers. Symptoms range from watery diarrhea to severe dysentery, which can be potentially life-threatening. Doxycycline (100 mg per day given for 14 days), a tetracycline-like antibiotic, was offered to 50 patients to prevent shigellosis. Coincidentally, an outbreak of shigellosis developed three days into the study and six patients tested positive for the bacteria. Over the eight-week period, eight patients (16 percent) in the group receiving doxycycline developed diarrhea, compared with 37 (74 percent) in the untreated group. Although the symptoms did not differ in both groups, the length of time the symptoms were present was shorter for the treated patients. Doxycycline appears to prevent the diarrheal disease shigellosis. The long-term protection provided by doxycycline is unknown. Since the risk of doxycycline as a preventative treatment is low, it may be useful to give the drug to people travelling to areas with a high prevalence of the disease.
