This article provides a consensus treatment pathway supporting timely identification and management of a suboptimal response to these therapies, thereby aiming to facilitate clinically meaningful outcomes and efficient management of service capacity under specific circumstances where ranibizumab biosimilars may be initiated as a first-line treatment.Methods: Two structured round-table meetings of UK medical retina specialists were held in person and virtually on September 22 and November 3, 2022, respectively. These meetings were organized and funded by Bayer.Results: The panel provided guidance on the implementation of an early treatment optimization pathway in cases where ranibizumab biosimilars are used as a first-line treatment, including recommendations on patient suitability and capacity requirements, and criteria for identification and strategies for management of a suboptimal response. The panel discussed the role of aflibercept treatment and its potential benefits and outlined recommendations on switching ranibizumab biosimilar suboptimal responders to an aflibercept treat-and-extend regimen, where appropriate.Conclusion: Developed by a retinal expert panel, this early treatment optimization pathway provides guidance to facilitate optimal long-term patient outcomes while addressing capacity and resourcing constraints in circumstances of first-line ranibizumab biosimilar use for nAMD, including how aflibercept may be used in cases with a suboptimal response. Many hospital services struggle to deliver efficient and effective MR care due, at least in part, to infrastructure that does not expand responsively enough to meet the increased demand. A steering committee of retinal specialists from a variety of UK NHS hospital ophthalmology departments with experience of leading and managing NHS retinal services in the intravitreal era came together for the generation of this document to review and compile key aspects that should be considered when optimising intravitreal treatment capacity within MR services. This article aims to provide a useful collation and signposting of key published evidence, consensus and insights on aspects of delivering an intravitreal service, including treatment regimens, virtual clinics, staff training and governance, telemedicine and information technology, and data collection and key performance indicators. 7. Defining the neuropsychological and neuroimaging phenotype of behavioural variant frontotemporal dementia

Abstract

Frontotemporal dementia (FTD) is the second most common cause of early-onset dementia after Alzheimer’s disease (AD). There exists a paucity of quantifiable, sensitive, and specific biomarkers to detect this disease and track its manifestation and progression. The primary aim of this thesis was to develop and investigate new biomarkers for FTD, and focused on the examination of neuropsychological biomarkers in the behavioural variant of FTD (bvFTD) and their neuroanaotmical correlates. Chapters 4 and 5 explored social cognition in patients with FTD and the neural correlates of this behaviour. bvFTD patients displayed gross dysfunction in the perception of sarcasm and the ability to understand basic social signals, and this mapped onto a larger social cognition neural network that has previously been defined in the literature. These findings delineate a brain network substrate for the social impairment that characterises FTD syndromes. In Chapters 6 and 7, I explored the executive functions of task switching, reaction time, and neural timing in patients with FTD. Results indicated several dissociable executive capacities, which mapped onto discrete neural areas as part of a larger executive function network, suggesting that structures implicated in aspects of executive functioning can be targeted by FTD and may underpin aspects of the bvFTD phenotype. In the final Chapter, I devised a novel battery to examine the bases of psychosis in FTD patients with the C9ORF72 mutation, which demonstrated a specific and unique impairment in the ability to interpret somatosensory proprioceptive information in these patients, which may represent a candidate mechanism for psychosis. The studies described in this thesis contribute to the growing interest in characterising and understanding the neuropsychological phenotypes of bvFTD. Improved understanding of the anatomical associations of neuropsycholgical performance in this patient population could potentially facilitate earlier and more accurate diagnosis and symptom management.

Recent clinical trials and real-world studies have demonstrated the advantages of a Treat-and-Extend (T&E) regimen, and many hospital departments are now in the process of adopting this new regimen in favor of the pro re nata regimen for initiating and continuing ranibizumab therapy for patients with wet AMD. The comprehensive spectrum of issues related to implementation of the regimen is covered qualitatively in ten didactic topics provided by a group of clinicians with direct experience of this regimen in their department. The topics include definition, new and previously treated eyes, management of high-frequency injections, maximum extensions, discontinuing T&E, bilateral cases, clerical, audit, and patient counseling. This article aims to provide a useful resource for the implementation of the T&E regimen. A quantitative summary of the visual outcomes in key publications is also provided in this article. 29. The Social Studies in Alberta--1975: A Summary of a Report of an Assessment.

Abstract

This summary report assesses the design implementation and classroom instruction of the Alberta, Canada, social studies curriculum. The purpose of the study was to present a synthesis of findings from five investigative activities undertaken by a research association in Alberta: (1) review of the background of the program, (2) critiques of the master plan, (3) questionnaire survey of teachers, students, and parents, (4) analysis of locally developed programs, and (5) series of interviews, observations, and site visits. The study is presented in four chapters. Chapter I, The Questions, asks how appropriate and effective are the original master plan, the programs which have been developed at the local level, and the typical classroom situation. Chapter II, Summary of Conclusions and Supporting Evidence, broadly generalizes conclusions for the master plan, program development, and the classroom situation. Chapter III, Summary Judgments, presents a profile of attainment of educational objectives in the three sectors of the curriculum, and Chapter IV, Recommendations, suggests nine specific steps which might improve the project. It is concluded that the social studies program is consistent with emerging objectives of education but is not being effectively implemented. (Author/DB)

30. The Social Studies in Alberta--1975: A Report of an Assessment.

Abstract

Design implementation and classroom instruction of the Alberta, Canada, social studies curriculum master plan are assessed in this report. The objective of the study was to synthesize findings of investigative activities into a set of general conclusions from which recommendations could be developed. It was concluded that internal inconsistencies of the plan plus a lack of teacher awareness of its subtle intents have rendered it less useful than it ought to be. The following reasons are given for this weak translation of the Master Plan into programs: lack of time for program development, lack of resources, insufficient consultative services, and teacher incompetence in program development. To counteract these difficulties, nine recommendations are offered, including creation of an ad hoc task force to reassess and revise the master plan, revision of the documents of the master plan by the Department of Education, and encouragement of teachers to achieve a broader social studies repertoire and a better balance of classroom activities. The final judgment was that the social studies plan is commendable and highly acceptable in its major orientations and should be continued, with certain refinements. Five appendices discuss the background of the study; critique the master plan; provide the survey of teachers, students, and parents; assess the instructional programs; and present observations of site visits. 31. The Alberta Academy for Educational Leadership. Report of an Appraisal.

Abstract

The Alberta Academy for Educational Leadership was created by a number of education-related organizations in response to provincial governmental pressure. To evaluate the program, at the end of a 3-year exploratory period, data were obtained from onsite observations, meetings with three sets of graduates, selected school staffs and colleagues, and with participant and interested observers. Additionally, questionnaires sent to 273 graduates were responded to by 195 (74 percent); and questionnaires sent to 92 superintendents were responded to by 83 (90 percent). A summary of a review and synthesis of the current literature on leadership and leadership development is presented. More than 75 percent of the responding participants indicate they have considered returning to the Academy for another program, and 95 percent would recommend the experience to a colleague. An 80 percent positive rating in terms of the applicability of knowledge acquired was given, and on a five-point scale rating, the culture of the Academy as an environment for professional growth, the mean rating was 4.7. The superintendents' views of the Academy are generally positive. Alternative futures for the Academy were explored at a meeting with the six stakeholder groups by giving them a summary of the appraisal data with an overview of seven alternative scenarios. The report concludes with a summary, speculations, and recommendations. (MLF)

Abstract

Previously, it was suggested that haemadipsid leeches represent an important vector of trypanosomes amongst native animals in Australia. Consequently, Chtonobdella bilineata leeches were investigated for the presence of trypanosome species by polymerase chain reaction (PCR), DNA sequencing and in vitro isolation. Phylogenetic analysis ensued to further define the populations present. PCR targeting the 28S rDNA demonstrated that over 95% of C. bilineata contained trypanosomes; diversity profiling by deep amplicon sequencing of 18S rDNA indicated the presence of four different clusters related to the Trypanosoma (Megatrypanum) theileri. Novy–MacNeal–Nicolle slopes with liquid overlay were used to isolate trypanosomes into culture that proved similar in morphology to Trypanosoma cyclops in that they contained a large numbers of acidocalcisomes. Phylogeny of 18S rDNA/GAPDH/ND5 DNA sequences from primary cultures and subclones showed the trypanosomes were monophyletic, with T. cyclops as a sister group. Blood-meal analysis of leeches showed that leeches primarily contained blood from swamp wallaby (Wallabia bicolour), human (Homo sapiens) or horse (Equus sp.). The leech C. bilineata is a host for at least five lineages of Trypanosoma sp. and these are monophyletic with T. cyclops; we propose Trypanosoma cyclops australiensis as a subspecies of T. cyclops based on genetic similarity and biogeography considerations.

Abstract

The study examines the effects of the antioxidant flavonoid Pycnogenol on a range of cognitive and biochemical measures in healthy elderly individuals. The study used a double-blind, placebo-controlled, matched-pair design, with 101 elderly participants (60-85 years) consuming a daily dose of 150 mg of Pycnogenol for a three-month treatment period. Participants were assessed at baseline, then at 1, 2, and 3 months of the treatment. The control (placebo) and Pycnogenol groups were matched by age, sex, body mass index, micronutrient intake, and intelligence. The cognitive tasks comprised measures of attention, working memory, episodic memory, and psychomotor performance. The biological measures comprised levels of clinical hepatic enzymes, serum lipid profile, human growth hormone, and lipid peroxidation products. Statistically significant interactions were found for memory-based cognitive variables and lipid peroxidation products, with the Pycnogenol group displaying improved working memory and decreased concentrations of F2-isoprostanes relative to the control group. 44. Against Amnesia: Re-Imagining Central Banking

Abstract

The purpose of the present paper is to identify and challenge contemporary adherence to the core of the prevailing monetary policy consensus. This consensus consists of what we call the holy trinity of the inflation targeting paradigm: price stability as the primary goal of the central bank; central bank independence as the institutional arrangement; and the short-term interest rate as the operational target. Drawing on the literature on the history and political economy of central banking, we argue that the inability to think beyond this holy trinity stems from a severe case of collective institutional amnesia and comes at a heavy cost. We highlight that monetary policy can be deployed towards social purposes other than controlling inflation, in institutional configurations other than isolation from the rest of the government and with instruments other than interest rate manipulation. One central message is that whereas central banks are commonly portrayed as commanding only one instrument, in reality they have a battery of instruments at their disposal. We should think of central banking not as a hammer – a tool to hit inflation where it rears its ugly head – but as a Swiss army knife – a multi-purpose tool with many instruments. Abstract

Aim: To assess the impact of deprivation on diabetic retinopathy presentation and related treatment interventions, as observed within the UK hospital eye service. Methods: This is a multicentre, national diabetic retinopathy database study with anonymised data extraction across 22 centres from an electronic medical record system. The following were the inclusion criteria: all patients with diabetes and a recorded, structured diabetic retinopathy grade. The minimum data set included, for baseline, age and Index of Multiple Deprivation, based on residential postcode; and for all time points, visual acuity, ETDRS grading of retinopathy and maculopathy, and interventions (laser, intravitreal therapies and surgery). The main outcome measures were (1) visual acuity and binocular visual state, and (2) presence of sight-threatening complications and need for early treatment. Results: 79 775 patients met the inclusion criteria. Deprivation was associated with later presentation in patients with diabetic eye disease: the OR of being sight-impaired at entry into the hospital eye service (defined as 6/18 to better than 3/60 in the better seeing eye) was 1.29 (95% CI 1.20 to 1.39) for the most deprived decile vs 0.77 (95% CI 0.70 to 0.86) for the least deprived decile; the OR for being severely sight-impaired (3/60 or worse in the better seeing eye) was 1.17 (95% CI 0.90 to 1.55) for the most deprived decile vs 0.88 (95% CI 0.61 to 1.27) for the least deprived decile (reference=fifth decile in all cases). There is also variation in sight-threatening complications at presentation and treatment undertaken: the least deprived deciles had lower chance of having a tractional retinal detachment (OR=0.48 and 0.58 for deciles 9 and 10, 95% CI 0.24 to 0.90 and 0.29 to 1.09, respectively); in terms of accessing treatment, the rate of having a vitrectomy was lowest in the most deprived cohort (OR=0.34, 95% CI 0.19 to 0.58). Conclusions: This large real-world study suggests that first presentation at a hospital eye clinic with visual loss or sight-threatening diabetic eye disease is associated with deprivation. These initial hospital visits represent the first opportunities to receive treatment and to formally engage with support services. Such patients are more likely to be sight-impaired or severely sight-impaired at presentation, and may need additional resources to engage with the hospital eye services over complex treatment schedules.
