Background: Umbralisib is an oral, once-daily, novel, dual inhibitor of phosphatidylinositol-3-kinase-delta (PI3Kδ) and casein kinase-1ε (CK1ε) that exhibits improved selectivity for the delta isoform of PI3K. Ublituximab is a novel anti-CD20 monoclonal antibody glycoengineered for enhanced antibody-dependent cellular cytotoxicity that targets a unique epitope on CD20. U2 has been well-tolerated and demonstrated promising activity in heavily pre-treated CLL patients. Herein, results are presented for the randomized, multicenter, Phase 3 UNITY-CLL trial (NCT02612311), which evaluated U2 vs O+Chl in patients with TN and R/R CLL. Methods: Patients ≥18 years of age with treatment-naïve (TN) or relapsed/refractory (R/R) CLL requiring treatment per iwCLL criteria with adequate organ function and ECOG PS ≤2 were eligible. Stratification factors included treatment status (TN vs R/R) and del(17p) status. Patients were initially randomized 1:1:1:1 to receive U2, O+Chl, umbralisib monotherapy, or ublituximab monotherapy. Following establishment of contribution comparing U2 to the single agents, patients were randomized 1:1 to U2 or O+Chl. Umbralisib was given orally at 800 mg once-daily until progression or removal from treatment for other reasons. Ublituximab was administered intravenously at 900 mg on Days 1/2 [split 150/750 mg], 8, and 15 of Cycle 1, Day 1 of Cycles 2 - 6, and on Day 1 every 3 cycles after Cycle 6. O was given intravenously at 1000 mg on Days 1/2 [split 100/900], 8, and 15 of Cycle 1, and Day 1 of Cycles 2 - 6. Chl was given orally at 0.5 mg/kg on Day 1 and 15 of Cycles 1 - 6. Each cycle was 28 days. The primary endpoint was independent review committee (IRC)-assessed progression-free survival (PFS) of U2 vs O+Chl. Key secondary endpoints included IRC-assessed overall response rate (ORR), complete response (CR), undetectable minimal residual disease (uMRD), duration of response (DOR), and safety (assessed from the first dose until 30 days after the last dose of study medication in each arm) as well as contribution of umbralisib and ublituximab to the U2 combination. Results: From Feb 2016 - Oct 2017, 421 pts were randomized to the U2 (n=210) or O+Chl (n=211) arms. The median age was 67 y (range, 36-91); 57% of patients (n=240) were treatment-naïve; 43% (n=181) had R/R CLL (median number of prior treatments = 1); 10% had del(17p), 20% del(11q), and 56% were IgHV unmutated. 66% were male. Demographics were well-balanced between treatment arms. At a median follow-up of 36.2 mos, U2 significantly prolonged PFS vs O+Chl (median 31.9 mos vs 17.9 mos; HR 0.546, 95% CI 0.413-0.720, P The median treatment duration was 23 mos for U2 (range, 0.1 - 49 mos) and 5 mos (range, 0.1 - 7 mos) for O+Chl. G3/4 AEs of interest regardless of causality (U2 vs O+Chl) included neutropenia (30.6% vs 34.7%), thrombocytopenia (3.4% vs 13.1%), diarrhea (12.1% vs 2.5%), infusion related reaction (1.9% vs 3.5%), elevated AST/ALTs (8.3% vs 2%), colitis (3.4% vs 0%) and pneumonitis (2.9% vs 0%). AEs led to treatment discontinuation in 34 patients (16.5%) on U2 and 16 patients (7.6%) on O+Chl. Conclusions: UNITY-CLL is the first randomized Phase 3 study in CLL of a PI3Ki vs. chemoimmunotherapy, and the first randomized study of a PI3Ki in treatment-naive CLL. U2 exhibited a well-tolerated safety profile, and significantly improved PFS vs. standard of care chemoimmunotherapy in patients with treatment-naive and relapsed/refractory CLL. Background: Umbralisib, a selective PI3Kδ and casein kinase-1epsilon (CK1ε) inhibitor, is pharmacologically distinct from other PI3K inhibitors and is administered orally once daily. Ublituximab is a novel anti-CD20 monoclonal antibody glycoengineered for enhanced antibody-dependent cellular cytotoxicity that targets a unique epitope on CD20. The primary analysis of the randomized, multicenter, Phase 3 UNITY-CLL trial (NCT02612311) demonstrated that the umbralisib+ublituximab (U2) combination prolonged progression-free survival (PFS) compared to chemoimmunotherapy in both treatment-naïve (TN) and previously treated (PT) populations (Gribben et al. 2020). Herein, results are presented for patients treated with U2 by treatment status. Methods: Patients ≥18 years of age with TN or PT CLL requiring treatment, per iwCLL criteria with adequate organ function and ECOG PS ≤2, were eligible. Patients were initially randomized 1:1:1:1 to receive U2, obinutuzumab+chlorambucil (O+Chl), umbralisib monotherapy, or ublituximab monotherapy. Stratification factors included treatment status (treatment-naïve vs. previously treated) and deletion 17p status. Umbralisib was administered orally at 800 mg once daily until progression or removal from treatment for other reasons. Ublituximab was administered intravenously at 900 mg on Days 1/2 [split 150/750 mg], 8, and 15 of Cycle 1, Day 1 of Cycles 2 - 6, and on Day 1 every 3 cycles after Cycle 6. The primary endpoint was independent review committee (IRC)-assessed PFS of U2 compared to O+Chl. Key secondary endpoints included IRC-assessed overall response rate (ORR), complete response, undetectable minimal residual disease (uMRD), duration of response, and safety, assessed from the first dose until 30 days after the last dose of study medication. While the primary analysis reported results for the pooled intent-to-treat population, the current analysis focuses on outcomes in patients treated with U2 by treatment status. Results: At data cut-off date of May 1, 2020, U2-treated patients had a median follow-up of 35.27 months. Of the 210 patients treated with U2, 119 were TN and 91 were PT. TN patients had a median age of 68 years (39 - 88 years) and 63% were male. Median PFS for U2 in TN patients was 38.5 mos (95% CI, 33.2, NE) with an estimated 24-mo PFS rate of 76.6%. IRC-assessed ORR was 84.0% (95% CI, 77.5%-90.6%). The median duration of exposure to umbralisib and ublituximab was 26.5 and 29.5 mos, respectively. In TN patients, AEs of special interest (AESI) of grade ≥3 included: neutropenia (24.1%), diarrhea (13.8%), ALT increased (12.1%), AST increased (7.8%), non-infectious colitis (2.6%), infusion-related reaction (IRR, 0.9%), and pneumonitis (0.9%). Discontinuation of either study drug due to these grade ≥3 AESI occurred in 5.2% (neutropenia), 2.6% (diarrhea), 3.4% (ALT increase), 1.7% (AST increase), 1.7% (colitis), 0.9% (IRR), and 0.9% (pneumonitis) of patients. PT patients had a median of 2 prior (1 - 9) lines of therapy, the median age was 65 years (43 - 87 years) and 65.9% were male. Median PFS was 19.5 mos (95% CI, 14.6-27.7) with an estimated 24-mo PFS rate of 41.3%. The IRC-assessed ORR was 82.4% (95% CI, 74.6%-90.2%). Among 14 patients previously treated with ibrutinib, the ORR was 57%. The median duration of exposure to umbralisib and ublituximab were 15.6 mos and 14.6 mos, respectively. In PT patients, AESI of grade ≥3 included: neutropenia (40.0%), diarrhea (10.0%), IRR (3.3%), ALT increase (3.3%), AST increase (2.2%), and non-infectious colitis (2.2%). Discontinuation of either study drug due to these grade ≥3 AESI occurred in 1.1% (ALT increase), 1.1% (AST increase), and 1.1% (colitis) of patients. Conclusions: U2 demonstrated a tolerable safety profile in both the TN and PT populations. These results mark the first randomized Phase 3 trial of a PI3K in TN CLL, establishing a new mechanism of action in this setting. A nonrandom structural gain of 1q may be seen in myelodysplastic syndrome (MDS) and acute myeloid leukemia (AML), and often it is due to an unbalanced translocation. Dup(1)(q21q32) as the sole abnormality has only rarely been reported. Reports have suggested that the dup(1)(q21q32) is predictive of a poor prognosis. We describe a case report of a 55 year old male who presented in 2002 with AML-M2, t(8;21)(q22;q22). He underwent induction with "7+3" followed by consolidation chemotherapy resulting in a complete remission. Two years later, his bone marrow revealed a dup(1)(q21q32) as an isolated aberration for the first time. In 2010, cytogenetic analysis of the bone marrow again confirmed this finding and FISH for AML1/ETO t(8;21) remained negative. Dup(1q) developed as an isolated abnormality two years after AML treatment, and to date, there is no evidence of progression to MDS. This is the first report of an acquired dup(1)(q21q32) as the sole abnormality in a patient treated for AML. This suggests that the dup(1q) may not be exclusively associated with a poor prognosis.

Neuroendocrine tumors comprise a large group of malignancies which share unique morphological features and are characterized by the presence of neuroendocrine markers such as synaptophysin, chromogranin-A, and CD56 (N-CAM), ranging from indolent tumors, such as carcinoid tumors, to aggressive tumors, such as small cell carcinoma. The lung is the most common site for primary neuroendocrine tumors. Extrapulmonary primary sites of small cell carcinoma are rare but have been documented arising from various sites including esophagus, stomach, colon and rectum, gallbladder, thymus, salivary gland, ovary, cervix, bladder, prostate, and skin. We present a case of small cell carcinoma arising from the thyroid gland, a site not previously described in the literature. A 59-year-old woman presented with a thyroid mass, which, after resection, showed small cell morphology and positive immunostains for TTF-1, synaptophysin, chromogranin-A, CD56, etc. Five months after diagnosis, she had widely metastatic disease. After a near-complete response to the first chemo-treatment, her disease progressed. Following local radiation and more rounds of chemotherapy, she succumbed to the disease, 15 months after diagnosis. Our patient had no pulmonary lesions at the time of diagnosis to suggest metastasis from the lung. Much like its pulmonary counterparts, this small cell carcinoma of primary thyroid origin displayed an aggressive clinical course and poor outcome. Although it shows early sensitivity to chemotherapy, small cell carcinoma remains a difficult-to-treat cancer with a poor prognosis and can rarely be seen originating in organs outside of the lung.

Background Non-cancerous medical conditions have a profound impact on the health of patients with malignancies. In individuals with lung cancer, the likelihood of death due to other causes is higher than that of the general population. It is therefore essential that patients with hematologic/oncologic diseases be effectively represented in non-oncologic clinical trials. Despite that truism, patients with cancer or with hematological impairments are typically excluded from clinical trials due to safety concerns. The frequency and degree to which hematological parameters or a diagnosis of cancer serve to exclude patients from practice-changing clinical studies in cardiology have yet to be determined. Whether studies that exclude patients based on such parameters have a lower incidence rate of major bleeding than those that do not is also unknown. Methods We focused on acute coronary syndrome (ACS). An investigation of the hematologic/oncologic exclusion profile of landmark studies of ACS was conducted. We searched The New England Journal of Medicine for all original research studies of ACS published between 2006 and 2016. Hematologic/oncologic exclusion criteria were reviewed. The nature of those parameters (laboratory value, disease state, medication use, clinical risk assessment) was analysed. The number of criteria per study as well as the percentage of participants excluded for meeting those criteria were recorded. The incidence of major bleeding (defined by TIMI bleeding criteria) among patients randomised into the interventional arms was compared between studies that excluded patients with particular derangements and those that did not. Results Thirty randomised clinical trials were found with a total number of patients of 235,981. Two thirds (20 out of 30) of the studies had at least one hematologic/oncologic exclusion criteria. The mean number of hematologic/oncologic exclusion criteria per study was 1.7. The most common exclusion parameter was laboratory-based. Ten studies (33.3%) specified cell count value as a cutoff for participation. Haemoglobin threshold (700k) was chosen in 9 publications (30%). Twelve studies (40%) issued active bleeding diathesis while six (20%) included "high-risk" for bleeding as exclusion criteria. Eight studies (26.6%) excluded patients who were chronically on oral anticoagulation. Six studies (20%) excluded patients with history of cancer. Seven studies (23.3%) had at least one subjective parameter in the list of exclusion criteria, such as "concerning lab abnormality", "high-risk" for bleeding, or "severe haematological disease". The aggregate incidence rate of major bleeding was significantly higher in studies that did not exclude for haemoglobin levels lower than 10 mg/dl compared with those that did (2.8% versus 1.6%, p value Conclusions Hematological parameters as well as a medical history of a cancer commonly serve as exclusion criteria in studies of patients with ACS. Our analysis suggests that there may be a need for better standardization of exclusion criteria in studies in cardiovascular medicine, given that almost a quarter of high-quality studies of ACS include at least one inherently subjective parameter. The results provide support for uniformly setting severe anemia as exclusion criteria in studies of ACS until further data is available. α-Amino-3-hydroxy-5-methyl-4-isoxazole propionic acid (AMPA)-type glutamate receptors (AMPARs) mediate excitatory neurotransmission at neuronal synapses, and their regulated localization plays a role in synaptic plasticity. In Caenorhabditis elegans, the PDZ and PTB domain-containing protein LIN-10 is required both for the synaptic localization of the AMPAR subunit GLR-1 and for vulval fate induction in epithelia. Here, we examine the role that different LIN-10 domains play in GLR-1 localization. We find that an amino-terminal region of LIN-10 directs LIN-10 protein localization to the Golgi and to synaptic clusters. In addition, mutations in the carboxyl-terminal PDZ domains prevent LIN-10 from regulating GLR-1 localization in neurons but do not prevent LIN-10 from functioning in the vulval epithelia. A mutation in the amino terminus prevents the protein from functioning in the vulval epithelia but does not prevent it from functioning to regulate GLR-1 localization in neurons. Finally, we show that human Mint2 can substitute for LIN-10 to facilitate GLR-1 localization in neurons and that the Mint2 amino terminus is critical for this function. Together, our data suggest that LIN-10 uses distinct modular domains for its functions in neurons and epithelial cells and that during evolution its vertebrate ortholog Mint2 has retained the ability to direct AMPAR localization in neurons.

e12008 Background: Because of the ubiquitous usage of cellphones, any health consequences, if found, would be of paramount importance. Many have questioned the relationship of cellphone radiofrequency waves and malignancies such as brain tumors. Because men usually store their phones in close proximately to the testicles, we analyzed the relationship between cell phone usage and testicular cancer. Methods: Testicular cancer data was obtained from SEER-9 United States Population Data Base of the National Cancer Institute. Cell phone subscription data was obtained from the World Health Organization publication. Initial data analysis was done from 1991 to 2008, with subsequent age specific and other analyses performed from 1999 to 2008. Trends over time were calculated as Annual Percentage Change of incidence rates (APC) by using log linear models. Results: Rates of United States mobile subscriptions increased exponentially since the late 1990’s reaching 88.87 percent of the population by 2008. There was no statistically significant change in incidence rates of all testicular tumors (APC 0.7; CI -0.2, 0.7), Seminoma (APC 0.2; CI 0.8, 1.1) and non-Seminoma (APC 1.4; CI -0.6, 3.4) from 1999 to 2008. In the age specific analysis, three groups showed an increase in incidence rates: all testicular cancer age 25-34 (APC 2; CI 0.6, 3.4), All testicular cancer age greater than 45 (APC 1.7; CI 0.1, 3.3) and Seminoma age 25-34 (APC 2.1; CI 0.6, 3.7). All these changes were only minor percentage increase when compared to mobile subscriptions. Conclusions: Based on incidence data, there is no convincing evidence of increased risk of testicular cancer from cell phone use. Given possible lag time incidence, continued monitoring is needed.

A 79-year-old man presented with a history of clear cell carcinoma of the right kidney, Fuhrman grade 2, 12 years after nephrectomy, and a history of low-risk prostate adenocarcinoma 11 years after brachytherapy. One year before presentation, the renal cell carcinoma had metastasized to his axial skeleton, and temsirolimus was started. Approximately 1 year later, he presented with a new, painful, lytic lesion in a rare site, his middle phalanx, which was biopsy proven to be clear cell carcinoma.

Abstract 4863 Introduction: Cytogenetic studies are an important prognostic tool in the management of hematologic malignancies such as myelodysplastic syndromes (MDS) and acute myeloid leukemia (AML). While a nonrandom structural gain of 1q may be seen in MDS and AML, it is most commonly due to an unbalanced translocation usually in association with other chromosomal abnormalities. Duplication of the long arm of chromosome 1 [dup(1)(q21q32)] as the sole abnormality in nonlymphoid hematologic malignancies has only rarely been reported. As a result, very little is known about the clinical significance of this chromosomal abnormality. However, it has been suggested that this single aberration is predictive of an increased risk of clonal evolution and poor overall prognosis. Case Presentation: A then 55 year old male presented in June, 2002 with a CBC showing: WBC 36,300/ul (74% blasts with Auer rods), Hgb 7.0 gm/dl, MCV 98fl, platelets 61,000/ul. Bone marrow biopsy was 90% cellular with sheets of blasts. The blasts were positive for CD34, CD117, HLA-DR, CD13, MPO, CD56, and Tdt. A diagnosis of AML M2 was made. Cytogenetics showed 45,X,-Y, t(8;21)(q22;q22). He underwent successful standard induction with “7+3” followed by consolidation with “5+2” and 4 cycles of high dose cytarabine. Subsequent bone marrows after induction and during consolidation showed no evidence of AML. The karyotype was 46,XY with no evidence of t(8;21) by FISH. He has had complete recovery of his blood counts, except for intermittent neutropenia. In May, 2004, G-banding of his bone marrow revealed the presence of a dup(1)(q21q32) as an isolated cytogenetic aberration for the first time. This finding was again seen on follow up studies in 2006, 2007, and 2009. His most recent evaluation in May 2010 showed WBC 5,500/ul (neutrophils 2,200/ul), Hgb 14.0 gm/dl, MCV 92fl, platelets 299,000/ul. Bone marrow biopsy was 40% cellular with trilineage hematopoiesis. The aspirate showed erythroid hyperplasia with megaloblastoid features. No dysplastic RBC or ring sideroblasts were seen. Cytogenetics showed 46,XY,dup(1)(q21q32)[15]/46,XY[5]. FISH for AML1/ETO t(8;21) and for abnormalities of chromosomes 5, 7, 8, and 20 were negative. Discussion: Dup(1q) has been reported as an isolated cytogenetic abnormality in a variety of MDS subtypes. It may be either a primary or secondary chromosomal aberration. It has been suggested that isolated dup(1q) in MDS is a harbinger of disease progression. Recent reports of MDS with either inverted dup(1)(q32 q21) or dup(1)(q21q32) of both chromosome 1 homologs showed no disease progression but had very short follow-up. Conclusion: To our knowledge, this case is the first report of an acquired duplication dup(1)(q21q32) as the sole abnormality in a patient previously treated for AML. This duplication developed approximately 2 years after induction and consolidation chemotherapy. The involved clone is not the original leukemic clone since there is no evidence for t(8;21) or -Y. Six years later, there has been no evidence of progression to MDS or sAML. It is not known why our patient has shown such long survivorship after discovery of dup(1)(q21q32), nor is it known if this chromosomal finding is a treatment related phenomenon. This case suggests that dup(1q) may not be exclusively associated with a poor prognosis. FISH analysis with a 1q21 probe is planned to confirm our G-banding observation.
