This study was conducted in Côte d’Ivoire to determine the respective prevalence of Candida species and to study their susceptibility profile to antifungal agents to identify the emergence of resistance.This retrospective study was conducted from 2017 to 2019 at the Institute Pasteur of Côte d’Ivoire on patients with fungal disease. The yeasts isolated and identified in Côte d’Ivoire were subcultured on a chromogenic medium and identified with MALDI-TOF-MS. Sensitivity tests were performed using the CLSI method and evaluated by the M27. In total, 227 Candida spp. were isolated from 1941 patients. Most of the samples were vaginal swabs (120/228). Using the conventional method, C. albicans was prevalent (52.6%). There were mixtures of 2 to 4 species in 36.8% of the samples. Thus MALDI-TOF-MS identified in 315 isolates 13 distinct species, C. tropicalis was prevalent at 35.2%, and the species in both C. albicans and C. parapsilosis complexes were differentiated. This study highlights the hurdle of correct yeast identification in developing countries. The transition from conventional to modern MALDI-TOF based identification of these life-threatening opportunistic pathogens is mandatory and should be emphasised as a public health priority.

Background: One of the main health problems in West Africa remains upsurge of emerging pathogens. Ebola virus disease outbreak occurred in 2014 in Liberia, Guinea and Sierra Leone, Monkeypox virus in Nigeria in 2017 and most recently Lassa virus in Nigeria, Togo and Benin in 2018. These pathogens have animal reservoirs as vectors for transmission. Proper investigation of the pathogens in their rodent vectors could help reduce and manage their emergence and spread. Methodology: This study was conducted with an approval from the Côte d’Ivoire Bioethics Community. Small mammal trappings were carried out in 9 sites within three zones namely, peri-urban, peri-rural and protected areas. Liver, lung and kidney tissues from trapped small mammals were sampled in accordance with the recommended conditions of biosafety and bioethics. The organs were transported in liquid nitrogen to the laboratory. Molecular tests were used to detect pathogens. Orthopoxviruses and Monkeypox virus were detected in the organs by PCR using consensus primers targeting the virus surface membrane haemagglutinin (HA) genes, while Leptospira species were detected by PCR using primers targeting the rrs and lfb1 genes. Results: Out of 4930 night-traps, 256 (5.19%) small mammals were trapped including Crocidura, Rattus, Lophuromys, Praomys, Mus and Mastomys. Leptospira species were detected in 6 genera from 7 study sites and the infected small mammals accounted for 13.3%. Leptospira sp was detected mainly in the rodent vector genera Rattus (32.3%), Lophuromys (29.0%), and Praomys (16.1%). Three species of Leptospira were detected and Leptospira interrogans was the most common frequent species (74.2%). Monkeypox virus was not detected from studied small mammals. Conclusion: The initial data from our investigation indicates the presence of Leptospira sp in rodent vectors, Rattus, Lophuromys and Praomys, which are the potential small mammalian reservoirs of this pathogen in Cote d’Ivoire.

Une belle exposition montée en 1990, exposée en 1998 lors du XVIe Congrès Mondial de Science du Sol à Montpellier, a continué de voyager après les années 2000. Elle s'est retrouvée à Alès en 2018 pour une exposition pour les jeunes et grand public. Deux scientifiques du sol, invités à donner leur opinion sur le contenu des panneaux, ont réalisé combien, en une trentaine d'années la communication sur les sols pour un grand public a pu changer, en particulier par l'accent mis actuellement sur les biens et les services écosystémiques rendus par les sols à l'humanité. L'exposition a donc été repensée en utilisant les panneaux initiaux et en proposant au public jeune diverses manipulations. Ce sont ces deux expositions, à 28 ans d'écart, qui sont racontées et illustrées dans cet article.

Buruli ulcer is a cutaneous mycobacterial disease that occurs in tropical countries in sub-Saharan Africa, South-East Asia, Australia and America. The responsible pathogen is Mycobacterium ulcerans. Cote d'Ivoire is the most affected country, with more than 30 endemic health districts reporting a large number of chronic skin lesions. The clinical forms and the severity of ulcers vary from one patient to another. Samples from suspected patients were analyzed by PCR at Pasteur Institute of Cote d'Ivoire, as recommended by WHO. IS2404 sequence was detected in 61% of cases, incriminating M. ulcerans in chronic cutaneous lesions. For the other cases the etiology was not identified, thus raising several questions. Are all reported "Buruli ulcer" cases really caused by M. ulcerans? Would other mycobacteria be involved in the occurrence of chronic skin lesions considered as "Buruli ulcer"? BU suspected patients were enrolled in endemic areas of Cote d’Ivoire. Samples were collected from cutaneous lesions and transported to the lab at +4°C in 2 ml of Middlebrook 7H9 medium supplemented by Cetylpiridium chloride. The centrifugation pellet was taken with saline buffer to perform microscopic examination and mycobacteria isolation on Lowenstein-Jensen medium. Biochemical characteristics were described by the nicotinic acid detection according to Konno protocol, by the Nitrates reduction test, by the catalase activity detection at 22° and 68°C and the Wayne's Tween 80 hydrolysis test. Genotypic characteristics were determined by PCR with 1 ml of bacterial suspension targeting the insertion sequences (IS6110, IS2404, and IS2606), the plasmid virulence genes and Miru-VNTR loci (Miru-1, VNTR 6, VNTR 19, ST-1). A total of 47 mycobacterial strains were isolated with 3 different types of colonies whose microscopic examination showed Acid-Alcohol-Resistant Bacilli. 65.9% of isolates expressed biochemical characters in favor of M. ulcerans strains and 6.4% in favor of M. marinum strains. For 29.8% of isolates, the characteristics were related to atypical mycobacterial species. The genotyping targeting the IS6110, IS2606 and IS2404 insertion sequences allowed simultaneous amplification in 53.2% of isolates. IS2404 was amplified in 93.6% of isolates; IS6110 was amplified in 74.5% of isolates and IS2606 was amplified in 70.2% of isolates. Five genotypes were identified corresponding to various species of mycobacteria: genotypes 1 and 2 accounted for 63.8% with all the 3 insertion sequences and biochemical characteristics in favor of M. ulcerans strains; genotype 4 accounted for 6.4% of isolates with insertion sequences and biochemical characteristics in favor of M. marinum strains; The strains of genotypes 3 and 5 expressed molecular and biochemical characters relating to various non-M. ulcerans mycobacteria. Virulence genes were found in 72.3% of isolates corresponding to 90% of M. ulcerans strains and 60.7% of non-M. ulcerans mycobacteria. This study confirmed the involvement of several genotypes of M. ulcerans and other mycobacteria in chronic cutaneous lesions suspected as cases of Buruli ulcer in Cote d’Ivoire.

The Orthopoxvirus (OPV) and the Molluscum contagiosum virus (MCV) are Poxviruses involved in viruses skin lesions in humans. OPV infects many vertebrates and MCV mainly infects humans. A diagnostic confusion is often observed between the clinical lesions due to the different Poxviruses firstly and secondly with other viruses like the virus of the chickenpox. In Côte d'Ivoire, the diagnosis of MCV remains essentially clinical and that of OPV is non-existent despite the risk of circulation of the virus. This study aims to implementthe molecular detection of the OPV and the MVC in Côte d'Ivoire. Material and method: Cowpoxvirus DNA and 21 DNA extracts from suspicious cutaneous lesions of the MCV were analyzed by conventional PCR. The consensus primers (EACP1, EACP2) designed from the surface hemagglutin gene were used for the detection of the OPVs and the primers (MCV1, MCV2) targeting the K fragment of the MCV were used for the MCV’s detection . A growing dilution series of the Cowpoxvirus DNA and the MCV allowed the study of the method’s sensitivity used. The DNAs of S.aureus, M. ulcerans, VZV, HSV, the Measles virus and Varicella virus were used for the specificity tests. Results: The detection of the OPV from the Cowpoxvirus viral strain was positive with a positivity threshold at 10-1 dilution. That of the MCV DNA from the suspected MCV's lesion was positive with a positivity threshold of up to 10 -6 dilution. No non-specific amplification was observed with the DNAs of the other pathogens responsible for lesions Cutaneous. The clinical diagnosis of the MCV was confirmed by PCR in 18 out of the 21 patients, ie 85.71%. On the 3 patients with a negative MCV PCR, 2 were positive for the OPV PCR , reflecting the risk of confusion between clinical lesions due to Poxviruses.Keyvords: Molecular diagnostic, Poxviruses, West Africa

SETTING World Health Organization advocates for integration of HIV-tuberculosis (TB) services and recommends intensive case finding (ICF), isoniazid preventive therapy (IPT), and infection control ("Three I's") for TB prevention and control among persons living with HIV. OBJECTIVE To assess the implementation of the "Three I's" of TB-control at HIV treatment sites in lower income countries. DESIGN Survey conducted between March-July, 2012 at 47 sites in 26 countries: 6 (13%) Asia Pacific, 7 (15%), Caribbean, Central and South America, 5 (10%) Central Africa, 8 (17%) East Africa, 14 (30%) Southern Africa, and 7 (15%) West Africa. RESULTS ICF using symptom-based screening was performed at 38% of sites; 45% of sites used symptom-screening plus additional diagnostics. IPT at enrollment or ART initiation was implemented in only 17% of sites, with 9% of sites providing IPT to tuberculin-skin-test positive patients. Infection control measures varied: 62% of sites separated smear-positive patients, and healthcare workers used masks at 57% of sites. Only 12 (26%) sites integrated HIV-TB services. Integration was not associated with implementation of TB prevention measures except for IPT provision at enrollment (42% integrated vs. 9% non-integrated; p = 0.03). CONCLUSIONS Implementation of TB screening, IPT provision, and infection control measures was low and variable across regional HIV treatment sites, regardless of integration status.
