3. Quality of Life in Children With Achondroplasia Undergoing Paired Limb Lengthening With an External Fixator and Modified Distraction Control: Observational Nonrandomized Study

BackgroundTransosseous distraction osteosynthesis is prioritized in orthopedic care for children with achondroplasia. However, difficulties encountered during treatment and rehabilitation directly impact patients’ quality of life. Using rod external fixators within a semicircular frame for osteosynthesis is less traumatic compared to spoke circular devices. Their straightforward assembly and mounting on the limb segment can help significantly reduce treatment duration, thereby improving children’s quality of life during treatment and rehabilitation. ObjectiveThis study aimed to conduct a comparative analysis of the quality of life (measured by postoperative pain syndrome, physical activity, and emotional state) among children with achondroplasia undergoing paired limb lengthening using either an external fixator with modified distraction control or a circular multiaxial system developed by the authors. MethodsThis was an observational, prospective, nonrandomized, and longitudinal study with historical control. The study group consisted of 14 patients ranging from 5 to 15 (mean 7.6, SD 2.3) years old with a genetically confirmed diagnosis of achondroplasia. All patients underwent paired limb lengthening with a rod external fixator and a modified distraction control developed by the authors. A total of 28 limb segments, among them 4 (14%) humeri, 8 (29%) femurs, and 16 (57%) tibias, were lengthened in 1 round. Unpublished data from the previous study served as the control group, comprising 9 patients (18 limb segments) of the same age group (mean age at surgery 8.6, SD 2.3 years), who underwent limb lengthening surgery using a circular multiaxial system—2 (11%) humeri, 6 (33%) femurs, and 10 (56%) tibias. The Wong-Baker Faces Rating Scale was used to measure pain symptoms, while the Russified Pediatric Quality of Life (PedsQL) v4.0 questionnaire assessed quality of life. ResultsDuring the latent phase (7 to 10 days after surgery), a more pronounced decrease in the indicators of physical activity and emotional state on the PedsQL v4.0 questionnaire was noted in the control group (mean 52.4, SD 4.8 versus mean 52.8, SD 5.5 points according to children’s responses and their parents’ responses, respectively) compared to the experimental group (mean 59.5, SD 6.8 points and mean 61.33, SD 6.5 points according to the children’s responses and their parents’ responses, respectively). 5. Reducing the Risk of Postoperative Genital Complications in Male Adolescents

- Abstract
The reproductive system of adolescents is exposed to a high risk of anomalies. In spite of the successes of surgical correction, the percentage of postoperative complications remains high. Special attention should be paid to circumcision, which is regarded as a religious tradition in many countries and carried out with sanitary violations. This research developed a systematic algorithm for the prevention of complications of surgical interventions in case of external genitalia anomalies. The article describes results of a statistical analysis related to the main risk factors of postoperative complications in primary genital surgery in male adolescents. The analysis implied calculating the frequency of value occurrence, the percentage of value occurrence, and cumulative frequency. Using the Spearman criterion, we carried out correlation analysis; 11 predicts of complications in the postoperative period during manipulations on the genitals were determined with regard to logistic regression. Comparison of the interdependent variables by using the Chi-square method and the Wilcoxon test gave the possibility to determine statistically significant differences in the frequency distribution of patients in terms of complications of the main group. A set of necessary measures for preventing postoperative complications was presented in the form of a prevention circle.

Abstract The work aimed to evaluate the effectiveness of the developed distraction system based on the rod external monolateral fixation mechanisms by comparing it with the classical technique of long tubular bones distraction based on the circular multi-axial system. The study included patients with a genetically confirmed diagnosis of achondroplasia. The experimental group consisted of 14 patients who underwent surgical limb lengthening by the rod monolateral external fixator with a distraction system developed by the authors. The lengthening was performed on 28 segments of tubular bones. The majority of the experimental group patients achieved the lengthening value close to the planned one and the deformation correction. The fixation period was averagely 83.8 ± 3.7 days, the regenerate length was 8.5 ± 0.6 cm, and the mechanical strength of the distraction regenerate was 10.3° ± 2.18°. The rod external fixator with a control distraction system developed by the authors has small dimensions and low weight of the external supporting elements of high durability. It is reported to provide a good psychological tolerance of the treatment process and significantly outperforms the circular multi-axis system. Considering the aforementioned, the proposed apparatus can grant good orthopedic care to patients with achondroplasia.

There is a long history of examining the connection between crime and delinquency and economic well-being and employment quality. Despite this vast literature, there still remains unanswered questions surrounding these associations, including whether the timing of adolescent delinquency is associated with adulthood economic disadvantage and job quality, whether different types of adolescent delinquency maintain differential associations with measures of economic disadvantage and employment quality in adulthood, and whether any associations between delinquency and economics/employment are invariant between males and females. The current study sought to address these issues by exploring the associations between adolescent involvement in nonviolent and violent delinquency (measured at two times in adolescence) and adulthood economic disadvantage and job quality. To do so, data drawn from the National Longitudinal Study of Adolescent to Adult Health were analyzed. The results revealed consistently null associations between the measures of adolescent delinquency and economic disadvantage and job quality for males. For females, however, the results were consistently significant, indicating that females who self-reported greater involvement in delinquency were more likely to be economically disadvantaged as adults and to have lower quality jobs. These results indicate that the associations between delinquency and economic disadvantage and job benefits differs between males and females, with females, in comparison with males, paying a significantly greater toll for engaging in acts of violent and nonviolent delinquency. The investigation of thermoelastic wave propagation in elastic media is bound to have much influence in the fields of material science, geophysics, seismology, and so on. The heat conduction equations and bound equations of motions differ by the difficulty level and presence of many physical and mechanical parameters in them. Therefore thermoelasticity is being extensively studied and developed. In this paper by using analytical matrizant method set of equation of motions in elastic media are reduced to equivalent set of first-order differential equations. Moreover, for given set of equations, the structure of fundamental solutions for the general case has been derived and also dispersion relations are obtained.

Mining activities are acknowledged to introduce contaminants into localised environments and cause wider spread diffuse pollution. The concentration, distribution and fate of arsenic (As) and antimony (Sb) were studied at the former metalliferous Louisa Mine at Glendinning, Scotland. Soils and surface water were sampled and subsequently analysed to map the distribution of contamination and identify pollution sources. The maximum concentrations of As and Sb of 15,490 and 1504.2 mg kg−1, respectively, were determined in soils associated with the ore processing area and spoil heaps. Abstract The inclusive photoproduction of D ∗ mesons and of D ∗-tagged dijets is investigated with the H1 detector at the ep collider HERA. The kinematic region covers small photon virtualities Q 21.8 GeV. The heavy quark production process is further investigated in events with at least two jets with transverse momentum p T(jet)>3.5 GeV each, one containing the D ∗ meson. Differential cross sections for D ∗-tagged dijet production and for correlations between the jets are measured in the range |η(D ∗)|2.1 GeV. The results are compared with predictions from Monte Carlo simulations and next-to-leading order perturbative QCD calculations.

Abstract A measurement of the inclusive ep scattering cross section is presented in the region of low momentum transfers, 0.2 GeV2 ≤Q 2≤12 GeV2, and low Bjorken x, 5⋅10−6≤x≤0.02. The result is based on two data sets collected in dedicated runs by the H1 Collaboration at HERA at beam energies of 27.6 GeV and 920 GeV for positrons and protons, respectively. A combination with data previously published by H1 leads to a cross section measurement of a few percent accuracy. A kinematic reconstruction method exploiting radiative ep events extends the measurement to lower Q 2 and larger x. The data are compared with theoretical models which apply to the transition region from photoproduction to deep inelastic scattering.

The work aimed to evaluate the effectiveness of the developed distraction system based on the rod external monolateral fixation mechanisms by comparing it with the classical technique of long tubular bones distraction based on the circular multi-axial system. The study included patients with a genetically confirmed diagnosis of achondroplasia. The experimental group consisted of 14 patients who underwent surgical limb lengthening by the rod monolateral external fixator with a distraction system developed by the authors. The lengthening was performed on 28 segments of tubular bones. The majority of the experimental group patients achieved the lengthening value close to the planned one and the deformation correction. The fixation period was averagely 83.8 ± 3.7 days, the regenerate length was 8.5 ± 0.6 cm, and the mechanical strength of the distraction regenerate was 10.3° ± 2.18°. The rod external fixator with a control distraction system developed by the authors has small dimensions and low weight of the external supporting elements of high durability. It is reported to provide a good psychological tolerance of the treatment process and significantly outperforms the circular multi-axis system. Considering the aforementioned, the proposed apparatus can grant good orthopedic care to patients with achondroplasia.

Mining activities are acknowledged to introduce contaminants into localised environments and cause wider spread diffuse pollution. The concentration, distribution and fate of arsenic (As) and antimony (Sb) were studied at the former metalliferous Louisa Mine at Glendinning, Scotland. The associated deposit is one of very few able to produce Sb in the UK and was mined for three brief periods between 1793 and 1922. The remnants of the mine consist of the ore processing area and two spoils. Soils withing these zones as well as around the mine were sampled and complemented by water samples from the adjacent stream, the Glennshanna Burn. All samples were subsequently analysed to map the distribution of contamination and identify pollution sources. The maximum concentrations of As and Sb, 15490 and 1504.2 mg kg−1 respectively, were determined in soils associated with the ore processing area and spoil heaps. Anthropogenic activities also redistributed As and Sb within these mine zones and altered their relative ratios. The fractions of dissolved As and Sb in soils were < 1 and < 5% of total soil content, respectively, confirming findings of previous studies that As and Sb are relatively immobile. Yet, the concentrations of As and Sb released by soils exceeded regulatory limits.Concentrations of As and Sb in surface water in the immediate vicinity of the mine were impacted by a gully discharge, but rapidly diluted. While the concentrations affected by the run-off waters did not exceed EU environmental standards for freshwater, the concentrations of As and Sb sharply increased to 11.43 ± 3.43 and 9.28 ± 0.59 μg l−1, respectively, approximately 100 m downstream of the mine site. The unaltered As to Sb ratios in water samples suggested a geogenic source of contamination.While there is a justifiable concern about the soil pollution caused by the historic mining in the studied area, the Glenshanna Burn is affected more by indigenous geochemical processes than the derelict mine.
