Objetivo: Evaluar si los corticoesteroides antenatales reducen el distrés respiratorio en recién nacidos por cesárea a término. Método: Estudio multicéntrico aleatorizado pragmático, realizado en 10 maternidades; 998 mujeres con consentimiento informado fueron randomizadas al momento de decidirse la cesárea electiva: 503 fueron randomizadas al grupo de tratamiento. El grupo de tratamiento recibió 2 dosis intramusculares de 12 mg de betametasona en las 48 horas anteriores al parto, el grupo control recibió tratamiento estándar. El resultado primario fue la admisión a la Unidad de Cuidados Intensivos (UCI) neonatal por distrés respiratorio. Los resultados secundarios fueron la severidad del cuadro y nivel de cuidados requeridos. Resultados: El sexo, peso y edad gestacional de los recién nacidos (RN) no presentaron diferencias significativas entre los dos grupos. De los 35 RN que fueron admitidos a la UCI neonatal debido a distrés respiratorio, 24 pertenecían al grupo control y 11 al grupo de intervención (p=0,02). La incidencia de admisión por distrés respiratorio fue de 0,051 en el grupo control y 0,024 en el grupo de tratamiento (RR: 0,46; 95% IC 0,23 a 0,93). La incidencia de taquipnea transitoria del recién nacido fue de 0,040 en el grupo control y 0,021 en el grupo de tratamiento (RR: 0,54; 95% IC 0,26 a 1,12). La incidencia del síndrome de distrés respiratorio fue 0,011 en el grupo control y de 0,002 en el grupo de tratamiento (RR: 0,21; 95% IC 0,03 a 1,32). Conclusiones: Tanto la betametasona antenatal como postergar el parto hasta después de las 39 semanas de gestación reducen la admisión del RN a la UCI neonatal después de una cesárea electiva al término del embarazo

Se presenta el caso de un embarazo gemelar monocorial con un gemelo muerto a las 24 semanas de embarazo, manejado en forma expectante luego de efectuada una transfusión intrauterina de rescate. La paciente evolucionó con análisis de coagulación dentro de límites normales y pruebas de bienestar fetal satisfactorias. Molecular structural elucidation can be accomplished by different techniques, such as nuclear magnetic resonance or X-ray diffraction. However, the former does not give information about the three-dimensional atomic arrangement, and the latter needs crystallizable solid samples. An alternative is direct, real-space visualization of the molecules by cryogenic scanning tunneling microscopy (STM). This technique is usually limited to thermally robust molecules because an annealing step is required for sample deposition. A landmark development has been the coupling of STM with electrospray deposition (ESD), which smooths the process and widens the scope of the visualization technique. In this work, we present the on-surface characterization of air-, light-, and temperature-sensitive rhamnopolyene with relevance in molecular biology. Supported by theoretical calculations, we characterize two isomers of this flexible molecule, confirming the potential of the technique to inspect labile, non-crystallizable compounds.

A set of novel Donor-Acceptor-Donor (D-A-D) benzoselenadiazole derivatives has been synthesized and crystallized in nanocrystals in order to explore the correlation between their chemical structure and the waveguided luminescent properties. The findings reveal that all crystals exhibit luminescence and active optical waveguiding, demonstrating the ability to adjust their luminescence within a broad spectral range of 550-700 nm depending on the donor group attached to the benzoselenadiazole core. Notably, a clear relationship exists between the HOMO-LUMO energy gaps of each compound and the color emission of the corresponding optical waveguides. These outcomes affirm the feasibility of modifying the color emission of organic waveguides through suitable chemical functionalization. Importantly, this study marks the first utilization of benzoseleniadiazole derivatives for such purposes, underscoring the originality of this research. In addition, the obtention of nanocrystals is a key tool for the implementation of miniaturized photonic devices.

Background: People with chronic pain might evade certain movements to prevent their experience of pain. Movement-evoked pain (MEP) might induce lower functionality during daily activities., Hypothesis: (1) MEP after physical fitness tests would vary depending on the main musculature involved in the test; (2) physical and psychological factors would be associated with MEP in patients with NSCLBP., Study Design: Cross-sectional design., Level of Evidence: Level 3., Methods: A total of 104 (69 women) patients aged 51.0 ± 10.3 years with NSCLBP participated. MEP was measured with a visual analog scale (VAS) at baseline and immediately after performing each physical fitness test, that is, the Biering-Sørensen, prone bridging, handgrip, chair-stand, and 8-foot time-up-and-go tests, measuring back extensor, back flexor, upper- and lower-body muscle strength, and motor agility, respectively. Global muscle strength was calculated with normalized index ( z -score) procedure. Depression, anxiety, pain catastrophizing, and central sensitization were assessed with the Beck Depression Inventory II, State Trait Anxiety Inventory I, Pain Catastrophizing Scale, and Central Sensitization Inventory (CSI), respectively., Results: Patients showed greater pain after completion of the Biering-Sørensen (mean difference 95% CI, 0.02, 1.11), prone bridging (0.15, 1.21), lower pain after handgrip (-1.46, -0.52), and the 8-foot time-up-and-go (-1.43, 0.46) tests (all P ≤ 0.04). Lower global muscular strength (β between -0.18 and -0.30), and greater pain catastrophizing (β = 0.16), and CSI scoring (β between 0.18 and 0.27) were associated with greater MEP (all P ≤ 0.04)., Conclusion: Greater MEP was observed after tests measuring core musculature strength than after tests measuring distal (limbs) strength and agility. Greater MEP was overall associated with lower muscle strength, greater pain catastrophizing, and central sensitization., Clinical Relevance: Fitness testing might be implemented as a complementary tool for the monitoring of NSCLBP in clinical settings., Competing Interests: The authors report no potential conflicts of interest in the development and publication of this article.

In order to achieve a multifunctional compound with potential application in organic photonics and electronics, a multidonor benzothiadiazole derivative was rationally designed and synthesized employing microwave irradiation as energy source, increasing the process efficiency about yields and reaction times in comparison with conventional conditions. This powerful compound displayed solvatochromism and showed efficient behavior as red optical waveguide with low OLC around 10 -2  dB μm -1 and with the capacity of light transmission in two directions. In addition, the proposed derivative acted as efficient p-type semiconductor in organic field-effect transistors (OFETs) with hole mobilities up 10 -1  cm 2  V -1  s -1 . This corroborates its multifunctional character, thus making it a potential candidate to be applied in hybrid organic field-effect optical waveguides (OFEWs)., (© 2023 Wiley-VCH GmbH.)

A series of donor-π-acceptor-π-donor (D-π-A-π-D) compounds based on naphthalendiimide (NDI) and perylenediimide (PDI) central cores combined with triphenylamine and phenylcarbazole donor groups have been synthesized, characterized and tested in top-contact/bottom gate organic field-effect transistors (OFETs). The results showed high electron mobilities, up to 0.3 cm 2  V -1  s -1 , in the case of NDI derivatives and moderate values of around 10 -3  cm 2  V -1  s -1 for PDI-based semiconductors. Quantum chemical calculations were performed in order to support the experimental data. The results suggest that adequate molecular characteristics and larger crystalline domains in NDI vs. PDI semiconducting films may be the reasons behind the enhanced electrical properties of NDI derivatives. Furthermore, when the lateral donor substituents are triphenylamine groups, the mobilities were slightly higher in comparison to phenylcarbazole donor groups due to an improved electron-donating character. Other characterization techniques, such as AFM, X-ray diffraction or spectroelectrochemistry, among others, have been performed to analyze supramolecular order, charge carriers' nature and stability, parameters closely related to charge transport characteristics., (© 2023 The Authors. In this work, as an equivocal proof of the potential of microwave irradiation in organic synthesis, a complex pyrazine-decorated benzotriazole derivative that is challenging to prepare under conventional conditions has been obtained upon microwave irradiation, thus efficiently improving the process and yields, dramatically decreasing the reaction times and resulting in an environmentally friendly synthetic procedure. In addition, this useful derivative could be applied in organic electronics, specifically in organic field-effect transistors (OFETs), exhibiting the highest electron mobilities reported to date for benzotriazole discrete molecules, of around 10 -2 cm 2 V -1 s -1 .

Objective: This study aimed to determine the ability of axillary ultrasonography to predict the number of lymph nodes with metastases found in sentinel node axillary surgery or axillary lymph node resection in patients recently diagnosed with breast cancer after percutaneous biopsy., Methods: This prospective study included patients diagnosed with invasive breast cancer by percutaneous biopsy. Axillary lymph nodes were classified at ultrasound examination as suspicious or not suspicious, and the number of suspicious nodes was compared with the number classified as suspicious in the surgical specimen., Results: We included 142 patients, 4 of whom had bilateral cancer; 133 of the 146 tumors were clinically classified as T1-T2 N0. The median number of suspicious lymph nodes at ultrasound was 2 (1-6), and the median number of suspicious lymph nodes in the surgical biopsy specimen was 1 (1-16); the difference was not significant (p=0.1). The correlation between the number of positive lymph nodes on axillary ultrasound and the number of metastatic lymph nodes in the surgical specimen was 72.7% p=0.0002 and the concordance was 79% (95%CI 62.4%-95.6%) p=0.0001. For diagnosing high axillary tumor load (≥3 metastatic lymph nodes) versus low axillary tumor load (<3 metastatic lymph nodes), axillary ultrasound had 86.6% sensitivity, 83.3% specificity, 92% PPV, and 71.4% NPV., Conclusion: Our results show that preoperative axillary ultrasound can differentiate between low and high tumor load and can be used as a tool to select the type of treatment. A series of donor-π-acceptor-π-donor (D-π-A-π-D) benzoazole dyes with 2H-benzo[d][1,2,3]triazole or BTD cores have been prepared and their photophysical properties characterized. The properties of these compounds display remarkable differences, mainly as a result of the electron-donor substituent. Dyes with the best properties have visible-light absorption over λ=400 nm, large Stokes shifts in the range of about 3500-6400 cm -1 , and good fluorescence emission with quantum yields of up to 0.78. The two-photon absorption properties were also studied to establish the relationship between structure and properties in the different compounds synthesized. These results provided cross sections of up to 1500 GM, with a predominance of S 2 ←S 0 transitions and a high charge-transfer character.
