Physicians completed questionnaires for their next five consecutively consulting patients with NASH or suspected NASH. Analyses were descriptive.Results: Seventy-six physicians provided data on 380 patients. The mean age was 58.5 ± 11.1 years and the mean body mass index was 31.8 ± 5.5 kg/m2. A total of 231 patients (61%) had no/non-advanced fibrosis as evaluated by liver biopsy or non-invasive tests. Common diagnostic assessments were cholesterol, hemoglobin A1c, absence of viral hepatitis, and alcohol assessment. At diagnosis, 87% (n=322/372) and 45% (n=169/372) of patients received an ultrasound and liver biopsy, respectively. Overall, 88% of patients were referred from primary to secondary care. Obesity (81%) and type 2 diabetes (62%) were the most commonly recorded comorbidities, with 70% of patients having ≥ 3 comorbidities. Vitamin E (13%) and GLP-1 receptor agonists (13%) were the most prescribed guideline-recommended treatments for all patients.Conclusion: Patients with NASH in Italy had high levels of obesity and comorbidities, while diagnosis and treatment frequently were not according to guidelines. Our data show an unmet need for more targeted diagnosis and treatment in Italian patients with NASH, in order to optimize outcomes.Plain language summary: Fat buildup in the liver, known as fatty liver disease, affects around 4-6% of people in Italy, and can lead to complications if left untreated. However, little is known about how doctors manage people with this disease. Fatty liver disease can only be diagnosed by a liver biopsy, but we found this is only performed in 45% of people, making accurate diagnosis difficult. We also found that 28% of people receive a recommended medication. Overall, increasing doctor awareness of fatty liver medical guidelines may help improve the diagnosis and treatment for people living with fatty liver disease in Italy.Keywords: NASH diagnosis, NASH treatment, NASH resource utilization

Abstract

Although onset/exacerbation of bullous Pemphigoid (BP) has been reported to occur frequently following exposure to the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), the link, if any, between BP dermatoses and the viral infection remains obscure. Therefore, searching for possible molecular mechanisms, we hypothesise that molecular mimicry between BP antigens and the SARS-CoV-2 proteins might lead to autoimmune responses cross-reacting with the BP proteins, thus triggering the dermatosis pathologies. Using this research paradigm, we analyzed the Bullous Pemphigoid antigen 1 (BP230) and the SARS-CoV-2 proteome to share minimal immune determinants, i.e., pentapeptides. Results indicate a high level of molecular mimicry between BP230 and SARS-CoV-2, thus supporting the hypothesis of cross-reactivity as a possible major mechanism in the SARS-CoV-2-associated BP etiopathogenesis.

Abstract

OBJECTIVE: Pemphigus Vulgaris (PV) is a mucocutaneous autoimmune disease mediated by autoantibodies that often affects oral mucosa. However, little is known about the connection between oral infections and PV. The present study addresses the hypothesis that immune responses following bacterial infections may cross-react with PV autoantigens, thus providing the pathogenic stimulus, eventually leading to clinical manifestations.METHODS: Available proteomic resources and immunologic data were explored. Searching for common peptides that underlie immune cross-reactions, the analyses focused on pathogenic oral bacteria and PV autoantigens, i.e., Desmoglein-3 (Dsg3).RESULTS: It was found that the analysed bacteria share numerous immunoreactive heptapeptide sequences with Dsg3.CONCLUSION: These data seem to support the hypothesis that the oral microbiome may also contribute to the pathogenesis of PV, with important implications for the treatment of this disease.

Abstract

Cytochrome P450 CYP1A1 and CYP1B1 enzymes are regulated by the aryl hydrocarbon receptor (AhR), a transcription factor activated by a variety of ligands among which Malassezia metabolites. In this study, we analyzed the modulation of CYP1A1, CYP1B1, and AhR in human keratinocytes infected with different strains of Malassezia pachydermatis, as well as the upregulation of some genes involved in the epidermal homeostasis. We demonstrated that all the strains induced AhR activation and its nuclear translocation in HaCaT cells infected for 24 h, compared to untreated cells. The expression of CYP1A1 and CYP1B1, prototypical markers of the AhR signaling pathway, were upregulated with the level of CYP1A1 mRNA approximately 100-fold greater than that for CYP1B1. Filaggrin, involucrin, and TGaseI, proteins involved in epidermal differentiation, were all modulated by Malassezia pachydermatis strains, with the strongest induction observed for filaggrin. By contrast, quinone oxidoreductase 1 (NQO1), which is part of the antioxidant defense system involved in detoxification, was not modulated in our experimental model. In conclusions, our findings suggest that Malassezia pachydermatis infection of human keratinocytes induces activation of the AhR, and increases the expression of its responsive genes and markers of epidermal differentiation, paving the way for occurrence/exacerbation of pathological skin conditions.

Abstract

Background The best treatment option for patients with type 2 diabetes in whom treatment with metformin alone fails to achieve adequate glycaemic control is debated. We aimed to compare the long-term effects of pioglitazone versus sulfonylureas, given in addition to metformin, on cardiovascular events in patients with type 2 diabetes. Methods TOSCA.IT was a multicentre, randomised, pragmatic clinical trial, in which patients aged 50–75 years with type 2 diabetes inadequately controlled with metformin monotherapy (2–3 g per day) were recruited from 57 diabetes clinics in Italy. Patients were randomly assigned (1:1), by permuted blocks randomisation (block size 10), stratified by site and previous cardiovascular events, to add-on pioglitazone (15–45 mg) or a sulfonylurea (5–15 mg glibenclamide, 2–6 mg glimepiride, or 30–120 mg gliclazide, in accordance with local practice). The trial was unblinded, but event adjudicators were unaware of treatment assignment. The primary outcome, assessed with a Cox proportional-hazards model, was a composite of first occurrence of all-cause death, non-fatal myocardial infarction, non-fatal stroke, or urgent coronary revascularisation, assessed in the modified intention-to-treat population (all randomly assigned participants with baseline data available and without any protocol violations in relation to inclusion or exclusion criteria). This study is registered with ClinicalTrials.gov, number NCT00700856. Findings Between Sept 18, 2008, and Jan 15, 2014, 3028 patients were randomly assigned and included in the analyses. 1535 were assigned to pioglitazone and 1493 to sulfonylureas (glibenclamide 24 [2%], glimepiride 723 [48%], gliclazide 745 [50%]). At baseline, 335 (11%) participants had a previous cardiovascular event. The study was stopped early on the basis of a futility analysis after a median follow-up of 57·3 months. Abstract

Malassezia pachydermatis is part of the normal cutaneous microbiota of most warm-blooded vertebrates and is associated with otitis externa and seborrhoeic dermatitis in dogs and cats. In this study, we evaluated the growth capacity of nine M. pachydermatis strains on Sabouraud medium in the presence of a high concentration of gentamicin. Strains of M. pachydermatis cultured on Sabouraud medium in the presence of 50 and 100 µg gentamicin ml(-1) displayed different growth patterns such as growth or lack of growth. We hypothesized that this difference in growth of M. pachydermatis strains was correlated with the different genotypes of the strains. Random amplification of polymorphic DNA (RAPD) was applied for genetic typing of M. pachydermatis isolates, derived from the external ears of house pet cats suffering from otitis externa. The M. pachydermatis strains were cultured on commercial or home-made Sabouraud medium supplemented or not with gentamicin. RAPD analysis demonstrated a genetic heterogeneity between each strain. In particular, five out of nine strains tested were able to form colonies in the presence of gentamicin. However, a correlation between M. pachydermatis genotype and growth capacity in the presence of gentamicin was not widely demonstrated.

Abstract

The fusion of virus and endosome membranes is an essential early stage in influenza virus infection. The low pH-induced conformational change which promotes the fusogenic activity of the haemagglutinin (HA) is thus an attractive target as an antiviral strategy. The anti-influenza drug Arbidol is representative of a class of antivirals which inhibits HA-mediated membrane fusion by increasing the acid stability of the HA. In this study two series of indole derivatives structurally related to Arbidol were designed and synthesized to further probe the foundation of its antiviral activity and develop the basis for a structure-activity relationship (SAR). Ethyl 5-(hydroxymethyl)-1-methyl-2-(phenysulphanylmethyl)-1. H-indole-3-carboxylate (15) was identified as one of the most potent inhibitors and more potent than Arbidol against certain subtypes of influenza A viruses. In particular, 15 exhibited a much greater affinity and preference for binding group 2 than group 1 HAs, and exerted a greater stabilising effect, in contrast to Arbidol. The results provide the basis for more detailed SAR studies of Arbidol binding to HA; however, the greater affinity for binding HA was not reflected in a comparable increase in antiviral activity of 15, apparently reflecting the complex nature of the antiviral activity of Arbidol and its derivatives.

Introduction: Infection with Helicobacter pylori (H. pylori) evokes inflammatory and immune responses of the host, which most likely determine the clinical outcome of H. pylori infection. In the gastric epithelium, expression of human alpha-defensins, beta-defensins (hBD)-1, -2 and -3 has been detected in vivo. In particularly hBD3 as well hBD2 is known to be induced in gastric epithelial cells infection by the H. pylori and may be involved in the pathogenesis of H pylori-associated gastritis, possibly through its function as immune and inflammatory mediator. However, during prolonged infection, hBD3 was subsequently downregulated by the H. pylori virulence determinant CagA. Materials and Methods: Patients. In this study, serum and biopsies of 33 healthy individuals stratified according to the H. pylori infection (17 positive and 11 negative) was analyzed. Elisa Assay and Real Time PCR. D’Alessandro”, Università degli Studi di Palermo - Italy Introduction: HBV is characterized by a high genetic variability and several studies have demonstrated the correlation between viral variants and severe forms of acute and chronic liver disease. Few data are available about the impact of genetic variability on the evolution of acute infection. Recently, 5 SNPs (C504, C801, G1171, T1785 and A1786) into HBV genome were reported as potential novel markers linked to a lower rate of chronicity. The aim of this study was to analyze the genetic variability of HBV genotypes A and D isolates from cases of self-limiting acute (AHB) and chronic hepatitis (CHB) in order to identify HBV variants associated with chronicity or resolution of infection. Patients and Methods: The preS-S, preC-C and the overlapped P and X genes, were sequenced from sera of 33 AHB and 46 CHB Sicilian patients and analyzed with 92 HBV-A and D sequences from Gene Bank in order to identify HBV geno/subtype by phylogenetic analysis and to study the genetic variability. Results: Eighteen strains of HBV-D and 15 of HBV-A from AHB, 42 of HBV-D and 4 of HBVA2 from CHB patients were obtained. Two in frame deletions, between preS1 and preS2 regions (Δ173-321, Δ1-29) and in the S gene (Δ489-516) were detected in an AHB-D3 strain while 2 CHB-D1 strains showed the deletions Δ37-52 and Δ36-57 into preS2 region. The mutation M01I at preS2 start codon was detected in 2 AHB-D1 and a CHB-D1strains. Mutations of immune-escape were not found but >30% of strains showed many amino acid polymorphisms in “a determinant” domain. Conclusion: Although it has been demonstrated that H.pylori infection was associated with significant upregulation of hBD-2 and hBD-3 transcript levels in gastric mucosa, our data suggest that no similar association was found on protein level in serum of patients. In addition to, the upregulation of hBD-2 and hBD-3 transcript levels is absent in gastric biopsies of some H. pylori- positive subjects. These results suggest that, most likely, H. pylori strains belonging to a particular genotype not only inhibit pathogen-derived hBD-2 and hBD- 3 induction, but also block their constitutive expression to escape from the strong antimicrobial effects.

Abstract

Whether the traditional treatment of chronic kidney disease (CKD)-mineral and bone disorder is effective in the setting of tertiary nephrology care is an unexplored question. We evaluated phosphate, calcium and PTH levels during the first year of nephrology care and the prognostic role of month-12 levels in non-dialysis patients referred prior to availability of the novel P binders. We studied a historical cohort of consecutive patients with CKD stage 3-5 at referral (baseline), and after 6 and 12 months of nephrology care; thereafter, patients were followed for renal survival (time to death or end-stage renal disease). At month 12, versus baseline, we detected a larger implementation of dietary protein restriction (P = 0.001), vitamin D and P binder (P < 0.0001 for both). Mean serum P remained unchanged (4.02 +/- A 0.77, 4.01 +/- A 0.79, 4.10 +/- A 0.85 mg/dL at baseline, month 6 and 12, respectively) with only 18, 16 and 21 % patients showing uncontrolled serum P at the three study visits. Similarly, calcium levels were unchanged and within the target in most cases. Conversely, intact PTH increased from 102 pg/mL (interquartile range 67-139) to 113 (68-179), P = 0.015, with 59, 60 and 53 % patients showing high values at the three study visits. During the subsequent follow-up (31 months), 96 renal deaths occurred. Cox analysis evidenced a significant prognostic role of the interaction P x PTH (P = 0.002), that is, the risk of renal death associated with serum P increased in the presence of higher PTH. In patients under nephrology care, P and PTH should be considered in concert to optimize risk stratification for renal death.

Abstract

Background and aims Metformin is the first-line therapy in type 2 diabetes. In patients inadequately controlled with metformin, the addition of a sulfonylurea or pioglitazone are equally plausible options to improve glycemic control. However, these drugs have profound differences in their mechanism of action, side effects, and impact on cardiovascular risk factors. A formal comparison of these two therapies in terms of cardiovascular morbidity and mortality is lacking. The TOSCA.IT study was designed to explore the effects of adding pioglitazone or a sulfonylurea on cardiovascular events in type 2 diabetic patients inadequately controlled with metformin. Methods Multicentre, randomized, open label, parallel group trial of 48 month duration. Type 2 diabetic subjects, 50–75 years, BMI 20–45 Kg/m 2 , on secondary failure to metformin monotherapy will be randomized to add-on a sulfonylurea or pioglitazone. The primary efficacy outcome is a composite endpoint of all-cause mortality, nonfatal myocardial infarction, nonfatal stroke, and unplanned coronary revascularization. Principal secondary outcome is a composite ischemic endpoint of sudden death, fatal and non-fatal myocardial infarction and stroke, endovascular or surgical intervention on the coronary, leg or carotid arteries, major amputations. Side effects, quality of life and economic costs will also be evaluated. Efficacy, safety, tolerability, and study conduct will be monitored by an independent Data Safety Monitoring Board. End points will be adjudicated by an independent external committee. Conclusions TOSCA.IT is the first on-going study investigating the head-to-head comparison of adding a sulfonylurea or pioglitazone to existing metformin treatment in terms of hard cardiovascular outcomes. Abstract

The metabolic fate of dietary anthocyanins (ACN) has not been fully clarified in humans. In all previous studies, the proportion of total ACN absorbed and excreted in urine was,1% intake. This study aimed to elucidate the human metabolism of cyanidin-glucosides (CyG) contained in blood orange juice (BOJ). One liter of BOJ, containing 71 mg CyG, was consumed by 6 healthy, fasting volunteers. Blood, urine, and fecal samples were collected at baseline and at different times up to 24 h after juice consumption. The content of native CyG, glucuronidated/methylated derivatives, and various phenolic acids was determined by HPLC/MS/MS. The serum maximal concentration of cyanidin-3-glucoside (Cy-3-glc) was 1.9 6 0.6 nmol/L and that of protocatechuic acid (PCA) was 492 6 62 nmol/L at 0.5 h and 2 h after juice consumption, respectively. The calculated total amounts in plasma corresponded for Cy-3-glc to 0.02% and for PCA to 44% of CyG ingested. CyG and glucuronidated/methylated metabolites, but not PCA, were detected in urine. ACN recovered in 24-h urine collections represented ;1.2% of the ingested dose. Both CyG (1.90 6 0.04 nmol/g) and PCA (277 6 0.2 nmol/g) were recovered in 24-h fecal samples. Data explained the metabolic fate of 74% of BOJ ACN. PCA was for the first time, to our knowledge, identified in humans as a CyG metabolite, accounting for almost 73% of ingested CyG. A high concentration of PCA may explain the short-term increased plasma antioxidant activity observed after intake of cyanidin-rich food and it can also contribute to the numerous health benefits attributed to dietary ACN consumption.

Abstract

Bovine lactoferrin catalyzes the hydrolysis of synthetic substrates (i.e., Z-aminoacyl-7-amido-4-methylcoumarin). Values of K-m and k(cat) for the bovine lactoferrin catalyzed hydrolysis of Z-Phe-Arg-7-amido-4-methylcoumarin are 50 muM and 0.03 s(-1), respectively, the optimum pH value is 7.5 at 25 degreesC. The bovine lactoferrin substrate specificity is similar to that of trypsin, while the hydrolysis rate is several orders of magnitude lower than that of trypsin. The bovine lactoferrin catalytic activity is irreversibly inhibited by the serine-protease inhibitors PMSF and Pefabloc. Moreover, both iron-saturation of the protein and LPS addition strongly inhibit the bovine lactoferrin activity. Interestingly, bovine lactoferrin undergoes partial auto-proteolytic cleavage at positions Arg415-Lys 416 and Lys440-Lys441.pK(a) shift calculations indicate that several Ser residues of bovine lactoferrin display the high nucleophilicity required to potentially catalyze substrate cleavage. However, a definitive identification of the active site awaits further studies.

Abstract

In this study the validity of the identification method of L. monocytogenes based on colony hybridization has been demonstrated. Similar results were obtained either with radioactively or nonradioactively labeled DNA probes, represented by the hly and inlA virulence-associated genes. The experimental procedure allows to discriminate L. monocytogenes among Listeria spp. and other Gram-positive bacteria able to grow on Listeria Selective Agar Base. This technique results highly specific and could be applied to detect L. monocytogenes in food samples.
