Abstract

Abstract Tourette syndrome (TS) is a neuropsychiatric disorder of complex genetic architecture involving multiple interacting genes. Here, we sought to elucidate the pathways that underlie the neurobiology of the disorder through genome-wide analysis. We analyzed genome-wide genotypic data of 3581 individuals with TS and 7682 ancestry-matched controls and investigated associations of TS with sets of genes that are expressed in particular cell types and operate in specific neuronal and glial functions. We employed a self-contained, set-based association method (SBA) as well as a competitive gene set method (MAGMA) using individual-level genotype data to perform a comprehensive investigation of the biological background of TS. Our SBA analysis identified three significant gene sets after Bonferroni correction, implicating ligand-gated ion channel signaling, lymphocytic, and cell adhesion and transsynaptic signaling processes. MAGMA analysis further supported the involvement of the cell adhesion and trans-synaptic signaling gene set. The lymphocytic gene set was driven by variants in FLT3, raising an intriguing hypothesis for the involvement of a neuroinflammatory element in TS pathogenesis. The indications of involvement of ligand-gated ion channel signaling reinforce the role of GABA in TS, while the association of cell adhesion and trans-synaptic signaling gene set provides additional support for the role of adhesion molecules in neuropsychiatric disorders. This study reinforces previous findings but also provides new insights into the neurobiology of TS.

Abstract

Introduction.Acute mesenteric ischemia is a surgical emergency that entails complex, multi-modal management, but its epidemiology and outcomes remain poorly defined. The aim of this study was to perform a population analysis of the contemporary incidence and outcomes of mesenteric ischemia.Methods.This was a retrospective analysis of acute mesenteric ischemia in the state of Maryland during 2009 – 2013 using a comprehensive statewide hospital admission database. Demographics, illness severity, comorbidities, and outcomes were studied. The primary outcome was inpatient mortality. Survivors and non-survivors were compared using univariate analyses, and multivariable logistic regression analysis was performed to evaluate risk factors for mortality.Results.During the 5-year study period, there were 3,157,499 adult hospital admissions in Maryland. 2,255 patients (0.07%) had acute mesenteric ischemia, yielding an annual admission rate of 10/100,000. Increasing age, hypercoagulability, cardiac dysrhythmia, renal insufficiency, increasing illness severity, and tertiary hospital admission were associated with development of mesenteric ischemia. Inpatient mortality was high (24%). After multivariate analysis, independent risk factors for death were age > 65 years, critical illness severity, mechanical ventilation, tertiary hospital admission, hypercoagulability, renal insufficiency, and dysrhythmia.Conclusions.Acute mesenteric ischemia occurs in approximately 1/1000 admissions in Maryland. Patients with mesenteric ischemia have significant illness severity, substantial rates of organ dysfunction, and high mortality. Patients with chronic comorbidities and acute organ dysfunction are at increased risk of death, and recognition of these risk factors may enable prevention or earlier control of mesenteric ischemia in high-risk patients.

Introduction.Vascular surgery patients have multiple risk factors for renal dysfunction, but acute kidney injury (AKI) is poorly studied in this group. The purpose of this study was to define the incidence, risk factors and outcomes of AKI in high risk vascular patients. Methods.Critically ill vascular surgery patients admitted during January – December 2012 were retrospectively analyzed with 1-yearfollow-up. The endpoint was AKI by established RIFLE creatinine criteria. The primary analysis was between patients with or without AKI, with secondary analysis of postoperative AKI. Outcomes were inpatient and 1-year mortality, inpatient lengths of stay, and discharge renal function. Results.136 vascular surgery patients were included, representing 27% of all vascular surgery admissions during the study period. 65 (48%) developed AKI. Independent global risk factors for AKI were diabetes, increasing critical illness severity, and sepsis. Whileintraoperative blood loss and hypotension were associated with subsequent renal dysfunction, postoperative AKI rates were similar for patients undergoing aortic, carotid, endovascular, or peripheral vascular procedures, All RIFLE grades of AKI were associated with worse outcomes. Overall, patients with AKI had significantly increased short and long-term mortality, longerinpatient lengths of stay, and worse discharge renal function. Conclusions.AKI is common among critically ill vascular surgery patients. Importantly, the type of surgical procedure appears to be lessimportant than intra- and perioperative management in determining renal dysfunction. Regardless of its severity, AKI is a clinically significant complication that is associated with substantially worse patient outcomes.

6. Reduced dominant motor cortex inhibition is associated with increased tic severity in children with Tourette syndrome

Abstract

Aim: Compare transcranial magnetic stimulation (TMS)-derived measures of motor cortex (M1) physiology between children with and without Tourette syndrome (TS) and dimensionally analyze TMS measures with TS-related tic and urge symptom severity.Method: 60 8-12-year-old children (30 TS, 3 girls, 10 years, 10 months [1 year, 3 months]; 30 typically developing children - TDC, 7 girls, 10 years, 7 months [1 year, 3 months]) participated. Within TS, 15 children (1 girl, 10 years, 11 months [1 year, 3 months]) had comorbid Attention-Deficit/Hyperactivity Disorder (ADHD), rated with the Conners 3rd Edition and the parent-reported ADHD rating scale. Tic severity was rated with the Yale Global Tic Severity Scale (YGTSS), and premonitory urge severity with the Individualized Premonitory Urge for Tics Scale (I-PUTS) for children with TS. M1 short interval cortical inhibition (SICI) and intracortical facilitation (ICF) were compared between diagnostic groups and, within TS, correlated with symptom severity using repeated measures regressions.Results: Accounting for ADHD, we found no difference in SICI or ICF in TS vs. TDC (p>.1). Within TS, reduced M1 SICI predicted greater total (p=.012) and global (p=.002) tic severity. There were no associations with urge severity (p>.5). Interpretation: Reduced M1 SICI is robustly associated with increased tic, but not urge, severity.

Abstract

Background. Individuals with Tourette syndrome (TS) often report that they express tics as a means of alleviating the experience of unpleasant sensations. These sensations are perceived as an urge to act and are referred to as premonitory urges. Premonitory urges have been the focus of recent efforts to develop interventions to reduce tic expression in those with TS. Increasing evidence points to the role of both structural and functional alterations of prefrontal and limbic brain regions regarding the experience of premonitory urges to tic in TS. This study examined the contribution of brain GABA and glutamate levels of the right primary sensorimotor cortex (SM1), supplementary motor area (SMA), and insular cortex (insula) to tic and urge severity in children with TS.Methods. Edited magnetic resonance spectroscopy was used to assess GABA+ (GABA + macromolecules) and Glx (glutamate + glutamine) of the right SM1, SMA and insula in 68 children with TS (MAge = 10.59, SDAge = 1.33) and 41 typically developing controls (MAge = 10.26, SDAge = 2.21). We first compared GABA+ and Glx levels of these brain regions between groups. We then explored the association between regional GABA+ and Glx levels with urge and tic severity. Results. GABA+ and Glx of the right SM1, SMA and insula were comparable between the children with TS and typically developing controls. In children with TS, lower levels of SMA GABA+ was associated with more severe and more frequent premonitory urges. Neither GABA+ nor Glx levels were associated with tic severity. Conclusions. These results broadly support the role of GABAergic neurotransmission within the SMA in the experience of premonitory urges in children with TS.

12. The Poetics of Friendship in Early Modern Spain

Abstract

This book posits the Aristotelian-Ciceronian notion of perfect male friendship as an independent poetic force within the development of Spanish literature in the sixteenth and early seventeenth centuries. Through a re-examination of Spanish critic Juan Bautista Avalle-Arce’s notion of the “tale of two friends” tradition, the book shows how the poetics of friendship evolves in relation to other key concepts from the period—most notably exemplarity and imitatio—in a series of carefully selected examples from several important genres including the pastoral novel, the picaresque, and the Spanish comedia. Particular attention is given to the trajectory whereby the highly formalized narrativization of the traditional Aristotelian paradigm for friendship gives way to representations of personal intimacy grounded in a recognition of the idiosyncratic particularity of human experience in the world beyond the text. This alternative modality for representing friendship, which encompasses a variety of relationships beyond the Aristotelian paradigm—between women, erstwhile lovers, and pícaros, to take just three examples—reaches its fullest expression in the depiction of the evolving intimacy that grows up between the two unlikely companions, Don Quixote and Sancho Panza, whose shared experiences provide the main focus for Cervantes’s most important work.

Abstract

María de Zayas’s La traición en la amistad stands out for its representation of female friendship and for its development of a new archetype within the tradition of writing friendship: the figure of the “bad” friend. Through an innovative use of theatrical hybridity, María de Zayas contrasts the false hyperbole of Fenisa, the bad friend, with the quiet intimacy that grows up around her rivals, Laura and Marcia. Through an inversion of traditional gender roles, the figure of Fenisa emerges as a female heir to Tirso de Molina’s Don Juan who is finally condemned less for her irrepressible desire than for her failure as a friend. Her lack of solidarity with the other women in the play functions as a point of contrast to a clearly gendered representation of good female friendship that is closely associated with the rhetoric of verisimilitude and intimacy.

Abstract

This introductory chapter traces the origin of the poetics of friendship to Aristotle’s Nicomachean Ethics and, by extension, Cicero’s De amicitia. Aristotle’s definition of the perfect friend in static categorical terms presents significant challenges to narrativization, that is, to a mode of literary expression based in conflict and change. Following Ullrich Langer, the chapter explores how Aristotle’s largely static categorical framework is filtered through the discourse of spiritual friendship in the Middle Ages, exposing the key problem of the perfect friend’s ultimate unknowability. Alternatively, following Cicero’s more fluid dialogical exploration of the problem of friendship, the chapter traces a second trajectory for a poetics of friendship that draws on Kathy Eden and Nancy Struever’s work on the more recognizably modern notion of intimacy. Taken together, these two genealogies provide the foundation for an evolving poetics of friendship in the narrative prose and dramatic works of early modern Spain.

In an essential rejection of the Aristotelian model for perfect friendship, the intensely detailed account of the relationship between Don Quixote and Sancho Panza inaugurates a radically new approach to writing narrative friendship. Freed from the determinism of the traditional tale of two friends, the introduction of Sancho Panza into the narrative drives a new poetics of friendship in which hyperbolic genuflecting before the Aristotelian ideal gives way to an interest in verisimilitude now understood as the act of representing the world beyond the text. More specifically, Cervantes’s novel proposes a vision of narrative friendship that is radically anti-exemplary, serving not as the model for some universal ideal, but rather as an expression of the idiosyncratic inimitability—both practical and theoretical—of human relationships in the world.

Avalle-Arce argues that the interpolated story from Don Quixote, “El curioso impertinente,” marks “the final step in the development of the tale of two friends and, at the same time, its destruction.” Here the test of perfect friendship, rather than affirm the Aristotelian ideal, exposes its insufficiency through a series of betrayals that leads to the final deaths of both friends and their contested lover. Not satisfied with merely critiquing the tradition of writing perfect friendship, Cervantes reveals the essential vacuity of the Aristotelian ideal, the fact, indeed, of its utter incompatibility with life in the world beyond the text. That this final destruction occurs in the midst of a radical reconceptualization of narrative friendship—the novel Don Quixote, the subject of the last chapter of this book—underscores the formal significance of Cervantes’s powerful repudiation of the traditional tale of two friends.

Guillén de Castro’s dramatic rewriting of “El curioso impertinente” provides a unique window onto the formal differences between prose narrative and theatrical approaches to the representation of perfect friendship. Key to the analysis of this chapter is the inherent hybridity of the Spanish early modern comedia, the tendency within one and the same work to engage in hyperbolic displays of emotional excess alongside more muted representations of feeling that feed an emerging interest in verisimilitude, especially as the basis for a new kind of audience identification within the theater. In practice, Castro´s play dramatizes the stark contrast between the exaggerated discourse of perfect friendship and other modes of personal intimacy not so much as a mode of critique of the Aristotelian ideal, but rather as two strategies that function simultaneously within the same work of dramatic literature.

Abstract

Culture of the Baroque offers José Antonio Maravall's most comprehensive vision of the baroque in Spain as a historical phenomenon that encompasses virtually all aspects of seventeenth-century social and cultural life. Maravall's study reappraises the conventional view of the baroque as the privileged locus of Spanish literary and historical production by appealing to the secularized post-Hegelianism of Jacob Burckhardt and Heinrich Wölfflin. In particular, Maravall's analysis draws on two related elements in the post-Hegelian approach to early modern historiography that contemporary work tends to ignore or even contradict: a nominalist view of the baroque that rejects the transcendent claims of Hegelian historiography and an explicit embrace of subjectivism as the necessary condition of historical scholarship. Maravall's adherence to these foundational principles of post-Hegelian historiography illuminates the original stakes of early modern periodization and their radical deformation over time. In the process, his work not only facilitates a reassessment of many of the conventional claims made for the Spanish baroque but, more important, establishes a theoretical perspective from which to evaluate contemporary scholarship on the baroque as a historical and aesthetic concept.

Este articulo examina el importante papel que juega la amistad en La Diana (1559) de Jorge de Montemayor como respuesta a las hiperbolicas muestras de amor que se encuentran a lo largo de la novela. A diferencia de las exageradas declaraciones de los amantes en el texto, la amistad tipicamente se caracteriza por muestras de simpatia sin pretensiones entre los personajes. Como consecuencia, la amistad proporciona un punto de referencia desde el cual se puede juzgar el amor en la novela, y asi poner en tela de juicio el significado de las quejas amorosas. Al mismo tiempo, el articulo propone que la representacion del amor en La Diana constituye un ejemplo del nuevo discurso de la 'vida privada', tal y como se la presenta en la obra de Philippe Aries (1989). Desde esta optica, la amistad en la novela pastoril de Montemayor se puede entender como una innovacion literaria vinculada al ambito historico.

Abstract

IThere is an anecdote that is frequently recounted about the death of Enrique de Villena. Upon his demise in 1434, the King of Castile, Juan II, ordered that Villena's library be seized. The collection was taken to the King's quarters where the royal tutor, Fray Lope de Barrientos, was instructed to make an examination of the contents. Fray Lope, who was not especially sympathetic to Villena's varied intellectual pursuits, reports that he discovered no less than fifty volumes dealing with what he describes as the "malas artes" (Gascon Vera 317). After a consultation with the King, the offending books were summarily destroyed, burnt in the flames of a kind of posthumous auto de fe. The destruction of Villena's library led to an animated debate among the younger intellectuals of the period, a debate that, according to Gascon Vera, highlights Villena's status as a kind of Renaissance precursor: "Es evidente que los estudios de Villena, expresion de una libertad intelectual y de un eclecticismo cientifico propio del incipiente Renacimiento, servian de piedra de toque entre la mentalidad tradicional de Fernan Perez de Guzman y Lope de Barrientos, que los consideran sospechosos, y la nueva mentalidad experimental y critica de Mena y Santillana, que los ensalzan" (318).In fact, Gascon Vera's association of Villena with an "incipient Renaissance" forms part of a larger debate among contemporary scholars over don Enrique's historical significance, both in terms of his relationship to the Italian humanists of his own age, and to the next generation of Spanish humanists that would inherit his legacy. The contours of this debate are somewhat elastic, reflecting the various scholarly interests of the participants. Nonetheless, one detects throughout a recurring return to the question of Villena's place within the evolution of humanistic discourse in the fifteenth century. The question, as it turns out, is a complicated one, and the collective impression left by this body of scholarship would seem to be well captured by Pedro Catedra, who remarks, "No creo que la busqueda a to largo de la obra de Villena de incipiente Humanismo sea un fracaso" ("Enrique de Villena" 187). Catedra's wonderful understatement of the case for Villena's humanist sympathies seems to exude precisely the kind of ambiguity that belies macrohistorical narratives that would conveniently define boundaries between the various "moments" of history. And as for Villena himself, it suggests the difficulties of any overly general assessment of his work as somehow prefiguring an emerging Renaissance in Spain.In the spirit of the more nuanced approaches to Villena of Catedra and others, this essay will undertake a historically sensitive reading of the Prohemio, or introduction, to what is arguably Villena's most important literary work, his vernacular translation of Vergil's Aeneid. Commissioned by Juan II, the King of Navarre, in 1427, the project's genesis reflects both the limited competence in Latin of the Castilian aristocracy and an emerging belief in the value of a mode of classical learning designed to address the needs of the ruling elite (Catedra, Introduccion xv-xvi). Indeed, against the traditional view of the Spanish aristocracy as essentially hostile to literary pursuits, the King's commission to Villena underlines the commitment to classical learning of various figures associated with Juan II's court. Thus, in this same context, one discovers the Epistula ad Comitem de Haro (c. 1440) in which Alfonso de Cartagena advocates for a kind of learning appropriate for the "militares viri," that is, precisely for those men of state, like Juan II, who combined "las vidas activa y contemplativa" (Lawrance, Un tratado 10, 12). Similarly, Villena's vernacular translation of the Aeneid attempted to make Vergil's work available in a form that might be appropriate for an aristocracy that did not possess either the language skills or the background necessary to understand the text in its original form. …

Abstract

This thesis proposes the use of plenoptic cameras for improving the robustness and simplicity of machine vision in field robotics applications. Dust, rain, fog, snow, murky water and insufficient light can cause even the most sophisticated vision systems to fail. Plenoptic cameras offer an appealing alternative to conventional imagery by gathering significantly more light over a wider depth of field, and capturing a rich 4D light field structure that encodes textural and geometric information. The key contributions of this work lie in exploring the properties of plenoptic signals and developing algorithms for exploiting them. It lays the groundwork for the deployment of plenoptic cameras in field robotics by establishing a decoding, calibration and rectification scheme appropriate to compact, lenslet-based devices. Next, the frequency-domain shape of plenoptic signals is elaborated and exploited by constructing a filter which focuses over a wide depth of field rather than at a single depth. This filter is shown to reject noise, improving contrast in low light and through attenuating media, while mitigating occluders such as snow, rain and underwater particulate matter. Next, a closed-form generalization of optical flow is presented which directly estimates camera motion from first-order derivatives. An elegant adaptation of this "plenoptic flow" to lenslet-based imagery is demonstrated, as well as a simple, additive method for rendering novel views. Finally, the isolation of dynamic elements from a static background is considered, a task complicated by the non-uniform apparent motion caused by a mobile camera. Two elegant closed-form solutions are presented dealing with monocular time-series and light field image pairs. This work emphasizes non-iterative, noise-tolerant, closed-form, linear methods with predictable and constant runtimes, making them suitable for real-time embedded implementation in field robotics applications.

45. Malignant melanoma arising from an end ileostomy.

Abstract

A 65-year-old woman with a history of proctocolectomy and end ileostomy for ulcerative colitis was referred to our clinic with a slowly growing mass around her ileostomy. She did not report any systemic symptoms. On examination, an exophytic mass was observed around her ileostomy and hard lymph nodes palpated in her groins bilaterally. Punch biopsy of the lesion established a diagnosis of invasive melanoma. Positron emission tomography revealed regional metastatic lymphadenopathy in the right axilla and both groins. There was no evidence of distant metastatic disease. The patient then underwent wide local excision of her ileostomy with bowel resection and ileostomy re-siting, bilateral complete ilioinguinal lymphadenectomy and a right Level III axillary dissection. She is doing well postoperatively and receiving adjuvant systemic therapy with BRAF and MEK inhibitors, now 17 months later with no signs of recurrent disease. [ABSTRACT FROM AUTHOR]
