- Abstract
Background: Anemia is a common complication of chronic kidney disease (CKD), but its incidence in nephrology settings is poorly investigated. Similarly, the risks of adverse outcomes associated with new-onset anemia are not known. Methods: We performed a pooled analysis of three observational cohort studies including 1031 non-anemic CKD patients with eGFR <60 mL/min/1.73 m2 regularly followed in renal clinics. We estimated the incidence of mild anemia (hemoglobin 11-12 g/dL in women and 11-13 g/dL in men) and severe anemia (hemoglobin <11 g/dL or use of erythropoiesis-stimulating agents) during a 3-year follow-up period. Thereafter we estimated the risk of end-stage kidney disease (ESKD) and all-cause death associated with new-onset mild and severe anemia. Results: The mean age was 63 ± 14 years, 60% were men and 20% had diabetes. The mean estimated glomerular filtration rate (eGFR) was 37 ± 13 mL/min/1.73 m2 and the median proteinuria was 0.4 g/day [interquartile range (IQR) 0.1-1.1]. The incidence of mild and severe anemia was 13.7/100 patients-year and 6.2/100 patients-year, respectively. Basal predictors of either mild or severe anemia were diabetes, lower hemoglobin, higher serum phosphate, eGFR <30 mL/min/1.73 m2 and proteinuria >0.50 g/day. Male sex, moderate CKD (eGFR 30-44 mL/min/1.73 m2) and moderate proteinuria (0.15-0.50 g/day) predicted only mild anemia. The incidence of anemia increased progressively with CKD stages (from 8.77 to 76.59/100 patients-year) and the proteinuria category (from 13.99 to 25.02/100 patients-year). During a median follow-up of 3.1 years, 232 patients reached ESKD and 135 died. Compared with non-anemic patients, mild anemia was associated with a higher adjusted risk of ESKD {hazard ratio [HR] 1.42 [95% confidence interval (CI) 1.02-1.98]} and all-cause death [HR 1.55 (95% CI 1.04-2.32)]. Severe anemia was associated with an even higher risk of ESKD [HR 1.73 (95% CI 1.20-2.51)] and death [HR 1.83 (95% CI 1.05-3.19)].

Il progetto Ancient Maritime Dynamics (AMD) promosso dall’Università di Koç di Istanbul, estende ai metodi dell’archeologia tradizionale i sistemi informativi geografici al fine di comprendere e modellare gli elementi del paesaggio marittimo del Mediterraneo nell’antichità. A partire dal 2016, questo progetto sta focalizzando i suoi sforzi sul tratto di costa tra Salerno e Punta Campanella, effettuando ricerche subacquee e terrestri per iniziare a decifrare attività marittime praticate nel passato in questa area e verificare ipotesi riguardanti la costruzione dello spazio marittimo mediterraneo. Questo studio è un resoconto dei risultati conseguiti sino ad oggi, oltre ad essere un’anticipazione dei progetti futuri.

The article analyzes morphodynamic aspects of dam-river-coast system and the related conflicts. New tools are required for the study of this system, with the aim of defining in the medium-short term river longitudinal zoning, concentration time evaluation and return time, catchment erosion rate, quantification of silting of reservoir, littoral drift and morho-evolutionary trend of littoral prism. The proposed systemic approach aims to select adaptive rather than maladaptive strategies, following a policy of forecast, prevention and protection. Ecological engineering techniques and environmental design are fundamental to mitigate risks, recover and remediate endangered transition environments, considering integrated interventions in dam-river-coast system based on resilience rather than resistance.

22. Geomorphological features of the archaeological marine area of Sinuessa in Campania, southern Italy

- Abstract
Submarine surveys carried out since the'90s along the coastland of Sinuessa allowed us to draw up a geomorphological map with archaeological findings. Along the sea bottom, 650 m off and −7 m depth, a Campanian Ignimbrite bedrock was detected: dated ∼39 kyr BP, its position is incompatible with the current sea level. Towards the northern edge of the shoal, a depressed area with 24 cubic elements in concrete was surveyed. These artefacts (pilae) are typical of Roman maritime structures widespread along the southernmost Phlegrean coast. Beachrocks and accessory landforms at the same depth of bedrock suggest that this was emerging and attended by man in Roman times, even for activities related to port facilities. Submerged palaeo-channels, in alignment of current watercourses on the mainland, dissect the shoal. These channels were moulded in subaerial environment during Würm glaciation, following the tuff deposition, and then were drowned by sea-level rise. The northernmost channel, next to the pilae, likely allowed transit and manoeuvring of Roman ships. The discovery along the seabed of a large stump of lead anchor, hundreds of Roman amphorae and fragments, as well as of a depression of millstone, confirm this finding. Probably the sinuous physiography favored the choice of this site for the docking of Sinuessa, as sheltered from storms.

This study was conducted within the framework of the “Coastal Morphodynamics” Working Group (WG) of the Italian Association of physical Geography and Geomorphology (AIGeo), according to the Institute for the Protection and Environmental Research (ISPRA) for the updating of the legend for the “Geomorphological Map of Italy”. The WG deals with the legend for the coastal areas, focusingits work on marine, lagoon and aeolian landforms, processes and deposits. In particular, the legend aims to classify coastal landforms in order to contribute to hazard and risk assessment,for supportingland-use planning and management. The legend allows the mapping of each landform in function of its genesis as well as its evolution and present dynamics, providing information about morphological characteristics at small and large scales. The relict morphological features and the active ones are reported along with the quantitative parameters useful for the description of the present wave/climate conditions and morphodynamics. As a result of the activities and experiments carried out by the “Coastal Morphodynamics” AIGeo WG during the last years, some examples of coastal geomorphological mappings at different scales (1:5,000 and 1:25,000) have been developed and are presented in this paper. The maps focus both on littoral plains and rocky coast dynamics as well as on the interactions with anthropic modifications.

Riassunto Il riconoscimento e l’interpretazione di elementi tettonici, litostratigrafici e geomorfologici, antichi o recenti, nonché di strutture geoarcheologiche in quanto geoindicatori, sono utili per ricostruire l’evoluzione dei vari morfotipi costieri, di cui si analizzano alcuni casi studio. I recenti studi dell’ambiente marino costiero della Campania hanno aggiornato il quadro geomorfologico e sedimentario. La suddivisione in principali ambienti (spiaggia emersa e sommersa, falesia e piede di falesia) ha permesso di raggruppare aree con processi simili, attivi o meno, e correlare le forme alle zone emerse. Va studiata anche la fascia dunare, poiché riveste un ruolo basilare nell’equilibrio ambientale del sistema di spiaggia. Gli effetti dell’erosione si riflettono pure sulla vegetazione, che mostra un forte stato di stress. I processi erosivi sono messi in relazione con le pressioni antropiche che hanno agito nel tempo in modo diretto o meno, innescando o accelerando fenomeni che riducono la resilienza del sistema naturale. Abstract Recognition and interpretation of tectonic, lithostratigraphic and geomorphic elements, ancient or recent, and geoarchaeological structures, represent useful geoindicators to reconstruct coastal dynamics. The recent coastal geological mapping of Campania provided a new reading of the coastland. In this way, the surveys update the whole geomorphologic and sedimentologic framework. Sea cliff and cliff base, emerged and submerged beach should be surveyed, with the aim to group waterscapes with same processes and correlate their shapes. Also the coastal dune should be studied, as it plays a key role in the balance of a littoral for its positive effect on beach system. In the last years, the activities of Working Groups (WGs), in the framework of Italian Association of Physical Geography and Geomorphology (AIGeo), were aimed at the new issue of the legend of the Italian Geomorphological Map. The activities were performed keeping in mind the necessity: 1) to add new symbols because the great progress made in geomorphology, in terms of new knowledge on geomorphological processes and landforms, after the first issue of the legend in 1994; 2) to have a legend usable with large scale maps, suitable for applied purposes; 3) to create a legend usable with softwares (GIS environment, etc). The WG on Coastal Morphodynamics contributed to this work, as well, promoting, through meetings and workshops, the debate among the Italian coastal geomorphologists on the approach needed to produce a modern geomorphological legend. The classic dichotomy forms-processes used in the geomorphological mapping is often based to static concepts, especially when applied to marine and coastal areas, affected by fast dynamic events, sometimes paroxysmal. We present the new geomorphological legend to represent both coastal and underwater contexts, aimed to the improvement of classic geomorphological legends. The main goal of the work was the production of a legend suitable to clarify the importance and the need of representing landforms in the context ofthe Quaternary evolution, as well as from more recent or modern processes.

Derangements of glucose metabolism are common among critically ill patients. Critical illness- associated hyperglycemia (CIAH) is characterized by raised blood glucose levels in association with an acute event that is reversible after resolution of the underlying disease. CIAH has many causes, such as changes in counter-regulatory hormone status, release of sepsis mediators, insulin resistance, drugs and nutritional factors. It is associated with increased mortality risk. This association appears to be strongly influenced by diabetes mellitus as a comorbidity, suggesting the need for an accurate individualization of glycemic targets according to baseline glycemic status. Hypoglycemia is also very common in this clinical context and it has a negative prognostic impact. Many studies based on intensive insulin treatment protocols targeting normal blood glucose values have in fact documented both an increased incidence of hypoglycemia and an increased mortality risk. Finally, glycemic control in the ICU is made even more complex in the presence of acute kidney injury. On one hand, there is in fact a reduction of both the renal clearance of insulin and of gluconeogenesis by the kidney. On the other hand, the frequent need for renal replacement therapy (dialysis / hemofiltration) may result in an energy intake excess, under the form of citrate, lactate and glucose in the dialysate/reinfusion fluids. With regard to the possible renal protective effects afforded by intensive glycemic control protocols, the presently available evidence does not support a reduction in the incidence of AKI and/or the need for RRT with this approach, when compared with standard glucose control. Thus, the most recent guidelines now suggest higher blood glucose targets (180 mg/dl or 140-180 mg/dl) than in the past (80-110 mg/dl). Albeit with limited evidence, it seems reasonable to extend these indications also to patients with AKI in the intensive care unit. Further studies are needed in order to better ascertain the effects of dysglycemia on the outcome of patients with AKI.

Geomorphological, stratigraphical and paleoecological studies carried out along the Vendicio coastal plain (southern Latium) allowed us to reconstruct the paleoenvironmental evolution of the plain in relation to Holocene palaeosea-level standings. On the basis of altitudes, texture, microfossil and pollen content of sediments recovered in a 20 m borehole realised on the backshore, three main transitional palaeoenvironments were recognised. Particularly, the 14C dating of a peat layer at the boring base, interbedded between sandy silt with organic matter and silty peat levels, gives an age of 7620 ±100 yr BP, equivalent to 8354-8524 cal yr BP. Over this deposits, sands with pebbles, sometimes alternated with silt and sandy silt levels, lay. Microfossil content, together with sediment faetures, confirms a transitional sequence from marsh (oligohaline) to lagoon (mesohaline-polyhaline), and finally to marine (littoral) environment, probably due to the last sea ingression related to the mid-Holocene Climatic Optimum peak (~6000 years BP). Pollen analysis reveals the existence of a deciduous forest association, rich in high-humidity demanding elements, by now not represented by a regional modern analogue. The present altitude of the dated peat palaeosea-level, correlated with the available eustatic curves, suggests that the Vendicio sedimentary succession probably has been affected by a slight subsidence phase during MIS 1. The occurrence of a slight subsidence may be also related to late Quaternary vertical displacement of adjacent coastal sectors, and fits with recent morphological evolutionary models of other minor coastal plains of Latium.

To analyze risk factors for type 2 diabetes among women attending menopause clinics in Italy for counselling about the menopause.Women attending a network of first-level outpatient menopause clinics in Italy for general counselling about menopause or treatment of menopausal symptoms.Cross-sectional study with no exclusion criteria. Type 2 diabetes was defined according to National Diabetes Data Groups Indications and the fasting blood glucose at an oral glucose tolerance test within the previous year.Out of the 44 694 considered in this analysis, 808 had a diagnosis of diabetes type 2 (1.8%). In comparison with women aged50 years, the multivariate odds ratios (OR) of type 2 diabetes were 1.31 (95% confidence interval (CI), 0.99-1.74) for women aged 50-52 years, 1.66 (95% CI, 1.27-2.17) at 53-56 years and 2.84 (95% CI, 2.20-3.67) in women agedor = 57 years. Type 2 diabetes was less frequently reported in more educated women (OR high school/university vs. primary school = 0.44 (95% CI, 0.36-0.55)). Being overweight was associated with an increased risk of type 2 diabetes. In comparison with women reporting a low level of physical activity, the multivariate OR of type 2 diabetes was 0.67 (95% CI, 0.54-0.84) for women reporting regular physical activity. In comparison with premenopausal women, the multivariate OR of type 2 diabetes was 1.38 (95% CI, 1.03-1.84) in women with natural menopause. This finding was present also after allowing for the potential confounding effect of age. The multivariate OR of diabetes for users of hormonal replacement therapy was 0.58 (95% CI, 0.46-0.73).This large cross-sectional study suggests that postmenopausal women are at higher risk of type 2 diabetes after allowance for the effect of age. Other main determinants of risk of type 2 diabetes in women around menopause were low socioeconomic status and being overweight. Diabetes was found less frequently in those taking hormone replacement therapy.

OBJECTIVE To obtain data on correlates of climacteric symptoms in women around menopause attending menopause clinics in Italy. METHODS Since 1997 a large cross sectional study has been conducted on the characteristics of women around menopause attending a network of first level menopause outpatient's clinics in Italy. A total of 66,501 (mean age 54.4 years) women are considered in the present paper. RESULTS The odds ratios of moderate and severe hot flashes/night sweats were lower in more educated women and (for severe symptoms only) in women reporting regular physical activity. Depression, difficulty to sleep, forgetfulness and irritability tended to be less frequent in more educated women and (depression only) in women reporting regular physical activity. Parous women reported more frequently these symptoms. CONCLUSIONS This large study confirms in Southern European population that low education, body mass index and low physical activity are associated with climacteric symptoms. Parous women are at greater risk of psychological symptoms.

La laguna di Sabàudia possiede una geometria a pettine, unica in Italia e forse in Europa, con sei bracci ortogonali alla linea di riva; si è impostata su un paesaggio ereditato e si è evoluta a partire dall'Olocene medio fino ad assumere l'attuale assetto fisiografico. Le indagini geomorfologiche condotte lungo le aree emerse e sommerse della laguna, unitamente ai risultati di analisi morfostratigrafiche, hanno consentito di definire l'evoluzione dell'ambiente lagunare in relazione alla paleomorfologia costiera, agli apporti sedimentari, alle variazioni del reticolo idrografico ed ai fenomeni eustatici tardo-quaternari.

Background/Aims: We evaluated prevalence and prognosis of mild anemia, defined as Hb (g/dl) 11–13.5 in males and 11–12 in females, in a prospective cohort of stage 3–5 chronic kidney disease (CKD) patients. Methods: We enrolled 668 consecutive patients in 25 renal clinics during 2003. Patients with frank anemia (Hb Results: Mild anemia was present in 41.3% at visit 1 and 34.1% at visit 2. We identified PER in 22% patients, RES in 10%, and PRO in 26%. In the subsequent 40 months, 125 patients developed end-stage renal disease (ESRD) and 94 died. At competing risk model, PER predicted ESRD (hazard ratio, HR, 1.82, 95% confidence interval, CI, 1.01–3.29) while PRO predicted both ESRD (HR 1.81, 95% CI 1.02–3.23) and death (HR 1.87, 95% CI 1.04–3.37). Conclusion: In non-dialysis chronic kidney disease, mild anemia is prevalent and it is a marker of risk excess when persistent or progressive over time.
