Abstract Aims Lower body half compression of bilateral secondary leg lymphoedema (LE) without relevant cardiac insufficiency gives rise to whether external leg compression may influence left ventricular (LV) function. Patients with LE were subjected to baseline two‐dimensional transthoracic echocardiography (2DTTE) for general assessment then three‐dimensional speckle‐tracking echocardiography (3DSTE) before and 1 h after lower body half external compression for LV torsion analysis. Methods and results Baseline 2DTTE was performed in the cohort of 25 LE patients, and the results were compared with those of age‐ and gender‐matched 52 healthy controls (mean age: 47.8 ± 12.8 vs. 40.7 ± 14.0 years, 24 women/1 man vs. 49 women/3 men, respectively). 3DSTE was conducted for the assessment of LV rotational mechanics where apical (AR), and basal rotations (BR) were measured before and 1 h after the use of compression class 2 (ccl 2) flat‐knitted medical compression pantyhoses (pressure range: 23–32 mmHg). 2DTTE showed significantly larger LV end‐diastolic volume and ejection fraction among LE patients compared with control subjects (108.3 ± 20.1 vs. 98.5 ± 21.7 mL, 69.8 ± 4.8 vs. 65.5 ± 4.3%, respectively) and notably smaller LV end‐systolic diameter and posterior wall thickness (28.9 ± 3.5 vs. 31.2 ± 3.4 mm, 8.1 ± 1.0 vs. 9.0 ± 1.7 mm, respectively). The results of 20 patients with LE were considered in 3DSTE examinations due to the drop‐out of five probands with technical failures. The data of four LE patients showing significant LV rotational abnormalities were managed separately, and the rotational parameters of the remaining sixteen patients did not differ significantly from those of matched controls except significant reduction of LV BR following the application of medical compression stockings (MCS) (−2.70 ± 1.26 degrees after 1 h use of pantyhose in patient group vs. −4.28 ± 2.18 degrees of the control group; P

Introduction and Objective: Acromegaly is a rare, chronic and slowly developing endocrine disorder caused by hypersecretion of human growth hormone and consequently of insulin-like growth factor-1 during adulthood. The present study was conducted to assess mitral annular (MA) size and function between acromegalic patients and age- and gender-matched healthy controls by three-dimensional speckle-tracking echocardiography (3D-STE). It also aimed to examine whether activity of the disease has any effect on MA parameters. Methods: This study included 27 patients with acromegaly, three of whom were excluded due to inferior image quality. The mean age of the remaining 24 patients was 55.7±14.0 years and seven were male. Complete two-dimensional Doppler echocardiography and 3D-STE were performed in all cases. Results: Significantly increased end-diastolic and end-systolic MA diameter (2.81±0.36 cm vs. 2.44±0.34 cm and 2.00±0.32 cm vs. 1.65±0.37 cm, respectively), area (9.67±2.33 cm2 vs. 7.38±1.93 cm2 and 5.14±1.62 cm2 vs. 3.74±1.19 cm2, respectively) and perimeter (11.76±1.42 cm vs. 10.27±1.33 cm and 8.61±1.23 cm vs. 7.36±1.10 cm, respectively) were demonstrated in acromegalic patients compared with control subjects. MA functional parameters were not significantly altered compared to those of healthy individuals. Conclusions: MA dilation could be seen in acromegaly regardless of its activity. Acromegaly is not associated with MA functional impairment. Resumo: Introdução e Objectivo: A acromegalia é uma doença endócrina rara, crónica e de desenvolvimento lento, causada pela hipersecreção da hormona do crescimento humano e, consequentemente, do fator de crescimento semelhante à insulina-1 durante a idade adulta. O presente estudo foi conduzido para avaliar o tamanho e a função do anel mitral (MA) nos doentes com acromegalia e controles saudáveis emparelhados por idade e sexo, por meio de ecocardiografia tridimensional com speckle-tracking (3DSTE). O objetivo foi também examinar se a atividade da doença tem algum efeito nos parâmetros da MA. Métodos: Este estudo incluiu 27 pacientes com acromegalia, três dos quais tiveram de ser excluídos devido à qualidade inferior de imagem. A média de idade dos 24 pacientes restantes foi de 55,7 ± 14,0 anos (7 homens). Foram realizados em todos os casos uma ecocardiografia Doppler bidimensional completa e um 3DSTE. Resultados: Quando comparados com os controlos, os doentes acromegálicos apresentaram diâmetros diastólico e sistólico finais significativamente dilatados (2,81 ± 0,36 cm versus 2,44 ± 0,34 cm e 2,00 ± 0,32 cm versus 1,65 ± 0,37 cm, respetivamente), assim como a área valvular (9,67 ± 2,33 cm2 versus 7,38 ± 1,93 cm2 e 5,14 ± 1,62 cm2 versus 3,74 ± 1,19 cm2, respetivamente) e o perímetro (11,76 ± 1,42 cm versus 10,27 ± 1,33 cm e 8,61 ± 1,23 cm versus 7,36 ± 1,10 cm, respetivamente) Os parâmetros funcionais da MA não estavam significativamente alterados em comparação com indivíduos saudáveis. Conclusões: A dilatação do MA pode ser observada na acromegalia, independentemente de sua atividade. A acromegalia não está associada ao comprometimento funcional da MA.

Introduction: Acromegaly is a relatively rare chronic hormonal disease resulting in disfigurement. In 90% of cases, acromegaly is caused by a benign pituitary monoclonal human growth hormone-secreting tumor. The aim of the present study was to determine the presence of left ventricular (LV) deformation abnormalities using three-dimensional speckle-tracking echocardiography in a group of acromegalic patients. Methods: Thirty-eight acromegalic patients were involved in the study. Thirteen patients were excluded due to inadequate image quality. The mean age of the remaining patients was 57.2±13.6 years and seven were male. Their data were compared to an age- and gender-matched control population, which consisted of 34 healthy volunteers (mean age: 52.7±4.9 years, 15 male). Results: Global and mean segmental LV radial strain (RS) (33.2±13.4% vs. 25.2±10.8%, p=0.01 and 36.0±12.1% vs. 28.2±10.0%, p=0.009, respectively) proved to be significantly higher in acromegaly compared to controls. Active acromegalic patients had significantly higher global and mean segmental LV-RS (35.5±14.4% vs. 25.2±10.8%, p=0.03 and 37.9±13.3% vs. 28.2±10.0%, p=0.03, respectively) compared to controls. Between the active and inactive acromegaly groups, only basal LV circumferential strain (-30.2±4.8% vs. -26.7±4.1%, p=0.02) was found to be significantly different. Conclusion: The presented clinical, demographic, therapeutic and echocardiographic features demonstrate that active acromegaly is associated with enhanced LV RS as compared to healthy controls and those with inactive acromegaly. Resumo: Introdução: A acromegalia é uma doença hormonal crónica relativamente rara, associada a dismorfismos somáticos. Em 90% dos casos, ela é causada por um tumor benigno monoclonal da hipófise, secretor de hormona do crescimento. O objetivo do presente estudo foi detetar a presença de alterações da deformação miocárdica do ventrículo esquerdo (VE) através de ecocardiografia tridimensional por speckle tracking num grupo de doentes portadores de acromegalia. Métodos: Foram incluídos 38 doentes portadores de acromegalia. Treze foram excluídos devido à má qualidade de imagem. A idade média dos restantes foi de 57,2±13,6 anos (7 homens). Os seus dados foram comparados com uma população controlo emparelhada por idade e género, que compreendeu 34 voluntários saudáveis (idade média: 52,7±4,9 anos, sendo 15 indivíduos do sexo masculino). Resultados: O strain radial (SR) global e segmentar VE (respetivamente 33,2±13,4% versus 25,2±10,8%, p=0,01 e 36,0±12,1% versus 28,2±10,0%, p=0,009) foi significativamente superior nos casos de acromegalia em relação aos casos controlo. Os doentes portadores de acromegalia ativa tiveram um SR global e segmentar significativamente superior (35,5±14,4% versus 25,2±10,8%, p=0,03 e 37,9±13,3% versus 28,2±10,0%, p=0,03, respetivamente), quando comparados com os grupos controlo. Entre os grupos portadores de acromegalia ativos e inativos, só foi considerado significativamente diferente o strain circunferencial basal VE (-30,2±4,8% versus -26,7±4,1%, p=0,02). Conclusões: Com base nas características clínicas, demográficas, terapêuticas e ecocardiográficas apresentadas, a acromegalia ativa apresenta SR-VE aumentado em relação à população saudável e ao grupo de portadores de acromegalia inativa.

Resumen: El lipedema es un trastorno crónico que se caracteriza por una hiperplasia del tejido adiposo subcutáneo simétrico, deformante, asociado a hematomas y dolor, que afecta fundamentalmente a mujeres tras la pubertad, que suele ser familiar y tiene influencia hormonal. Sin tratamiento, el lipedema puede progresar a linfedema, y se ha asociado al aumento en la rigidez de la aorta. La mecánica rotacional del ventrículo izquierdo (VI) tiene un papel significativo en la circulación normal. En circunstancias normales, la base del VI rota en el sentido de las agujas del reloj, mientras que el ápex del VI rota en sentido contrario, provocando un movimiento como de retorcer una toalla llamado torsión del VI. En el presente caso, mediante ecocardiografía speckle-tracking tridimensional se reveló una torsión del VI demostrando un ápex VI en el sentido de las agujas del reloj y la rotación antihoraria de la base del VI en un paciente con características clínicas de lipedema. Abstract: Lipoedema is a chronic disorder that is characterised by a symmetric, deforming, hyperplasia of the subcutaneous adipose tissue, and is associated with haematomas and pain. It mainly affects women after puberty, is usually familial and is influence by hormones. Untreated, the lipoedema can progress to lymphoedema, and has been associated with the increase in aortic stiffness. The mechanical rotation of the left ventricle (LV) has an important role in normal circulation. In normal circumstances, the base of the LV rotates clockwise, while the LV apex rotates in the opposite direction, leading to a movement like wringing a towel, called LV twist. In the present case, using three-dimension speckle-tracking echocardiography, an LV twist is observed, showing an LV apex in the clockwise direction and the anti-clockwise rotation of the LV base in a patient with clinical characteristics of lipoedema. Introduction: While cardiac amyloidosis (CA) is a rare systemic disease characterized by extracellular deposition of protein-derived fibrils, hypertrophic cardiomyopathy (HCM) is histopathologically characterized by myocyte hypertrophy and disarray, interstitial fibrosis, and small intramural coronary arteriole dysplasia. The aim of the present study was to compare left atrial (LA) volumetric and functional characteristics between light-chain (AL) CA and HCM by three-dimensional (3D) speckle-tracking echocardiography (STE). Methods: The AL-CA group initially consisted of 17 patients with AL-CA, but one patient was excluded due to inadequate image quality, and so the study population consisted of 16 patients (mean age: 64.0±9.6 years, five men). Their results were compared with data on 20 age-matched HCM patients (mean age: 59.8±5.2 years, 10 men) and on 16 age-matched healthy controls (mean age: 58.2±7.2 years, six men). Complete two-dimensional Doppler echocardiography and 3D-STE were performed in all cases. Results: Significantly increased LA volumes were observed in both AL-CA and HCM compared with the control group. Only active atrial emptying fraction was found to be significantly reduced in AL-CA patients compared to controls. Peak global and mean segmental circumferential, longitudinal and area strains showed significant reductions in AL-CA patients compared with controls, but only peak mean segmental longitudinal strain differed significantly between HCM patients and controls. While no differences were demonstrated in global and mean segmental strain at atrial contraction between HCM patients and controls, AL-CA patients showed reductions in certain strain parameters compared to controls and HCM patients. Conclusions: Different patterns of LA functional characteristics were demonstrated in AL-CA and HCM patients by 3D-STE. Resumo: Introdução: A amiloidose cardíaca (CA) é uma doença sistémica rara, caracterizada pela deposição extracelular de fibrilhas derivadas das proteínas; por outro lado, a miocardiopatia hipertrófica (HCM) é caracterizada do ponto de vista histopatológico por hipertrofia e desorganização dos miócitos, fibrose intersticial e displasia das pequenas arteríolas coronárias intramurais. No presente estudo fomos comparar as características volumétricas e funcionais da aurícula esquerda (AE) entre a CA de cadeias leves (AL) e a HCM com ecocardiografia speckle-tracking tridimensional (3DSTE). Métodos: A população do estudo foi constituída por 17 doentes com AL-CA, foi excluído um doente por deficiente qualidade de imagem. Desse modo, o grupo final de AL-CA consistiu em 16 doentes (média de 64,0 ± 9,6 anos, cinco homens). Os resultados desse grupo foram comparados com os de 20 doentes com HCM emparelhados por idade (idade média: 59,8 ± 5,2 anos, 10 homens) e com 16 controlos saudáveis igualmente emparelhados por idade média: 58,2 ± 7,2 anos, seis homens). Em todos os doentes foi feito ecocardiograma Doppler bidimensional e 3DSTE. Resultados: Os volumes da AE foram significativamente maiores nos doentes com AL-CA e com HCM comparados com o grupo controlo. Apenas a fração de esvaziamento auricular ativo foi significativamente mais baixa nos doentes com AL-CA em comparação com o grupo de controlo. Apesar de os strains de pico e médio segmentares, circunferenciais, longitudinais e em área serem significativamente mais baixos nos doentes com AL-CA por comparação com o grupo controlo, apenas o strain longitudinal segmentar médio de pico foi significativamente diferente entre os doentes com HCM e os do grupo de controlo. Apesar de não ter sido possível demonstrar diferenças no strain segmentar global e médio entre os doentes com HCM e o grupo de controlo, no caso dos doentes com AL-CA observou-se redução em alguns parâmetros de strain por comparação quer com o grupo de doentes com HCM quer com o grupo de controlo. Conclusão: Foi possível demonstrar a presença de diferentes padrões funcionais da AE por 3DSTE em doentes com amiloidose cardíaca e miocardiopatia hipertrófica. Objective: Isolated left ventricular noncompaction (ILVNC) is a rare cardiomyopathy characterized by a prominent trabecular meshwork and deep intertrabecular recesses. The present study aimed to examine right atrial (RA) volumes, volume-based functional properties and strains by three-dimensional speckle-tracking echocardiography (3DSTE) in ILVNC patients. Methods: The study group consisted of 13 patients with ILVNC (mean age: 54.6±14.1 years, six male) and 23 healthy age- and gender-matched volunteers (mean age: 50.4±12.4 years, 10 male), who served as normal controls. Complete two-dimensional Doppler echocardiography and 3DSTE were performed in all cases. Results: Increased systolic maximum (58.2±15.3 ml vs. 40.5±11.8 ml, p=0.0004) and diastolic pre-atrial contraction (39.6±16.1 ml vs. 28.2±9.2 ml, p=0.01) and minimum (46.2±17.5 ml vs. 34.6±11.6 ml, p=0.02) RA volumes were detected in ILVNC patients. Only total (18.6±8.5 ml vs. 12.2±5.9 ml, p=0.01) and passive (12.0±13.3 vs. 5.9±3.7 ml, p=0.05) RA stroke volumes, representing features of RA reservoir and conduit phases, were increased in ILVNC; active RA stroke volume and all emptying fractions did not differ between ILVNC patients and matched controls. Moreover, global, mean segmental and regional peak strains and strains at atrial contraction showed no differences between ILVNC patients and controls. Conclusions: 3DSTE-derived volumetric analysis confirmed increased cyclic RA volumes in ILVNC. Only mild RA functional alterations were demonstrated in ILVNC. Resumo: Objetivo: A miocardiopatia não compactada isolada do ventrículo esquerdo (NCIVE) foi apresentada como uma miocardiopatia rara, caracterizada por uma trabeculação proeminente e por profundos recessos trabeculares na cavidade ventricular. Este estudo tem como objetivo examinar os volumes da aurícula direita (AD) e a mecânica auricular direita por ecocardiografia tridimensional de speckle-tracking nos doentes com NCIVE. Métodos: O grupo de estudo é composto por 13 doentes com NCIVE (idade média: 54,6 ± 14,1 anos, seis homens) e 23 voluntários saudáveis com idade e género correspondentes (idade média: 50,4 ± 12,4 anos, homens) que são controlos normais. Foram realizadas em todos os casos ecocardiografia Doppler bidimensional e ecocardiografia tridimensional de speckle-tracking. Resultados: Foi detetado nos doentes com NCIVE um aumento do volume da AD na fase sistólica (58,2 ± 15,3 ml versus 40,5 ± 11,8 ml, p = 0,0004), na fase de contração diastólica pré-auricular (39,6 ± 16,1 ml versus 28,2 ± 9,2 ml, p = 0,01) e na fase diastólica final (46,2 ± 17,5 ml versus 34,6 ± 11,6 ml, p = 0,02). Apenas o volume da fase de reservatório (18,6 ± 8,5 ml versus 12,2 ± 5,9 ml, p = 0,01) e da fase de condução da AD (12,0 ± 13,3 versus 5,9 ± 3,7 ml, p = 0,05) foram superiores no grupo NCIVE em comparação com o grupo de controlos. O volume de ejeção da aurícula direita foi semelhante entre ambos os grupos. Os grupos foram também homogéneos relativamente ao strain e ao strain rate da AD nas diferentes fases do ciclo cardíaco. Conclusão: A análise volumétrica detetada pela ecocardiografia tridimensional de speckle-tracking confirmou o aumento cíclico dos volumes da AD na NCIVE. Apenas alterações funcionais suaves da AD podem ser demonstradas na NCIVE. Introduction: The present study was designed to compare three-dimensional speckle tracking echocardiography (3DSTE)-derived left atrial (LA) volumetric, volume-based functional and strain parameters between patients with hypereosinophilic syndrome (HES) and matched controls. Methods: A total of 10 HES patients and 19 age- and gender-matched healthy controls were included in the study. Complete two-dimensional Doppler echocardiography and 3DSTE were performed in all HES cases and controls. Results: Significantly increased maximum (72.9±38.8 ml vs. 45.6±15.5 ml, p=0.01) and minimum (46.3±33.3 ml vs. 26.0±15.0 ml, p=0.03) LA volumes and LA volume before atrial contraction (62.0±36.0 ml vs. 36.5±16.6 ml, p=0.01) were found in HES patients compared to controls. Both peak global (18.3±6.7% vs. 25.6±9.0%, p=0.03) and mean segmental (22.2±6.0% vs. 31.0±12.1%, p=0.04) circumferential strains were significantly reduced in HES patients, suggesting decreased LA reservoir function. Conclusion: Increased LA volumes can be demonstrated in HES patients, accompanied by reduced LA peak circumferential strain as assessed by 3DSTE, suggesting LA remodeling. Resumo: Introdução: O presente estudo foi concebido para comparar parâmetros volumétricos auriculares esquerdos derivados por ecocardiografia tridimensional speckle tracking, parâmetros funcionais e strain baseados no volume entre doentes com síndrome hipereosinofílica e controlos equiparados. Métodos: Um total de dez doentes HES e de 19 controlos saudáveis, equiparados para idade e género, foram incluídos no estudo. O estudo completo ecocardiográfico bidimensional, Doppler e 3DSTE, foi realizado em todos os casos HES e controlos. Resultados: Encontrou-se em doentes com HES, comparativamente com os controlos, um aumento significativo dos volumes auriculares esquerdos (72,9 ± 38,8 ml versus 45,6 ± 15,5 ml, p = 0,01), mínimo (46,3 ± 33,3 ml versus 26,0 ± 15,0 ml, p = 0,03) e volume auricular esquerdo antes da contração auricular (62,0 ± 36,0 ml versus 36,5 ± 16,6 ml, p = 0,01). Ambos strains circunferenciais pico, global (18,3 ± 6,7% versus 25,6 ± 9,0%, p = 0,03) e segmentar médio (22,2 ± 6,0% versus 31,0 ± 12,1%, p = 0,04) eram significativamente reduzidos em doentes HES, sugerindo diminuição de função auricular esquerda de reservatório. Conclusão: O aumento de volumes auriculares esquerdos foi demonstrado nos doentes HES, sendo acompanhado por redução de strain circunferencial pico da aurícula esquerda conforme avaliado por 3DSTE, sugerindo remodelagem auricular esquerda. Introduction and Objective: Three-dimensional (3D) speckle tracking echocardiography (3DSTE) is a novel method for assessment of left atrial (LA) volumes and function without geometrical assumptions. 3DSTE allows detailed assessment of LA features including volume measurements, strain assessments and calculation of LA ejection force (LAEF). LA strain and volume-based functional parameters originate from the same 3D dataset, but assessment of LAEF requires more data including measurement of mitral annular dimensions and Doppler-derived inflow velocities. The present study was designed to find correlations between LAEF and 3DSTE-derived LA volume-based functional properties and strain parameters in healthy subjects. Methods: The study population comprised 34 randomly selected healthy subjects (age 36.1±11.2 years, 15 men) in sinus rhythm, all of whom had undergone standard two-dimensional transthoracic Doppler echocardiographic study extended with 3DSTE. Results: Mitral annulus diameter-based LAEF correlated with global LA peak circumferential (r=0.39, p=0.02), longitudinal (r=0.32, p=0.05) and area (r=0.43, p=0.01) strain, total atrial stroke volume (r=0.30, p=0.05) and total atrial emptying fraction (r=0.31, p=0.05) characterizing (systolic) LA reservoir function and global LA 3D strain at atrial contraction (r=−0.44, p=0.01) and active atrial emptying fraction (r=0.36, p=0.04) characterizing (diastolic) LA contraction function (booster pump phase). Conclusions: Complex LA functional assessment can be provided by 3DSTE, including calculation of LAEF and volume-based and strain functional properties, with significant correlations between these parameters. Resumo: Introdução e objetivos: A ecocardiografia tridimensional (3D) com speckle tracking (E3DST) é um novo método para avaliação do volume e da função da aurícula esquerda (AE) sem pressupostos geométricos. A E3DST permite a avaliação detalhada das características da AE, incluindo as medições volumétricas, as avaliações da pressão e o cálculo da força de ejeção da AE (FEAE). A força da AE e os parâmetros funcionais baseados no volume são provenientes dos mesmos dados 3D, mas a avaliação da FEAE requer mais dados incluindo a medição das dimensões do anel mitral e das velocidades do fluxo derivado do Doppler. Este estudo foi concebido para encontrar correlações entre a FEAE e a E3DST derivadas das propriedades funcionais baseadas no volume da AE e nos parâmetros de pressão em indivíduos saudáveis. Métodos: Este estudo incluiu 34 indivíduos saudáveis selecionados aleatoriamente (36,1 ± 11 anos, 15 homens) em ritmo sinusal, submetidos a estudo ecocardiográfico Doppler bidimensional transtorácico padrão associado a E3DST. Resultados: A FEAE no anel mitral baseada no diámetro correlacionada com o pico global da AE circunferencial (r = 0,39, p = 0,02), longitudinal (r = 0,32, p = 0,05) e área (r = 0,43, p = 0,01) força, volume arterial total de acidente vascular cerebral (r = 0,30, p = 0,05) e fração auricular total de esvaziamento (r = 0,31, p = 0,05) caracterizando a função de reservatório da AE (sistólica) e a força 3D da AE na contração auricular (r = 0,44, p = 0,01) e fração auricular de esvaziamento ativa (r = 0,36, p = 0,04) caracterizando a função da contração da AE (diastólica) (fase da bomba de reforço). Conclusões: A E3DST podia providenciar a avaliação funcional complexa da AE incluindo o cálculo da FEAE e das propriedades funcionais baseadas no volume e na força com correlações significativas entre estes parâmetros. Noncompaction cardiomyopathy (NCCM) is a rare congenital cardiomyopathy characterised by a distinctive 2-layered appearance of the myocardium due to hypertrabecularisation and deep intertrabecular recesses. The present study was designed to assess left atrial (LA) volumes and volumetric and strain-based functional properties by three-dimensional speckle-tracking echocardiography (3DSTE) in NCCM. Methods: The study included 12 consecutive NCCM patients. Their results were compared to 20 age- and sex-matched healthy controls. Complete two-dimensional Doppler echocardiography and 3DSTE were performed in all cases. Results: Calculated LA maximum (76.5 ± 26.8 mL vs. 45.3 ± 15.1 mL, p=0.0002) and minimum (56.9 ± 27.3 mL vs. 25.3 ± 15.2 mL, p=0.0002) volumes and LA volume before atrial contraction (67.1 ± 28.2 mL vs. 35.7 ± 16.4 mL, p=0.0004) were significantly greater in NCCM patients. Total, active, and passive LA emptying fractions proved to be smaller in NCCM. Peak global radial (−9.3 ± 7.8% vs. −16.8 ± 10.2%, p=0.05), circumferential (12.8 ± 8.4% vs. 26.2 ± 9.2%, p=0.0003), longitudinal (12.8 ± 8.2% vs. 22.5 ± 8.5%, p=0.004), and area (26.7 ± 18.5% vs. 51.6 ± 20.3%, p=0.001) strains were significantly smaller in NCCM patients as compared to matched controls. Conclusions: Significantly greater LA volumes and compromised LA functional properties could be demonstrated by 3DSTE in patients with NCCM.

Abstract Background: Light-chain (AL) cardiac amyloidosis (CA) is characterized by fibril deposits, which are composed of monoclonal immunoglobulin light chains. The right ventricle is mostly involved in AL-CA and impairment of its function is a predictor of worse prognosis. Objectives: To characterize the volumetric and functional properties of the right atrium (RA) in AL-CA by three-dimensional speckle-tracking echocardiography (3DSTE). Methods: A total of 16 patients (mean age: 64.5 ± 10.1 years, 11 males) with AL-CA were examined. Their results were compared to that of 15 age- and gender-matched healthy controls (mean age: 58.9 ± 6.9 years, 8 males). All cases have undergone complete two-dimensional Doppler and 3DSTE. A two-tailed p value of less than 0.05 was considered statistically significant. Results: Significant differences could be demonstrated in RA volumes respecting cardiac cycle. Total (19.2 ± 9.3% vs. 27.9 ± 10.7%, p = 0.02) and active atrial emptying fractions (12.1 ± 8.1 vs. 18.6 ± 9.8%, p = 0.05) were significantly decreased in AL-CA patients. Peak global (16.7 ± 10.3% vs. 31.2 ± 19.4%, p = 0.01) and mean segmental (24.3 ± 11.1% vs. 38.6 ± 17.6%, p =0.01) RA area strains, together with some circumferential, longitudinal and segmental area strain parameters, proved to be reduced in patients with AL-CA. Global longitudinal (4.0 ± 5.2% vs. 8.2 ± 5.5%, p = 0.02) and area (7.8 ± 8.1% vs. 15.9 ± 10.3%, p = 0.03) strains at atrial contraction and some circumferential and area strain parameters at atrial contraction were reduced in AL-CA patients. Conclusion: Significantly increased RA volumes and deteriorated RA functions could be demonstrated in AL-CA.

Secondary myocardial involvement by diffuse large B-cell lymphoma is a rare occurrence. Left ventricular (LV) twist is considered an essential part of LV function. In normal circumstances LV twist results from the movement of two orthogonally oriented muscular bands of a helical myocardial structure with consequent clockwise rotation of the base and counterclockwise rotation of the apex. Three-dimensional (3D) speckle-tracking echocardiography (3DSTE) has been found to be feasible for non-invasive 3D quantification of LV wall motion and rotational mechanics. The present report aimed to assess LV twisting motion in a patient with diffuse large B-cell lymphoma with positron emission tomography/computer tomography-proven cardiac involvement by 3DSTE. During 3DSTE, reduction in some segmental radial, longitudinal, circumferential, area and 3D LV strains were found. Apical and basal LV rotations were found to be in the same counterclockwise direction, confirming near absence of LV twist – so-called rigid body rotation. Resumo: O envolvimento do miocárdio secundário ao linfoma difuso de células grandes B é uma ocorrência rara. A torção ventricular esquerda (VE) é considerada uma parte essencial da função VE. Em circunstâncias normais, a torção do VE resulta do movimento de uma estrutura miocárdica helicoidal de duas bandas musculares orientadas ortogonalmente, com a consequente rotação no sentido dos ponteiros do relógio da base do VE e rotação contra os ponteiros do relógio do ápex do VE. A ecocardiografia tridimensional (3D) de speckle-tracking (3DSTE) foi considerada adequada para a quantificação 3D não invasiva da contratilidade segmentar VE e da mecânica de rotação. Este artigo tem como objetivo a avaliação do movimento de torção VE por 3DSTE num doente com linfoma difuso de células grandes B e envolvimento cardíaco comprovado por tomografia de emissão de positrões/tomografia. Por 3DSTE foi verificada uma certa redução do strain radial, longitudinal, circunferencial do VE. A rotação dos segmentos basais e apicais do VE foi semelhante, num mesmo sentido, contra os ponteiros do relógio. Tal conduziu a uma redução marcada da torção do VE – rigid body rotations. Detetaram-se rotações VE apicais e basais no sentido contrário ao dos ponteiros do relógio, confirmando a ausência próxima da torção VE conforme referido «rotação rígida do corpo».
