Copyright © 2016 by the Society of Critical Care Medicine and the World Federation of Pediatric Intensive and Critical Care Societies.Objectives: To describe the epidemiology, morbidity, and mortality of new or progressive multiple organ dysfunction syndrome in children with severe sepsis. Design: Secondary analysis of a prospective, cross-sectional, point prevalence study. Setting: International, multicenter PICUs. Patients: Pediatric patients with severe sepsis identified on five separate days over a 1-year period. Interventions: None. Measurements and Main Results: Of 567 patients from 128 PICUs in 26 countries enrolled, 384 (68%) developed multiple organ dysfunction syndrome within 7 days of severe sepsis recognition. Three hundred twenty-seven had multiple organ dysfunction syndrome on the day of sepsis recognition. Ninety-one of these patients developed progressive multiple organ dysfunction syndrome, whereas an additional 57 patients subsequently developed new multiple organ dysfunction syndrome, yielding a total proportion with severe sepsis-associated new or progressive multiple organ dysfunction syndrome of 26%. Hospital mortality in patients with progressive multiple organ dysfunction syndrome was 51% compared with patients with new multiple organ dysfunction syndrome (28%) and those with single-organ dysfunction without multiple organ dysfunction syndrome (10%) (p < 0.001). Survivors of new or progressive multiple organ dysfunction syndrome also had a higher frequency of moderate to severe disability defined as a Pediatric Overall Performance Category score of greater than or equal to 3 and an increase of greater than or equal to 1 from baseline: 22% versus 29% versus 11% for progressive, new, and no multiple organ dysfunction syndrome, respectively (p < 0.001). Conclusions: Development of new or progressive multiple organ dysfunction syndrome is common (26%) in severe sepsis and is associated with a higher risk of morbidity and mortality than severe sepsis without new or progressive multiple organ dysfunction syndrome. Our data support the use of new or progressive multiple organ dysfunction syndrome as an important outcome in trials of pediatric severe sepsis although efforts are needed to validate whether reducing new or progressive multiple organ dysfunction syndrome leads to improvements in more definitive morbidity and mortality endpoints.

Background Although high-flow nasal cannula therapy (HFNC) has become a popular mode of non-invasive respiratory support (NRS) in critically ill children, there are no randomised controlled trials (RCTs) comparing it with continuous positive airway pressure (CPAP). We performed a pilot RCT to explore the feasibility, and inform the design and conduct, of a future large pragmatic RCT comparing HFNC and CPAP in paediatric critical care. Methods In this multi-centre pilot RCT, eligible patients were recruited to either Group A (step-up NRS) or Group B (step-down NRS). Participants were randomised (1:1) using sealed opaque envelopes to either CPAP or HFNC as their first-line mode of NRS. Consent was sought after randomisation in emergency situations. The primary study outcomes were related to feasibility (number of eligible patients in each group, proportion of eligible patients randomised, consent rate, and measures of adherence to study algorithms). Data were collected on safety and a range of patient outcomes in order to inform the choice of a primary outcome measure for the future RCT. Results Overall, 121/254 eligible patients (47.6%) were randomised (Group A 60%, Group B 44.2%) over a 10-month period (recruitment rate for Group A, 1 patient/site/month; Group B, 2.8 patients/site/month). In Group A, consent was obtained in 29/33 parents/guardians approached (87.9%), while in Group B 84/118 consented (71.2%). Intention-to-treat analysis included 113 patients (HFNC 59, CPAP 54). Most reported adverse events were mild/moderate (HFNC 8/59, CPAP 9/54). More patients switched treatment from HFNC to CPAP (Group A: 7/16, 44%; Group B: 9/43, 21%) than from CPAP to HFNC (Group A: 3/13, 23%; Group B: 5/41, 12%). Intubation occurred within 72 h in 15/59 (25.4%) of HFNC patients and 10/54 (18.5%) of CPAP patients (p = 0.38). HFNC patients experienced fewer ventilator-free days at day 28 (Group A: 19.6 vs. 23.5; Group B: 21.8 vs. 22.2). Conclusions Our pilot trial confirms that, following minor changes to consent procedures and treatment algorithms, it is feasible to conduct a large national RCT of non-invasive respiratory support in the paediatric critical care setting in both step-up and step-down NRS patients. The National High Magnetic Field Laboratory has designed and will build a large bore, 60 tesla, 100 ms (quasi continuous) flat-top magnet for installation and use at Los Alamos. The magnet consists of eight concentric, resistive, solenoid coils fabricated from high strength, high conductivity copper supported by external stainless steel shells. Before each pulse the magnet is cooled to 77 K with liquid nitrogen. The energy for the magnet is provided through converter power supplies from a 1430 MVA, 24 kV alternating current energy storage generator. Plans for prototype tests of full scale portions of the magnet are discussed. A detailed description of the magnet is presented along with available information on fabrication methods to be employed in its manufacture.

Copyright © 2016 by the Society of Critical Care Medicine and the World Federation of Pediatric Intensive and Critical Care Societies.Objectives: Pediatric severe sepsis remains a significant global health problem without new therapies despite many multicenter clinical trials. We compared children managed with severe sepsis in European and U.S. PICUs to identify geographic variation, which may improve the design of future international studies. Design: We conducted a secondary analysis of the Sepsis PRevalence, OUtcomes, and Therapies study. Data about PICU characteristics, patient demographics, therapies, and outcomes were compared. Multivariable regression models were used to determine adjusted differences in morbidity and mortality. Setting: European and U.S. PICUs. Patients: Children with severe sepsis managed in European and U.S. PICUs enrolled in the Sepsis PRevalence, OUtcomes, and Therapies study. Interventions: None. Measurements and Main Results: European PICUs had fewer beds (median, 11 vs 24; p < 0.001). European patients were younger (median, 1 vs 6 yr; p < 0.001), had higher severity of illness (median Pediatric Index of Mortality-3, 5.0 vs 3.8; p = 0.02), and were more often admitted from the ward (37% vs 24%). Invasive mechanical ventilation, central venous access, and vasoactive infusions were used more frequently in European patients (85% vs 68%, p = 0.002; 91% vs 82%, p = 0.05; and 71% vs 50%; p < 0.001, respectively). Raw morbidity and mortality outcomes were worse for European compared with U.S. patients, but after adjusting for patient characteristics, there were no significant differences in mortality, multiple organ dysfunction, disability at discharge, length of stay, or ventilator/vasoactive-free days. Conclusions: Children with severe sepsis admitted to European PICUs have higher severity of illness, are more likely to be admitted from hospital wards, and receive more intensive care therapies than in the United States. The lack of significant differences in morbidity and mortality after adjusting for patient characteristics suggests that the approach to care between regions, perhaps related to PICU bed availability, needs to be considered in the design of future international clinical trials in pediatric severe sepsis.

The Near East and the Caucasus regions are considered as gene and domestication centre for grapevine. In an earlier project “Conservation and Sustainable Use of Grapevine Genetic Resources in the Caucasus and Northern Black Sea Region” (2003-2007) it turned out that 2,654 accessions from autochthonous cultivars maintained by Armenia, Azerbaijan, Georgia, Moldova, Russian Federation and Ukraine in ten grapevine collections may belong to 1,283 cultivars. But trueness to type assessment by morphology and genetic fingerprinting still needed to be done. In COST Action FA1003 a first step in that direction was initiated. The following countries participated: Albania, Armenia, Austria, Azerbaijan, Bulgaria, Croatia, Georgia, Hungary, Latvia, Moldova, Romania, Slovakia, Slovenia and Ukraine. Mainly Vitis vinifera accessions (1098 samples) and 76 Vitis sylvestris individuals were analyzed by nine SSR-markers (VVS2, VVMD5, VVMD7, VVMD25, VVMD27, VVMD28, VVMD32, VrZag62, VrZag79). Cultivar identity confirmation/rejection was attempted for 306 genotypes/cultivars by comparison of the generated genetic profiles with international SSR-marker databases and ampelographic studies. The outcome proved unambiguously the necessity of morphologic description and photos (a) for comparison with bibliography, (b) for a clear and explicit definition of the cultivar and (c) the detection of sampling errors and misnomers. From the 1,098 analyzed accessions, 997 turned out to be indigenous to the participating countries. The remaining 101 accessions were Western European cultivars. The 997 fingerprints of indigenous accessions resulted in 658 unique profiles/cultivars. From these 353 (54 %) are only maintained in the countries of origin and 300 (46 %) unique genotypes exist only once in the Eastern European collections. For these 300 genotypes duplicate preservation needs to be initiated. In addition, the high ratio of non redundant genetic material of Eastern European origin suggests an immense unexplored diversity. - Abstract
Objectives: The prevalence of septic acute kidney injury and impact on functional status of PICU survivors are unknown. We used data from an international prospective severe sepsis study to elucidate functional outcomes of children suffering septic acute kidney injury. Design: Secondary analysis of patients in the Sepsis PRevalence, OUtcomes, and Therapies point prevalence study: acute kidney injury was defined on the study day using Kidney Disease Improving Global Outcomes definitions. Patients with no acute kidney injury or stage 1 acute kidney injury ("no/mild acute kidney injury") were compared with those with stage 2 or 3 acute kidney injury ("severe acute kidney injury"). The primary outcome was a composite of death or new moderate disability at discharge defined as a Pediatric Overall Performance Category score of 3 or higher and increased by 1 from baseline. Setting: One hundred twenty-eight PICUs in 26 countries. Patients: Children with severe sepsis in the Sepsis PRevalence, OUtcomes, and Therapies study. Interventions: None. Measurements and Main Results: One hundred two (21%) of 493 patients had severe acute kidney injury. More than twice as many patients with severe acute kidney injury died or developed new moderate disability compared with those with no/mild acute kidney injury (64% vs 30%; p < 0.001). Severe acute kidney injury was independently associated with death or new moderate disability (adjusted odds ratio, 2.5; 95% CI, 1.5-4.2; p = 0.001) after adjustment for age, region, baseline disability, malignancy, invasive mechanical ventilation, albumin administration, and the pediatric logistic organ dysfunction score. Conclusions: In a multinational cohort of critically ill children with severe sepsis and high mortality rates, septic acute kidney injury is independently associated with further increased death or new disability.

- Abstract
Objectives: Pediatric severe sepsis remains a significant global health problem without new therapies despite many multicenter clinical trials. We compared children managed with severe sepsis in European and U.S. PICUs to identify geographic variation, which may improve the design of future international studies. Design: We conducted a secondary analysis of the Sepsis PRevalence, OUtcomes, and Therapies study. Data about PICU characteristics, patient demographics, therapies, and outcomes were compared. Multivariable regression models were used to determine adjusted differences in morbidity and mortality. Setting: European and U.S. PICUs. Patients: Children with severe sepsis managed in European and U.S. PICUs enrolled in the Sepsis PRevalence, OUtcomes, and Therapies study. Interventions: None. Measurements and Main Results: European PICUs had fewer beds (median, 11 vs 24; p < 0.001). European patients were younger (median, 1 vs 6 yr; p < 0.001), had higher severity of illness (median Pediatric Index of Mortality-3, 5.0 vs 3.8; p = 0.02), and were more often admitted from the ward (37% vs 24%). Invasive mechanical ventilation, central venous access, and vasoactive infusions were used more frequently in European patients (85% vs 68%, p = 0.002; 91% vs 82%, p = 0.05; and 71% vs 50%; p < 0.001, respectively). Raw morbidity and mortality outcomes were worse for European compared with U.S. patients, but after adjusting for patient characteristics, there were no significant differences in mortality, multiple organ dysfunction, disability at discharge, length of stay, or ventilator/vasoactive-free days. Conclusions: Children with severe sepsis admitted to European PICUs have higher severity of illness, are more likely to be admitted from hospital wards, and receive more intensive care therapies than in the United States. Introduction: In this study were compared the effects of the Conventional Flipped Classroom (virtual component for independent study + face-to-face component for the interactive discussion of clinical cases) and the Remote Flipped Classroom (FRFC) (virtual component for independent study + “online” component for the interactive discussion of clinical cases), as a teaching / learning strategy in the face of the COVID- 19 pandemic, on the self-directed learning of surgical students. Methods: Self-directed learning levels, as a result indicator of the change in methodological strategy, were compared in a group of undergraduate students participating in a conventional flipped classroom in 2017 to those of a group of students participating in a remote flipped classroom in 2020, during the surgery course at the Universidad de La Sabana, in Chía - Colombia. For this purpose, the Preparedness Scale for Self-Directed Learning (EPAD), validated to the Spanish language, was used. Results: In both groups, the levels of self-directed learning were acceptable. No significant effect (positive or negative) of either of the two models of flipped classroom was identified on the self-directed learning (d Cohen = -0.08; IC95% -0.42 – 0.24). Conclusions: The remote flipped classroom is an alternative to the conventional inverted classroom that does not compromise the self-directed learning of surgery students.

Introduction Consensus criteria for pediatric severe sepsis have standardized enrollment for research studies. However, the extent to which critically ill children identified by consensus criteria reflect physician diagnosis of severe sepsis, which underlies external validity for pediatric sepsis research, is not known. We sought to determine the agreement between physician diagnosis and consensus criteria to identify pediatric patients with severe sepsis across a network of international pediatric intensive care units (PICUs). Methods We conducted a point prevalence study involving 128 PICUs in 26 countries across 6 continents. Over the course of 5 study days, 6925 PICU patients Results Of the 706 patients, 301 (42.6 %) met both definitions. The inter-rater agreement (κ ± SE) between physician diagnosis and consensus criteria was 0.57 ± 0.02. Of the 438 patients with a physician’s diagnosis of severe sepsis, only 69 % (301 of 438) would have been eligible to participate in a clinical trial of pediatric severe sepsis that enrolled patients based on consensus criteria. Patients with physician-diagnosed severe sepsis who did not meet consensus criteria were younger and had lower severity of illness and lower PICU mortality than those meeting consensus criteria or both definitions. After controlling for age, severity of illness, number of comorbid conditions, and treatment in developed versus resource-limited regions, patients identified with severe sepsis by physician diagnosis alone or by consensus criteria alone did not have PICU mortality significantly different from that of patients identified by both physician diagnosis and consensus criteria. Conclusions Physician diagnosis of pediatric severe sepsis achieved only moderate agreement with consensus criteria, with physicians diagnosing severe sepsis more broadly. Consequently, the results of a research study based on consensus criteria may have limited generalizability to nearly one-third of PICU patients diagnosed with severe sepsis.

We study the connections between the Kadec-Klee property for local convergence in measure H`, the Kadec-Klee property for global onvergence in measure Hg and the ∆2-condition for Orlicz function spaces Lϕ equipped with either the Luxemburg norm k · kϕ or the Orlicz norm k · k0 ϕ. Nominally, we prove that for (Lϕ, k · kϕ) the conditions: ϕ satisfies an appropriate ∆2-condition and Lϕ ∈ H`, Lϕ ∈ Hg are equivalent, although Lϕ ∈ Hg is not equivalent to Eϕ ∈ Hg. In contrast, we also prove that, in the case of a non-atomic infinite measure space, properties H` and Hg for (Lϕ, k · k0 ϕ) do not coincide. More precisely, we prove that if ϕ vanishes only at zero, then both these properties coincide and they are equivalent to ϕ ∈ ∆2. However, if ϕ vanishes outside zero, then (Lϕ, k · k0 ϕ) ∈ Hg if and only if ϕ ∈ ∆2(∞). Since in the last case (Lϕ, k k0 ϕ) is not order continuous, properties H` and Hg differ. Analogous results are also proved for the subspace Eϕ of Lϕ. It is also worth mentioning that the criteria for Eϕ ∈ H` as well as for Eϕ ∈ Hg were not previously known. It follows from the criteria that the appropriate regularity ∆2-condition for ϕ is necessary for Eϕ ∈ H`, E ϕ 0 ∈ H`, Eϕ ∈ Hg and E ϕ 0 ∈ Hg although these spaces are order continuous for any ϕ.

Oxidation of carbon based materials - Abstract
Following an overview on kinetics of carbon/gas-reactions, status and further needs in selected safety relevant fields of graphite oxidation in HTR and fusion reactors are outlined: Considering severe air ingress, lack of experimental data on Boudouard reaction and in the field of advanced oxidation kinetics was detected. Development of coatings, protecting against oxidation, has to focus on stability under neutron irradiation and on general feasability of coatings on HTR pebble fuel graphite. Oxidation in normal operation of direct-cycle HTR requires examinations on gas atmospheres and on catalytic effects. Advanced carbon materials like CFCs and mixed materials should be developed and tested with respect to their oxidation resistance in a common HTR/fusion task. In an interim HTR fuel storage radiolytic oxidation in normal operation and thermal oxidation in accidents have to be considered. Plans for future work in these fields are described.

- Abstract
High penetration of renewable technologies, especially of wind and solar photovoltaic, has added further requirements to the control and supervision of power systems. In particular, their dispersion and their non-manageable variability involve the need to increase the attention and readiness to act of power system operators. In order to cope with these challenges in an efficient manner, control centers must be equipped with tools that automate processes in order to aid operators. One example of such a control center is the Spanish TSO's CECRE, equipped with tools and communication capabilities to supervise all the commercial wind and 60% of the solar photovoltaic generation installed in the Spanish peninsular system. In this system wind energy accounted for 16% and solar for 2.5% of the total electrical energy produced in the year 2010 and is one of the systems in the world with the highest non-manageable renewable energy penetration levels. In addition, the CECRE checks whether the real time generation scenario of these technologies is admissible and issues limitations as set-points to each single plant when the technical constraints detected can only be solved by means of reducing the non-manageable renewable generation. This paper describes the tools involved in this process and how the three presently possible types of constraints to integration are managed: congestions either in the transmission or distribution system, generation trips due to voltage dips and off-peak power balance feasibility.
