Objective: To determine the roles of psychologists in Spain who dedicate totally or partially to the study and treatment of pain, as well as their training, clinical assistance, research and teaching activities. Material and method: An observational, cross-sectional study was carried out based on two on-line surveys: one addressed to the head of the pain unit, and the other, to the psychologists. The first one requested information on the level of the unit, the typology and the number of patients attended, as well as the number and specialties of the professionals working within it. The perception of the need for a psychologist was also requested. The second survey, which was addressed to psychologists, required information on the scope and conditions in which they performed their professional work, the type of professional activity, their training and their teaching or research activities. Results: 72 out of the 187 surveys (39 %) sent to the heads of pain units responded. In addition, 27 responses from psychologists were obtained. After debugging the data, the sample finally consisted of 70 surveys of heads of pain units and 25 psychologists. According to the data, 29 % of the pain units had a psychologist. Among the units defined as multidisciplinary (47 %), 51 % reported to have a psychologist. Among the pain units that did not have a psychologist, 94 % considered this professional necessary. Psychologists who responded to the survey were on average almost 44 years old. They were mostly women (64 %) and had on average 8 years tenure working in pain. With regards to their training, 56 % had a specialty in clinical psychology and 28 % had a doctoral degree in psychology. 32 % reported no specific training in pain. 60 % of the psychologists were integrated into pain units, although mostly with part-time dedication. 28 % were working in the university and 20% of them collaborated with the pain units, in spite of having their professional activity in other care servi, Objetivo: Determinar los roles de los profesionales de la Psicología que en España se dedican de forma total o parcial al estudio y tratamiento del dolor, así como a su formación y actividad asistencial, investigadora y docente. Material y métodos: Estudio observacional, transversal, basado en dos encuestas online: la primera orientada al responsable de la unidad de dolor, y la segunda al profesional de la Psicología. En la primera se solicitaba información sobre el nivel de la unidad, la tipología y el número de pacientes atendidos, así como el número y la especialidad de los profesionales con que contaba. También se solicitaba la percepción sobre la necesidad de contar con un profesional de la psicología. En la segunda encuesta, dirigida a los psicólogos/as, se requería información sobre el ámbito y las condiciones en que realizaba su labor profesional, el tipo de actividad, su formación, así como la realización de actividades docentes o de investigación. Resultados: De las 187 encuestas enviadas a los responsables de las unidades de dolor que figuran en el directorio de la SED, se obtuvo respuesta de 72 (39 %). Asimismo, se obtuvieron 27 respuestas de profesionales de la Psicología. Tras depurar los datos, la muestra quedó finalmente formada por 70 encuestas de responsables de unidades de dolor y 25 psicólogos/as. El 29 % de las unidades disponían de psicólogo/a. Entre las unidades que se definían como multidisciplinares (47 %), el 51 % informaba disponer de psicólogo/a. Entre las unidades que no disponían de psicólogo/a, el 94 % consideraba necesaria esta figura profesional. Los psicólogos/as que respondieron a la encuesta tenían una edad media próxima a los 44 años, eran mayoritariamente mujeres (64 %) y llevaban poco más de 8 años trabajando en dolor. El 56 % disponía de la especialidad en Psicología Clínica y el 28 % tenía el grado de doctor en Psicología. El 32 % refería no tener formación específica en dolor. Introduction: A review of the CAD-R (Cuestionario de afrontamiento al dolor reducido) (1) is presented in a sample of 4.389 patients diagnosed with chronic pain derived from a spanish multicenter study (2). Material and method: The factorial structure is analyzed by exploratory factor analysis (EFA) and confirmatory (CFA), as well as reliability (internal consistency and test-retest at three months, the latter of which is 2.825 patients). Four models are tested proving that correlated first-order strategies are those that provide better adjustment indices. Results: Both internal consistency and test-retest indices are acceptable. Conclusion: CAD-R appears as an instrument that presents good reliability and a factorial structure of six first-order strategies., RESUMEN Introducción: Se presenta una revisión del CAD-R (Cuestionario de afrontamiento al dolor reducido) (1) en una muestra de 4.389 pacientes diagnosticados con dolor crónico derivada de un estudio multicéntrico de toda España (2). Material y método: Se analiza la estructura factorial mediante análisis factorial exploratorio (AFE) y confirmatorio (AFC), así como la fiabilidad (consistencia interna y test retest a los tres meses, esta última sobre 2.825 pacientes). Se ponen a prueba cuatro modelos constatando que las estrategias de primer orden correlacionadas son las que aportan mejores índices de ajuste. Resultados: Tanto los índices de consistencia interna como los test-retest resultan aceptables. Conclusiones: El CAD-R aparece como un instrumento que presenta una buena fiabilidad y una estructura factorial de seis estrategias de primer orden.

Introduction: Lattinen Index (IL) is an instrument of measurement widely used in the area of the treatment and investigation of the chronic pain. Nevertheless its source is confused, and the reliability and validity criteria for the use of this instrument with all the scientific guarantees, don’t exist. All that has generated the need of studies directed to provide information on the content of IL and its validity to allow the confirmation for the best use. Material and method: A sample of 112 subjects diagnosed of chronic pain was analyzed. On one hand (study 1) the relations between the IL, coping and quality of life were analyzed by means of a correlational analysis. Secondly (study 2) discriminant capacity of IL was analyzed by means of a cluster analysis between different groups of subjects. Finally, and by means of a regression analysis (study 3) the predictive capacity of quality of life of IL was analyzed. All these studies were analyzed by means of the utilization of the IL, the visual analogical scale (EVA) for the intensity of pain, the health questionnaire SF-36 as quality of life criterion and the coping questionnaire CAD-R to evaluate coping strategies. Results: The results show an index of internal consistency of alpha=0,56. Significant correlations appear fundamentally with quality of life variables (SF-36), as well as with the intensity of the pain (EVA), and with passive coping strategies (CAD-R). As for its discriminat capacity, different values appear depending on the IL items, differences being obtained depending on the symptomatology, pain type, the sex/gender and age. As for the predictive capacity the IL shows its capacity fundamentally of these items: intensity of pain, activity self-sufficiency, frequency of pain and sleep quality. Conclusions: In spite of showing a low index of reliability, the results of the study show the usefulness of the IL for the valuation of chronic pain patient, mainly due to its predictive capacity of different qu, Introducción: El Índice de Lattinen (IL) es un instrumento de medida ampliamente utilizado en el ámbito del tratamiento e investigación del dolor crónico. Sin embargo su origen es confuso, no existiendo criterios de fiabilidad y validez que permitan utilizar este instrumento con todas las garantías científicas. Todo esto ha generado la necesidad de estudios encaminados a proporcionar datos sobre el contenido del IL y su validez que permitan confirmar su adecuada utilización. Material y método: Mediante el análisis de una muestra de 112 sujetos diagnosticados de dolor crónico se analiza, por una parte (estudio 1), las relaciones entre el IL, el afrontamiento y la calidad de vida mediante un análisis correlacional de los datos. En segundo lugar (estudio 2) se analiza la capacidad discriminante del IL mediante un análisis cluster entre distintos grupos de sujetos. Por último, y a través un análisis de regresión (estudio 3), se analiza la capacidad predictiva de calidad de vida del IL. Todos estos estudios se realizaron mediante la utilización del IL, la Escala Visual Analógica (EVA) para la intensidad del dolor, el Cuestionario de Salud SF-36 como criterio de calidad de vida y el Cuestionario de Afrontamiento al Dolor CAD-R para evaluar las estrategias de afrontamiento. Resultados: Los resultados muestran un bajo índice de consistencia interna de alfa=0,56. Aparecen correlaciones significativas fundamentalmente con variables de calidad de vida (SF-36), así como con la intensidad del dolor (EVA) y con estrategias de afrontamiento pasivas (CAD-R). En cuanto a su capacidad discriminante aparecen distintos valores dependiendo de los ítems del IL, obteniéndose diferencias en función de la sintomatología, el tipo de dolor, el sexo/género y la edad. Objective: The objective of this research was to study the relations and interactions between cognitive assessment procedures and coping strategies proposed in the Lazarus and Folkman model (1984) as processes mediating the adaptation of patients to their pain, using as adaptation criteria the level of anxiety (STAI/R) and depression (BDI), in order to determine variables and coping strategies that predict a better adaptation to chronic pain, taking the variables separately. Material and methods: The population study included 168 patients with chronic pain attending the Multidisciplinary Pain Unit at the General University Hospital of Valencia. The patients answered to a first set of questionnaires used to record social and demographical data (age, sex, marital status, education level), clinical diagnosis and pain severity measured on a visual analogical scale (VAS). Afterwards, they answered to another set of questionnaires focused on the assessment of pain, such as pain assessment variables, pain coping strategies (PCS), and level of anxiety and depression measured on STAI/R and BDI, respectively. Results and conclusions: On a descriptive level and considering the whole study population, the results show that pain severity reported by patients ranged between moderate and severe, that patients perceived their pain as exceeding their own coping resources and that patients rated low their self-effectiveness in order to manage, control or relieve pain, this being a disabling and limiting factor in their everyday life. In our study population, the most frequently used strategies for coping with pain were self-assertion, search of information and religion, and their anxiety and depression scores were moderate or high. On a predictive level, both anxiety and depression can be mainly explained by assessment variables, being the emotional repression the most relevant variable for predicting anxiety and the feeling of loss the most relevant variable for predicting depressio, Objetivo: El objetivo de la investigación consiste en estudiar las relaciones e interacciones existentes entre los procesos de valoración cognitiva y estrategias de afrontamiento afrontamiento propuestos en el modelo de Lazarus y Folkman (1984) como procesos mediadores en el ajuste de los pacientes su dolor, utilizando como criterio de adaptación el nivel de ansiedad (STAI/R) y depresión (BDI), con la finalidad de poder determinar qué variables de valoración y qué estrategias de afrontamiento predicen un mejor ajuste al dolor crónico, tomando las variables criterio por separado. Material y métodos: La muestra del estudio está compuesta por 168 pacientes con dolor crónico que acuden a la Unidad Multidisciplinar del Dolor en el Hospital General Universitario de Valencia. Los pacientes completaron una bateria de cuestionarios donde, en una primera parte se recogían los datos sociodemográficos (edad, sexo, estado civil, nivel de estudios), el diagnóstico clínico, y el nivel de dolor a través del EVA (escala visual analógica). Posteriormente completaban un conjunto de cuestionarios centrados en la evaluación de su dolor como son las variables de valoración del dolor del paciente, las estrategias de afrontamiento al dolor (CAD), y el nivel de ansiedad rasgo y depresión, evaluados a partir del STAI/R y del BDI, respectivamente. Resultados y conclusiones: A nivel descriptivo, y considerando a la totalidad de la muestra, los resultados muestran que el nivel de dolor informado por los pacientes oscila entre moderado e intenso, percibiendo que su dolor supera sus propios recursos para hacerle frente, y percibiéndose a sí mismos con una baja autoeficacia para manejar, controlar o disminuir el dolor, siendo este un factor incapacitante y limitante en su vida diaria. Las estrategias de afrontamiento ante el dolor más utilizadas por la muestra han sido la autoafirmación, la búsqueda de información y la religión, mostrando puntuaciones en ansiedad y depresión moderadamente elevadas

Introduction: The development of the science depends on two dimensions that interact between the development of ideas, and the social interaction of those who develop them. There are many the researches that analyze the first dimension, but there are limited the studies that approach the second one. In the area of the study of pain these studies are non-existent. Coinciding with the tenth anniversary of the Journal of the Spanish Society of Pain (Revista de la Sociedad Española del Dolor), our aim is analyze the relations of the groups of work that appear during this period in the mentioned journal, realizing a quantitative analysis of the authors who sign the works in it published between 1994 and 2003. Material and method: There have been analyzed the review articles, original articles, clinical notes, special collaborations, clinical cases and continued formation articles of the Journal of the Spanish Society of Pain (Revista de la Sociedad Española del Dolor) between 1994 and 2003. A quantitative analysis of the signatories and of the articles in the studied period and of the groups of work and the invisible colleges was realized. Results and conclusions: They are 520 works the included ones in our study, signed by a whole of 1172 authors. The average of signatures for article is 3.5. Analyzing the relations between authors arise 5 invisible colleges that publish in the studied period more than 10 articles. More than 75% of the authors are considered to be transients since only they sign a unique article in the mentioned period. The results are analyzed and discussed., Introducción: El desarrollo de la ciencia depende de dos dimensiones que interactúan entre sí, el desarrollo de ideas, y la interacción social de quienes las desarrollan. Muchas son las investigaciones que analizan la primera dimensión, pero son limitados los estudios que abordan la segunda. En el ámbito del estudio del dolor estos estudios son inexistentes. Coincidiendo con el décimo aniversario de la Revista de la Sociedad Española del Dolor, nuestro objetivo es analizar las relaciones de los grupos de trabajo que aparecen durante ese periodo en la mencionada revista, realizando un análisis cuantitativo de los autores que firman los trabajos en ella publicados entre 1994 y 2003. Material y método: Se han analizado los artículos de revisión, originales, notas clínicas, colaboraciones especiales, casos clínicos y los artículos incluidos en formación continuada de la Revista de la Sociedad Española del Dolor entre 1994 y 2003, realizándose un análisis cuantitativo de los firmantes y de los artículos en el periodo estudiado y de los grupos de trabajo y los colegios invisibles. Resultados y conclusiones: Son 520 trabajos los incluidos en nuestro estudio, firmados por un total de 1172 autores. La media de firmas por artículo es de 3,5. Analizando las relaciones entre los autores surgen 5 colegios invisibles que publican en el periodo estudiado más de 10 artículos. Más del 75% de los autores son considerados transeúntes ya que sólo firman un único artículo en el periodo mencionado. Se analizan y discuten los resultados.

Fibromyalgia is a chronic and complex pathology that provokes muscular pain which may become invalidant, associated to a badly night rest and fatigue that affects the biological, psychological and social environment of the patients. Its high prevalence makes fibromyalgia a first magnitude sanitary problem. The fact that its diagnostic criteria is only clinical, and that its aetiopathogenesis has not yet been clarified. This makes very difficult the study and therapeutical approach of the disease. The multidisciplinary approach for its treatment is very important, against the traditional biomedical approach, because of the high complexity of the patients. In this review, we try to bring together the current knowledgements in the literature, though it has to be highlighted that many studies and medical and paramedical references approach the topic with complete scientific rigour., La fibromialgia es una patología crónica y compleja que provoca dolor muscular generalizado que puede llegar a ser invalidante, asociado a mal descanso nocturno y fatigabilidad, y que afecta a las esferas biológica, psicológica y social de los pacientes. Además su elevada prevalencia hace de ella un problema sanitario de primera magnitud. Dificultad añadida supone el que sus criterios diagnósticos únicamente sean clínicos y que su etiopatogenia todavía no haya sido aclarada, lo que dificulta aún más su estudio y por supuesto su abordaje terapéutico. En su tratamiento resulta fundamental el abordaje multidisciplinar en contraposición a un abordaje biomédico tradicional, dada la enorme complejidad que suelen presentar estos pacientes. En esta revisión intentamos aunar los conocimientos actuales en la literatura médica aunque hay que resaltar que diariamente multitud de estudios y referencias médicas y paramédicas abordan el tema con mayor o menor rigor científico.

Objective: The objective of this research focused on the study of coping strategies used by patients to face pain, these being defined as the thoughts and actions taken by individuals in their efforts to handle pain and their impact on the patient quality of life. Material and methods: The study population included 32 women diagnosed of fibromyalgia that attended for the first time the Psychology Service of the Multidisciplinary Pain Unit at the Consorcio Hospital General Universitario de Valencia prior medical examination. The patients completed a set of questionnaires that, in addition to obtain sociodemographic variables (age, sex, marital status, educational level), the following was assessed: pain severity determined using a VAS (visual analogic scale), pain valuation (for example, "pain depends on me", "I can anticipate when it is going to hurt"...), pain coping strategies based on the pain coping questionnaire (PCQ) and the quality of life questionnaire ICV-2R. Results and conclusions: At the descriptive level, the results show that the patients reported a moderately severe pain, being self-assertion and information seeking the pain coping strategies more frequently used. Furthermore, they presented a quality of life index lower than the general population. At the correlational level, no significant relationship has been found between the general quality of life index and the pain coping strategies, although the use of more passive coping strategies (such as religion) are significantly and positively associated to dimensions of quality of life that involve certain degree of maladjustment of the patient to the disease, while the use of more active pain coping strategies (such as distraction and mental self-control) are significantly and negatively associated to such dimensions., Objetivo: El objetivo de la investigación se centra en el estudio de las estrategias de afrontamiento utilizada por el paciente para hacer frente a su dolor, siendo definidas estas como los pensamientos y acciones que los individuos realizan en sus esfuerzos por manejar el dolor, y su repercusión sobre la calidad de vida del paciente. Material y métodos: La muestra del estudio está formada por 32 mujeres diagnosticadas de fibromialgia que acuden por primera vez al Servicio de Psicología de la Unidad Multidisciplinar del Dolor en el Consorcio Hospital General Universitario de Valencia previa exploración médica. Los pacientes completaron una batería de cuestionarios donde además de recoger las variables sociodemográficas (edad, sexo, estado civil, nivel de estudios), se evaluó el nivel de dolor informado a través del EVA (escala analógica visual), valoración del dolor (por ejemplo, "el dolor depende de mí", "puedo anticipar cuándo me va a doler"...), las estrategias de afrontamiento al dolor a partir del cuestionario de afrontamiento al dolor (CAD) y el cuestionario de calidad de vida ICV-2R. Resultados y conclusiones: A nivel descriptivo, los resultados muestran que las pacientes informan de un nivel de dolor moderadamente elevado, siendo las estrategias de afrontamiento al dolor más utilizadas, la autoafirmación y la búsqueda de información, y presentando un índice de calidad de vida inferior al de la población general. A nivel correlacional, no se ha encontrado ninguna relación significativa entre el índice general de calidad de vida y las estrategias de afrontamiento al dolor, aunque el uso de estrategias de afrontamiento más bien pasivas (como por ejemplo, la religión) se asocian significativa y positivamente con la dimensiones de calidad de vida que implican cierto grado de desadaptación por parte del paciente a la enfermedad, mientras que el uso de estrategias más activas ante el dolor (por ejemplo, distracción y autocontrol mental) se relacionan significativa

Uno de los tópicos de mayor relevancia en las últimas décadas en el estudio del paciente con dolor crónico ha sido el afrontamiento. Dada la importancia de medir y evaluar las diferentes maneras con las que el individuo afronta el dolor, en las últimas décadas se han desarrollado diferentes instrumentos para evaluar dicha dimensión. El cuestionario de afrontamiento al dolor (CAD), inicialmente compuesto por 31 ítems, incluye 6 factores: autoafirmación, búsqueda de información, religión, distracción, catarsis y autocontrol mental. En este estudio se reduce el CAD, proponiendo una versión más corta del mismo, el CAD-R. Se evaluaron 314 sujetos por el Servicio de Psicología de la Unidad Multidisciplinar para el Tratamiento del Dolor del Consorcio Hospital General Universitario de Valencia. Del cuestionario original de 31 ítems (CAD) se eliminaron 7 ítems reduciéndose a una versión con un total de 24 ítems (CAD-R). Se evalúa la validez de esta nueva versión, su consistencia interna y estabilidad temporal, así como las relaciones entre afrontamiento y afecto para determinar las estrategias más adaptativas., Coping has been turned over the last decades in one of the most important topics to board the patient with chronic pain. Given the importance of considering and evaluating the different forms that the individual has to cope with pain, different instruments in the last decades have been developed to evaluate the above mentioned dimension. The coping pain questionnaire (in Spanish, CAD), initially composed by 31 items, gathers 6 factors: autoaffirmation, Look for information, religion, distraction, catharsis and mental autocontrol. In this study the length of the CAD diminishes, proposing a small version of the same (CAD-R). 364 subjects were evaluated by the psychology service of the Multidisciplinary Pain Treatment Unit of the Valencia University General Hospital. Of the original questionnaire of 31 items (CAD), there was eliminated a whole of 7 items remaining reduced to a whole of 24 items (CAD-R). Assessing the validity of this new version designed for chronic pain patients with a sample of Spanish population, its consistence and stability are analyzed again, and the relationships between coping and affect are defined to determine the most adaptative coping skills.
