Abstract

BACKGROUND AND OBJECTIVES: The inability of individuals in the advanced stage of dementia to communicate about preferences in care at the end-of-life poses a challenge for healthcare professionals and family carers. The proven effective Family Carer Decision Support intervention has been designed to inform family carers about end-of-life care options available to a person living with advanced dementia. The objectives of the mySupport study were to adapt the application of the intervention for use in different countries, assess impact on family satisfaction and decision-making, and identify costs and supportive conditions for the implementation of the intervention. RESEARCH DESIGN AND METHODS: A multiple-case study design was chosen where the nursing home was the case. Nursing homes were enrolled from six countries: Canada, Czech Republic, Italy, Netherlands, Republic of Ireland, and United Kingdom. RESULTS: Seventeen cases (nursing homes) participated, with a total of 296 interviews completed including family carers, nursing home staff, and health providers. Five themes relevant to the implementation of the intervention were identified: supportive relationships; committed staff; perceived value of the intervention; the influence of external factors on the nursing home; and resource impact of delivery. DISCUSSION AND IMPLICATIONS: There is a commonality of facilitators and barriers across countries when introducing practice innovation. A key learning point was the importance of implementation being accompanied by committed and supported nursing home leadership. The nursing home context is dynamic and multiple factors influence implementation at different points of time.

10. Increasing pre, peri and postmenopausal women's activity levels : a theory of planned behaviour approach to a behaviour change intervention

Abstract

Background: Given that the menopause brings additional risk of non-communicable diseases preventable by undertaking physical activity, action is needed to increase the proportion women undertaking recommended levels of physical activity as they transition from pre- to post-menopause. The aim of this thesis was to develop and test the feasibility and acceptability of an intervention to promote recommended levels of physical activity among pre-, peri- and post-menopausal women using a two component model of the Theory of Planned Behaviour. Methods: This study employed a mixed method multiphase research design and as such three phases of research guided by the Theory of Planned Behaviour were undertaken: (1) the cognitive processes underlying pre-, peri- and post-menopausal women’s decision to perform recommended levels of moderate physical activity and muscle strengthening activities were explored using qualitative methodology and analysed using thematic analysis; (2) significant predictors of intention and the relative importance of those predictors were explored using quantitative methodology. Descriptive statistics, bivariate correlations, regression analyses and analysis of variances; were used to analysis the data and: (3) a mixed methods study involving a randomised controlled trial and a qualitative evaluation was undertaken to determine the feasibility and acceptability of an intervention designed based on the findings from phase one and two. Descriptive statistics, independent samples T-Tests, mixed within and between subjects repeated measures analysis of variance were used to analysis the quantitative data, qualitative data was analysed using thematic analysis. Results: Phase one: Attitudes toward undertaking moderate physical activity were positive across groups however, motivational differences were apparent. Attitudes vi towards undertaking muscle strengthening activities were negative and knowledge was identified as the main factor that influenced women’s attitudes and motivation towards undertaking this mode of physical activity. Phase two: The results showed that selfefficacy followed by affective attitude and descriptive norms were the main predictors of women’s intentions to undertake moderate physical activities. With regards to muscle strengthening activities, self-efficacy followed by affective attitudes were the main predictors of intention. No significant differences between groups were evident. However, belief based measures supported the findings from phase one illustrating slight differences across groups. Phase three: The results showed that the intervention was feasible and acceptable to pre- and post-menopausal women. However, the results indicated that the acceptability of the intervention could be improved by removing or altering the reward, monitoring of emotional consequences and practical social support elements of the intervention. Conclusions: Overall, the findings from this thesis provide support for the feasibility and acceptability of an intervention based on the Theory of Planned Behaviour. However, the intervention should be refined and a larger scaled evaluation undertaken to determine the effectiveness of this intervention.

15. Menopause as a window of opportunity: the benefits of designing more effective theory-driven behaviour change interventions to promote healthier lifestyle choices at midlife.

Abstract

Menopause is a natural developmental phase that all women go through from their early forties to mid-fifties, marking the transition from the reproductive to the non-reproductive years. This is characterised as the permanent cessation of menses due to progressive ovarian failure. Each woman's experience of the menopause is unique. Biopsychosocial changes occur during this time with some symptoms affecting up to 80 % of women and lasting for 4–5 years from the peri- to post-menopause. Reduced oestrogen may predispose some women to health issues following menopause, such as heart disease, diabetes, stroke and cognitive decline. It is vital to understand how to promote health and well-being to reduce the risk of developing chronic conditions in later life. Increased symptoms and concerns about health during the menopausal transition can be cues to action for some women to actively maintain their health. Menopause represents a window of opportunity to promote health, and to support women to make healthier lifestyle choices, part of the National Institute for Clinical Excellence guidelines for menopause management. Identifying appropriate strategies to change behaviour is less clear. Theories of behaviour change can provide frameworks to gain more insight into the facilitators and barriers to behaviour and can inform the researcher on what needs to change. This information can be used to inform the design, content, implementation and evaluation of a lifestyle intervention. This review paper will explore the impact of menopause on health and well-being generally, and the benefits of designing more effective theory-driven behaviour change interventions for menopause. Abstract

This paper reports the quantitative findings of a study on conceptions of positive career outcomes held by Chinese students who returned home to work after gaining higher education qualifications in Australia. The study explores what positive career outcomes mean to Chinese returnees, the actual positive career outcomes Chinese returnees have experienced and contributing factors to the positive career outcomes achieved. Both quantitative and qualitative research methods are used and here the analysis of the quantitative findings is presented. The survey data show that Chinese returnees assign greater importance to 'soft outcomes' relating to personal and life fulfilment than 'hard outcomes' such as employment results. Also, the discrepancy between the returnees' ideal and actual outcomes is greater in the soft outcomes than the hard outcomes. Statistical analysis indicates three emergent, distinct groups of conceptions of positive career outcomes. The qualitative findings will be presented separately.

Abstract

Objective: The objective of this scoping review is to identify and explore available evidence for the efficacy of treatment models integrating substance use disorder (SUD) treatment and obstetric care, compared to a non-integrated models where treatment and obstetric care occur in separate contexts. Introduction: Prior research has demonstrated the superiority of treatment for SUD during pregnancy for improving both perinatal and SUD outcomes, in comparison to not receiving SUD treatment. The benefits of engagement in prenatal care for people with SUD are also well studied. However, fewer studies have compared the effectiveness of different models of SUD treatment delivery during pregnancy, and no clear methodology for studying this topic has been described. Inclusion criteria: Inclusion criteria include peer reviewed, published papers comparing integrated with non-integrated treatment for SUD, excluding treatment for tobacco and cannabis use disorders, for pregnant people in an outpatient treatment setting; and explore at least one outcome of interest including perinatal complications, prematurity, engagement in obstetric care, retention in SUD treatment, neonatal or pediatric outcomes, or maternal mortality. Methods: The proposed review will follow Arksey’s and O’Malley’s (2005) framework for scoping reviews1 with an enhanced approach recommended by Daudt2 and will be conducted in accordance with the PRISMA Extension for Scoping Reviews (PRISMA-ScR).3 A comprehensive search of MEDLINE, PsychInfo, CINAHL, the Cochrane Library, Sociological Abstracts, Scopus and Web of Science will be conducted. Full text papers will be validated and abstracted by two independent reviewers, using a standardized tool, and data extracted will be analyzed descriptively.

Abstract

Background Advance care planning in nursing homes is important to ensure the wishes and preferences of residents are recorded, especially during the COVID-19 pandemic. However, care staff and family members frequently report feeling unprepared for these conversations. More resources are needed to support them with these necessary discussions. This research aimed to develop, implement and evaluate a website intervention for care staff and family members to provide training and information about advance care planning during COVID-19. Methods The research was a primarily qualitative case study design, comprising multiple UK nursing home cases. Data collection included semi-structured interviews with care staff and family members which were coded and analysed thematically. A narrative synthesis was produced for each case, culminating in a thematic cross-case analysis of the total findings. Theoretical propositions were refined throughout the research. Results Eight nursing homes took part in the study, involving 35 care staff and 19 family members. Findings were reported according to the RE-AIM framework which identified the reach, effectiveness, adoption, implementation and maintenance of the intervention. Themes included: website content that was well received; suggestions for improvement; implementation barriers and facilitators; examples of organisational and personal impact. Conclusions Four theoretical propositions relating to advance care planning in nursing homes are presented, relating to: training and information needs, accessibility, context, and encouraging conversations. Implications for practice and training include an awareness of diverse learning styles, re-enforcing the right to be involved in advance care planning and encouraging opportunities for facilitated discussion.

Abstract

Aim: This study aimed to identify early clinical biomarkers from birth to 16 weeks corrected age to predict typical outcome and developmental delay in infants born very preterm or with very low birthweight. Method: A prospective cohort of infants on the Sunshine Coast, Australia, was assessed using the Premie‐Neuro Examination, the General Movement Assessment (GMA), the Alberta Infant Motor Scale, and the Infant Sensory Profile 2. At 24 months corrected age, delay was identified using the Bayley Scales of Infant and Toddler Development, Third Edition (Bayley‐III) and Neurosensory Motor Developmental Assessment (NSMDA). Results: One hundred and four infants were recruited; 79 completed outcome assessments (43 females, 36 males; mean gestational age 30 weeks [SD 1 week 6 days], mean birthweight 1346 g [SD 323]). The incidence of developmental delay (motor or cognitive) was 6.3%. Suboptimal quality of fidgety general movements (temporal organization) at 16 weeks corrected age demonstrated the best predictive accuracy (Bayley‐III motor: sensitivity 100% [95% confidence interval {CI} 3–100], specificity 75% [95% CI 63–84], area under the curve [AUC] 0.87); Bayley‐III cognitive: sensitivity 100% [95% CI 3–100], specificity 75% [95% CI 64–84], AUC 0.88); NSMDA motor: sensitivity 100% [95% CI 40–100], specificity 81% [95% CI 70–90], AUC 0.91 [95% CI 0.86–0.95]). GMA trajectories that combined abnormal writhing general movements at 4 to 5 weeks corrected age with suboptimal quality of fidgety movement at 16 weeks corrected age were strongly predictive of developmental delay, superior to all other clinical tools, and perinatal and demographic variables investigated (p = 0.01, Akaike information criterion method 18.79 [score corrected for small sample size], accounting for 93% of the cumulative weight). Interpretation: Only the GMA had sufficient predictive validity to act as a biomarker for both conditions: typical outcome and developmental delay (motor or cognitive). GMA trajectories that assessed both writhing general movements at 4 to 5 weeks corrected age and quality of fidgety movement at 16 weeks corrected age predicted adverse neurodevelopmental outcome, accurately differentiating between infants with typical outcomes and those at increased risk for motor or cognitive delay. Abstract

Background: Nursing home residents are typically older adults with high levels of chronic illness and impairment. As such, they are particularly susceptible to severe complications and mortality from COVID-19. Since all nursing home residents are at increased risk, nursing home care staff need to know what residents would want to happen should they become infected with COVID-19. This study aims to develop and evaluate advance care planning (ACP) COVID-centric online training and information resources for nursing home staff and family members of residents, to improve care at the end of life during a COVID-19 outbreak. Based on the findings we will develop implementation guidelines for nursing homes to ensure wider impact and application during the pandemic and beyond. Methods: The content of the training and information resources will be based on a rapid review of literature and guidance on ACP in the context of COVID-19 and consultation with the study expert reference group. An integrated communications company will then work alongside the research team to design the online training and information resources. To evaluate the resources, we will employ a multiple case study design where a nursing home (defined as an institutional setting in which nursing care is provided to older adults on-site 24 hours a day) will be the unit of analysis or ‘case’. The RE-AIM (reach, effectiveness, adoption, implementation, maintenance) framework will guide the evaluation of implementation of the training and information resources. We will recruit and interview staff and family members from between 6-9 nursing homes across Northern Ireland, England and Scotland and gather quantitative data from a feedback survey included in the training and information resources. Abstract

Background Levels of physical activity decline with age. Some of the most disadvantaged individuals in society, such as those with a lower rather than a higher socioeconomic position, are also the most inactive. Peer-led physical activity interventions may offer a model to increase physical activity in these older adults and thus help reduce associated health inequalities. This study aims to develop and test the feasibility of a peer-led, multicomponent physical activity intervention in socioeconomically disadvantaged community-dwelling older adults. Objectives The study aimed to develop a peer-led intervention through a rapid review of previous peer-led interventions and interviews with members of the target population. A proposed protocol to evaluate its effectiveness was tested in a pilot randomised controlled trial (RCT). Design A rapid review of the literature and the pilot study informed the intervention design; a pilot RCT included a process evaluation of intervention delivery. Setting Socioeconomically disadvantaged communities in the South Eastern Health and Social Care Trust and the Northern Health and Social Care Trust in Northern Ireland. Participants Fifty adults aged 60–70 years, with low levels of physical activity, living in socioeconomically disadvantaged communities, recruited though community organisations and general practices. Interventions ‘Walk with Me’ is a 12-week peer-led walking intervention based on social cognitive theory. Participants met weekly with peer mentors. During the initial period (weeks 1–4), each intervention group participant wore a pedometer and set weekly step goals with their mentor’s support. During weeks 5–8 participants and mentors met regularly to walk and discuss step goals and barriers to increasing physical activity. In the final phase (weeks 9–12), participants and mentors continued to set step goals and planned activities to maintain their activity levels beyond the intervention period. The control group received only an information booklet on active ageing. Main outcome measures Rates of recruitment, retention of participants and completeness of the primary outcome [moderate- and vigorous-intensity physical activity measured using an ActiGraph GT3X+ accelerometer (ActiGraph, LLC, Pensacola, FL, USA) at baseline, 12 weeks (post intervention) and 6 months]; acceptability assessed through interviews with participants and mentors. Results The study planned to recruit 60 participants. In fact, 50 eligible individuals participated, of whom 66% (33/50) were female and 80% (40/50) were recruited from general practices. At 6 months, 86% (43/50) attended for review, 93% (40/43) of whom returned valid accelerometer data. Intervention fidelity was assessed by using weekly step diaries, which were completed by both mentors and participants for all 12 weeks, and checklists for the level of delivery of intervention components, which was high for the first 3 weeks (range 49–83%). However, the rate of return of checklists by both mentors and participants diminished thereafter. Outcome data indicate that a sample size of 214 is required for a definitive trial. Limitations The sample was predominantly female and somewhat active. Conclusions The ‘Walk with Me’ intervention is acceptable to a socioeconomically disadvantaged community of older adults and a definitive RCT to evaluate its effectiveness is feasible. Some modifications are required to ensure fidelity of intervention delivery is optimised. Future research needs to identify methods to recruit males and less active older adults into physical activity interventions.

Abstract

Tactical decision making occurs in environments characterized by high uncertainty, high-valued outcomes, and time stress. The critical relation of decision outcomes to uncertain events pushes decision makers to resolution of uncertainty. Time stress, however, works in the opposite direction, precluding ideal planning efforts. It is thus crucial to systematically characterize the impacts of uncertainty and time stress on tactical decision making to facilitate the design of decision aids that will be effective in the environments described. The present study employs a theoretical framework that extends Kahneman and Tversky's prospect theory (1979) to goal-directed decisions such as those involved in battlefield tactical decisions. The framework is used to provide a basis for characterizing the critical issues involved in such environments and to design decisions aiding concepts based on studies employing the framework. Four studies were conducts. The first study provided participants with choices involving gains and losses in goal-directed tactical and nontactical situations. The purpose was to examine preferences among risky options and to validate the extended theory as an appropriate characterization of the tactical decision making process. The results strongly confirmed the theoretical formulation. The second study involved a more rigorous test of the framework using a more realistic tactical problem that provided both for a test of the framework and development of concepts for aiding decisions under time stressed conditions. Tradeoffs between effort allocated to uncertainty reduction and option generation were examined in detail, and aid concepts were elicited. Study two results were used to design aiding concepts that were further refined and tested in study three to yield a final set of aiding concepts to be tested in study four.

Abstract

Background and Objectives: The inability of individuals in the advanced stage of dementia to communicate about preferences in care at the end-of-life poses a challenge for healthcare professionals and family carers. The proven effective Family Carer Decision Support intervention has been designed to inform family carers about end-of-life care options available to a person living with advanced dementia. The objectives of the mySupport study were to adapt the application of the intervention for use in different countries, assess impact on family satisfaction and decision-making, and identify costs and supportive conditions for the implementation of the intervention. Research Design and Methods: A multiple-case study design was chosen where the nursing home was the case. Nursing homes were enrolled from six countries: Canada, Czech Republic, Italy, Netherlands, Republic of Ireland, and United Kingdom. Results: Seventeen cases (nursing homes) participated, with a total of 296 interviews completed including family carers, nursing home staff, and health providers. Five themes relevant to the implementation of the intervention were identified: supportive relationships; committed staff; perceived value of the intervention; the influence of external factors on the nursing home; and resource impact of delivery. Discussion and Implications: There is a commonality of facilitators and barriers across countries when introducing practice innovation. A key learning point was the importance of implementation being accompanied by committed and supported nursing home leadership. The nursing home context is dynamic and multiple factors influence implementation at different points of time.
