Abstract

A case study of an 85-year old woman with atrial fibrillation is examined. A pacemaker was inserted in the patient following dysrhythmia and stroke symptoms. Evaluation of pacemaker functioning was made with electrocardiograms and consideration of the placement of the pacer wires, insertion time, and location of pacer spikes. Though initially the patient continued to exhibit atrial fibrillation, her condition improved with ongoing anticoagulant drug therapy.

21. Long-term evaluation of cardiac function in obese patients treated with a very-low-calorie diet: a controlled clinical study of patients without underlying cardiac disease

Abstract

Very-low-calorie diets (VLCDs) are used for treating obese people who were unable to lose weight while following a less extreme diet restriction. VLCDs are low in fat and high in protein, providing 45 to 100 grams protein per day in 400 to 800 calories. The average weekly weight loss on this type of diet ranges from three to four and a half pounds. Some of the early VLCDs, available in 1976 and 1977, were associated with fatal heart disorders because of protein, vitamin, and mineral deficiencies, and prolonged periods of dieting. The VLCDs that are available today are high in protein, vitamins and minerals, and studies have indicated that these diets do not cause heart problems. However, these studies were limited to a period of seven weeks or less, and VLCDs are usually followed for 12 to 16 weeks. Therefore, in the current study, the effect of a VLCD on the heart was determined over a 16-week period in 12 obese women, with no history of heart disease, who followed a 420 calorie/day liquid diet for 16 weeks and then were switched to a 1,200 calorie/day balanced diet for the remainder of the study period (until 45 weeks). A different group of eight obese women followed a 1,200 calorie/day balanced diet, and six others did not follow any form of diet. Electrocardiograms were performed prior to the beginning of the study and again at weeks 3, 9, 13, 17, 19 and 45, in order to detect changes in the electrical activity of the heart. The average weight loss during the 16-week VLCD was 47 pounds. During the same period, those following the balanced diet lost an average of 23 pounds and those who were not dieting gained an average of 6 pounds. There were no symptoms or abnormalities in heart function during the study period. It is concluded that VLCDs, when used for up to 16 weeks under appropriate medical supervision, provide a safe and effective means for weight loss in obese patients who do not have a prior history of heart disease. Abstract

Interleukin-2 (IL-2) is a naturally occurring protein which is made by white blood cells (leukocytes) and which improves the response of leukocytes to infection. IL-2 is being studied as a possible treatment for cancer, and several side effects from its use have been reported. A case is reported in which a 63-year-old woman being treated with IL-2 for kidney cancer developed dysfunction in electrical conduction in the heart. Following six treatments with IL-2, the patient developed heart arrhythmias, which developed into heart block, in which the upper and lower chambers beat independently rather in the normal coordinated fashion. The basis for the reaction is unclear, but the study suggests that heart complications of IL-2 treatment may be common, and suggest that potential candidates for IL-2 therapy should be screened for underlying heart disease. Abstract

Anticoagulation for atrial fibrillation has become more complex due to the introduction of new anticoagulant agents, the number and kinds of patients requiring therapy, and the interactions of those patients in the matrix of care. The management of anticoagulation has become a "team sport" involving multiple specialties in multiple sites of care. The American College of Cardiology, through the College's Anticoagulation Initiative, convened a roundtable of experts from multiple specialties to discuss topics important to the management of patients requiring anticoagulation and to make expert recommendations on issues such as the initiation and interruption of anticoagulation, quality of anticoagulation care, management of major and minor bleeding, and treatment of special populations. The attendees continued to work toward consensus on these topics, and present the key findings of this roundtable in a state-of- the-art review focusing on the practical aspects of anticoagulation care for the patient with atrial fibrillation.

Abstract

Cardiovascular disease continues to be the leading cause of death in women over age 50 in the United States. Significant differences exist between men and women in the prevalence, incidence, and treatment outcomes of cardiovascular disease, particularly coronary artery disease. The presenting features of coronary artery disease may vary between men and women, and the diagnostic evaluations of such symptoms may differ based solely on gender. Outcomes of angioplasty and bypass surgery, although not as extensively studied in women as in men, may be less favorable. The impact of risk factor modification may also differ due to the effects of menopause on cardiovascular events. More studies of cardiovascular outcomes in women are required, particularly now that hormone replacement therapy has been shown to increase cardiovascular event rates in most of the recent trials. Until such studies are available, our focus should therefore remain on treatment of risk factors where such treatment has clearly been shown to impact favorably on cardiovascular morbidity and mortality. Abstract

We have evaluated the influence of several factors on the binding of disopyramide to protein in human serum using a new ultrafiltration system and the enzyme multiplied immunoassay technique (EMIT) for disopyramide immuno-assay. From these studies and those of other investigators, we conclude that the most significant in vivo factors influencing disopyramide binding in human serum are total disopyramide concentration, the concentration of α4-acid glycoprotein, and the concentration of mono-N-dealkyldisopyramide. Significant variation in binding occurs in patients whose total disopyramide concentration is within the therapeutic range. The proportion of free disopyramide ranged from 16% to 54% in the sera of SO cardiac patients whose total disopyramide concentration ranged from 0.5 to 5.8 mcg/ml. Since there is so much variation in the proportion of disopyramide that is free, and since it is the unbound form of the drug that is pharmacologically active, it is likely in patients whose arrhythmia responds to disopyramide therapy that unbound drug concentration will be a better indicator of drug efficacy and toxicity than will be total drug concentration. To test this hypothesis, we are investigating the relationship between steady state total and unbound disopyramide plasma levels in patients with supraventricular or ventricular arrhythmias, and pharmacologic response as determined by standard intracardiac electrophysiologic studies. Abstract

The past decade has witnessed important advances in the understanding of the mechanism underlying ventricular arrhythmias. It has become clear that sustained ventricular arrhythmias can generally be reproduced with programmed ventricular stimulation in the clinical electrophysiology laboratory. False positive results may, however, occur with very vigorous stimulation techniques, particularly in patients without documented arrhythmias. False negative results are not infrequent in victims of cardiac arrest. Ability or inability to initiate ventricular tachycardia during acute and chronic drug testing has predicted clinical failure or success, at least for conventional antiarrhythmics. Patients with sustained ventriculartachycardia and cardiac arrest occurring outside the peri-infarction period are those most likely to benefit from study. Conventional antiarrhythmic agents are successful in about one-third of patients with a high degree of concordance among these drugs. Amiodarone is frequently effective in patients with drug-refractory ventricular arrhythmias. However, its efficacy cannot be predicted by programmed stimulation. 49. Validation of fourier amplitude ratio to quantitate valvular regurgitation

Abstract

It has previously been shown that patients with valvular regurgitation can be identified by the ratio of left and right ventricular amplitude values obtained from first harmonic Fourier analysis of the gated blood pool scan. The present study was designed to validate the accuracy of this technique for quantifying the amount of valvular regurgitation. In a blinded analysis of 19 patients who underwent cardiac catheterization, there was a close correlation between the radionuclide and hemodynamic determination of the amount of regurgitation (r=0.84). The interobserver agreement for calculating the radionuclide data was also high (r=0.88). These results suggest that the Fourier ventricular amplitude ratio is an accurate and reproducible technique for quantifying valvular regurgitation by gated blood pool scanning.

50. Limited role of intravenous propafenone hydrochloride in the treatment of sustained ventricular tachycardia: Electrophysiologic effects and results of programmed ventricular stimulation

Abstract

The electrophysiologic effects and response to programmed ventricular stimulation of intravenous propafenone, an experimental antiarrhythmic agent, were studied in a group of 14 patients with both clinical and induced sustained ventricular tachycardia. Twelve of the 14 patients had not responded to conventional antiarrhythmic drug therapy. Propafenone had no significant effect on sinus cycle length (836 ± 170 ms before and 750 ± 124 ms after propafenone), P wave duration (108 ± 24 ms before and 106 ± 23 ms after propafenone) or PR interval (181 ± 45 ms before and 194 ± 53 ms after propafenone). QRS duration and ventricular effective refractory periods increased significantly (109 ± 20 to 130 ± 21 ms and 235 ± 24 to 256 ± 19 ms, respectively).
