This study aimed to better demonstrate the long-term outcomes and effectiveness of crystallized phenol treatment (CPT) by presenting our 20 years experience. One thousand and twenty-six patients who have primer pilonidal sinus were enrolled. The operation was applied outpatient clinic under local anesthesia. Recurrence rate, application number, healing time, and factors affecting them were analyzed. The success rate was 84.3%. The mean number of CPT applications was 2.1 and the mean recovery time was 8.9 weeks. The rate of recurrence was higher in patients with a long disease duration prior to initiating treatment (p =.04). The rate of recurrence was higher in patients with a higher number of opening and CPT application, those with longer application time, and those with a positive family history (respectively p =.01, p =.002, p =.008). Long-term outcomes are also very successful in the treatment of pilonidal disease with CPT. We believe that CPT should be the first choice in the treatment of pilonidal disease because it is inexpensive, can be applied as an outpatient treatment, is a noninvasive procedure, has results comparable to surgical procedures, and does not cause loss of work and strength. In this study, the effects of melatonin (M) and lactulose (L) on the liver and kidneys were investigated by inducing OJ in 30 rats in five groups (n = 6): controls, sham, M, L, and M + L. In the treatment group, after the rats' biliary canals were tied and cut, 10 mg/kg M IM and 2 ml/day L p.o. was administered for 7 days. The histopathologic findings in the liver and kidneys, tissue malonyl dialdehyde (MDA) levels, and serum biochemistry were evaluated. In the M group, pathological histologic findings were less marked than in the other groups investigation of kidney cross-sections revealed no significant differences among groups. In the jaundiced rats liver MDA levels were significantly higher compared to the control group (P &lt 0.001), but no such difference was observed in kidney MDA levels (P &gt 0.05). L did not cause any significant changes in tissue MDA levels. There were no differences among groups with regard to serum levels of liver enzymes and bilirubin. Serum urea was significantly less in the group that received L (P &lt 0.001), but the groups showed no significant differences with respect to creatinine values (P &gt 0.05). The increase in serum total cholesterol was significantly less in the M + L group than in the other groups (P &lt 0.001). We conclude that in the rats in which obstructive jaundice was induced, M administration reduced liver and kidney injury, but L and M + L did not lead to significant improvement. Multiple treatment options for hidradenitis suppurativa (HS) have been suggested; none 100% successful. We assessed crystallized phenol treatment (CPT) for sacral HS. Twenty-five patients with sacral HS were enrolled. HS severity was assessed by Hurley Score and HS-Physician's Global Assessment (HS-PGA). All the leaking openings were dilated by a thin mosquito clamp after local anesthesia. Any hair present was removed, and phenol was applied. Patients were followed-up for a mean 36.8 months. The mean number of CPT application was 3.98. After CPT mean Hurley score decreased from 2.1 ± 0.68 to 1.2 ± 0.4 and HS-PGA score from 3 ± 1.22 to 0.72 ± 8.4 (p =.01). Recurrence was seen in 10 patients during follow-up period but they were re-treated with CPT. CPT may be useful as a first-line treatment of sacral HS. [ABSTRACT FROM AUTHOR]

In this article, it is claimed that Turkey's problematic situation before the ECHR is generally rooted in two main reasons: the problem of effectiveness and the problem of pluralism in the Turkish legal system. For the authors, the problem of effectiveness stems from the fact that investigations were not carried out effectively in cases of death and torture, especially involving public officials. In fact, this ineffectiveness leads to impunity and it gives a message to the security forces that they do need not hesitate to apply violence. On the other hand, the problem of pluralism arises from the disproportionate sanctions imposed on persons with opposing views, in the context of a democratic society's rights, in particular in the context of political freedom of expression. This disproportionate violence applied to people who criticise the government and intolerance gives a message to the society that they have to be prepared to get into hot water if they oppose state authority. In this article, the authors reveal the events involving the messages in question, through the "chilling effect" and "impunity" lenses, and question whether the Turkish Constitutional Court has contributed to these problems. In this respect, the article also examines whether there is a concordance between the judgments of the European Court of Human Rights and the Turkish Constitutional Court, which has been reviewing national individual applications since 2012. Nodular formation of the thyroid tissue can occur as a result of exposure to radiation. The nodular goiter is a common disease seen in the city of Elazig and its surroundings, in the eastern part of Turkey. A prospective study was conducted in an effort to identify the role of 129I in drinking water supply. Specimens obtained from nodular and normal thyroid tissue during surgery and also water specimens were counted by nuclear spectrometric system. 129I radioactivity in nodular tissue was noted to be higher compared to normal tissue and the difference was statistically significant (p < 0.05). There was no statistically significant difference (p > 0.05) between 129I radioactivity in the water supply and tissue obtained from the patients who have malignant or benign nodular lesions. These results support that the 129I radioactivity level in the water supply is one of the risk factors of the nodular formation of the thyroid tissue in the eastern part of Turkey. Aim: To share our experience of the management and outcomes of patients with pneumatosis cystoides intestinalis (PCI)., Methods: The charts of seven patients who underwent surgery for PCI between 2001 and 2009 were reviewed retrospectively. Clinical features, diagnoses and surgical interventions of patients with PCI are discussed., Results: Seven patients with PCI (3 males, 4 females; mean age, 50 ± 16.1 years; range, 29-74 years) were analyzed. In three of the patients, abdominal pain was the only complaint, whereas additional vomiting and/or constipation occurred in four. Leukocytosis was detected in four patients, whereas it was within normal limits in three. Subdiaphragmatic free air was observed radiologically in four patients but not in three. Six of the patients underwent an applied laparotomy, whereas one underwent an applied explorative laparoscopy. PCI localized to the small intestine only was detected in four patients, whereas it was localized to the small intestine and the colon in three. Three patients underwent a partial small intestine resection and four did not after PCI was diagnosed. Five patients were diagnosed with secondary PCI and two with primary PCI when the surgical findings and medical history were assessed together. Gastric atony developed in one case only, as a complication during a postoperative follow-up of 5-14 d., Conclusion: Although rare, PCI should be considered in the differential diagnosis of acute abdomen. Diagnostic laparoscopy and preoperative radiological tests, including computed tomography, play an important role in confirming the diagnosis.

OBJECTIVES: This study was undertaken to investigate the effects of hypovolemic and hypertonic treatments on plasma vasopressin (AVP) levels and fluid balance in thyroidectomy-induced hypothyroidism in the rat. The influence of hypothyroidism on AVP responsiveness to hypertonic and hypovolemic stimuli were compared. MATERIALS & METHODS: Adult male rats were divided into two groups. The rats were surgically thyroidectomized (hypothyroid) or sham-operated (euthyroid). Two weeks later these groups were further divided in three subgroups each containing six rats. The first subgroup consisted of unchallenged rats. The second group underwent hypovolemic treatment by using I.P. 700 mg polyethylene glycol. The third subgroup consisted of hypertonic (1.5 M NaCl; 1 ml/100 g) stimulated animals. All rats were decapitated and trunk blood collected in heparinized tubes. Plasma samples were stored at -20 degrees C until assayed. Plasma AVP, T3 and T4 levels were measured by radioimmunoassay. Hematocrit values and plasma Na and K concentrations were also determined. RESULTS: In the hypothyroid rats, hypovolemic treatment significantly reduced the expected increases in plasma AVP levels (p<0.05) compared to the respective intact animals. In the hypertonic group, similar increases occurred in plasma AVP levels of hypothyroid and euthyroid rats. Hematocrit values and plasma Na concentrations were not significantly different in the hypothyroid rats compared to euthyroid rats. CONCLUSION: In conclusion, thyroidectomy-induced hypothyroidism may affect AVP response to hypovolemic stimulus although it has no important effect on basal AVP levels nor AVP response to hypertonic stimulus.
