Abstract

This guideline is aimed at registrars and consultants in dermatology, ophthalmology, ENT, pediatrics, neurology, virology as well as infectiology, anaesthesia and generell medicine as well as policymakers and payers and purchasers of care. It was developed by dermatologists, virologists, ophthalmologists, ENT physicians, neurologists, pediatrician and anesthetists using a formal consensus process (S2k).The guideline provides an overview of clinical and molecular diagnostics as well as antigen detection, antibody culture and viral culture. Diagnostic special situations and complicated courses of the disease are also considered. The antiviral therapy of zoster and postzoster neuralgia is presented in general and for special situations. Detailed information on the treatment of pain is mentioned and presented in an overview. Likewise, the local therapeutic measures are discussed., Diese Leitlinie richtet sich an Dermatologen, Ophthalmologen, HNO-Ärzte, Pädiater, Neurologen, Virologen sowie Infektiologen, Anästhesisten und Allgemeinmediziner in Klinik und Praxis und dient zur Information für andere medizinische Fachrichtungen, die an der Behandlung des Zoster beteiligt sind. Darüber hinaus soll die Leitlinie Kostenträgern und politischen Entscheidungsträgern zur Orientierung dienen. Die Leitlinie wurde im formellen Konsensusverfahren (S2k) von Dermatologen, Virologen/Infektiologen, Ophthalmologen, HNO-Ärzten, Neurologen, Pädiatern und Anästhesisten/Schmerzmedizinern erstellt. Die Leitlinie stellt einen Überblick über die klinische und molekulare Diagnostik sowie den Antigennachweis, die Antikörperkultur und Viruskultur dar. Diagnostisch besondere Situationen und komplizierte Verläufe der Erkrankung finden ebenfalls Berücksichtigung. Die antivirale Therapie des Zoster und der Postzosterneuralgie wird im Allgemeinen und für besondere Situationen dargelegt. Detaillierte Angaben zur Schmerzbehandlung finden Erwähnung und sind in einer Übersicht dargestellt. Ebenso werden die lokaltherapeutischen Maßnahmen thematisiert.

Abstract

Neutralizing antibodies against poliovirus types 1, 2 and 3 were determined in serum of 4,039 persons of different ages and of 879 persons at the beginning of their training (male recruits at their initial examination; female teaching trainees and student nurses). The sera were supplied by eleven institutions. Compared with similar studies in 1969, 1972 and 1978, the immunity against individual poliovirus types as well as that against the three types in the different age groups has improved. About three quarters of those older than seven years have antibodies against types 1, 2 and 3. The present study, which for the first time used a more sensitive neutralization test, revealed that about 85% of probands had antibodies against all three types. The trivalent immunity of those aged from 0 to six years has improved by 11 percent points since 1978. There was no decrease in immunity among those older than 20 years.

Abstract

Background Cytomegalovirus (CMV) infection is associated with acute and chronic allograft rejection. We have recently shown that rat CMV increases portal inflammation and bile duct destruction in a model of rat liver allograft rejection. Desferrioxamine (DFO), an iron chelator and antioxidant, has recently been demonstrated to have antiviral as well as immunomodulatory effects in vitro. We therefore investigated whether DFO inhibits (a) CMV infection and (b) graft destruction in our rat model. Method One day after liver transplantation, PVG (RT1c) into BN(RT1n), the rats were infected with rat CMV (RCMV, Maastricht strain; 10(5) plaque-forming units i.p.). The effects of 100 mg/kg body weight and 200 mg/kg body weight DFO were examined. Results In the untreated group, the grafts were uniformly RCMV culture-positive. In the group receiving 200 mg/kg DFO, RCMV replication was effectively inhibited. Inflammatory response in the graft, and especially the number of macrophages, was significantly reduced by DFO. Portal inflammation and bile duct destruction were also significantly reduced. In the untreated group, the bile duct epithelial cells were found to be strongly positive for tumor necrosis factor-alpha and this expression was clearly decreased by DFO. In addition, DFO significantly inhibited vascular cell adhesion molecule-1 expression on sinusoidal endothelial cells. Conclusions Our in vivo transplant study strongly supports the inhibitory effects of metal chelators on CMV infection and their possible usefulness in the treatment of CMV-induced pathogenic changes.

Abstract

Transplantation-related pathogenic factors such as ischemia or allograft-directed inflammation are associated with oxidative changes that might lead to cellular oxidative stress. The aim of this study was to investigate the impact of oxidative stress on: (1) CMV replication in cultured human endothelial cells and (2) the stimulation of endothelial cells by proinfiammatory cytokines. Both pathomechanisms are known to contribute to graft rejection crises in vivo. Oxidative stress was induced in endothelial cell cultures with 10-200 microM buthionine sulfoximine. Western blotting showed a significant increase in the production of CMV-specific immediate early and late proteins in buthionine sulfoximine-treated cultures. Immunocytochemical staining suggested that this effect was caused by increased numbers of CMV antigen expressing cells (66% immediate early; 78%, late). Quantitative polymerase chain reaction for CMV-specific DNA and virus titration revealed that enhanced viral replication levels correlated with increased virion production. As a measure for the endothelial cell activation status, the surface expression of HLA-ABC and HLA-DR and adhesion molecules (ICAM-1, ELAM-1, VCAM-1) was quantified by fluorometric methods. Whereas oxidative stress alone did not modulate any surface molecule expression, the IFN-gamma-mediated expression of HLA-ABC and HLA-DR and the IL-1-mediated expression of ICAM-1, but not of ELAM-1 and VCAM-1 (IL-1 + TNF-alpha), was amplified. Interestingly, the amplification of HLA molecule expression was even higher in CMV-infected endothelial cells. This study provides evidence that oxidative stress contributes to the regulation of CMV replication, virus shedding, and the activation of endothelial cells by proinflammatory cytokines as it is observed in transplant recipients.

Abstract

Gegenstand der vorliegenden Arbeit sind diagnostische und epidemiologische Gesichtspunkte bei der Bestimmung von CMV-Antikörpern im Serum bei Blut- und Organspendern sowie Dialysepatienten und Drogenabhängigen (darunter ein Teil HIV-positiv) mittels Enzymimmunoassay (ElA). Der hierfür eingesetzte ELA-Test (CMV-IgG-ELA u. CMV-IgM-ELA) wurde mit dem «CMV total AB EIA», der seit Mai 1985 routinemäβig im Blutspendedienst Frankfurt/M. bei «Haustermin»-Blutspendern eingesetzt wird, verglichen: Bei 221 Seren zeigte sich in 214 Fallen (96,8%) übereinstimmende Ergebnisse, was einer hohen Korrelation entspricht (räquiv = 0,99, p < OOOl). Der CMV-Durchseuchungsgrad der einzelnen Kollektive war wie folgt: Blutspender 65/102 (63,7%), Organspender 10/13 (76,9%), Dialysepatienten 88/106 (83,0%), Drogenabhängige 131/227 (57,7%), HIV-AK-positive Dro-genabhängige 17/31 (54,8%). Abstract

Congenital HCMV infection was diagnosed at the 22th week of gestation. The infection was suspected because HCMV IgM was detected in a serum sample obtained from the woman's husband. HCMV infection was detected in the amniotic fluid by polymerase chain reaction, shell vial culture (immunoperoxidase assay) and conventional virus isolation. Serologic testing in paired sera of the woman and in umbilical cord blood for specific IgM and IgA remained negative. As serological data (preconceptional HCMV serostatus) were incomplete, a clear differentiation between primary and secondary infection could not be achieved; consequently, risk quantification could not be determined. Viruria was detected in the offspring during the 1st week post partum. No clinical signs of cytomegalic inclusion disease were diagnosed up to six weeks post partum. Our case report indicates that for pregnancy surveillance, serologic testing for HCMV antibody should also be performed in the spouse.

Abstract

Bisher wurden 100 Patienten mit unterschiedlich ausgepragten Gerinnungsstorungen auf das Vorhandensein von Antikorpern gegen Hepatitis C (Anti-HCV ELISA von Ortho Diagnostica) untersucht. Da anzunehmen ist, das nicht die Gerinnungsstorung an sich, sondern deren Behandlung mit Plasmakonzentraten als Ursache der HCV-Ubertragung anzusehen ist, fanden nur Patienten, die zuvor mindestens einmalig mit Plasmakonzentraten behandelt wurden, Einlas in die nachfolgende Untersuchung. Ferner gingen wir der Frage nach, ob die Haufigkeit der Behandlungen mit Plasmakonzentraten mit der Pravalenz von Anti-HCV korreliert. Dazu unterteilten wir die Gesamtzahl der Patienten in drei Gruppen: Patienten, die bisher an 1–10 Tagen, an 10–100 Tagen und an uber 100 Tagen mit Plasmakonzentraten behandelt wurden. Wir verglichen diese Befunde mit den derzeitigen Transaminasen und Hepatitis B- (Anti-HBs und Anti-HBc) sowie HIV-Antikorpern.

35. Are medical students sufficiently vaccinated?

Abstract

Medical students are exposed to infectious diseases during the course of their clinical training. Unfortunately, vaccination rates among medical students remain insufficient. However, immunizations against vaccine-preventable diseases should be carried out before the students enter clinical courses. This is vital in order to prevent nosocomial infections. We screened 366 medical students in their first clinical year for hospital-related viral diseases. Serum samples were collected between April and May 2007. Antibody testing was carried out using commercial ELISA systems against measles, mumps, rubella, varicella, hepatitis B (HBV), hepatitis C (HCV), and human immunodeficiency virus (HIV). Overall, 63.9% (n=234) of the students were sufficiently vaccinated against HBV. In contrast, 31.7% (n=116) had not received any HBV vaccine dosage, and 4.4% (n=16) had not completed the full vaccine cycle (<3 dosage). Remarkably, two students showed serological markers of resolved HBV infection. In addition, one student was HCV-positive and one was HIV-positive, respectively. The following seronegative rates were found: measles (7.9%), mumps (17.5%), rubella (6.5%), and varicella (2.2%). Further work is needed to identify optimal strategies for improving vaccination rates among medical students. It is imperative to identify and limit possible disparities in immunity of vaccine-preventable diseases before initial patient contact. With regard to the primary diagnosis of serious virus diseases including HBV, HCV and HIV, medical students should be screened for these blood borne pathogens. [ABSTRACT FROM AUTHOR]

40. Primary cytomegalovirus infection in an outpatient setting -- laboratory markers and clinical aspects.

Abstract

BACKGROUND: Occasionally, primary cytomegalovirus (CMV) infection may give rise to more or less severe clinical illness in immunocompetent adults. We retrospectively analyzed cases of acute CMV infection in medical outpatients. PATIENTS AND METHODS: Over a 6-year period, we identified 22 patients with a febrile illness and hepatitis suffering from primary CMV infection. This was diagnosed on the basis of a strongly positive CMV IgM antibody test result and/or CMV IgG seroconversion. Clinical features as well as relevant laboratory results were analyzed. We also tested available samples for CMV glycoprotein B-specific antibodies and CMV IgG avidity and analyzed results of Epstein-Barr virus (EBV)-specific antibody assays. In addition, current age-specific CMV IgG seroprevalence rates were determined using 9,870 routine patient samples. RESULTS: At presentation, all patients complained of malaise and fever higher than 38 degrees C, and many also complained of cephalgia. Most patients who underwent abdominal ultrasonography had an enlargement of the spleen. Most patients had a relative lymphocytosis but only three had a mild leukocytosis. C-reactive protein was only slightly elevated in 13 patients; all 22 patients had elevated levels of alanine aminotransferase (ALT) and lactate dehydrogenase (LDH). Half the patients reported travel to areas outside western Europe, mostly to tropical and subtropical areas, within 3 weeks before onset of illness. Primary CMV infection was confirmed by negative anti-gB antibody test results and the absence of high-avidity CMV antibodies. In contrast, despite past EBV infection demonstrated by positive anti-EBNA-1 results, 15 out of 21 patients tested for EBV markers had positive or nonspecific IgM test results. The overall CMV IgG seroprevalence rate in the routine samples was 64.4%, with marked age-dependent increases. CONCLUSION: CMV is a relevant differential diagnosis in feverish illnesses accompanied by hepatitis in otherwise healthy adults, about 40% of whom are CMV-naive. Half our patients seem to have acquired their CMV infection abroad, so that a diagnosis of CMV infection needs to be taken into account in travelers, in addition to infectious illnesses more commonly considered in this context, such as dengue or hepatitis A. For diagnosis, both CMV and EBV antibody studies should be performed and the inclusion of assays able to demonstrate past infection is helpful for achieving a definite diagnosis. 41. Prions and orthopedic surgery.

Abstract

Prions are a novel class of infectious agents that cause subacute encephalopathy in man and animals as human Creutzfeldt-Jakob disease (CJD), sheep scrapie and bovine spongiform encephalopathy (BSE). Previously, prions were shown to be transmitted by neuro- and ophthalmosurgical measures and by application of brain-derived therapeutic hormones. Recently, prions have been detected in blood specimens of experimentally infected monkeys indicating a principal threat to transfusion medicine, furthermore in human or bovine materials used in reconstructive surgery. In this article the risk of prion transmission from the surgeon to the patient or vice versa during (orthopedic) surgery is reevaluated including the issues of blood transfusion. This is accomplished based on recent epidemiologic findings and biometric calculations on the spread of prions in animals and humans as well as in terms of experimental data on artificially contaminated medical materials and devices. The overall risk of prion transmission in orthopedic surgery is considered very low if adequately prepared and sterilized materials and devices are used. [ABSTRACT FROM AUTHOR]

Abstract

Six virus laboratories from various parts of the German Federal Republic and West Berlin undertook investigations into polio-myelitis immunity in 1972. A total of 267 persons aged up to 20 years were investigated for neutralizing antibodies against the three types of polio virus. The study showed that from the fourth year of life approximately 70% of the persons investigated had antibodies against all three types of polio virus. In some younger children the equivalent values were considerably lower. After the fourth year of life there were only slight differences in the humoral immunity against the three polio virus types. There was no evidence that the immunity level had deteriorated up to the 20th year of life and thus there is no indication of the necessity for a repeat immunisation at a certain time. The investigation confirmed that three oral administrations of trivalent vaccine, as are now generally recommended as basic immunization in the German Federal Republic, result in a conversion rate of over 90% against each of the three virus types.

Abstract

Over a period of 16 months acute mycoplasma pneumonia was diagnosed in 20 patients. All were rapidly and completely cured by ampicillin and cephalosporins, which are ineffective against mycoplasma, indicating that acute mycoplasma infection in children does not require selective treatment, the pneumonia probably being caused by other micro-organisms. There is no prove that mycoplasma pneumonia heals spontaneously, without any therapy.

Abstract

The Pelargonium sidoides extract EPs® 7630 is an approved drug for the treatment of acute bronchitis in Germany. The postulated mechanisms underlying beneficial effects of EPs® 7630 in bronchitis patients include immunomodulatory and cytoprotective effects, inhibition of interaction between bacteria and host cells, and increase of cilliary beat frequency on respiratory cells. Here, we investigated the influence of EPs® 7630 on replication of a panel of respiratory viruses. Determination of virus-induced cytopathogenic effects and virus titres revealed that EPs® 7630 at concentrations up to 100 μg/ml interfered with replication of seasonal influenza A virus strains (H1N1, H3N2), respiratory syncytial virus, human coronavirus, parainfluenza virus, and coxsackie virus but did not affect replication of highly pathogenic avian influenza A virus (H5N1), adenovirus, or rhinovirus. Therefore, antiviral effects may contribute to the beneficial effects exerted by EPs® 7630 in acute bronchitis patients. [ABSTRACT FROM AUTHOR]

Abstract

The immunity of the populations of several countries of the GFR against poliovirus 1, 2 and 3 was investigated in 11 laboratories. Sera of 4707 persons aged 0 to 30 years were assessed for neutralising antibodies (serum dilution 1:4) against the 3 types of poliovirus. 34.5% of the 4707 investigated sera had no neutralising antibodies at least against one type of the poliovirus, 4.7% showed no antibodies against all 3 types of the virus. Especially children aged 0 to 4 years were protected incompletely against the 3 types of poliovirus. In comparison with similar investigations of 1969 and 1972 no decisive change of immunity of the population of the GFR against poliomyelitis has occurred.
