Abstract

The purpose of this paper is to examine the role of fiscal discipline on the dynamics of exchange rate pass-through during inflation targeting. The present study analyses the Turkish economy for the period 2006–2022 by the use of threshold regression models. Our results suggest that fiscal discipline, measured by the IMF-defined primary budget balance as a share of GDP (which excludes the one-off government revenues balance such as privatization, interest earnings, and real estate sales) has a significant role in inflation dynamics. The degree of exchange rate pass-through is significantly lower and thus raises the effectiveness of inflation targeting when the government ensues strict fiscal discipline. [ABSTRACT FROM AUTHOR]

Abstract

PURPOSE: To investigate differences in athletes' knowledge, beliefs, and training practices during COVID-19 lockdowns with reference to sport classification and sex. This work extends an initial descriptive evaluation focusing on athlete classification. METHODS: Athletes (12,526; 66% male; 142 countries) completed an online survey (May-July 2020) assessing knowledge, beliefs, and practices toward training. Sports were classified as team sports (45%), endurance (20%), power/technical (10%), combat (9%), aquatic (6%), recreational (4%), racquet (3%), precision (2%), parasports (1%), and others (1%). Further analysis by sex was performed. RESULTS: During lockdown, athletes practiced body-weight-based exercises routinely (67% females and 64% males), ranging from 50% (precision) to 78% (parasports). More sport-specific technical skills were performed in combat, parasports, and precision (∼50%) than other sports (∼35%). Most athletes (range: 50% [parasports] to 75% [endurance]) performed cardiorespiratory training (trivial sex differences). Compared to prelockdown, perceived training intensity was reduced by 29% to 41%, depending on sport (largest decline: ∼38% in team sports, unaffected by sex). Some athletes (range: 7%-49%) maintained their training intensity for strength, endurance, speed, plyometric, change-of-direction, and technical training. Athletes who previously trained ≥5 sessions per week reduced their volume (range: 18%-28%) during lockdown. The proportion of athletes (81%) training ≥60 min/session reduced by 31% to 43% during lockdown. Males and females had comparable moderate levels of training knowledge (56% vs 58%) and beliefs/attitudes (54% vs 56%). CONCLUSIONS: Changes in athletes' training practices were sport-specific, with few or no sex differences. Team-based sports were generally more susceptible to changes than individual sports. Abstract

Objective: To investigate the effect of 1) lockdown duration and 2) training intensity on sleep quality and insomnia symptoms in elite athletes. Methods: 1,454 elite athletes (24.1 ± 6.7 years; 42% female; 41% individual sports) from 40 countries answered a retrospective, cross-sectional, web-based questionnaire relating to their behavioral habits pre- and during- COVID-19 lockdown, including: 1) Pittsburgh sleep quality index (PSQI); 2) Insomnia severity index (ISI); bespoke questions about 3) napping; and 4) training behaviors. The association between dependent (PSQI and ISI) and independent variables (sleep, napping and training behaviors) was determined with multiple regression and is reported as semi-partial correlation coefficient squared (in percentage). Results: 15% of the sample spent < 1 month, 27% spent 1-2 months and 58% spent > 2 months in lockdown. 29% self-reported maintaining the same training intensity during-lockdown whilst 71% reduced training intensity. PSQI (4.1 ± 2.4 to 5.8 ± 3.1; mean difference (MD): 1.7; 95% confidence interval of the difference (95% CI): 1.6-1.9) and ISI (5.1 ± 4.7 to 7.7 ± 6.4; MD: 2.6; 95% CI: 2.3-2.9) scores were higher during-compared to pre-lockdown, associated (all p < 0.001) with longer sleep onset latency (PSQI: 28%; ISI: 23%), later bedtime (PSQI: 13%; ISI: 14%) and later preferred time of day to train (PSQI: 9%; ISI: 5%) during-lockdown. Those who reduced training intensity during-lockdown showed higher PSQI (p < 0.001; MD: 1.25; 95% CI: 0.87-1.63) and ISI (p < 0.001; MD: 2.5; 95% CI: 1.72-3.27) scores compared to those who maintained training intensity. Although PSQI score was not affected by the lockdown duration, ISI score was higher in athletes who spent > 2 months confined compared to those who spent < 1 month (p < 0.001; MD: 1.28; 95% CI: 0.26-2.3). Abstract

OBJECTIVE: In a convenience sample of athletes, we conducted a survey of COVID-19-mediated lockdown (termed 'lockdown' from this point forward) effects on: (i) circadian rhythms; (ii) sleep; (iii) eating; and (iv) training behaviors. METHODS: In total, 3911 athletes [mean age: 25.1 (range 18-61) years, 1764 female (45%), 2427 team-sport (63%) and 1442 elite (37%) athletes] from 49 countries completed a multilingual cross-sectional survey including the Pittsburgh Sleep Quality Index and Insomnia Severity Index questionnaires, alongside bespoke questions about napping, training, and nutrition behaviors. RESULTS: Pittsburgh Sleep Quality Index (4.3 ± 2.4 to 5.8 ± 3.1) and Insomnia Severity Index (4.8 ± 4.7 to 7.2 ± 6.4) scores increased from pre- to during lockdown (p < 0.001). Pittsburgh Sleep Quality Index was predominantly influenced by sleep-onset latency (p < 0.001; + 29.8%), sleep efficiency (p < 0.001; - 21.1%), and total sleep time (p < 0.001; - 20.1%), whilst Insomnia Severity Index was affected by sleep-onset latency (p < 0.001; + 21.4%), bedtime (p < 0.001; + 9.4%), and eating after midnight (p < 0.001; + 9.1%). During lockdown, athletes reported fewer training sessions per week (- 29.1%; d = 0.99). Athletes went to bed (+ 75 min; 5.4%; d = 1.14) and woke up (+ 150 min; 34.5%; d = 1.71) later during lockdown with an increased total sleep time (+ 48 min; 10.6%; d = 0.83). Lockdown-mediated circadian disruption had more deleterious effects on the sleep quality of individual-sport athletes compared with team-sport athletes (p < 0.001; d = 0.41), elite compared with non-elite athletes (p = 0.028; d = 0.44) and older compared with younger (p = 0.008; d = 0.46) athletes. CONCLUSIONS: These lockdown-induced behavioral changes reduced sleep quality and increased insomnia in athletes. Abstract

OBJECTIVE: Our objective was to explore the training-related knowledge, beliefs, and practices of athletes and the influence of lockdowns in response to the coronavirus disease 2019 (COVID-19) pandemic caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). METHODS: Athletes (n = 12,526, comprising 13% world class, 21% international, 36% national, 24% state, and 6% recreational) completed an online survey that was available from 17 May to 5 July 2020 and explored their training behaviors (training knowledge, beliefs/attitudes, and practices), including specific questions on their training intensity, frequency, and session duration before and during lockdown (March-June 2020). RESULTS: Overall, 85% of athletes wanted to "maintain training," and 79% disagreed with the statement that it is "okay to not train during lockdown," with a greater prevalence for both in higher-level athletes. In total, 60% of athletes considered "coaching by correspondence (remote coaching)" to be sufficient (highest amongst world-class athletes). During lockdown, < 40% were able to maintain sport-specific training (e.g., long endurance [39%], interval training [35%], weightlifting [33%], plyometric exercise [30%]) at pre-lockdown levels (higher among world-class, international, and national athletes), with most (83%) training for "general fitness and health maintenance" during lockdown. Athletes trained alone (80%) and focused on bodyweight (65%) and cardiovascular (59%) exercise/training during lockdown. Compared with before lockdown, most athletes reported reduced training frequency (from between five and seven sessions per week to four or fewer), shorter training sessions (from ≥ 60 to < 60 min), and lower sport-specific intensity (~ 38% reduction), irrespective of athlete classification. Abstract

Background/aim: Platelet-rich plasma (PRP) application has gained widespread interest for musculoskeletal injuries. Nonsteroidal antiinflammatory drugs are frequently used in sports medicine before and/or after PRP application. Our study seeks to determine whether serum levels of platelet-derived growth factor-AB (PDGF-AB) and vascular endothelial growth factor (VEGF) levels of PRP would be affected by nonsteroidal antiinflammatory drugs. Materials and methods: Two different final concentrations of diclofenac (0.5 µg mL–1 and 2.5 µg mL–1), meloxicam (0.8 µg mL–1 and 2.0 µg mL–1), and acetylsalicylic acid (final concentration 450 µm) were obtained in separate tubes with PRPs prepared from 20 healthy male volunteers. Medicine-free PRP was the control group. Growth factors were measured using ELISA. Results: PDGF-AB and VEGF serum levels did not change with diclofenac, meloxicam, or acetylsalicylic acid addition. PDGF-AB and VEGF serum levels correlated with each other. Conclusion: Diclofenac, meloxicam, and acetylsalicylic acid did not affect PDGF-AB and VEGF serum levels.

Abstract

Metastasis of gastric carcinoma to the breast is relatively uncommon. It may cause difficulty in differentiating metastasis from primary breast cancer. Our patient was 64 year-old woman presented with dysphagia, weight loss and mass on right breast. Mammography showed mass on the right breast and computerized tomographic scan of the abdomen showed thickening of the gastric cardia and small curvature. Pathological evaluation of both breast mass and gastric lesion showed that there is a primary gastric carcinoma and the mass on the breast is the metastasis of this primary gastric carcinoma. In this report; we present a case of gastric carcinoma with metastasis to the breast. © 2011 OMU All rights reserved.

Abstract

Meniscal injury is a common cause of knee pain and major cause of functional impairment of the knee at young athletic population. Although the menisci were thought to be a vestigial tissue and treated with disrespect as an unnecessary appendage for many years; today it is known that the menisci provide mechanical support, localized pressure distribution, and lubrication to the knee joint. Therefore, a shift toward meniscal preservation has led to the development of new surgical techniques in recent decades as removal of the meniscus constitutes a risk factor for osteoarthritis. Therefore, total meniscectomy has been replaced by arthroscopic partial meniscectomy rather than total meniscectomy. Treatment choice of a meniscal pathologies also depend on patient's age, health, lifestyle, sporting activity level, and request to undergo major surgery and also the location and type of meniscal tear. In this article, we provide a general perspective on evolution of surgical management especially better understanding on meniscectomy procedures in various types of meniscal tears. Abstract

This study was performed to evaluate the effects of enalapril, an angiotensin converting enzyme inhibitor and losartan an angiotensin II receptor antagonist on the level of plasma nitrate in untreated essential hypertensive patients. The study was carried on 28 mild or moderate essential hypertensive patients (9 of them were men, mean age: 46±8 years). After the clinical and plasma nitrate level assessment, patients were randomised to one of the 2 treatment protocols: enalapril 20 mg/d to group I, losartan 50 mg/d to group II. After patients were given antihypertensive treatment for 8 weeks, clinical and plasma nitrate level controls were performed again. Two treatment protocols were observed to decrease the clinical blood pressure meaningfully. With enalapril treatment, the level of plasma nitrate increased from 13.20±4.75 μM/L to 13.71 ±5.71 μM/L (p>0.05). With losartan treatment, the level of plasma nitrate rose from 14.09±5.21 μM/L to 15.12±8.77 μM/L (difference not significant). Thus both enalapril and losartan treatment were thought to increase the level of plasma nitrate in untreated essential hypertensive patients. We believe that randomised studies with larger patient populations should be conducted to evaluate the effect of antihypertensive treatment on plasma nitrate level.

Abstract

Goniumsp. was tested for the bioremoval of Reactive Blue 220 (RB220) dye at different conditions such as pH values, initial dye concentrations and biomass concentrations to evaluate the possibility of using this microalga in treating wastewaters. According to the data obtained from the experiments, microalgae removed RB220 with the highest yield (54.2%) at pH 8, and could treat the applied dye with the highest removal percentage as 84.2% at the lowest dye concentration (26.2 mg/L RB220). IncreasingGoniumsp. biomass concentration from 0.21 to 0.53 g/L stimulated RB220 removal rate from 87.7% to 96.8%. The present study clearly indicated thatGoniumsp. biomass could be used as a bioremediation biosorbent in treating RB220 dye in the related wastewaters. [ABSTRACT FROM PUBLISHER]

Abstract

Biosorption of copper(Il) ions from aqueous solutions was studied to compare the binding capacities of untreated (live) or treated (dried, autoclaved, perchloric acid (HClO[SUB4])-washed, ethanol-treated, formaldehydetreated) Candida sp. Due to the ion exchange nature of metal biosorption, copper(II) uptake was strongly affected by the solution pH and optimum adsorption pH value was determined as 5.0 for all the live and treated Candida sp. At the optimal conditions, metal ion uptake increased with increasing initial copper(II) concentration up to 300 mg 1[SUP-1] for all the biomass types. It is found that all the treatment methods used to kill the yeast (except for formaldehyde treatment) increased the copper(II) biosorption capacity of Candida cells. The results also indicated that copper(II) biosorption by live, autoclaved and formaldehyde-treated yeasts was consisted of an initial, rapid surface binding of copper(II) followed by a second, slower intracellular uptake of copper(II), but copper(II) uptake by the other treated yeasts took place only via surface binding. Both the Freundlich and Langmuir adsorption models were suitable for describing the short-term biosorption of copper(II) by all the untreated and treated Candida. [ABSTRACT FROM AUTHOR]

Abstract

The purpose of this paper is to examine the role of fiscal discipline on the dynamics of exchange rate pass-through during inflation targeting. The present study analyses the Turkish economy for the period 2006–2022 by the use of threshold regression models. Our results suggest that fiscal discipline, measured by the IMF-defined primary budget balance as a share of GDP (which excludes the one-off government revenues balance such as privatization, interest earnings, and real estate sales) has a significant role in inflation dynamics. The degree of exchange rate pass-through is significantly lower and thus raises the effectiveness of inflation targeting when the government ensues strict fiscal discipline

PubMed ID: 22257801, Objectives: A growing body of data illustrates the importance of microalbuminuria (MAU) as a strong predictor of cardiovascular risk in the hypertensive population. The present study was designed to define the prevalence of MAU and associated cardiovascular risk factors among Turkish hypertensive outpatients. Study design: Representing the Turkish arm of the multinational i-SEARCH study involving 1,750 sites in 26 countries around the world, a total of 1,926 hypertensive patients from different centers were included in this observational and cross-sectional survey study. Patients with reasons for a false-positive MAU test were excluded. The prevalence of MAU was assessed using a dipstick test, and patients were inquired about comorbidities, comedication, and known cardiovascular risk factors. Results: The overall prevalence of MAU was 64.7% and there was no difference between genders. Most of the patients (82.5%) had uncontrolled hypertension, 35.6% had dyslipidemia, and 35.5% had diabetes, predominantly type 2. Almost one-third of the patients (26.4%) had at least one cardiovascular-related comorbidity, with 20.3% having documented coronary artery disease (CAD). Almost all patients (96.8%) had one or more risk factors for cardiovascular disease in addition to hypertension, including family history of myocardial infarction or CAD, diabetes, dyslipidemia, lack of physical exercise, and smoking. A trend towards higher MAU values in the presence of CAD was determined. Conclusion: Microalbuminuria tests should be routinely used as a screening and monitoring tool for the assessment of subsequent cardiovascular morbidity and mortality among hypertensive patients. © 2011 Turkish Society of Cardiology.

Abstract

Yoksulluk olgusu tüm dünyada yüzyıllardır süregelen bir problemdir. Bu problemle mücadeledeki ilk adım yoksulluğun mümkün olduğunca doğru bir şekilde ölçülmesidir. Sonrasında yoksulluğun sebepleri ortaya çıkarılarak yoksullukla mücadele stratejileri geliştirilebilir. Bu nedenle bu çalışmanın amacı öncelikle Türkiye’deki yoksulluğu ölçmek ve ardından yoksulluğun olası sebeplerini irdelemektir. Ulusal düzeydeki yoksulluk ölçümleri genel bir izlenim sağlasa da bölgeler arasında önemli farklılıklar olabileceği için bu çalışmada İBBS-2 düzeyinde bölgesel yoksulluk ölçümleri yapılmıştır. Yoksulluk ölçümlerinde seçilen yoksulluk sınırı sonuçları önemli ölçüde etkileyebilmektedir. TÜİK’in 2010 senesine kadar yayımladığı ancak daha sonra AB istatistiklerine uyum sağlamak adına bıraktığı mutlak yoksulluk sınırı ve buna dayanan mutlak yoksulluk ölçümleri Türkiye gibi halen önemli ölçüde mutlak yoksulluğa ev sahipliği yaptığı düşünülen gelişmekte olan ülkeler için önemlidir. Bu nedenle bu çalışmada TÜİK’in son olarak 2010 senesinde yayımlamış olduğu ulusal düzeydeki mutlak yoksulluk sınırı, bölgesel fiyat farklılıkları göz önünde bulundurularak İBBS-2 düzeyinde her bölge için ayrı ayrı türetilmiştir. Daha sonra, elde edilen bölgesel mutlak yoksulluk sınırları tüketici fiyat endeksi kullanılarak sonraki yıllar için fiyat düzeyi artışlarına göre güncellenmiştir. Bölgesel yoksulluk ölçümleri için TÜİK’in Gelir ve Yaşam Koşulları anketinde yer alan hane geliri verisi kullanılmıştır. Bu anketin 2014 senesinden itibaren bölgesel düzeyde olması ve gelir verisinin bir yıl gecikmeli olması nedeniyle yoksulluk ölçümleri 2013 ile 2019 yılları arasındaki dönem için gerçekleştirilmiştir. Sonuçlara göre ülkenin Doğu ile Batı kesimleri arasında mutlak yoksulluk açısından ciddi bir ayrışma olduğu açık bir şekilde ortaya çıkmıştır. Özellikle TRC2 (Şanlıurfa alt bölgesi), TRB2 (Van alt bölgesi), TRC3 (Mardin alt bölgesi) ve TRA2 (Ağrı alt bölgesi) bölgelerinde mutlak yoksulluk oranı oldukça yüksek (bazı bölgeler için bazı yıllarda %50’ye yakın) seyrederken, ülkenin Batı kesimlerinde bu oranlar genellikle %10’un altında kalmaktadır. Yoksulluk trendleri incelendiğinde genel olarak yoksulluk oranlarının azalma eğiliminde olduğu görülmektedir. Çalışmanın devamında yoksulluğun olası sebepleri analiz edilmektedir. Bu bağlamda, ilk kısımda ölçülen bölgesel mutlak yoksulluk oranları ile bölgesel düzeyde doğurganlık hızı, işsizlik oranı, eğitim düzeyi ve finansal kapsayıcılık gibi literatürde yoksullukla ilişkili olabileceği ileri sürülen faktörler arasındaki ilişkiler 2014 ile 2019 yılları arasındaki dönem için geleneksel panel veri analizi yöntemi ve ayrıca mekânsal modeller ile incelenmektedir. Elde edilen ön bulgulara göre işsizlik oranı ve doğurganlık hızı ile yoksulluk oranı arasında pozitif yönlü ilişkiler bulunurken, eğitim düzeyi ve finansal kapsayıcılık arttıkça yoksulluğun azaldığı görülmektedir.

Abstract

The current study proposes RBBR biosorption by Clostridium beijerinckii DSMZ 6422 biomass remaining after biobutanol production from pumpkin peel (PP) by a zero-waste approach. Efficient biobutanol production was achieved by investigating initial PP concentrations (5-20% without or with enzymatic hydrolysis) and fermentation time. According to this, the highest concentrations of biobutanol and total ABE were obtained as 4.87 g/L and 8.13 g/L in the presence of 10% PP without enzymatic hydrolysis at 96 h. Furthermore, based on the zero-waste approach, C. beijerinckii DSMZ 6422 biomass obtained after biofuel production was used as a biosorbent for the removal of RBBR dye. Response surface methodology (RSM), commonly utilized for the experimental design, was used to specify the optimized biosorption conditions of RBBR, including initial dye concentration (50-200 mg/L), initial pH (2-6), biosorbent concentration (1-3 g/L), and contact time (0-240 min). The highest biosorption under optimized conditions with RSM was 98% in the presence of 194.36 mg/L RBBR and 2.65 g/L biosorbent at pH 2 and 15 min. This is the first report in the literature about the biosorption of RBBR dye by anaerobic C. beijerinckii biomass after the biobutanol production process. Abstract

This study aims the third generation biobutanol production in P2 medium supplemented D. salina biomass mixotrophically cultivated with marble waste (MW). The wastes derived from the marble industry contain approximately 90% of carbon-rich compounds. Microalgal growth in mixotrophic conditions was optimized in the 0.4-2 g/L of MW concentration range. The highest microalgal concentration was obtained as 0.481 g/L in the presence of 1 g/L MW. Furthermore, some important parameters for the production of biobutanol, such as microalgal cultivation conditions, initial mixotrophic microalgal biomass loading (50-300 g/L), and fermentation time (24-96 h) were optimized. The highest biobutanol, total ABE, biobutanol yield and productivity were determined as 11.88 g/L, 13.89 g/L, 0.331 g/g and 0.165 g/L/h at the end of 72 h in P2 medium including 60 g/L glucose and 200 g/L microalgal biomass cultivated in 1 g/L MW, respectively. The results show that D. salina is a suitable raw material for supporting Clostridium beijerinckii DSMZ 6422 cells on biobutanol production. To the best of our knowledge, this is the first study on the use of MW which is a promising feedstock on the mixotrophic cultivation of D. salina for biobutanol production.

43. Operative treatment in non-emergency sports injuries: a sports medicine clinic experience.

Abstract

Purpose: To elucidate referrals from sports medicine clinic (SMC) to orthopedics, consensus rates among physicians and surgeons on surgical need in these patients, and reasoning of the patients who refused to have surgery despite the recommendations by both groups in non-emergency sports injury patients., Methods: We conducted a retrospective cohort analysis by using the administrative and referral records of a sports medicine clinic for the 15 months between October 2017 and January 2019. Gender, age, diagnosis, and type of sport were compared between two groups (patients referred to orthopedics vs patients treated in SMC). Agreement between orthopedics and SMC on surgical need in those patients who were referred to orthopedics was evaluated. We additionally screened the hospital archive for surgery reports of patients recommended surgery, and, if a patient did not have a surgery record, we investigated the reasons by telephone interview and categorized these reasons., Results: Surgical evaluation needed for 4.7% ( n = 155) of the patients. Patients referred to orthopedics were younger (median: 22.0 years), mostly male ( n = 119, 76.8%), and involved in contact sports ( n = 108, 69.7%) ( p < 0.001 for all) compared to patients treated conservatively. Majority of the consultations were made owing to certain diagnoses such as cruciate/collateral ligament sprain ( n = 70, 45.2%), meniscal tear ( n = 21, 13.5%), and cartilage injuries ( n = 15, 9.7%). Most of the patients diagnosed with Achilles tendon rupture ( n = 2, 100%) and patellar instability ( n = 13, 81.3%) were referred to surgery. Orthopedics agreed with the sports medicine physicians' opinions in most cases ( n = 110, 71.0%). Thirty-four patients (30.9%) recommended surgery by the surgeon postponed/refused the treatment for various reasons or had surgery in another hospital., Conclusion: Most non-emergency sports injuries can be treated conservatively. Young and male patients participating in contact sports are more likely to need surgical treatment. Non-surgeon musculoskeletal health-care providers may consider referring patients to orthopedics considering the data demonstrated in this study. Nevertheless, patient's motivation to undergo an operation should be considered before referring to orthopedics.

Abstract

Objective: A 1-day point prevalence study was planned to obtain country data by determining the clinical characteristics, follow-up and treatment methods of coronavirus disease 2019 (COVID-19) cases that required intensive care unit (ICU) treatment in the second year of the pandemic., Material and Methods: All patients who were hospitalized in the ICUs due to COVID-19 between March 11, 2022, 08.00 am, and March 12, 2022, 08.00 am, were included in the study. Demographic characteristics, intensive care and laboratory data, radiological characteristics, and follow-up results of the patients were recorded., Results: A total of 811 patients from 59 centers were included in the study, 59% of the cases were male, and the mean age was 74 ± 14 years. At least one comorbid disease was present in 94% of the cases, and hypertension was the most common. When ICU weight scores were examined, Acute Physiology and Chronic Health Evaluation-II: 19 (15-27) and Sequential Organ Failure Assessment: 7 (4-10) were seen. Sepsis was present in 37% (n = 298) of cases. PaO2/FiO2 ratios of the patients were 190 the highest and 150 the lowest and 51% of the cases were followed via invasive mechanical ventilation. On the study day, 73% bilateral involvement was seen on chest x-ray, and ground-glass opacities (52%) were the most common on chest tomography. There was growth in culture in 40% (n = 318) of the cases, and the most common growth was in the tracheal aspirate (42%)., Conclusion: The clinical course of COVID-19 is variable, and ICU follow-up was required due to advanced age, comorbidity, presence of respiratory symptoms, and widespread radiological involvement. The need for respiratory support and the presence of secondary infection are important issues to be considered in the follow-up. Despite the end of the second year of the pandemic and vaccination, the high severity of the disease as well as the need for follow-up in ICUs has shown that COVID-19 is an important health problem.

Patients aged ≥ 18 years with confirmed COVID-19 who were admitted to the ICU were included. The present study had two phases. The first phase was designed as a one-day point prevalence study, and demographic and clinical findings were evaluated. In the second phase, the 28-day mortality was evaluated., Results: As of August 11, 2021, 921 patients were enrolled in the study. The mean age of the patients was 65.42 ± 16.74 years, and 48.6% (n = 448) were female. Among the critically ill patients with COVID-19, 52.6% (n = 484) were unvaccinated, 7.7% (n = 71) were incompletely vaccinated, and 39.8% (n = 366) were fully vaccinated. A subgroup analysis of 817 patients who were unvaccinated (n = 484) or who had received two doses of the CoronaVac vaccine (n = 333) was performed. The 28-day mortality rate was 56.8% (n = 275) and 57.4% (n = 191) in the unvaccinated and two-dose CoronaVac groups, respectively. The 28-day mortality was associated with age, hypertension, the number of comorbidities, type of respiratory support, and APACHE II and sequential organ failure assessment scores ( p < 0.05). The odds ratio for the 28-day mortality among those who had received two doses of CoronaVac was 0.591 (95% confidence interval: 0.413-0.848) ( p = 0.004)., Conclusion: Vaccination with at least two doses of CoronaVac within six months significantly decreased mortality in vaccinated patients than in unvaccinated patients.

Abstract

Objective: Vaccination against severe acute respiratory syndrome coronavirus-2 (SARS-2) prevents the development of serious diseases has been shown in many studies. However, the effect of vaccination on outcomes in COVID-19 patients requiring intensive care is not clear., Methods: This is a retrospective multicenter study conducted in 17 intensive care unit (ICU) in Turkey between January 1, 2021, and December 31, 2021. Patients aged 18 years and older who were diagnosed with COVID-19 and followed in ICU were included in the study. Patients who have never been vaccinated and patients who have been vaccinated with a single dose were considered unvaccinated. Logistic regression models were fit for the two outcomes (28-day mortality and in-hospital mortality)., Results: A total of 2968 patients were included final analysis. The most of patients followed in the ICU during the study period were unvaccinated (58.5%). Vaccinated patients were older, had higher Charlson comorbidity index (CCI), and had higher APACHE-2 scores than unvaccinated patients. Risk for 28-day mortality and in-hospital mortality was similar in across the year both vaccinated and unvaccinated patients. However, risk for in-hospital mortality and 28-day mortality was higher in the unvaccinated patients in quarter 4 adjusted for gender and CCI (OR: 1.45, 95% CI: 1.06-1.99 and OR: 1.42, 95% CI: 1.03-1.96, respectively) compared to the vaccinated group., Conclusion: Despite effective vaccination, fully vaccinated patients may be admitted to ICU because of disease severity. Unvaccinated patients were younger and had fewer comorbid conditions. Abstract

There is emerging data indicating that long-standing vigorous exercise may be associated with atrial structural remodelling. This remodelling process is may be the cause of the increasing frequency of atrial arrythmias in athletes. Early diagnosis of atrial remodelling by atrial imaging could have a role in management of atrial arrythmias in elite athletes. In this study we aimed to diagnose early phases of atrial remodelling in elite athletes. Two groups of athletes including professional weight lifters (n = 33), professional marathoners (n = 32) and sedentary participants (n = 30) were enrolled. We also studied patients who received cardiotoxic chemotherapy (n = 10) for comparison. Serum TGF-beta level as a marker of fibrosis was measured. Both left atrial (LA) 3D volume and strain values were analysed. There was a positive correlation between serum TGF-beta levels and LA volumes and negative correlation between TGF-beta levels and strain values. TGF-beta levels were higher among chemotherapy and weight lifter groups, compared to control and marathoner groups [mean 0.57 ± 0.3 and 0.55 ± 0.2 vs. 0.45 ± 0.2 and 0.47 ± 0.2, respectively, p = 0.005]. LA volumes were higher among chemotherapy and weight lifter groups [median 33 (26-38) and 31 (23-36) respectively, p = 0.005], and strain values were lower in these two groups [mean 20.3 ± 2.5 and 24.6 ± 4.5, respectively, p < 0.005] compared to control and marathoner groups. Total exercise volume was higher in weight lifter group compared to marathoners [13,780 (2496-36,400) vs. 4732 (780-44928), respectively, p = 0.001]. There wasn't any difference between any group regarding left ventricular systolic and diastolic functions. Vigorous exercise causes atrial remodelling and fibrosis in elite athletes. Strength exercise carries higher risk for atrial fibrosis than endurance exercise. Burden of exercise is correlated with the severity of cardiac fibrosis. Echocardiographic evaluation of the left atrium and TGF-beta levels may help to detect subclinical cardiac remodelling and fibrosis., (© 2023. Objective: To determine what combinations of self-reported factors distinguish patellar tendinopathy (PT) from other knee problems, and explain PT severity variance., Design: Case-control study., Setting: Social media, private practice and National Health Service., Participants: An international sample of jumping athletes diagnosed with either PT (n = 132; 30.7 ± 8.9 years; 80 males; VISA-P = 61.6 ± 16.0) or another musculoskeletal knee condition (n = 89; 31.8 ± 9.9 years; 47 males; VISA-P = 62.9 ± 21.2) by a clinician in the last 6 months., Main Outcome Measures: We considered clinical diagnosis (case = having PT vs control = having other knee problems) as the dependent variable. Severity and sporting impact were defined by VISA-P and availability, respectively., Results: A model comprising seven factors distinguished PT from other knee problems; training duration (OR = 1.10), sport type (OR = 2.31), injured side (OR = 2.28), pain onset (OR = 1.97), morning pain (OR = 1.89), condition acceptability (OR = 0.39) and swelling (OR = 0.37). Sports-specific function (OR = 1.02) and player level (OR = 4.11) explained sporting availability. 44% of PT severity variance was explained by quality of life (β = 0.32), sports-specific function (β = 0.38) and age (β = -0.17)., Conclusion: Sports-specific, biomedical and psychological factors partially distinguish PT from other knee problems. Availability is mainly explained by sports-specific factors, while psychosocial factors impact on severity. Abstract

Boron (B) is one of the most important trace elements. Turkey has the largest B reserves in the world with 3 billion tons of B. Therefore, the toxicity of B is more important in Turkey. In this study, effective B removal was provided by different types of microalgae and then B recovery in culture media by bio-mining was detected. The water of Lake Mogan in Gölbaşı/Ankara/Turkey was used as the microalgae culture medium as a cost-reducing factor. The effects of light, temperature, NaNO 3 and K 2 HPO 4 stresses on B removal were determined. The highest B removal was 17.19% at 25 μmol/m 2 s light intensity and 25 °C for Phormidium animale in Lake Mogan culture medium. Boron removal of Scenedesmus sp. was 36.42% at 25 μmol/m 2 s light intensity, at 25 °C, at 1.5 g/L NaNO 3 and 40 mg/L K 2 HPO 4 concentrations in BG11 medium on the 15th day. The chl (a + b) concentration of Scenedesmus sp. was 1.63 μg/mL at 75 μmol/m 2 s light intensity and the chl (a) concentration of P. animale was 2.01 μg/mL at 25 μmol/m 2 s light intensity. Considering all parameters, Scenedesmus sp. and P. animale are recommended as effective biomaterials for the B removal process.
