Introduction: The study aims to evaluate the motor, non-motor, cognitive and psychiatric conditions of our patients diagnosed with Idiopathic Parkinson's Disease (IPD) before and after the COVID-19 pandemic and to investigate the effect of the pandemic on male and female genders. Methods: Ninety patients were included in the study. Demographic data such as age, gender, and duration of disease, type of disease of the patients were recorded. Patients were then divided into 2 groups: male and female. Movement disorders of both groups before the pandemic, disease stage determined by H&Y and UPDRS at the admission and after the pandemic, Levodopa equivalent dose used were recorded and all patients were surveyed to evaluate their motor, non-motor, cognitive, and psychiatric conditions during the COVID-19 pandemic. The effects of the COVID-19 pandemic on male and female genders were investigated. Results: Of the patients, 40 were male and 50 were female. Daytime drowsiness, weakness, impaired walking, constipation, sleep disorders, and inability to turn in the bed worsened compared to the period before the pandemic. Yet, of the psychiatric symptoms, boredom, unhappiness, anhedonia, irritability, and tension were found to increase during the pandemic. There was a statistically significant difference in UPDRS, H&Y and disease stages before and after the COVID-19 pandemic in both groups, especially in the female group (p<0.05). Conclusion: Although it was more pronounced in female patients with IPD diagnosis, both motor and non-motor symptoms were found to worsen during the COVID-19 pandemic in both groups. Amaç: Guillain–Barré sendromu (GBS)akut yada subakut başlayabilen genellikle ilerleyici flask paralizi ileseyreden inflamatuar demyelinizan poliradikülonöropatiler içerisinde yer alanbir hastalıktır. Bu çalışmanın amacı kendi hasta grubumuzda GBS'nin mevsimleregöre dağılımını, mevsimle GBS alttipleri arasındaki ilişkiyi ve mevsimleprognoz arasındaki ilişkiyi araştırmaktır.Gereç ve Yöntem: Bu çalışmada 2007-2017 yılları arasında kliniğimizdeyatarak tedavi görmüş Asbury tanı kriterlerine göre kesin GBS tanısı almış erişkin hastaların dosyalarıretrospektif olarak tarandı.Hastaların yaş, cinsiyet, geçirilmiş enfeksiyonlar, mevsimlere göre dağılımı, klinikalt tipleri , hastanede kalış süreleri, Hughes Fonksiyonel Ölçeği Skoru (HFÖS),yoğun bakım ihtiyaçları, entübasyon , mortalite durumları kaydedildi.Bulgular:Çalışmaya 63 hasta alındı. Hastaların 35'i erkek, 28'i kadındı. Yaş ortalaması erkeklerde 51,4±18,2,kadınlarda 47,07±18,9idi. AIDP hastaların 37(%58,7)'sinde, AMANhastaların 8 (%12,7)'inde,AMSAN hastaların 6 (%9,5)'sında,Miller-Fisher Sendromu hastaların 5(%7,9)'inde görüldü. GBS semptomları başlamadan önceki 2-4 haftadahastaların 30 (%47,6)'undaÜSYE, 15 (%2,8)'inde GİS enfeksiyonu, 1 (%1,6)'indeaşı öyküsü vardı. Mevsimsel dağılıma bakıldığında hastaların 13(%20,6)'ükış mevsiminde, 21 (%33,3)'iilkbaharda, 19 (%30,2)'uyaz mevsiminde, 10 (%15,9)'usonbaharda idi. Mevsimsel dağılım ile klinik alt tipler arasındaki ilişkiyebakıldığında AIDP formunda mevsimsel fark saptanmazken aksonal formlar ilkbaharve yaz mevsiminde diğer mevsimlere göre daha sık ortaya çıkmıştır. Hastaneyeilk başvuru HFÖS ile mevsimsel dağılıma bakıldığında en düşük HFÖS ilkbahardasaptandı. Sonuç:Çalışmamızda, GBS hastalarında aksonal form, ilkbahar ve yazmevsiminde daha sıktı. Buna ek olarak, ilkbaharda görülenler vakalarda prognozdaha iyi iken kış aylarında daha kötüdür. Hastaların mevsimsel dağılımlarıarasındaki ilişkinin daha fazla sayıda hasta ile değerlendirilmesi gerektiğinidüşünüyoruz., Background: Guillain-Barrésyndrome (GBS) is a disease that occurs with in the inflammatory demyelinating polyradiculoneuropathiesof the common flaccid paralysis that can begin in the acute or subacute stage. The aim of this study was to investigate the relationshipbetween seasonal distribution of GBS in our own patient group, the relationshipbetween seasonal GBS subtypes and seasonal prognosis.Material and Method: This study retrospectively reviewed the files ofadult patients who were diagnosed as GBS-diagnosed according to Asbury diagnosticcriteria in our clinic between 2007 and 2017. Age, sex, past infections,seasonal distribution, clinical subtypes, hospitalization times, Hughes FunctionalScale HFRS), intensive care needs, intubation, and mortality were recorded.Results: Sixty-three patients were included in the study. Thepatients were 35 males and 28 females. The mean age was 51.4 ± 18.2 in males and47.07 ± 18.9 in females. In 37 (58.7%) of AIDP patients, 8 (12.7%) in AMANpatients, 6 (9.5%) in AMSAN patients and 5 (7.9%) in Miller-Fisher syndrome patientswere found to be indent. Before the onset of GBS symptoms, there were 30(47.6%) Upper respiratory tract infection patients, 15 (23.8%) indent GISinfections, and 1 (1.6%) indent vaccination history at 2-4 weeks. When the seasonaldistribution was examined, 13 (20.6%) of the patients were in the winter, 21(33.3%) were in the spring, 19 (30.2%) were in the summer and 10 (15.9%) werein the autumn. Given the relationship between seasonal distribution and clinicalsubtypes, no seasonal difference was found in AIDP form, where asaxonal forms appearedmore frequently in spring and summer than in other seasons. When the first applicationto the hospital and the seasonal distribution with HFRS were taken, the lowestHFRS was determined in the spring.Conclusion: Inour study, axonal form in GBS patients was more frequent in spring and summer seasons.In addition, those who are seen in the spring are better be haved and the treatmentis worse in the winter months while the treatment is better. We think that the relationshipbetween seasonal distributions of patients should be evaluated with higher numberof studies.

Introduction: Given the limited data on autonomic dysfunction in patients with primary restless legs syndrome (pRLS), we compared autonomic dysfunction and presence of irritable bowel syndrome (IBS) between patients with pRLS and control patients.Methods: Consecutive adult drug-naïve patients with pRLS, and age- and gender-matched healthy control patients were enrolled in this study. Diagnoses, based on validated self-reported questionnaires, were made using the following guidelines: Rome III classification system for functional gastrointestinal disorders for IBS; Beck Anxiety Inventory (BAI) and Beck Depression Inventory (BDI) for the presence of anxiety and depression, respectively; Pittsburgh Sleep Quality Index (PSQI) for severity of sleep disturbances; and Scales for Outcomes in Parkinson's disease-Autonomic (SCOPA-AUT) for autonomic dysfunction.Results: There were 88 patients with pRLS (18 male, 70 female) and 128 control patients (40 men, 88 women). The mean age of the pRLS patients and control patients was 50.3 ± 9.3 years and 49.7 ± 8.2 years, respectively. Overall, 41 (46.6%) of the patients with pRLS and 16 (12.5%) of the control patients had IBS. Among patients with pRLS, IBS was significantly more common and the total autonomic SCOPA-AUT scores were higher than those found among control patients. Among pRLS patients with IBS, total autonomic SCOPA-AUT, PSQI, BAI and BDI scores were significantly higher than among pRLS patients without IBS. The presence of IBS did not affect the severity of restless legs syndrome.Conclusion: The presence of autonomic nervous system impairment in patients with pRLS and the strong link between IBS and pRLS merit further, more extensive investigation. Introduction: Uremia is a metabolic disorder that affects the functions of the central nervous system (CNS). Visual evoked potentials (VEPs) can be used to demonstrate uremia-induced effects on the CNS. Data on VEPs of patients with chronic renal failure (CRF) are limited. The purpose of this study was to examine the effects of renal failure and the ultrafiltration rate (UFR) on CNS functions in patients undergoing hemodialysis (HD) using VEPs. Methods: The study consisted of 30 CRF patients undergoing HD, and a control group of 28 individuals, matched in terms of age and gender. Patients with mental retardation, diabetes mellitus, demyelinating disease, stroke, use of neurotoxic drugs, and ophthalmologic diseases were excluded from the study. The pattern VEP (PVEP) parameters recorded were latencies to N75 (N1), P100 (P1) and N135 (N2) waves, and the peak-to-peak amplitude of a P100 wave. The PVEP was taken twice just before and 24 h after HD, and the PVEP parameters were compared. Additionally, the effect of the UFR on the PVEP parameters was examined. Results: There was no difference between the HD group and control group in terms of gender and age (mean ages of 49.56±12.52 and 48.53±13.28 years, respectively). The average HD period was 87.53±56.34 months. The P1 latencies between the two eyes the patients with CRF were prolonged when compared to those of the controls (right eye p=0.002; left eye p=0.019). A single HD session significantly shortened the latency of the P1 waves compared to that of the baseline waves (p<0.000). Additionally, there was a positive correlation between the difference in the P1 latency before and after HD and the UFR (right eye p=0.03, r=0.525; left eye p=0.022, r=0.417, respectively) Conclusion: We conclude that VEP, which is an easy, cheap, and simple electrophysiological method, can be used to determine subclinical CNS dysfunction in patients with CRF and show the effect of fluid removal during HD on the visual pathways, and to show the corrective effect of effective fluid imaging on the visual pathways. AMAÇBu çalışmada polikliniğimize başağrısı şikayetiyle başvuran ve migren tanısı alan hastalarda istenen kranial MRG sonuçları gözden geçirilmiştir. Bu sayede migren tanısı alan hastalarda yapılan kranial görüntülemelerde saptanan patoloji tipleri, oranları ve patolojik lezyonların sınıflandırılması amaçlanmıştır.GEREÇ VE YÖNTEM Bu çalışma Ocak 2011-Nisan 2011 tarihleri arasında Sağlık Bakanlığı İstanbul Eğitim ve Araştırma Hastanesi nöroloji polikliniğine başağrısı şikayetiyle başvuran ve nörolojik muayeneleri normal olan 105 migrenli hasta (79 kadın, 26 erkek; ortalama yaş 35+_10) üzerinde retrospektif olarak yapıldı. Migren tanısı Uluslararası Başağrısı Derneğinin tanı kriterlerine göre konuldu. 23 hasta auralı migren, 82 hasta aurasız migren tanısı aldı. Hastaların dosyaları taranarak klinik özellikleri, kranial görüntüleme endikasyonları ve sonuçları belirlendi. Literatür bilgileriyle sonuçlarımız karşılaştırılarak tartışıldı. İstatistiksel analiz için çapraz analiz yapılırken ki-kare (chi-square) ve bağımsız t testi kullanıldı.BULGULAR105 Migrenli hasta çalışmaya alındı. Migren şikayeti ile gelen hastalarda farklı lezyon olma ihtimaline karşı MRG tetkikleri yapılarak incelenmiştir. 53 hastada normal kranial MRG bulguları saptandı. 52 migrenli hastada çoğunluğu benign değişik patolojik görüntüler saptandı. Bulgular literatür bilgileriyle sonuçları karşılaştırılarak tartışıldı.SONUÇPolikliniğimize migren anamnezi ile başvuran hastalarda yapılan retrospektif çalışmada; hastaların klinik özelliklerinin migren ile ilgili literatür bilgileriyle uyumlu olduğu ve nörolojik görüntülemede nedensel lezyonların saptanma olasılığının olduğu saptandı. Şiddetli kronik başağrısı ile başvuran vakalarda klinik bulgular migren kriterleri ile uyumlu olsa dahi kranial görüntüleme yapılması diğer organik nedenlerin ekartasyonu ve tedavi edilebilir patolojilerin tespiti açısından gerekli görünmektedir. Yine de klinisyen bu türlü patolojilerin bir arada olabileceğini akılda tutarak vakaya yaklaşmalıdır. Eşlik eden patolojilerin tespiti ve tedavisi kronik başağrısını tamamen tedavi edebileceği gibi eşlik eden patolojilerin tedavisi migren sıklığını da azaltabilmektedir. AIM: In the following study cranial magnetic resonance imaging (MRI) findings of patients applied to our clinics with headache are evaluated. It is aimed to classify the types and ratios of the pathologies detected in the MRI of the patients who are diagnosed as migren. MATERIALS AND METHODS: Patients with headahce without any neurologic findings who applied to Ministery of Health Istanbul Reasearch and Training Hospital Neurology department are included in the study. Total number of patients is 105 (79 female, 26 male) with mean age 35 (+-10). The study is designed as retrospective. Migren diagnosis is based on the criteria of International Headache Society. 23 of the cases are diagnosed as Migren with aura, remaining 82 are diagnosed as Migren with out aura. Patients archieves are reviewed for clinical properities, cranial imaging indications and result. For statistical analysis Chi squre and independent t test are used. RESULTS: 105 patients with migren are included in the study. According to the MRI findings accompaning lesion are listed. 53 of them hve norma MRI findings. Among 52 of patients most of them have benign pathologic findings. Results are discussed in the light of literature. COMMENT:In the retrospective study of patients applied to our clinics with history of migren; clinical properities of patients are compatible with the literture. It is also shown that causative lesion can be determined in the Neurologic Imaging. For the severe chronic headache patients, even the clinical findings are compatible with the migren, it seems tobe necessary to get cranial imaging. Nevertheless during initial approach the clinician should bare in mind that that kind of pathologies can accompany each other. To define and treat the accompaning pathologies can both decrease the severity and frequency of the headache, even it can help in cure. 62

11. The Association Between Childhood Traumatic Events and Headache-Related Parameters in Patients with Migraine: A Cross-Sectional Study in Turkish Population.

Introduction: The aim of this study is to investigate the association between childhood traumatic events and headache-related clinical parameters in migraine patients. Methods: 95 patients diagnosed with migraine and 50 healthy controls were included in the study. A socio-demographic form, the Childhood Trauma Questionnaire (CTQ), the Beck Depression Inventory (BDI) and the Beck Anxiety Inventory (BAI) were completed by all participants. Additionally, the Visual Analog Scale (VAS) and the Migraine Disability Assessment Test (MIDAS) were applied to migraine patients. Results: Positive correlations were found between emotional abuse, physical abuse, physical neglect, CTQ total scores, and headache frequency (r=0.33, r=0.24, r=0.26 and r=0.28 respectively) in migraine patients. A positive correlation was found between physical neglect and headache duration (r=0.28). Positive correlations were also found between emotional abuse and physical neglect, and MIDAS total scores (r=0.22 and r=0.23, respectively). Emotional abuse and CTQ total scores were associated with younger mean age of headache on-set (r=-0.24 and r=-0.23) Conclusion: Childhood traumatic events are associated with more frequent and more severe headache episodes, and younger headache onset in migraine patients. Miller Fisher Syndrome (MFS) is an acute, autoimmune polyneuropathy usually associated with a good prognosis which is clinically characterized by ataxia, ophtalmoplegia, and areflexia. MFS has a monophasic course, double recurrence can be seen in rare cases. But three attacks are very rare. In this article, we discussed a rare case of MFS and approach to the treatment that recurred 3 times within a period of 8 years. It is emphasized that MFS cases may occur a recurring pattern, and plasma exchange should be definitely tried as a therapeutic approach when clinical deterioration occurs under intravenous immunoglobulin (IVIg) therapy. - Abstract
Behçet's syndrome is a recurrent systemic inflammation with an unknown etiology. The onset of the condition often occurs in the second decade of life and it is more common in males than in females. One of the most feared organ involvements associated with BS is neurological involvement, which often affects the brain stem, although hemispheric involvement may be primary in a limited number of patients. Parenchymal neurological involvement in BS affects the prognosis unfavorably, as it may result in severe sequelae or mortality. Early diagnosis and treatment is important to prevent sequelae.
