Background: The Appropriateness of Aspirin Use in Medical Outpatients: A Multicenter, Observational Study trial has been the largest study ever conducted among patients in Turkey regarding aspirin treatment. In the subgroup analysis of the hypertensive group of the Appropriateness of Aspirin Use in Medical Outpatients: A Multicenter, Observational Study trial, we aimed to evaluate the physicians' adherence to current guidelines regarding their aspirin treatment preferences. Methods: The Appropriateness of Aspirin Use in Medical Outpatients: A Multicenter, Observational Study trial is a cross-sectional and multicenter study conducted among 5007 consecutive patients aged ≥18 years. The study population consisted of outpatients on aspirin treatment (80-300 mg). The patient data were obtained from 30 different cardiology clinics of 14 cities from all over Turkey. In this subgroup analysis, patients were divided into 2 groups: the hypertensive group (n=3467, 69.3%) and the group without hypertension (n=1540, 30.7%) according to the 2018 European Society of Cardiology/ European Society of Hypertension Guidelines for the Management of Arterial Hypertension. Results: Aspirin use for primary prevention was higher in patients with hypertension compared to patients without hypertension [328 (21.3%); 1046 (30.2%); P < .001]. Treatment with a dose of 150 mg aspirin (n=172, 5%) was mostly preferred by internists for hypertensive patients (n =226, 6.5%); however, a daily dose of 80-100 mg aspirin therapy (n=1457, 94.6%) was mostly prescribed by cardiologists (n=1347, 87.5%) for patients without hypertension. Conclusion: Aspirin was found to be used commonly among patients with hypertension for primary prevention despite the current European Society of Cardiology Arterial Hypertension Guideline not recommending aspirin for primary prevention in patients with hypertension.

Amaç: Over kanseri jinekolojik kanserler arasında en yüksek ölüm oranına sahip olan kanserdir.İnflamasyon temelli skorlar olan nötrofil lenfosit oranı (NLR), trombosit lenfosit oranı (PLR), CRP albümin oranı (CRP/Alb) çoğu kanserde prognoz üzerine etkisi gösterilmiştir. Biz de bu çalışmada NLR, PLR, CRP/Albümin oranlarının over kanserinin prognozu üzerine olan etkisini değerlendirmeyi amaçladık. Gereç ve Yöntem: Çalışmamıza Eylül 2014 - Aralık 2020 tarihleri arasında Pamukkale Üniversitesi Tıp Fakültesi Tıbbi Onkoloji Bilim Dalı polikliniğine başvuran over karsinom tanılı 124 hasta çalışmaya alındı. Evre 1-4 arasında değişen toplam 124 hasta hasta çalışmaya dahil edildi. NLR, PLR, CRP/Albumin kesme noktaları ROC analizi ile yapıldı. Bulgular: Hastalarımızın medyan yaşı 56 (sınırlar 24-84 ) idi. NLR oranı 63 hastada (%50), PLR oranı 56 hastada (%45), CRP/Alb oranı 61 hastada (%49) kesme değerine göre yüksek saptandı. PLR değeri ile progresyon sağkalım arasında istatistiksel olarak anlamlı ilişki yokken, genel sağkalımla istatistiksel olarak anlamlı ilişki bulunmaktaydı (p=0.01). PLR değeri yüksek olanlarda medyan genel sağkalım 23 ay iken PLR değeri düşük olanlarda 45 aydır olarak saptandı. CRP/Alb oranının ise hem progresyonsuz sağkalım hem de genel sağkalım ile istatistiksel olarak anlamlı ilişkisi mevcuttu (p=0.011; p=0.008). CRP/Alb değeri düşük olanlarda daha uzun progresyonsuz sağkalım ve genel sağkalım olduğu saptandı. Sonuç: Diğer kanser tiplerinde olduğu gibi over kanserinde de inflamasyon belirteçlerinin kanser progresyonu üzerine dolayısıyla prognoza olan etkisi dikkat çekmektedir. PLR oranının genel sağkalımla, CRP/alb oranının ise hem genel sağkalım hemde progresyonsuz sağkalımla istatistiksel olarak anlamlı bir ilişkisi olduğunu gösterdik. Bu oranların gelecekte farklı tedavi gruplarını oluştururken hastaların seçiminde yardımcı olabileceği kanısındayız.

Bunu gerçekleştirmek için bölgede yer alan devletler veya beyler ile kız alıp vererek akrabalık kurmuşlardır. Böylece yapılan evlilikler tarafların siyasi hadiselerinde önemli rol oynamıştır. Diğer taraftan Türkiye Selçuklu Devleti'nde önemli devlet adamlarının yapmış olduğu evlilikler de birçok olayın vuku bulmasında etkili olmuştur. Buhadiselerin yaşanması gerek devlet içerisinde gerek halk arasında tepkiyle karşılanmıştır. Bütün bunlar devlet kademesi dâhil birçok alanda değişimi beraberinde getirmiştir. Cardiovascular diseases are the leading causes of mortality in patients with chronic kidney disease. Nitric oxide has a critical role in both endothelial dysfunction and the atherosclerosis process. We aimed to investigate the relationships between serum asymmetric dimethyl arginine (ADMA), LOX-1, and Apelin-13 levels, which are known to act over nitric oxide with endothelial dysfunction and cardiac morphology as well as with each other in hemodialysis patients. The study comprised a total of 120 patients (53 females and 67 males) receiving hemodialysis three times a week for at least 6 months and an age-gender matched control group (55 females and 58 males). Serum ADMA, LOX-1, and Apelin-13 levels were measured using the ELISA technique. Echocardiography, 24-h blood pressure monitoring by the Holter and carotid artery intima-media thickness (CIMT) measurement was performed on all of the included subjects. The associations between serum ADMA, LOX-1, and Apelin-13 levels with CIMT, echocardiographic parameters [left ventricular mass (LVM) and left ventricular mass index (LVMI)], and inflammatory markers [high sensitive C-reactive protein (hsCRP) and neutrophil lymphocyte ratio (NLR)] were evaluated by correlation analysis. Serum ADMA, Apelin-13, and LOX-1 levels were significantly higher in the hemodialysis group than the controls (P < 0.001, P < 0.001, and P < 0.001, respectively). CIMT, hsCRP, and NLR levels were also significantly higher in the hemodialysis group (P < 0.05, P < 0.001, P < 0.001, respectively). Significant correlations were observed among the serum ADMA, Apelin-13, and LOX-1 levels. Moreover, notably positive correlations were found between these three biochemical markers and LVM, LVMI, hsCRP, and CIMT. Serum ADMA, Apelin-13, and LOX-1 levels can be indicators not only for the inflammatory process but also for the pathogenesis of cardiovascular diseases in hemodialysis patients. © 2017 International Society for Apheresis, Japanese Society for Apheresis, and Japanese Society for Dialysis Therapy

Epinefrin anafilaktik reaksiyonun tedavisinde sıklıkla kullanılan bir ajandır. Anafilaktik reaksiyonlarda epinefrin kullanımına bağlı göğüs ağrısı, miyokardiyal enfarktüs ve aritmi olabileceği bildirilmiştir. Biz bu makalede ciddi anafilaksisi olan bir hastada, subkutan olarak order edilen ancak yanlışlıkla intravenöz olarak uygulalan epinefrinin neden olduğu miyokardiyal enfarktüsü sunduk. Epinephrine is an agent which is often used in the treatment of anaphylactic reaction. It has been reported that chest pain, myocardial infarction and arrhythmia may occur secondary to epinephrine use in anaphylactic reactions. In this report, we presented a case with anaphylaxis who developed myocardial infarction secondary to epinephrine, which was ordered to be administered subcutaneously but administered intravenously by mistake.

factPresentation Amaç Kounis Sendromu (KS) allerjenle temas sonrası koroner arterlerde geçici vazospazm gelişmesi olarak tarif edilir. Şiddetli göğüs ağrısı, ST eleve ya da non-ST eleve miyokardiyal infarkt şeklinde kendini gösterebilir. KS nedenleri arasında arı sokmaları, her türlü ilaçlar ve yiyecekler sayılabilir. Olgu 25 yaşındaki bayan hastada metamizol infüzyonu sonrası dispne ve senkop gelişmiştir. Acil servisteki takibi esnasında, şiddetli göğüs ağrısı oldu ve kan sonuçlarında Troponin I seviyesi yüksekliği saptandı. Koroner anjiyograma alındı ve koroner arterlerinde darlık saptanmadı. Yoğun bakıma yatırılıp destek tedavisi aldıktan sonra Troponin I seviyesi düştü ve hasta tam iyilik haliyle taburcu edildi.( Sakarya Tıp Dergisi 2016, 6(3):170-174 )Sonuç Mortalite ve morbiditeyi azaltmak için acil doktorları bu nadir hastalık için dikkatli olmalıdırlar. Objective Kounis Syndrome (KS) is described as temporary vasospasm of coronary arteries after exposure to an allergen. It may present as severe chest pain, ST elevated or non-ST elevated myocardial infarction. Among factors causing KS are bee stings, all kinds of medications and food. Case A 25-year-old female patient developed dyspnea and syncope following metamizole infusion. During follow-up in Emergency department, she developed severe chest pain and blood results revealed Troponin I elevation. She underwent coronary angiogram and any narrowing in coronary arteries was not determined. After hospitalization in the Intensive Care Unit with supportive therapy, levels of Troponin I decreased and the patient was discharged with a total recovery. Conclusion Emergency physicians must be aware of this rare entity in order to prevent mortality and morbidity. ( Sakarya Med J 2016, 6(3):170-174 )

Bu çalışmada tek taraflı yapıştırmalı bağlantılı kompozitler deneysel olarak araştırılmıştır. Farklı yüzey özellikleri, farklı kürlenme sıcaklıkları, farklı yapıştırıcı kalınlıklarının kompozit bağlantı üzerindeki etkileri araştırılmıştır. İlk olarak yapıştırıcı ve kompozit malzemelerin mekanik özellikleri tespit edilmiştir. Daha sonra çekme testleri, eksenel ve transvers darbe testleri, dört nokta eğme testi sırasıyla gerçekleştirilmiştir. Bütün bu testler farklı koşullar altında örneğin çalışma sıcaklığı, yüzey kalitesi, kürlenme zamanı, darbe enerjisi vb. yapılmıştır. Son olarak bu testlerden sonra farklı ortam koşullarında daha iyi mukavemet özelliklerine sahip olabilmesi için kompozit bağlantı üzerinde modifikasyonlar çalışılmış ve farklı konfigürasyonlar test edilmiştir. Bu çalışma yapıştırıcı kalınlığı, kürlenme sıcaklığı, yüzey kalitesi ve operasyon sıcaklığının, darbe enerjisinin kompozit bağlantının mukavemeti üzerinde önemli etkileri olduğunu göstermiştir. Bu etkiler genellikle olumsuz olsa da bazı durumlarda fayda sağlamışlardır. Bu çalışmada kompozit bağlantıların hasar analizlerine ek olarak bu faydalı etkilerden yaralanılarak kompozit bağlantı modifiye edilmiş ve daha yüksek operasyon sıcaklıklarında daha iyi mukavemet sağlayan kompozit bağlantı sunulmuştur. In this study adhesively bonded composite single-lap joints are investigated experimentally. Different surface properties, different curing temperatures of the adhesive, and the effects of different adhesive thickness on the composite joint strength are investigated. First, mechanical properties of the adhesive and adherend are investigated. Then tensile tests, axial impact tests, transverse impact tests and four point bending tests are conducted, respectively. All of these test are made in different conditions like operation temperature, surface quality, curing time, impact energy etc. Finally, after these series of the tests, for having good strength in different conditions, joint modifications are studied and some configurations are made on the composite joint and tested. This study showed that adhesive thickness, curing temperature, surface quality, operation temperature and impact energies have an important effect on the composite joints. Generally these effects were adverse but in some conditions these effects were useful. Additionally to the failure analysis, in this study, we used these useful effects to modify composite joints and we developed high strength composite joints in higher operation temperatures. 81
