Amaç: Çalışmamızdaki amaç çocuk sağlığı ve hastalıkları uzmanları, kulak burun boğaz uzmanları, pratisyen hekimler ve aile hekimleri arasında akut bakteriyel rinosinüzit tanı ve tedavisi konusunda farkındalık yaratarak güncel kılavuzlara uyum oranlarını analiz etmektir.Yöntem: Marmara Bölgesi'nde görev yapan hekimlere 18 sorudan oluşan bir anket gönderilmiştir. Yaş, cinsiyet, mesleki deneyim süresi, uzmanlık alanı ve bir ay içinde teşhis edilen vaka sayısı gibi yanıtlar ve değişkenler, Amerikan Enfeksiyon Hastalıkları Derneği'nin güncel kılavuzlarına göre değerlendirildi.Bulgular: Bu çalışmaya toplam 500 hekim katıldı. Hekimlerin çoğunluğunun [91,4% (n=458)] akut rinosinüziti viral üst solunum yolu enfeksiyonundan klinik olarak nasıl ayırt edeceklerini bilmediği gözlemlendi. Doğru antibiyotiği yanlış dozda reçete eden hekim sayısı 186 (%37,2) idi. Pratisyen hekimlerin diğer branşlara göre doğru kriterlerle tanı koymada daha fazla hata yaptıkları görüldü (p=0,03).Sonuç: Türkiye'de acil serviste pediatrik akut bakteriyel rinosinüzit tanı ve tedavisinde, muhtemelen duruma aşina olmama, aşırı hasta yükü, performans ücretleriyle ilgili endişeler ve mevcut kılavuzlarla ilgili tıbbi bilgilerin güncellenmemesi nedeniyle klinik atalet sorunu olduğu görülmektedir. Sürekli mesleki eğitime artan vurgu ve birinci basamak hekimleri için çeşitli destekleyici önlemler bu sorunu çözmeye yardımcı olabilir., Objective: Our aim in this study is to analyze the compliance rates with current guidelines by raising awareness among pediatricians, otolaryngologists, general practitioners and family physicians about the diagnosis and treatment of acute bacterial rhinosinusitis.Methods: A survey containing 18 questions was sent to physicians working in the Marmara Region of Turkey. The responses and variables, including age, gender, duration of professional experience, specialty area, and the number of cases diagnosed in one month, were evaluated concerning the current guidelines from the American Society of Infectious DiseasesResults: Totally 500 physicians were enrolled into this study. The majority of the physicians participating in the study [91,4% (n=458)] did not know how to differentiate acute rhinosinusitis from viral upper respiratory tract infection clinically. The number of physicians prescribing the correct antibiotic but at the wrong dosage was 186 (37.2%). It was observed that general physicians made more mistakes in determining the diagnosis with the correct criteria than the other specialties (p=0.03).Conclusion: There appears to be a problem of clinical inertia in the diagnosis and treatment of pediatric acute bacterial rhinosinusitis at the emergency department in Turkey, possibly due to unfamiliarity with the condition, patient overload, worries about performance fees, and failure to update medical knowledge concerning current guidelines. Increased emphasis on continuing professional education and various supportive measures for primary care physicians may help to resolve this.

Daha önce yerleşim ve imar durumu hemen hemen bulunmayan Ankara?yı, modern ve etkili bir ulusal mimari üslubun uygulanmasında görüyoruz. 1932-1938 döneminde ise batılı akımlar ve Avrupalı mimarların etkileri görüldükten sonra, ulusal mimari üsluba değinilmiş olup daha sonraları bu zaman diliminde Avrupa?da ve Türkiye?de görülen neo-klasik üslubun özellikleri üzerinde durulmuştur. Ankara da yapılan kamu binalarının planları, geçmiş dönemlere ait resimleri ortaya konmuş olup mimari üslupları, yapım aşamaları, dönemin koşulları, inşaat faaliyetlerinin durumu hakkında bilgiler verilmiştir. Sonuç bölümünde ise 1920-1938 döneminde genç Cumhuriyetin geçirmiş olduğu mimari üsluplar ve projeler ortaya konmuştur. Despite being a new state having risen from the ashes of wars, invasions and financial impossibilities, the young Turkish Republic gave priority to tackling the important problem of public works in the period of 1920-1932. One can observe that Ankara, with nearly no settlements and poor zoning status previously, was reconstructed in a modern and efficient national architectural style. In this study, the effects of the western trends and European architects in the period of 1932-1938 have been analyzed and then the national architectural style has been touched upon. This study has also laid emphasis on the features of the neo-classical architectural style in Turkey and Europe within this period. The study contains information about the plans, architectural styles and construction phases of the public buildings in Ankara and also about the conditions of that period and the situation of construction activities along with the old-dated illustrations of those buildings. In the final chapter, the architectural styles and projects which were produced by the young Turkish Republic in the era of 1920-1938 have been introduced. 215

Amaç: Ülkemizde çocuk sağlığı ve hastalıkları, kulak-burun-boğaz ve aile hekimliği alanında çalışan ve pratisyen hekimlerin pediyatrik akut bakteriyel rinosinüzitin tanı ve tedavisine ilişkin bilgi ve uygulamalarının güncel kılavuzlara uygunluk derecesini saptamak ve bu konuda farkındalık sağlamak.Materyal ve Yöntem : On dokuz soru içeren anket formu, Marmara Bölgesi'nde görev yapan ilgili hekimlere ulaştırıldı. Çalışmaya 500 hekim dahil edildi. Yanıtlar hekimlerin yaşları, cinsiyetleri, mesleki deneyim süreleri, uzmanlık alanları, bir ayda tanı koydukları vaka sayısı gibi değişkenlerle birlikte Amerikan Enfeksiyon Hastalıkları Derneği ve Amerikan Pediyatri Akademisi'nin güncel kılavuzları referans alınarak değerlendirildi.Bulgular : Akut bakteriyel rinosinüzit tanısı koyduğu her hastaya hemen antibiyotik tedavisi başlamayan hekim sayısı 409'du (%81,8). Doğru antibiyotiği başlayıp yanlış dozajdan reçete eden hekim sayısı 186 (%37,2) idi. Pratisyen hekimlerin ve ayda 20 ve daha fazla sayıda akut bakteriyel rinosinüzit hastası gören hekimlerin akut bakteriyel rinosinüzit tanısını koyma sürecinde daha yüksek oranda yanlış ölçütleri kullandıkları gözlendi (sırasıyla p=0,002 ve p=0,01). Kulak-burun-boğaz hekimlerinin ampirik antbiyotik tedavisini yanlış zamanda başlama oranı, diğer branş hekimlerine kıyasla daha yüksekti (p=0,02). Tanı doğruluğu, meslekte geçirilen süre ile ters orantılıydı.Sonuç: Çalışmamız, Türkiye'de pediyatrik akut bakteriyel rinosinüzitin tanı ve tedavisinde birinci basamak sağlık hizmetleri düzeyinde önemli bir klinik eylemsizlik sorunu bulunduğunu ortaya koymuştur. Bunun başlıca nedenlerinin çocukluk çağı akut bakteriyel rinosinüzitiyle az karşılaşma, hasta yükü, performans ücreti kaygısı ve kılavuzları takip ederek tıbbi bilgiyi güncel tutmada eksiklik olduğu düşünülmektedir.Tıbbi bilgileri güncellemeye yönelik özendirme çabaları ve çeşitli destek önlemleri, bu sorunu çözmede etkili olabilir.Anahtar sözcükler: sinüzit, tanı, tedavi, sağaltım harcamaları, potasyum klavulanat-amoksisilin kombinasyonu Objective: To determine the degree of concordance of the knowledge and the practice of physicians working in the fields of pediatrics and child health, otolaryngology, and family medicine in pediatric acute bacterial rhinosinusitis to the current guidelines and to raise awareness on this issue.Material and Methods: A questionnaire containing 19 questions was sent to physicians working in Marmara Region. Five hundred physicians were enrolled in the study. The answers and variables, such as age, gender, duration of professional experience, specialty areas, and number of cases diagnosed in one month, were evaluated with reference to the current guidelines of the American Society of Infectious Diseases and the American Academy of Pediatrics.Findings: The number of physicians who did not immediately start antibiotic treatment for each patient diagnosed as acute bacterial rhinosinusitis was 409 (81.8%). The number of physicians prescribing the correct antibiotic at the wrong dosage was 186 (37.2%). Family practitioners and physicians who had 20 or more acute bacterial rhinosinusitis patients per month had more tendency to use inappropriate tools in the process of diagnosis (p=0.002 and p=0.01, respectively). The time at which otolaryngologists started empirical antibiotic treatment was more frequently incorrect than that determined for other branch physicians (p=0.02). The diagnostic accuracy was inversely proportional to the duration of professional experience.Conclusion: Our study revealed an important problem of clinical inertia in the diagnosis and treatment of pediatric acute bacterial rhinosinusitis at the primary care level in Turkey. Its main reasons are thought to be infrequent encounters with childhood acute bacterial rhinosinusitis, patient overload, worries about performance fee, and failure to update medical knowledge by following current guidelines.Efforts in encouragement to update medical knowledge and various supportive measures for primary care physicians may be effective in solving this problem.Keywords: sinusitis, diagnosis, therapy, treatment costs, potassium clavulanate-amoxicillin combination 60

Bu çalışmada, Batı Anadolu?nun önemli kentlerinden biri olan, seramik ithalatı ve yerel üretimi konusunda arkeolojik veriler sunan Phokaia?da, siyah firnisli seramiklerin üretimi, ayrıntılı incelenmesi ve tanıtımının yapılması amaçlanmaktadır. Bu çalışmadan çıkacak sonuçların Phokaia?dan ya da diğer merkezlerden gelebilecek seramik malzemeleri için de, referans niteliği taşıması beklenmektedir.Tez çalışması kapsamında Foça İsmetpaşa Mahallesi 1056 Ada 13 Parselde yapılan sondaj kazı çalışmalarında ele geçen siyah firnisli ve siyah firnis bant bezemeli seramik örnekleri incelenmektedir. Bu seramik buluntularının el ve bilgisayar çizimleri yapıldıktan sonra, benzer örneklerden yola çıkılarak ana gruplara ayrılmaktadır. Ana grup içerisinde yer alan örnekler profil ve bezeme özelliklerine göre birden çok alt grupta yer alabilmektedir. Phokaia ve diğer merkezlerde ele geçen benzer örnekler dikkate alınarak tarihlendirilen siyah firnisli seramiklerin Phokaia?da, İ.Ö. 7. ve 2. yüzyıllar arasında sürekli bir kullanımının olduğu anlaşılmaktadır. Bu örnekler içerisinde ithal Attika siyah firnisli seramiklerinin yanında, çok sayıda Attika dışı yerel üretimin olması, Phokaia?da ithal seramiklerin yanısıra yoğun bir yerel üretimin yapıldığını göstermektedir. Anahtar Kelimeler: Phokaia, Siyah Firnisli Seramik, Seramik, İonia, Yerel Üretim The objective in this study is to make a detailed survey and presentation of the Black- Glazed Pottery finds from Phokaia which was one of the most important sites in Western Asia Minor yielding archaeological data concerning with the production and import of this specific type of pottery. The conclusions of this work will also provide comparative material for the same group that could come from other sites and will be a reference for the future studies.Within the scope of this thesis, the Black-Glazed and Black-Glazed Banded Ware from İsmetpaşa district in Foça (island no. 1056, parcel no.13) are evaluated. After the visual recording (drawing and photographing) of the material, the main grouping of the material was made. Various profiles and decorations on the sherds provided the necessary data for fixing sub-groups. The analogies proved that the Black-Glazed pottery from Phokaia can be dated to the long period of time starting from the seventh century BC and ending in the second century BC. An important concern is that the Black-Glazed pottery from Phokaia was not only composing of imported wares. ÖZET Hazar Havzası, 20. yüzyıl sosyoekonomik politikalarını büyük ölçüde etkileyen ve jeopolitik teorilerde yer işgal eden çok önemli bir coğrafi alandır. Dünya enerji kaynaklarının yükselen değeri olarak görülen Hazar Havzası muazzam enerji kaynaklarına sahip olması nedeniyle, uluslararası terörizm, nükleer silahların denetimi ve en önemlisi de enerji arz talebinin güvenliği ve çeşitlendirilmesi gibi kaygılarla dolu dünya ülkeleri için çok büyük bir önem taşımaktadır. Hazar Havzası petrollerinin dünya pazarlanna çıkış yönü batı ülkeleri, Türkiye ve de bölge ülkeleri için çok büyük bir önem taşımaktadır. Tezimizde, Hazar Havzası petrollerinin ihraç yönünün Bakü-Ceyhan doğrultusunda olması halinde Kafkasya ve Orta Asya ülkeleri ile Türkiye arasındaki ekonomik ve siyasi ilişkilerin gelişerek süreklilik kazanacağı üzerinde durulmuş, ayrıca Hazar Havzası petrollerinin taşınmasında çevresel açıdan kabul edilebilir, ekonomik yönden tercih edilebilir ve de siyasal açıdan uygulanabilir en güçlü boru hattı seçeneğinin hangi güzergahta kurulması gerektiği ele alınmıştır. Araştırmamızın birinci bölümünde Hazar Havzası'nda petrol rezervleri olan ülkelerin gelecekteki petrol üretim, tüketim ve ihraç potansiyelleri üzerinde durulmuştur. Bölüm içinde Azerbaycan petrolünün tarihçesi ve Azerbaycan'ın yaptığı petrol anlaşmaları gözden geçirilmiştir. İkinci bölümde, Hazar havzası petrollerinin hangi yöntemle dünya pazarlanna ulaştırılması gereği üzerinde durularak, belirlenen taşıma yöntemi çerçevesinde `alternatif olarak öne sürülen boru hatları incelenmiştir. Bölüm içinde, petrol taşımasından kaynaklanan risklerin Türk Boğazlarına olabilecek etkilerine değinilmiş ve bu anlamda gündeme getirilen ve Türk boğazlarını by-pass eden alternatif boru hattı olasılıklarının ekonomik ve çevresel anlamda incelemesi yapılmıştır. Araştırmamızın sonuncu bölümünde, Türkiye-Azerbaycan ekonomik ilişkileri incelenmiştir. Bölüm; Bakü-Tiflis-Ceyhan Petrol Boru Hattı'nın Türkiye ve Azerbaycan ekonomilerine olası katkılarının değerlendirilmesiyle sonuçlandırılmıştır. Çalışmamızın sonuna Azerbaycan'ın petrol yataklarının işletilmesi çerçevesinde yaptığı `Üretim Paylaşım Anlaşmalarının` ayrıntılarını gösteren bir ek bölüm konulmuştur. ABSTRACT Hazar Region as the oil and natural gas fields has become one the most important geographical regions that has not only influential on the socio-economical policies, but also on the geopolitical theories and interests of many states. As a result of consisting large amount of oil and natural gas resources, the region has been viewed as the rising star of the world's energy resources. Therefore, the region has become a very important place for world community which has been threatened and suffered by international terrorism, control of nuclear weapons, and the rapid decrease in the energy, possible variation in the energy sources and most importantly sharp increase in the energy demand. In my thesis, I have dwelled on the issue of the Bakü-Ceyhan Oil Export Pipe Line and if this project is accepted and made possible, it will increase and give more stabilized shape to the economical and political relations between Turkey and the countries in Caucasus and Central Asian. I have also discussed and questioned economically the most preferred, politically the most applicable Oil Pipe Line among the competitive pipeline offers. In the first chapter, I have discussed the future oil production; consumption and oil export potential of the countries in the Hazar Region. I have also provided information on the historjr of oil production in Azerbaijan, and the international agreements on the oil export between Azerbaijan and the other countries. The second chapter of my thesis is about the discussed oil export pipelines. I have discussed the most effective route for the oil pipelines, and in the frame of alternative routes, I have also evaluated the advantages and disadvantages of the each routes. This chapter also provides information on the possible threat for Turkish straights that may occur while the passage of the ships loaded with oil from Hazar region. While dwelling upon the question of the possible threats for Turkish straights, I have discussed the advantages of the pipeline routes that by-pass this danger. In the last chapter of my study, I have evaluated the economical ties and relations between Turkey and Azerbaijan. This chapter is concluded with the discussion on the Bakii-Tiblisi-Ceyhan Pipeline's possible contributions for Turkish and Azerbaijani economies. I have added an `Appendix` of the `Oil Production Sharing Agreements` that has been signed between the governments of Azerbaijan and various oil consortiums. 172
